1.Circulating fibrocytes and chronic wound healing

BACKGROUND:Circulating fibrocytes originated from the peripheral blood are a fibroblast-like cel subpopulation of leukocytes. Circulating fibrocytes play an essential role in wound repair by secreting extracel ular matrix proteins and cytokines, initiating antigen-specific immunity, accelerating wound contraction as wel as promoting angiogenesis. However, the potential role for improving chronic wound healing and its mechanism is stil unclear. OBJECTIVE:To summarize the biological features of circulating fibrocytes and its potential role and mechanism in accelerating chronic wound healing. METHODS:A computer-based search of CNKI database from 2000 to 2014, PubMed database from 1994 to 2014 and Foreign Medical Journal Ful-Text Service database from 2000 to 2014 was performed using“circulating fibrocytes, non-healing wounds, diabetic foot ulcer, wound healing, cel therapy”in Chinese and English as the keywords to retrieve articles related to circulating fibrocytes, refractory mechanism of chronic wound healing and cel therapy for chronic wounds. Total y 54 articles meeting the inclusive criteria were included in result analysis. RESULTS AND CONCLUSION:Cel therapy with circulating fibrocytes is a safe and effective treatment for improving wound healing and has been used to repair various chronic wounds in the recent decades. Circulating fibrocytes are a distinct subpopulation of fibroblast-like leukocytes in the peripheral blood. It is able to migrate into wound sites at the early stage of wound repair and plays an active role in the wound healing process by secreting extracel ular matrix proteins, cytokines, presenting antigens, contracting wound and promoting neovascularization. Recently, it has been proved in animal experiments that circulating fibrocytes can accelerate chronic wound healing, especial y diabetic chronic wound.
2.Study on establishment of temporal lobe epilepsy model by administrating drug in brain region

Objective To explore a better method to establish temporal lobe epilepsy model by administrating drug in brain region.Methods Kainic acid(KA)4 ?g/kg was injected into rat's hippocampus by stereotactic operation.The rat's behavior,EEG and pathological changes were observed.Results After the rat's hippocampus injected with KA,staring,wet-dog shakes,masticatory movement and clonus of limbs occurred successively.The seizures were paroxysmal with rotation,unsettled state of jump and tic of limbs.The rats' behavior gradually recovered to normal after 10 hours.Then the spontaneous seizure(mostly rating 2~4)occurred 1~3 times every week.Cluster electric discharge,spike waves and sharp waves were recorded in cerebral cortex.KA-treated rats could result in hippocampal CA1 and CA3 fields neuronal degeneration and necrosis,especially significant neuron loss was observed in the CA3 field of KA injected ipsilateral side.Conclusions Injection KA in brain region of rat can establish temporal epilepsy model.The symptom,electrophysiology and pathological changes of temporal lobe epilepsy in the rat model are almost the same as those in human being.The KA induced rat model is an ideal tool to research human temporal lobe epilepsy.

BACKGROUND:Potential mechanisms of scar formation and scar prevention strategies have become more important in current medical research. Scar assessment scales have the potential to contribute to better evaluation of scar properties in both research and clinical diagnosis. OBJECTIVE: To summarize the literatures addressing scar prevention and clinical assessment. METHODS: A computer search of CNKI database from 2000 to 2014, PubMed database from 1975 to 2014 and Foreign Medical Journal Ful-Text Service Database from 2000 to 2014 was performed using the keywords of scar, prevention, treat, cicatrix, assessment, measure in Chinese and English to retrieve articles related to scar prevention strategies and scar assessment scales. Totaly 39 articles meeting the inclusive criteria were included in result analysis. RESULTS AND CONCLUSION: During wound healing, the unbalance between colagen anabolism and katabolism can lead to the formation of pathological scar. Prevention and treatment of pathological scars should be combined, and early intervention was used in wound healing. General scar assessment scales include Vancouver Scar Scale, Visual Analog Scale, Patient and Observer Scar Assessment Scale, Manchester Scar Assessment Scale, Stony Brook Scar Assessment Scale, al of which have the potential to contribute to better evaluation of scar properties in both research and clinical diagnosis.
5.Advances in p73-targeting anti-tumor drugs

The transcription factor p73 belongs to the p53 family of tumor suppressors,and can be transcribed into different isoforms with either pro-or anti-apoptotic(TAp73 and △Np73)functions.However,the tumor suppressor activity of TAp73 is inhibited through complex formation with inhibitory proteins(e.g.△Np73,mutant p53,MDM2 and iASPP).Therefore,it is a kind of tumor therapy strategy to reactivate TAp73 through targeting these inhibitors directly or release TAp73 from the complex by targeting their interaction.This review discusses the possible strategies of targeting p73 for its reactivation and the acting mechanism of related compounds.
6.Application of QCC in Improving the Compliance of Asthma/COPD Outpatients Handling Inhaler Devices

OBJECTIVE:To evaluate the effects of quality control circle(QCC)in improving the compliance of asthma/COPD outpatients handling inhaler devices. METHODS:By simple random sampling method,90 asthma/COPD outpatients receiving Tiotropium bromide powder for inhalantion,Salmeterol xinafoate and fluticasone propionate powder for inhalantion and Budesonide and formoterol fumarate powder for inhalantion were selected from our hospital during Apr. to Nov. 2015. The reasons for poor com-pliance of handling inhaler devices were analyzed by QCC. The countermeasures were formulated to improve the compliance,in-cluding clinical pharmacist education,follow-up,issuing education manuals,developing WeChat consulting. Operation step score of inhaler devices,compliance score and activity ability score of QCC members were analyzed statistically before and after QCC. RESULTS:After the development of QCC,operation step score of inhaler devices and compliance score in 90 asthma/COPD outpa-tients increased from 7.7 and 7.7 before activity to 8.8 and 9.3 after activity,the achievable rate of compliance was 114.3%,and the improvement rate was 20.8%. The sense of honor,responsibility,self-confidence,QCC techniques,communication and coordi-nation,team cohesion of all members were all improved obviously. CONCLUSIONS:QCC activity improves the compliance of asthma/COPD outpatients handling inhaler devices and shows the professional service level of clinical pharmacists,which is of sig-nificance to the improvement of pharmaceutical care.
7.Influence of Electro-acupuncture on E-Selectin and Resistin Protein Expression in Hippocampus of Rats with Chronic Stress Depression

Objective To observe the effects of electro-acupuncture on the E-selectin and resistin protein expression in the hippocampus of rats of chronic stress depression;To explore the role and mechanism of electro-acupuncture in the micro-environment of brain with depression. Methods 40 Fourty rats were randomly divided into four groups:blank group, model group, model+EA group, and model+fluoxetine group by using chronic stress combined with solitary raising methods, 10 rats in each group. Electro-acupuncture intervention was used 1 hour before the modeling. Electro-acupuncture was given at points Baihui (Du 20) and Yintang (Extra) for twenty minutes (2 Hz, 0.6 mA) every day. Fluoxetine was given at the volume of 5 mL/kg, dosage of 10 mg/kg. E-selectin and resistin protein expression in hippocampus were determined by biotin label-based antibody array. Results Compared with the blank group, the protein expression of E-selectin and resistin in the model group increased (fold change=1.23, 1.22). Compared with the model group, E-selectin and resistin expression decreased (fold change=0.65, 0.62;fold change=0.76, 0.65). Conclusion Electro-acupuncture intervention could down-regulate E-selectin and resistin expression in the hippocampus of chronic stress depression model.
8.Multicenter investigation of treatment for acute phase infant wheezing in outpatient departments in Shanghai areas

Objective To investigate the treatment of infant wheezing during acute phase in the pediatric out-patient departments of two tertiary hospitals and two secondary hospitals in Shanghai.Methods Simple random sam-pling method was adopted.Standardized questionnaires were given to the parents and physicians of infants who suffered wheezing during the acute phase to survey the treatment methods in the pediatric outpatient departments of two tertiary hospitals(Xinhua Hospital,Shanghai Children's Medical Center)and two secondary hospitals(Putuo District People's Hospital,Renhe Hospital)from February to May 2015 in Shanghai area.All data were analyzed by using SPSS 20.0 software.The enumeration data were used to describe the frequency distribution,composition ratio,and the rates among different groups were compared by the χ2test.Results The survey questionnaires were withdraw from 264 cases,of which 166 were boys and 98 were girls.In the treatment of acute exacerbation,the rate of antibiotic use was 94.7%(250/264 cases),in which 67.0%(177/264 cases)were intravenous drugs;62.1%(164/264 cases)patients were using inhaled glucocorticoid and short-acting beta-agonists as the main drugs for treatment;the ratio of systemic glu-cocorticoid using was 52.3%(138/264 cases),and the ratio of intravenous method was 43.2%(114/264 cases) among the cases.The usage ratio of systemic beta-agonist,anti-leukotrienes and antihistamines were 83.0%(219/264 cases),30.3%(80/264 cases)and 41.3%(109/264 cases),respectively.Of the 206 cases with respiratory in-fection,the rate of antibiotics use reached 98.5%(203/206 cases),and among the 58 cases without respiratory infec-tion,and the rate of antibiotics use acounted to 84.5%(49/58 cases).The difference between the two groups was sta-tistically significant(χ2=20.6,P<0.01). There were 200 cases of children with atopic physique background,of whom,42.5%(85/200 cases)were treated with antihistamine and 34%(68/200 cases)were treated with anti-leu-kotriene drugs.There were 64 cases without atopic physique background,of whom,37.5%(24/64 cases)were treated with antihistamine and 18.8%(12/64 cases)were treated with anti-leukotriene drugs.The proportion of anti-leuko-triene drug use in children with atopic physique background was significantly higher than that in children without atopic physique background,and the difference between two groups was statistically significant(χ2=5.34,P<0.05).Except for antihistamine treatment,the distribution of other treatments in different hospitals or outpatient departments was simi-lar(all P>0.05).Conclusions Nebulized glucocorticoid and beta-agonist therapy has become the routine treatment for infant wheezing in the acute phase in the tertiary and secondary level hospitals in Shanghai.The treatment of infant wheezing needs to be further standardized since the use of antibiotics,systemic glucocorticoids and intravenous drugs is excessive.

Background:
In China, secondary cytoreductive surgery (SCR) has been widely used in ovarian cancer (OC) over the past two decades. Although Gynecologic Oncology Group-0213 trial did not show its overall survival benefit in first relapsed patients, the questions on patient selection and effect of subsequent targeting therapy are still open. The preliminary data from our pre-SOC1 phase II study showed that selected patients with second relapse who never received SCR at recurrence may still benefit from surgery. Moreover, poly(ADP-ribose) polymerase inhibitors (PARPi) maintenance now has been a standard care for platinum sensitive relapsed OC. To our knowledge, no published or ongoing trial is trying to answer the question if patient can benefit from a potentially complete resection combined with PARPi maintenance in OC patients with secondary recurrence.
Methods
SOC-3 is a multi-center, open, randomized, controlled, phase II trial of SCR followed by chemotherapy and niraparib maintenance vs chemotherapy and niraparib maintenance in patients with platinum-sensitive second relapsed OC who never received SCR at recurrence. To guarantee surgical quality, if the sites had no experience of participating in any OC-related surgical trials, the number of recurrent lesions evaluated by central-reviewed positron emission tomography–computed tomography image shouldn't be more than 3. Eligible patients are randomly assigned in a 1:1 ratio to receive either SCR followed by 6 cyclesof platinum-based chemotherapy and niraparib maintenance or 6 cycles of platinum-based chemotherapy and niraparib maintenance alone. Patients who undergo at least 4 cycles of chemotherapy and must be, in the opinion of the investigator, without disease progression, will be assigned niraparib maintenance. Major inclusion criteria are secondary relapsed OC with a platinum-free interval of no less than 6 months and a possibly complete resection. Major exclusion criteria are borderline tumors and non-epithelial ovarian malignancies, received debulking surgery at recurrence and impossible to complete resection. The sample size is 96 patients. Primary endpoint is 12-month non-progression rate.
