1.Diagnostic value of MR mammography in breast ductal carcinoma

Objective To study the diagnostic value of MR mammography(MRM) in judging the nature of breast disease.Methods 145 cases with breast disease were retrospectively studied to investigate the relationships between two parameters(early-phase enhancement rate and the type of the delayed enhancement) and different breast diseases.All patients underwent MRM examination before surgical operation.Results ①Strengthen multiple in early stage larger than 3 times was inclined to malignant lesion;Strengthen multiple smaller than 1 time was inclined to benign lesion.②Drop type of delayed enhancement was the important parameter in diagnosis of ductal carcinoma.If drop type of delayed enhancement was viewed as the diagnosis standard of the ductal carcinoma of breast only:81% cases with invasive type of the ductal carcinoma,65% cases with noninvasive type of the ductal carcinoma had been diagnosed correctly;14% cases with benign breast lesions had been diagnosed exaggeratedly.③If strengthen multiple in early stage larger than 2.5 times in rising type of delayed enhancement and drop type of delayed enhancement was viewed as the diagnosis standard of the ductal carcinoma of breast,87.9% cases with invasive ductal carcinoma,73.1% cases with noninvasive ductal carcinoma had been diagnosed correctly;21.4% cases within benign breast lesions had been diagnosed exaggeratedly.Conclusion Compared with differential of disease nature by the type of the delayed enhancement only,the correct rate of diagnosis of breast ductal carcinoma mentioned above is raised if two parameters are combined.But the correct diagnosis rate of benign lesions is decreased.

3.The advanced techniques of dentoalveolar surgery.

During the recent decade, the advanced medical technology has brought the rapid development in the dentoalveolar surgery, and the medical concepts of humanity, painless, minimally invasive, safe and comfortable are gradually accepted by the patients and doctors. Many advanced techniques and equipments have been used in the dentoalveolar surgery. This paper would like to make a review on the clinical application and experience of the advanced medical technologies in the tooth extraction, such as the general anaesthetic technique, the nitrous oxide sedative technique, the computer-controlled local anesthesia delivery devices, the electrocardiogram montior, the location of embedded tooth by cone beam CT and minimally invasive surgical technique. Additionally, in this paper we also described the indication, contraindication, advantages and operating essentials of the orthodontic traction technique, ultrasonic bone surgery (piezosurgery) device and the surgery technology for alveolar bone shape and functional preservation in the extraction of tooth. Finally, we introduced the immediate implantation technique used in the dentoalveolar surgery.

4.Research on CT diagnostic analysis of three-dimensional magnetic palatal maxillary dentition direction

Objective:To discuss the changing position in three dimensions of maxillary dentition magnetic arch expansion after.Methods: Thirty patients who were in-patient cases were selected, and received three-dimensional data measured before and after treatment it its spiral CT, observed the three-dimensional position of maxillary dentition before and after treatment, and the angle of landmarks change data paired sign test for comparison.Results: Patients in the vertical position of maxillary dentition before and after treatment, sagittal and horizontal direction are changed. First premolars, first molars occurred cheek overall movement and dumping; the maxillary first premolar region is the largest widening position; three-dimensional position of the maxillary central incisor forward, the next move.Conclusion: Patients before and after the change of maxillary dentition MPE can be clearly presented in accordance with spiral CT, lay the foundation for further research MPE mechanism.
5.Vacuum sealing drainage plus skin transplantation repair skin soft tissue defects in tibia and fibula complicated fractures

BACKGROUND:Traditional treatment of tibia and fibula fractures complicated with skin soft tissue defect is mainly external fixation, wound debridement and dressing plus skin graft or skin flap transplantation after the infection is control ed and granulation grow. This method is disadvantageous due to high cost, great pain, long hospitalization stay, infection and necrosis, even the possibility of amputation.
OBJECTIVE:To explore the clinical efficacy of vacuum sealing drainage (VSD) and external fixator in the treatment of tibia and fibula open fractmes complicated with skin defects.
METHODS:A total of 38 patients with severe open fractures of tibia and fibula complicated with skin defects were selected from Benxi Central Hospital between January 2009 and September 2013. The size of skin defects was more than 5 cm2. After external fixation, the patients underwent complete debridement and dressing, fol owed by VSD treatment for 5-7 days. After granulation tissue grew and the wound became fresh, the patients received intermedite thickness skin graft. And 15 cases treated without VSD were retrospectively selected, serving as control group. The infection control rate and wound healing time in the two groups were compared.
RESULTS AND CONCLUSION:After 5-7 days of VSD treatment, VSD dressings were removed, wound infection was control ed, local wound granulation tissue was fresh. Both the wound and fractures were healed wel , no osteomyelitis or amputation occurred. Among the involved 38 patients, 23 cases were cured, 12 cases were effective, and three cases were ineffective. The total effective rate showed significant differences compared with the control group (92%vs. 53%;P<0.05). VSD can completely remove the wound secretions and necrotic tissue, improve local microcirculation, eliminate the infection, VSD plus external fixator and skin grating is a simple and effective method in treatment of tibia and fibula open fractures with soft tissue defects.
6.The analysis for images of multi-slice spiral CTE performance levels in active ulcerative ;colitis

Objective: To investigate the imaging characteristics of the intestinal multi-slice spiral CT enterography (CTE) in active ulcerative colitis(UC), analyze and summarize its application value. Methods: A total 43 cases diagnosed with active UC from April 2014 to April 2015 in our hospital were selected and divided into mild, moderate and severe three levels with each level as a group, treated by multi-slice spiral CTE examination system based on the improved Mayo system, comparing different performances of CTE levels. Results:43 cases of UC patients participating in the study, eight patients were mild, 16 cases were moderate and 19 cases were severe. There was statistically different in the intestinal mucosa in the presence of air bubbles between the mild and moderate group (x2=21.62; P<0.05). There were statistical differences between moderate group and severe group in the case of colon bags disappearing, as well as the lymph nodes of the intestinal wall layering (x2=25.95, x2=17.46, x2=19.46;P<0.05). At the same time, the differences of three groups of patients were not statistically significant on an increase in mesenteric vascular (P<0.05), the degree of enhancement in the intestinal mucosa, intestinal wall thickening, perirectal fat deposition, luminal stenosis and other projects (F=0.36, F=1.58, F=0.31;P>0.05). Conclusion:UC patients CTE examination, which can effectively conduct a comprehensive assessment against bowel, intestinal and parenteral, provide favorable conditions for the activity of UC clinical diagnosis and classification.
7.Clinical efficacy of transcatheter arterial embolization in the treatment of intractable hematuria

Objective To investigate the clinical effect of interventional embolization in the treatment of intractable hematuria after percutaneous nephrolithotripsy. Methods A retrospective analysis of the First Affili?ated Hospital of Kunming Medical University from February 2014 to March 2016 after percutaneous nephrolithot?omy appears intractable hematuria patients in 36 cases was conducted. All patients were treated with conservative treatment but invalid,then lesion was confirmed by renal artery angiography,after that,line embosphere micro?spheres or polyvinyl alcohol particles combined with spring ring interventional the treatment was conduc?ted. Postoperative urine color changes, postoperative complications and follow?up were observed whether have bleeding again. Results ( 1) The success rate:36 cases patients all with successful embolization,the symptoms of hematuria in 1?3 days gradually disappeared,and the effective rate was 100%. ( 2) Complications:the compli?cations after operation in 10 patients appeared different degree of embolism syndrome,given analgesic and antie?metic,the symptoms gradually ease after treatment. One case patient with a transient increase of creatinine after operation,renal function returned to normal 7 days after the treatment. ( 3) Follow up:there was no recurrence of hemorrhage after 3?6 months of follow?up. Conclusion Transcatheter embolization in the treatment of intracta?ble hematuria has definite curative effect,less trauma,high safety,less complications and faster postoperative re?covery.
8.Recent progress in partial splenic embolization treatment for hypersplenism due to hepatitis and cirrhosis

Nowadays partial splenic embolization (PSE) is an important therapeutic means to treat secondary hypersplenism caused by portal hypertension. By reducing the splenic blood flow and increasing hepatic blood supply, the peripheral blood picture and the liver function indexes as well as the portal hypertension can be effectively improved. This article aims mainly to make a comprehensive review on the therapeutic effect and mechanism of PSE, its clinical efficacy, as well as the factors affecting the clinical results, and to discuss the postoperative complications, the advantages and disadvantages of different embolic materials.
9.Applied distally based peroneus brevis muscle flaps for coverage of the soft tissue defects over the lower one-third of the leg, ankle and foot

Objective To investigate a simple, valuable new method for coverage of the soft tissue defect over the lower one-third of the leg, ankle and the foot. Methods The distally based peroneus brevis muscle flaps were applied for coverage of the soft tissue defects over the lower one-third of the leg, the ankle and the foot. The muscle flaps were covered with split-thickness skin grafts. Results The distally based peroneus brevis muscle flaps were applied for coverage of the soft tissue defects over the lower one-third of the leg, the ankle and the foot in 16 cases. The largest area of the soft tissue defect was 5 cm?7 cm. The smallest was 3 cm?4 cm. Primary healing occured in 14 cases undergoing muscle flap construction, second-stage healing occured in 2 cases, no total flap necrosis occured in any cases. Conclusion This technigue is a simple and complications are lesser. The successful rates are higher. This muscle flap is suitable to the mudium or small soft tissue defect especially.
10.Construction of immune inhibitor of new rhTNF-?

Objective:To construct an immune inhibitor of new TNF-?,C-terminal sequence of new rhTNF-? was replaced with sequence of T-help cell of hen egg-white lysozyme(HEL).Methods:The rhTNF-? mutant was cloned 、expressed and purified.Results:The DNA sequencing analysis showed that the C-terminal sequence of new rhTNF-? mutant was correct.The mutant was inserted into pBV220 expression vector .After the recombinant bacteria was incubated at 42℃ for 4 h,a new band of the protein with relative molecular weight of 1.7 kD was shown on the gel.The band amounted to 30 % of total bacteria protein.Western blot showed that the mutant protein could associated with anti-TNF-? antibody.After the protein was purified by through a column of Q-Sepharose Fast Flow, the purity of the protein was above 90%.The biological activity of the protein was measured with L929 cells.The result showed that biological activity of the protein was totally lost.Conclusion:The experimental evidence demonstrated that the construction of new rhTNF-? mutant was successful.The mutant not only can be associated with anti-TNF-? antibody, but also lose the biological activity of original TNF-?. [
