Objective: To explore the preventive measures for early stage pulmonary complications in old patients following thoracic surgery.Methods: We conducted psychotherapy,pulmonary infection control,training in breathing and instructions in expectorating phlegm in 127 old patients(over 70 years) undergoing thoracic surgery.Results: The patients experienced difficult expectoration on the first postoperative day because of increased sputum viscoelasticity,which was gradually improved later.Analgestic and mucokinetic measures produced intended results on the first three postoperative days,with significant improvement of sputum fluidity.And the concept of fast track surgery promoted the recovery of the patients.Conclusion: The preventive measures proved essential for the prevention of early stage pulmonary complications in old patients following thoracic surgery.

Many methods have been established in developing artificial airway since the first time that tracheotomy was developed by Egyptians and Indians 3000 years ago.Artificial airway is currently widely applied in critical care medicine,and plays an important role in the rescue of critically ill patients.It aims to improve ventilation and correct anoxia,to disencumber airway obstruction,to clean away airway secretions effectively and protect airway from risk of aspiration.This paper discussed about the nursing of tracheotomy and phlegm sucking,phlegm sucking related problems such as hypoxia,trachea hemorrhage and atelectasis,and ways to clean away secretions on air pocket.The risks of ventilator associated pneumonia,airway humidification and the physical and psychological needs were also discussed.

Objective To investigate pharmacodynamics of propofol for smooth exchange of tracheal tube for u laryngeal mask airway after surgery in elderly patients. Methods Twenty elderly patients, ASA Ⅰ or Ⅱ, aged 65-70 yr, scheduled for elective laparoscopic cholecystectomy were enrolled in this study. Anesthesia was induced with propofol, atracurium and remifentanil. The patients were mechanically ventilated after the trachea was intubated. Anesthesia was maintained with propofol combined with remifentanil. After surgery, a predetermined propofol concentration was achieved and a steady state maintained for at least 5 min. The target plasma concentration of propofol at which the laryngeal mask airway following tracheal extubation was attempted was determined by Dixon's up-and-down method with 0.5 μg/ml as the step size, The probit model was used to calculate the EC_(50) and EC_(95) and 95% confidence interval (95% CI) of propofol for inhibiting patient's response induced by smooth excharge of tracheal tube. Results The EC50 of propofol to achieve laryngeal mask airway following tracheal extubation was 2.79 μg/ml (95% CI 2.44-3.04 μg/ml) and EC_(50) 3.61 μg/ml (95% CI 3.27-4.78 μg/ml). Conclusion The target plasma concentration of propofol for laryngeal mask airway following tracheal extubation in elderly patients is 3.61 μg/ml.

Objective To evaluate the frequently used electrocardiographic criteria and propose a modified algorithm for differentiating typical atrioventricular node reentrant tachycardia(AVNRT)from atrioventricular reciprocating tachycardia(AVRT).Methods Twelve-lead electrocardiograms(ECGs)during sinus rhythm and atrioventricular node reentrant tachycardia(AVNRT)or atrioventricular reciprocating tachycardia(AVRT)with a narrow QRS complex were obtained from 154 patients who had received successful radiofrequency catheter ablation from Jan.2003 to Nov.2005.The ECGs of initial 104 patients were analyzed by 3 observers without knowledge of the electrophysiological diagnosis.According to these initial results,we proposed a modified stepwise ECG algorithm which used pseudo r′/S/Q waves,RP interval,and ST-segment elevation in lead aVR during tachycardia.Two observers assessed the algorithm in additional 50 patients.Results The algorithm was able to increase the overall accuracy from 77% and 79% with original algorithm to 84% and 87% with the modified algorithm,respectively.The inter-observer concordance was 85%.The intra-observer concordance was 89% in both investigators.Conclusion The modified algorithm can improve the accuracy of differential diagnosis between typical AVNRT and AVRT via concealed accessory pathway.

Objective To assess the clinical application value of color Doppler ultrasound guided puncture in peripherally inserted central catheter (PICC). Methods Thirty-two patients needed long-term intravenous infusion underwent PICC. Color Doppler ultrasound was used to select the puncture vascular,the best puncture point and angle, and the entire process was monitored and guided dynamically, and the initial position of the catheter tip was located. Results Color Doppler ultrasound-guided puncture was successful in all 32 patients, and the successful rate was 100%. The guided puncture time was 22 s to 19 min, and the first puncture succeeded in 30 patients (93.75%). Conclusion Color Doppler ultrasound-guided puncture in PICC can obviously raise the success rate of puncture, shorten puncture time and reduce the complications. It is an easy, safe and certain method.

Objective Aortic dissection is accute in occurrence along with fast change , which is inclined to to induce psy-chotic symptoms and affect the lives of patients .The study was to investigate the effects of healthcare cooperative intervention on pa-tients with psychiatric symptoms secondary to aortic dissection . Methods We established medical cooperation organization structure to clarify the division of responsibility along with a quick assessment form , a process chart and a green channel for emergencey treat-ment.The above method was applied to intervene 32 patients with psychiatric symptoms secondary to aortic dissection and observe the diagnosis time , the time from diagnosis to surgery , the remission time of psychiatric symptoms , the incidence of adverse events and the length of stay. Results 32 patients were diagnosed within 30min followed by emergency surgery within 30min after diagnosis, which was 1-2 hours shorter than the previous diagnosis and surgery preparation time .5-21 d after surgery , psychiatric symptoms have been effectively controlled.Extubation, falling out of bed, self-injury or injury and other adverse events did not occur .No patient died, and the average length of stay was 13.5d, which was 1.75d shorter than the length of stay of previous similar patients . Conclusion The healthcare cooperation method can quickly identify patients with psychiatric symptoms secondary to aortic dissection , prevent adverse events and shorten the length of stay .

Objective Comparing three screening schemes of colorectal cancer in general population to provide scientific basis for coiorectal cancer screening in communities. Methods Questionnaire screening among high-risk population and immunoassay fecal occult blood test (FOBT) were used for colorectal cancer detection. Colonescopy and pathological test were considered as gold standard of screening results. Results Nine coloreetal cancers and 796 colorectal adenomas were detected in 68 953 participants. Mean onset age of coloreetal adenoma was 7. 7 years earlier than colorectal cancer. For questionnaire and FOBT,the sensitivity was 44.4% ,77.8% and 100.0% ,while the specificity was 98.6%, 95.5% and 94.4%, respectively. The Youden index of questionnaire, FOBT, and questionnaire-FOBT sequential method was 0.4,0.7,and 0.9,and the likelihood ratio ( +/- ) was 32.7/0.6,17. 2/0.2,and 17.8/0.0,respectively. When 50 was used as initial age of screening, screening receivers accounted for 9.7% ,and 66. 7% colorectal cancer could be detected. When 40 was used as initial age of screening, screening receivers and cost increased by 57.1% ,and only 11.1% more patients with colorectal cancer could be detected. Conclusions Questionnaire-FOBT sequential method has high sensitivity and specificity of colorectal cancer screening. Screening at 50 year-old may be better for community residents. The time of colorectal adenoma developed to colorectal cancer is about 8 years.

Thirty-six male adult rats were divided into 12 groups. The rats, except the control group, were partially (68％) hepatectomized and then killed at intervals between 12-120h after operation. Isolated hepatocytes were prepared and flow cytometry was used to study the proliferative cycle. Mitotic index and binuclear cell count have been performed in liver sections and smears of isolated hepatocytes separately. Tetraploid cells acounted for 76% of all hepatocytes in normal rats and they also constituted the main proliferative population in regenerating liver. During 12-20h after operation, some of the tetraploid cells that remained in G_2 phase entered into mitosis. At 24h after operation, peaks of S phase in tetraploid cells and of octoploid cells occurred. At 36h after operation, mitotic index reached a maxium value and thereafter the percentage of binuclear cells were reduced rapidly. At 48-72h after operation, second peak of DNA synthesis occurred, but showed wide individual variations in time and cell proportion.

To find a way to improve the effects of clinical health education,the clinical health education nurses collocating qualifications to hold their posts, job duties, examinations of training, and practical efficacies were recommended. The setup of clinical education nurses had the advantages as follow: First, the clinical health education with high levels and high qualities can effectively strengthen the patients attending consciousness;second,scientific trainings of health education nurses were useful to pre control the qualities of the health education and improve the level; At last, respecting and caring for patients were the key points to make the patients satisfy the health education.

Objective To explore the influence of individualized nursing intervention on life quality of patients with rheumatoid arthritis. Methods 60 patients were divided into the intervention group and the control group with 30 cases in each group. The intervention group adopted individualized nursing projects according to the influencing factors of life quality based on routine nursing. The control group received routine nursing only. The life quality of the two groups was appraised by inventory before and after intervention. Results The status such as physiological function, mental function, social function and self-recognized health in the intervention group was greatly ameliorated and was statistically different from that in the control group(P＜0.05).Conclusion Individualized nursing intervention could dramatically improve the life quality of patients with rheumatoid arthritis
