1.Effect of Billroth Ⅱ Gastrectomy on Type 2 Diabetes Mellitus in Patients with Gastric Cancer

Objective To discuss the effect of Billroth Ⅱ gastrectomy on glucose metabolism in patients with gastric cancer complicated with type 2 diabetes mellitus(T2 DM).Methods A retrospective study on the serum level of glucose in 7 patients with gastric cancer complicated with T2 DM undergone Billroth Ⅱ gastrectomy.The therapeutic regime for T2DM after the operation was also analyzed.Results Billroth Ⅱ gastrectomy was completed in all of the 7 patients by open surgery(3) or laparoscopy(4).No serious intra- and post-operative complications occurred in the series.The preoperative level of fasting plasma glucose(FPG) was between 6.6 and 9.0 mmol/L(mean 8.1 mmol/L) and glycosylated hemoglobin concentrations(HbA1c) between 6.8% and 9.5%(mean 7.8%).While 1 to 8 months after the surgery,the FPG decreased to 4.8-7.9 mmol/L(mean 6.4 mmol/L) and HbA1c 5.5% to 7.2%(mean 6.3%).According to the criteria from ADA,4 patients were cured and 3 were improved.Conclusion Billroth Ⅱ gastrectomy is effective for type 2 diabetes mellitus in patients with gastric cancer.
2.Clinical Experiences on Endoscopic Thyroidectomy

Objective To investigate the causes and prevention of hemorrhage and recurrent laryngeal nerve injury during the endoscopic thyroidectomy via the breast areola approach.Methods Totally 36 patients underwent endoscopic thyroidectomy via the breast areola approach from January 2006 to January 2008.Results All the procedures were completed under an endoscope with a mean operation time of 105 min(50 to 180 min),and mean blood loss of 30 ml(20 to 150 ml).None of the patients were converted to an open surgery.One patient developed fat liquefaction,and one showed subcutaneous emphysema,no patient had injuries to any of the nerves or the parathyroid glands.The drainage tube was removed in 24 to 72 months after the surgery.The patients were discharged from the hospital 3 to 7 days postoperation.Postoperative pathological examination showed thyroid adenoma in 7 cases and thyroid nodular goiter in 29.The 36 patients were followed up for 3 to 24 months with a mean of 12 months.During the period,3 patients complained of chest symptoms and were cured spontaneously in 3 to 7 months.All the patients were satisfied with the aesthetic outcomes,none of them had local recurrence of the tumor.Conclusions Endoscopic thyroidectomy via the breasts areola approach is feasible and effective with good short-term and aesthetic outcomes.
3.Laparoscopic management of 54 cases of Mirizzi syndrome

As a result tentative diagnosis of Mirizzi's syndrome was established in 54 cases accounting for 0 56% of all LC patients.Results Laparoscopic procedure was successful in 46 out of 50 type Ⅰ cases. Four cases suffering from iatrogenic bile duct injury received primary repairment under laparoscopy. Fistulae repairment was successful laparoscopically in 3 out of 4 cases of type Ⅱ Mirizzi syndrome.Conclusion Type Ⅰ and type Ⅱ Mirizzi syndromes can be safely and successfully managed under laparoscopy in the hand of careful and skilled operators.
4.Impact of pelvic muscle training on quality of life in postoperative female patients with stress urinary incontinence

Objective To investigate the impact of pelvic muscle training (PFMT) on the quality of life (QOL) and incidence of postoperative complications in stress urinary incontinence (SUI) patients after tension-free vaginal tape surgery (TVT). In order to provide evidence for clinical nurses taking measures to promote rehabilitation of postoperative patients. Methods A total of 101 female SUI patients who underwent TVT surgery were included in this study. 50 patients in the trial group received pelvic muscle training guidance by one-to-one for 3months while 51 patients in the control group were instructed to the routine discharge guidance. All patients were asked to finish the Incontinence-specific Quality of Life questionnaire (I-QOL) at the day before surgery and 3 months after the surgery, respectively. And incidence of postoperative complications were evaluated at 3 months after the surgery. Results A total of 94 participants completed all questionnaires, including 48 patients of the trial group and 46 patients of the control group. The postoperative score of I-QOL in the trial group were higher than that of control group (94.32 vs. 90.34, Z=3.863, P<0.01). Further analysis found that avoidance and limiting behaviors domain score (90.63 vs.87.50, Z=3.227, P<0.01) and psychosocial impacts domain score (97.22 vs. 90.28, Z=4.186, P<0.01) in the trial group were higher than that of control group. The incidence of postoperative urgency urinary incontinence was significantly difference between the two groups (χ2=4.953,P=0.026), the proportion were 16.67% (8/48) in the trial group and 36.96% (17/46) in the control group, respectively. According to the results, the patients′overall quality of life of the trial group was better than that of control group after surgery. Conclusions Postoperative SUI patients′implementation of pelvic muscle training can significantly increase their quality of life and can decrease the incidence of urgency urinary incontinence.
5.Gastrointestinal stromal tumors of the stomach

Objective To investigate the clinical characteristics of gastrointestinal stromal tumor (GIST) of the stomach and to analyze the corresponding prognostic factors. Methods We retrospectively reviewed the clinical data of 121 patients in our hospital from March 1996 to March 2008. Gender、age、tumor size and histological type were analyzed. Results For these 121 cases the median survival time is 73.2 months. The overall 1-、3-、and 5-year survival rates were 82%, 59% and 50% respectively. The tumor-free survival rates for 1-、3-、and 5-yeas were 65%, 46% and 33% respectively. Cox univariale analysis revealed that gender、tumor size、organ involvement and recurrence were factors impacting prognosis. Cox multivariate analysis revealed that gender、tumor size、organ involvement、recurrence were prognostic factors for gastric GIST. Conclusions Male sex、tumor size (> 10 cm) and involvement of organ were independent indicators for a poor prognosis in patients with primary malignant gastric GIST.
6.Determination of taurocholic acid in Shedanchuanbei Oral Liquid and snake bile by SPE-HPLC

Objective: To establish the method for determination of taurocholic acid in the Traditional Chinese Medicine Shedanchuanbei Oral Liquid and snake bile. Methods: The sample was prepared as mixed solution containing methanol and KH 2PO 4. The mixed solution was injected into Sep Pak C 18 cartridge for the purpose of sample purity. In this processing, the substances which having strong retain action and could harm analytic column were hold in the Sep Pak C 18 cartridge. The eluting solution that the Sep Pak C 18 cartridge had be over loading for taurocholic acid was used as the test solution. The test solution was measured by RP HPLC. The chromatographic conditions were as followed: Supelcosil LC 8 column(150mm?4.6nm,5?m) as analytic column, detect wavelength at 203nm, and MeOH 0.4%KH 2PO 4 mixed solution(56∶44, V/V ) as mobile phase. The inject volume was 50?L. Results: The linear response range of sodium taurocholate was from 0.0253mg?mL -1 to 0.253mg?mL -1 , and the correlation coefficient was 0.9999. The average recovery rate was 101.3%, RSD was 0.40%( n =6). Conclusion: This method was simple, efficient and suitable to the quality control for Shedanchuanbei Oral Liquid and snake bile.
7.A study on the relationship between maxillary protraction treatment and changes of nasomaxillary complex facial soft tissue of children with skeletal Class m malocclusion

Objective To investigate nasomaxlllary complex facial soft tissue changes after the treatment with maxillary protraction appliance with skeletal Class Ⅲ malocclusion with a retruded maxilla.Methods Thirty growing subjects with skeletal Class Ⅲ malocclusions with maxillary retrognathism were selected and treated by facial mask(male 15,female 15,with an average age of 10.5).They were given a maxillary protraction treatment with face mask for 6-8 months.Cephalometric measurements about nasomaxillary complex soft tissue changes were analyzed to draw the statistic conclusion.Results After maxillary protraction treatment,PraY,nasofrontal angle,As-Y,UL-Y,UL-E,S-Ns-Sn increased (P < 0.01) ; M-Y increased (P < 0.05) ; LL-E,PosY,nasolabial angle decreased (P < 0.05).There were no significant differences in the Ns-Y and columella-tip angle.Conclusion After maxillary protraction treatment,nasomaxillary complex area becomes more marked.Both the nasomaxillary complex soft tissue and lower facial profile are dramatically improved.The combining effect of these two changes results in a more harmony profile.
8.Effects of Duraphat varnish on enamel demineralization after orthodontic treatment

Objective To explore the effect of duraphat varnish on reducing orthodontic tooth enamel demineralization around brackets.Methods We Selected 30 patients aged 12 to 14 years old in orthodontic Departrnent of Shenyang Stomatological Hospital from Jan 2011 to Dec 2013 and carried out rectification scheme to pull out the first premolar after test.The full mouth dental were divided into four parts by the quadrant and the first premolars of different groups were coated with Tooth Mousse,Fluor Protector and saline (as control group),duraphat varnish (as experimental group) respectively.Every group included 30 teeth.Three months later,We observed the demineralization of the teeth.The enamel decalcification of all quarters were detected by DI-AGNOdent.Results The rate of enamel demineralization in the experimental group was 10.0%,that in the Tooth Mousse group was 13.3%,the 0.1% Fluor Protector group 23.3%,the saline group 53.3%.There were significant statistical difference of the rate of enamel demineralization between the Duraphat varnish group and 0.1% Fluor Protector group,and that between the Duraphat varnish group and the saline group (P < 0.01).There was no statistical difference of that between the Duraphat varnish group and the Tooth Mousse group (P >0.05).There were no statistical difference of DIAGNOdent reading between the experimental group and the control groups before bonding(P > 0.05).After bonding,one month later and three months later,there was no statistical difference of DIAGNOdent reading between the Duraphat varnish group and the Tooth Mousse group (P > 0.05).There was significant statistical difference of that between the Duraphat varnish group and 0.1% Fluor Protector group(P < 0.01).Conclusion Duraphat varnish can reduce the tooth enamel demineralization more effectively than 0.1% Fluor Protector and saline in orthodontic treatment,and also can be used for children who were wearing fixed orthodontic appliances.
9.Changes of rat penumbral glucose transporter-3 expression following cerebral ischemia-reperfusion injury

BACKGROUND: Recent researches indicate that ischemia and hypoxia can lead to abnormal brain metabolism and even energy failure, which is an important reason for brain damage and necrosis and identifies energy metabolism disorder as the key event in brain ischemia-reperfusion (IR)injury. Glucose transporter-3 plays the vital role in brain energy metabolism.OBJECTIVE: To observe the changes of cerebral infarct volume and glucose transporter-3 mRNA and protein expressions in cerebral cortical penumbra at different stages of focal cerebral ischemia and reperfusion in rats.DESIGN: Randomized controlled experiment.SETTING: Department of Neurosurgery, Second Hospital Affiliated to Sun Yat-sen University.MATERIALS: This experiment was conducted in the Animal Laboratory of Medical Research Center, Second Hospital Affiliated to Sun Yat-sen University between August and October 2002.Totally 56 SD rats were randomized into 3 groups which were subjected to ① ischemia for 1 hour followed by reperfusion (n=28), ② ischemia for 3 hours followed by reperfusion (n=24), and ③ sham operation (n=4). The rats in the first group were subdivided into 7 subgroups for examination at 1, 3, 6, 12, 24, and 72hours and 1 week after ischemia, with 7 rats in each subgroup; the rats in the second ischemia group were also subdivided in similar manner but without a 1 hour postischemic subgroup. The rats in the sham operation group only received the operation but without arterial occlusion.METHODS: Focal cerebral ischemia-reperfusion (IR) injury model was induced in the rats in the two ischemic groups by means of insertion of suture for arterial occlusion, and the ratio of central ischemic area to cerebral infarct volume in the ischemic penumbra was examined at the specified time points. Reverse transcription-PCR (RT-PCR) was used to detect the expression of glucose transporter-3 mRNA in the cerebral cortex in ischemic penumbra region, and semi-quantitative immunohistochemistry (IHC) employed to detect the level of glucose transporter-3 protein.MAIN OUTCOME MEASURES: Cerebral infarct volume after IR injury, changes of transporter-3 mRNA and protein expressions after IR injury.RESULTS: Totally 56 rats were used in this experiment and all entered result analysis. The post-IR cerebral infarct volume was obviously smaller in 1-hour ischemia group than in 3-hour ischemia group. Glucose transporter-3 mRNA expression began to increase 3 hours after ischemia in 1-hour ischemia group, reaching the peak level at 24 hours and still mainrained higher level than that of the sham operation 1 week; in 3-hour ischemia group, the mRNA expression was slightly decreased at 3 hours but began to increase afterwards till reaching the peak level at 24 hours, followed then by recovery of normal level at 1 week. The changes in glucose transporter-3 protein and mRNA expressions followed almost the same pattern.CONCLUSION: Glucose transporter-3 expression is up-regulated in the ischemic penumbra region, possibly as a protective response to cerebral IR injury.
10.Compound flaps of foot with joint transplantation bridging severed thumbs and fingers

ObjectiveTo report the methods and clinical efficacy of bridging severed thumbs and fingers with foot joint compound flaps.Methods Ten patients with completely amputated thumbs and fingers with extensive defects in the proximal phalanx had bridged and repaired by using composite-free flap with joints from foot. Three of 4 cases of thumb amputation were repaired by using composite flaps of the second metatarsophalangeal and proximal interphalangeal joints in foot,and the other was used composite flap of the second metatarsophalangeal joints in foot.Donor sites underwent the second metatarsals distal osteotomy associated with free skin graft. And 6 cases with absent fingers by using composite-free flaps of the proximal interphalangeal joint to repair. The donor site in foot underwent metatarsophalangeal and toe joints osteotomy.ResultsAll flaps and replanted thumbs and fingers survived. Hand wounds recovered by primary repair.Donor site in foot all healed by primary repair except for 1 case,which healed after several dressing changes.All 10 cases were followed-up from 6 to 28 months,averaged of 9 months.The metacarpophalangeal joint of 4 replanted thumbs extension between -10° and 0°,and flexion between 20° and 50°.The function of fullfield digital mammography recovered well in 3 replanted thumbs, as well as the function of thumb-middle in the other. Three of them could completely 2-5 fingers tapping,one case could complete middle finger tapping,four cases could complete radial abduction.The proximal interphalangeal joints of 6 replanted fingers extension between -10° and 0°,and flexion between 30° and 90°,averaged of 50°.Sensory recovery of 1 case reached S4,two cases reached S3+,five cases reached S3,and 2 cases of S2.All replanted bones and joints healed after transplantations(bone healing time was 6 to 16 weeks), with no occurrence of re-fracture nor nonunion. Walking function was not significantly affected. According to the evaluation criteria of replanted severed fingers by Chinese Medical Association,one was excellent,eight were good,and 1 was poor.The excellent and good rate was 90%. Conclusion Repairing amputated thumbs and fingers with foot joint compound flaps could not only maximize the recovery of replanted fingers shape,but also get some function,which to meet everyday needs of patients.
