Objective To evaluate the clinical application significance of the direct drug susceptibility test and the conventional drug susceptibility test in blood bacterial infection .Methods 280 specimens of positive blood culture in our hospital from March 2012 to April 2014 were randomly selected and performed the the direct drug susceptibility test and the conventional drug suscepti‐bility test .The correlation of the bacterial identification results and the drug susceptibility test results between these two kinds of method was investigated .Results The consistence of the direct bacterial identification method and bacterial identification method was 92 .5% ,the consistence rate of sensitivity ,drug resistance and moderate sensitivity had no statistical differences between the two kinds of method (P>0 .05) .Conclusion The direct drug susceptibility test can shorten the report time of blood culture posi‐tive specimen and effectively guide the rational use of antibiotics in clinic .

Objective Through epidemiological analysis of prehospital trauma care in Guangzhou during 1996～2004,to explore common characteristics and trends of it.Methods The data of the prehospital trauma patient were collected by Guangzhou First Aid Service Command Center from 1996 to 2004 and was prospectively analyzed. Results The number of prehospital care had been increased year by year(increased by 3.56 times in the nine years).The number of the trauma patients had been increased by 7.98 times.Head injury was in the first place(6.86%). Most of casualties fell into the age of 21～40 years old.The male patients were the two times that of female patients.Conclusion Trauma has become an serious problem. The improvement of prehospital emergency network, ability of on-the-spot emergency care and popularizing the knowledge of emergency are key to reduce the casualties.

Objective To investigate and evaluate the feasibility and analgesic effect of ultrasound guided intercostobrachial nerve (ICBN) blockade in patients with persistent pain after radical mastectomy with ICBN preservation.Methods In a total of 53 following-up female cases,aged 18-55 years,ASA Ⅰ or Ⅱ,receiving radical mastectomy with ICBN preservation during January,2014 to February,2016,19 patients complained persistent pain in the lateral chest,axilla and/or upper arm more than 3 months after the surgery were enrolled.ICBN blockade was performed using 0.5% ropivacine 10 mL injected in the potential space between the superior of pectoralis minor muscle and the inferior of serratus anterior muscle in the second intercostal space guided by ultrasound.Visual analogue scale (VAS) was applied to assess the pain intensity at rest,on movement,and with 100 kPa pressure before blockage and 30 min thereafter,and then the pain intensity (SPI) was calculated.All nerve blockade-related adverse events including local anesthetic-related allergy,central nervous system and cardiovascular toxicity were recorded.Seven-day follow-ups were required in all patients to assess the pain intensity.Results All 19 cases completed ICBN blockade guided by ultrasound,of which 17 cases completed the seven-day follow-ups.The mean difference in SPI was-9.19 VAS points (95%CI-11.24--7.14,P<0.01).Compared with each mean VAS and SPI before block,all that after block at day 1-6 were significantly decreased (P<0.01 or P<0.05).There were 14(82.4%),13(76.5%),7(41.2%) and 3(17.6%)cases that SPI was decreased over 5 points after block at day 1-4.No nerve blockade-related adverse event was observed.Conclusion Ultrasound guided ICBN blockade can relieve persistent post-mastectomy pain safely and effectively.

ObjectiveTo observe effects of ginkgo bilobate extract injection on the hemorheology,blood lipid and c-reactive protein in serum of patients with cerebral infarction. Methods62 cases patients with cerebral infarction were randomly divided into two groups by means of random number table. Two groups were both given routine treatment. The control group was treated with 20ml compound salvia injection, which was added to 250 ml normal saline, once a day, continuous therapy for 14 days, and the treatment group was treated with 20ml ginkgo bilobate extract injection, which was added to 250 ml normal saline, once a day, continuous therapy for 14 days. The clinical symptoms were observed, hemorheology related index, level of blood lipid and c-reactive protein were detected before and after the treatment. ResultsCompared with the control group,hemorheology related index decreased significantly in the treatment group, especially whole blood viscosity ( 1.57±0.28) mpa ·s (P＜0.01). Blood reduced viscosity (13.95 ± 2.07) mpa ·s, erythrocyte aggregation index (1.37±0.12) , hematocrit (42.36±8.16) were also decreased significantly (P＜0.05) .Triglyceride (TG)(1.23±0.12) mmol/L、 cholesterol (TC) (4.29±1.17) mmtmol/L、 low density lipoprotein cholesterol (LDIL-C)(2.31± 0.39) mmol/L and c-reactive protein ( 10.48 ±- 3.82) mg/L in serum in the treatment group were lower than those of the control group (P＜0.01 orP＜0.05) , high density lipoprotein cholesterol (HDL-C) ( 1.78±0.76) mmol/L in serum was higher than that of the control group (P＜0.01) .The total effective rate of the treatment group and the control group was 87.1％ and 67.7％ respectively, there was a significant difference (x2=3.553, P＜0.05). Conclusion The ginkgo bilobate extract injection can protect cerebral infarction from injury,which may be related with its action on regulating the blood viscosity and level of blood lipid.

Objective To investigate the change of high‐sensitivity C‐reactive protein(hs‐CRP) and white blood cell(WBC) in patients with positive sputum culture results .Methods hs‐CRP and WBC were determined in 116 patients with positive sputum culture results ,which were divided into 5 groups according to strains ,and control group of patients with negative sputum culture re‐sults .Levels and abnormal rates of various parameters were compared between each group .Results hs‐CRP levels significantly in‐creased in the 5 sputum culture positive groups ,compared with control group(P<0 .05) ,and that of Candida albicans group was the most high .WBC and neutrophile granulocyte(NEU) levels of 4 positive groups were obviously raised(P<0 .05) besides Haemophi‐lus influenzae group ,and the elevation tendency was the most obvious in Streptococcus pneumoniae group .The 5 positive groups were all with high detectable rates of hs‐CRP elevation .The detectable rate of WBC elevation in Streptococcus pneumoniae group was 64 .7% ,higher than Haemophilus influenzae group(P<0 .05) .Conclusion The majority of patients with positive sputum cul‐ture results could be with inflammatory response because of bacterial infection or colonization .

Objective To investigate the diagnostic value of procalcitonin(PCT) ,high‐sensitivity C‐reactive protein(hs‐CRP) andwhitebloodcelltestsinpatientswithbrucellosis.Methods 37blood‐culturepositivepatientsweredividedinto4groupsac‐cording to the species of bacteria infected .In addition to that ,PCT ,hs‐CRP and white blood cell were tested and the tests results were compared .Results The PCT and hs‐CRP concentrations of the 4 groups all increased and while the increasing extent of bru‐cella group were lower than the other 3 groups ,the differences were statistically significant(P<0 .05) .The counting of white blood cell(WBC) ,neutrophil(NEUT) and lymphocyte(LYMPH) were all in normal range while the monocyte(MONO) raised to 80 .0%in brucella group .WBC and NEUT apparently were higher in the other 3 groups than in brucella group ,while LYMPH and MONO were lower than brucella group ,the differences were statistically significant(P<0 .05) .Conclusion The determinations of PCT ,hs‐CRP and white blood cell may act as the indicators for early stage of brucellosis .

Objective:To systematically analyse the blood coagulation features of coronavirus disease 2019 (COVID-19) patients.Methods:An electronic search in PubMed, Scopus, Web of Science, EMbase, and CNKI to collect studies related to the blood coagulation features of COVID-19 patients from 1 January 2020 to 1 May 2020. Two reviewers independently screened literatures, extracted data and assessed the risk of bias of included studies. Then, the platelet count, D-dimer value, prothrombin time (PT), activated partial thromboplastin time (APTT) and fibrinogen of patients with different types of diseases were analyzed by using Stata12.0 software.Results:Thirty-nine retrospective studies involving 6 994 COVID-19 patients were included. The results of meta-analysis showed that:(1) compared with severe group, the platelet count (Weighted mean difference; WMD=20.11, 95% CI 11.53 to 28.69, P<0.001) and APTT (WMD=1.30, 95%CI 0.31 to 2.30, P=0.01) were found to be higher while D-dimer (WMD=-0.41, 95%CI-0.58 to-0.24, P<0.001), fibrinogen (WMD=-0.58, 95% CI-0.76 to-0.39, P<0.001) and PT (WMD=-0.51, 95%CI-0.92 to-0.10, P<0.001) were lower in mild group; the platelet count (WMD=-14.75, 95% CI-29.73 to-0.23, P=0.044) was found to be lower while D-dimer (WMD=1.06, 95% CI 0.65 to 1.47, P<0.001) was found to be higher in critical ill patients. (2)Compared with the survival group, the patients in death group displayed elevated levels of D-dimer (WMD=6.86, 95% CI 4.15 to 9.57, P<0.001) and PT (WMD=1.37, 95% CI 0.73 to 2.02, P<0.001) while platelet count (WMD=-36.40, 95% CI-63.23 to-9.58, P=0.008) remained low. Conclusion:Coagulation dysfunction was common in severe, critical ill and dead COVID-19 patients. Platelet count, D-dimer and PT levels were associated with the severity of the disease, and thus could be used as early warning indicators for the deterioration of the disease during hospitalization.

Aim To investigate the proliferative effect and the apoptosis of rat epididymal epithelial cells induced by podophyllotoxin and its underlying mechanisms.Methods Primary epididymal epithelial cells were cultured in vitro.CCK-8 assay was used to analyze proliferation of epididymal epithelial cells induced by podophyllotoxin on 24, 48 and 72 h.The ultra structural changes of the epididymal epithelial cells were observed by transmission electron microscope.AnnexinV-FITC/PI staining was used to quantify the percentages of apoptosis in the total cell population.The TdTmediated dUTP nick end labeling(TUNEL) technique was applied to observe the morphological changes of apoptotic cells.The expression of tumor necrosis factor alpha(TNF-α) mRNA was investigated by real-time RT-PCR.The level of TNF-α in cell culture supernatant was measured by enzyme-linked immunosorbent assay(ELISA) technology.Western blot was per-formed to determine the protein expression of cytochrome C, caspase-3, caspase-8, caspase-9.Results Podophyllotoxin significantly inhibited the activity of proliferation and induced apoptosis of epididymal epithelial cells in a dose-and time-dependent manner(P<0.05), with a 50％ inhibitory concentration(IC50) value and corresponding 95％ confidence intervals(CI) of 59.36(15.50～227.41), 0.37(0.080～1.70), 0.077(0.017～0.35) μmol·L-1 at 24, 48 and 72 h, respectively.Podophyllotoxin induced cell volume turned round and cell nuclear fragmentation and mitochondrial vacuolation.RT-PCR and ELISA results showed that podophyllotoxin improved the expression of TNF-α mRNA and protein.Western blot results demonstrated that podophyllotoxin activated the protein expression of cytochrome C, caspase-3, caspase-8 and caspase-9.Conclusion Podophyllotoxin can induce rat epididymal epithelial cell apoptosis through both the mitochondria-regulated intrinsic pathway and the TNF receptor-mediated extrinsic pathway.

A reversed phase high performance liquid chromatography （HPLC） method was established for the simultaneous determination of 12, 13-dihydroxyeuparin and glycyrrhizic acid in Yanyanfang mixture. A Grace Apollo Cl8 column （250 mm × 4.6 mm, 5 μm） was used as the stationary phase and the mobile phase was composed of acetonitrile and aqueous phosphoric acid （0.2%, v/v）. Gradient elution was carried out at the flow rate of 1.0 mL/min and the column temperature was 30 ℃. An ultraviolet （UV） detector was used with a selected wavelength of 240 nm. Calibration curves were linear within the concentration range of 4.6-45.75 μg/mL for 12, 13-dihydroxyeuparin （r〉0.9999） and 106.9-1068.9μg/mL for glycyrrhizic acid （r〉0.9999）, respectively. Recoveries were 102.18% for 12, 13-dihydroxyeuparin and 101.17% for glycyrrhizic acid. The method developed could be applied to the simultaneous determination of 12, 13- dihydroxyeuparin and glycyrrhizic acid in Yanyanfang mixture.

A high-speed counter-current chromatography (HSCCC) method was successfully developed for the preparative separation and purification of deoxyschizandrin from Schisandrae Sphenantherae Fructus in one step. The purity of deoxyschizandrin was 98.5%, and the structure was identified by MS, UV and NMR. This method was simple, fast, convenient and appropriate to prepare pure compound as reference substances for related research on Schisandrae Sphenantherae Fructus.
