1.Transvaginal Surgery for Cesarean Scar Pregnancy: Clinical Analysis of 30 Cases

Objective To evaluate the clinical effect and safety of transvaginal lesion resection for cesarean scar pregnancy (CSP).Methods A total of 30 patients with CSP in our hospital received the transvaginal CSP lesion resection from January 2013 to January 2016 and were followed-up to observe the clinical efficacy and safety.Results The operations were successfully completed except one case of conversion to open surgery.The operation time was 30-120 min (53.4±26.0 min), the intraoperative hemorrhage was less than 50 ml, the hospital time was 3-7 d (4.3±1.3 d), and the postoperative time of serum β-hCG decreased to normal value was 14-36 d (24.1±13.2 d).No continuous pregnancy or complications such as incision non-healing, infection, or bladder injury after operation.Conclusion The clinical effect of vaginal CSP lesion resection is good and has fewer complications, being worthy of application.
2.Epidemiological investigation on pre-hospital trauma first aid in Guangzhou

Objective Through epidemiological analysis of prehospital trauma care in Guangzhou during 1996~2004,to explore common characteristics and trends of it.Methods The data of the prehospital trauma patient were collected by Guangzhou First Aid Service Command Center from 1996 to 2004 and was prospectively analyzed. Results The number of prehospital care had been increased year by year(increased by 3.56 times in the nine years).The number of the trauma patients had been increased by 7.98 times.Head injury was in the first place(6.86%). Most of casualties fell into the age of 21~40 years old.The male patients were the two times that of female patients.Conclusion Trauma has become an serious problem. The improvement of prehospital emergency network, ability of on-the-spot emergency care and popularizing the knowledge of emergency are key to reduce the casualties.
3.Effect of ultrasound-guided intercostobrachial nerve blockade in patients with post-mastectomy pain syndrome

Objective To investigate and evaluate the feasibility and analgesic effect of ultrasound guided intercostobrachial nerve (ICBN) blockade in patients with persistent pain after radical mastectomy with ICBN preservation.Methods In a total of 53 following-up female cases,aged 18-55 years,ASA Ⅰ or Ⅱ,receiving radical mastectomy with ICBN preservation during January,2014 to February,2016,19 patients complained persistent pain in the lateral chest,axilla and/or upper arm more than 3 months after the surgery were enrolled.ICBN blockade was performed using 0.5% ropivacine 10 mL injected in the potential space between the superior of pectoralis minor muscle and the inferior of serratus anterior muscle in the second intercostal space guided by ultrasound.Visual analogue scale (VAS) was applied to assess the pain intensity at rest,on movement,and with 100 kPa pressure before blockage and 30 min thereafter,and then the pain intensity (SPI) was calculated.All nerve blockade-related adverse events including local anesthetic-related allergy,central nervous system and cardiovascular toxicity were recorded.Seven-day follow-ups were required in all patients to assess the pain intensity.Results All 19 cases completed ICBN blockade guided by ultrasound,of which 17 cases completed the seven-day follow-ups.The mean difference in SPI was-9.19 VAS points (95%CI-11.24--7.14,P<0.01).Compared with each mean VAS and SPI before block,all that after block at day 1-6 were significantly decreased (P<0.01 or P<0.05).There were 14(82.4%),13(76.5%),7(41.2%) and 3(17.6%)cases that SPI was decreased over 5 points after block at day 1-4.No nerve blockade-related adverse event was observed.Conclusion Ultrasound guided ICBN blockade can relieve persistent post-mastectomy pain safely and effectively.
4.Simultaneous determination of 12, 13-dihydroxyeuparin and glycyrrhizic acid in Yanyanfang mixture by high performance liquid chromatography

A reversed phase high performance liquid chromatography (HPLC) method was established for the simultaneous determination of 12, 13-dihydroxyeuparin and glycyrrhizic acid in Yanyanfang mixture. A Grace Apollo Cl8 column (250 mm × 4.6 mm, 5 μm) was used as the stationary phase and the mobile phase was composed of acetonitrile and aqueous phosphoric acid (0.2%, v/v). Gradient elution was carried out at the flow rate of 1.0 mL/min and the column temperature was 30 ℃. An ultraviolet (UV) detector was used with a selected wavelength of 240 nm. Calibration curves were linear within the concentration range of 4.6-45.75 μg/mL for 12, 13-dihydroxyeuparin (r〉0.9999) and 106.9-1068.9μg/mL for glycyrrhizic acid (r〉0.9999), respectively. Recoveries were 102.18% for 12, 13-dihydroxyeuparin and 101.17% for glycyrrhizic acid. The method developed could be applied to the simultaneous determination of 12, 13- dihydroxyeuparin and glycyrrhizic acid in Yanyanfang mixture.

A high-speed counter-current chromatography (HSCCC) method was successfully developed for the preparative separation and purification of deoxyschizandrin from Schisandrae Sphenantherae Fructus in one step. The purity of deoxyschizandrin was 98.5%, and the structure was identified by MS, UV and NMR. This method was simple, fast, convenient and appropriate to prepare pure compound as reference substances for related research on Schisandrae Sphenantherae Fructus.

An efficient method for the isolation and purification of 12,13-dihydroxyeuparin from Radix Eupatorii Chinensis by high speed counter-current chromatography (HSCCC) was established in this paper.The ether extracts of Radix Eupatorii Chinensis were purified by HSCCC with a solvent system of hexyl hydride ethyl acetate-methanol-water (1∶2∶1∶2,v/v/v/v).The upper phase was used as the stationary phase and the lower phase as the mobile phase.About 8.4 mg of 12,13-dihydroxyeuparin was obtained from 200 mg of ether extracts from Radix Eupatorii Chinensis in one-step HSCCC separation,with the purity of 96.71%,as determined by HPLC.After methanolwater recrystallization,the purity of 12,13-dihydroxyeuparin reached 99.83%.Such a simple and effective method was fairly useful to prepare pure compound as reference substances for related study on Radix Eupatorii Chinensis.

9.The neuroprotective machanism of Delta-opioid receptor activator against global cerebral ischemia-reperfusion

Objective To investigate the effects of Delta-opioid receptor activator DADLE on the expression of the p38 MAPK and the neuronal apoptosis after global cerebral ischemia-reperfusion and explore the neuroprotective mechanisms of DADLE.Methods Global cerebral ischmemia-reperfusion models in rats were induced by bilateral common carotid artery occlusion combined with hypotension.Rats were randomly divided into sham group (n=10),ischemia-reperfusion group(n=10) and three treatment groups with different doses of DADLE (2 mg group,3 mg group,5 mg group,n=10).TUNEL method and Western blot analysis were used to measure apoptotic neurons and expression levels of p38 MAPK phosphorylation,repectively.Results DADLE treatment significantly reduced neuron apoptosis (P<0.05).The expression levels of p38 MAPK were increased in ischemia-reperfusion group than in sham group (P< 0.05).In DADLE treated groups,the expression levels of p38 MAPK were dose-dependently decreased compared with the ischemia-reperfusion group.Conclusion Delta-opioid receptor activator DADLE can be neuroprotective against global I/R injury.Attenuation of apoptosis and p38 MAPK signal pathway might be involved in the neuroprotective mechanism of DADLE.
10.Angiotensin II type 1 receptor is required for the cardiac fibrosis triggered by mechanical stress independent of Ang II in mice

AIM:We investigated how AT 1-R stimulated by mechanical stresses induces cardiac fibrosis .METHODS:We produced in vivo cardiac pressure overload model in angiotensinogen knockout ( ATG-/-) mice and in vitro mechanically-stretched cell model in cultured neonatal cardiac cells of ATG-/-mice both lack the participation of Ang II .RESULTS: Pressure overload for 4 weeks in ATG-/-mice induced myocardial hypertrophy accompanied by the significant interstitial fibrosis , however , the TGF-β, a key regulatory factor of fibrosis, was not significantly increased in these ATG-/-mice.Meanwhile, the inhibitor for AT1-R significantly inhibited mechani-cal stress-induced cardiac fibrosis in these ATG-/-models whereas inhibition of TGF-βdid not.CONCLUSION:The results showed that mechanical stress-induced fibrotic responses through AT 1-R required the phosphorylation of Smad 2 but not the involvement of TGF-β.
