Objective:To analyze the usage of anti-hypertensive drugs in our hospital,and provide a reference for clinical rational drug use.Method:With the consumption sort and DDDs analysis,the consumption of anti-hypertensive drugs used in 2006-2008 was analyzed.Result:The consumption of anti-hypertensive drugs in our hospital increased every year,and the consumption of the angiotensinⅡreceptor blockers(ARB),calcium channel blockers(CCB),angiotensin-converting enzyme inhibitor(ACEI) in three years were the top three of the total drug consumptions.Conclusion:The use of anti-hypertensive drugs was basically rational in our hospital and matched the domestic usage of anti-hypertensive drugs.
2.The study of uterine artery blood flow at 11-16 weeks normal gestation

Objective To analyze the Doppler indices of uterine artery at 11-16 weeks normal gestation. Methods Two hundred and ninty-seven normal pregnant women were consecutively recruited to take routine ultrasound examination at 11-16 weeks' gestation. According to gestational week, they were divided into 6 groups (11-11+6 weeks, 12-12+6 weeks, 13-13+6 weeks, 14-14+6 weeks, 15-15+6 weeks, 16-16+6 weeks). According to the location of placenta, they were divided into 3 groups (central placenta, right placenta, left placenta). Finally, the pregnant women were divided into 5 groups (RI<0.60 group, 0.60-0.69 group, 0.70-0.79 group, 0.80-0.85 group and ≥ 0.85 group) according to resistance index (RI). Doppler indices of uterine artery were measured in each case. Results (1) The PIm, RIm, S/Dm values (the mean PI, RI, S/D values of bilateral uterine artery) were decreased with the progress of pregnancy, but the difference of the RIm, S/Dm were not significant. Only the decrease of PIm after 15 weeks was significant (P<0.05). The mean PIm of each group was:11 weeks 1.96±0.39, 12 weeks 1.94±0.45, 13 weeks 1.79±0.43, 14 weeks 1.79±0.36, 15 weeks 1.51±0.43, 16 weeks 1.50±0.30. (2) The PI, RI, S/D values of uterine artery with placenta attached were lower than the other side. In the left placenta group, bilateral RI difference was-0.04 (t=-3.095, P=0.005), bilateral PI difference was -0.24 (t=-3.232, P=0.004), bilateral S/D difference was-1.00 (t=-2.965, P=0.007);in the right placenta group, bilateral RI difference was 0.04 (t=6.159, P=0.000), bilateral PI difference was 0.43 (t=6.614, P=0.000), bilateral S/D difference was 2.05 (t=6.378, P=0.000);in the middle placenta group, bilateral RI difference was 0.02 (t=4.150, P=0.000), bilateral PI difference was 0.14 (t=4.475, P=0.000), bilateral S/D differencewas 0.54 (t=4.376, P=0.000). (3) According to the RI, incidence rates ofα-notch detected:in<0.60 group both sides were 0;in 0.60-0.69 group, left uterine artery was 0.08, right uterine artery was 0.08;in 0.70-0.79 group, left uterine artery was 0.34, right uterine artery was 0.31;in 0.80-0.85 group, left uterine artery was 0.65, right uterine artery was 0.72;in≥0.85 group, left uterine artery was 0.81, right uterine artery was 0.87. Conclusion The uterine artery Doppler indices of 11-16 weeks maybe a reliable and non-invasive method for examining uteroplacental perfusion.
3. MNAT1 expression in non-small cell lung cancer and its biological cellular impact

Objective:
To investigate the expression and significance of MNAT1 in non-small cell lung cancer (NSCLC) and to explore the biological impact of MNAT1 expression in lung cancer cells at the cellular level and related signaling pathway.
Methods:
Forty-eight cases of NSCLC tissues and paired normal tissues was collected at Nanfang Hospital, Southern Medical University from 2015 to 2017. The expression level of MNAT1 was detected by immunohistochemistry, and the relationship between MNAT1 and clinicopathological features was analyzed. The expression of MNAT1 was detected in lung cancer cells, MNAT1 level was analyzed after knocking down in A549 and H322 cells by siRNA; Plasmid vector of overexpressing MNAT1 was constructed, followed by transfecting H1299 cells and observing proliferation and migration at the cellular level. Flow cytometry was used to analyze the effect of the expression of MNAT1 on cell cycle, and Western blot was used to explore the possible molecular mechanism of MNAT1 on cell proliferation and cell cycle.
Results:

Purpose To explore the clinicopathologic fea-tures of Epstein-Barr virus-positive inflammatory follicular den-dritic cell sarcoma(EBV+IFDCS).Methods The clinico-pathologic features of 9 cases of EBV+IFDCS were retrospective-ly analyzed and followed up.Results The age of 9 patients with EBV+IFDCS ranged from 22 to 78 years(mean 44.7 years).7 cases occurred in the liver and 2 in the spleen.Fi-brinoid degeneration and hyaline degeneration in the vessel walls(6/9),eosinophilic infiltration(3/9),and epithelioid granulo-mas(2/9)were seen in some cases.The tumor cells expressed CD21(7/9),CD23(8/9)and CD35(9/9),partially ex-pressed SMA(6/9)and D2-40(1/9).It was noteworthy that 2 cases from the spleen accompanied by high expression of IgG4 plasma cells(80-135/10 HPF),and in the liver(0-36/10 HPF).All cases were followed up for 3-84 months,with 6 pa-tients disease-free,2 patients underwent metastasis,1 patient lost of follow-up.Conclusion EBV+IFDCS is a rare low-grade malignant tumor.EBER in situ hybridization and immunohisto-chemical detection play important roles in the diagnosis and dif-ferential diagnosis of EBV+IFDCS.Surgical resection is the main therapeutic intervention for EBV+IFDCS,and patients re-quire long-term post-surgical follow-up.

6.Clinical and pathological features of Dubin-Johnson syndrome

Objective:To explore the clinical and pathological features of Dubin-Johnson syndrome.Methods:The clinical and pathological characteristics of 18 cases of Dubin-Johnson syndrome diagnosed in the Department of Pathology of the Third Affiliated Hospital of Sun Yat-sen University from 2008 to 2018 were analyzed. Related literature was reviewed.Results:There were 15 male and three female patients. The male-to-female ratio was 5∶1. The age of the patients ranged from 17 to 73 years (median 24 years). Common clinical manifestations were jaundice, anorexia, and abnormal liver function tests. The pathological feature was the deposition of dark brown particles in the hepatocytes, especially those around the central veins.Conclusions:Dubin-Johnson syndrome mainly occurs in young people. The characteristic pathological changes are the deposition of dark brown particles in the hepatocytes around the central veins of the liver. The diagnosis of Dubin-Johnson syndrome mainly relies on clinical manifestations, laboratory examinations, histopathological examinations and genetic testing. The understanding of Dubin-Johnson syndrome can help reduce the rates of missed diagnosis and misdiagnosis of the disease.
7.Comparative study in the effects of two different cleaning methods combined with hypothermia sterilization for rigid endoscopic instruments

Objective To investigate the effects of different cleaning methods combined with hypothermia sterilization for rigid endoscopic instruments.Methods A total of 300 thoracoscopes, 60 mediastinoscopes and 200 titanic clips were selected in the supply room of Nanjing Chest Hospital from March to September 2015 and randomly divided into two groups. The control group underwent traditional hand-washing combined with hypothermia sterilization, while the observation group underwent enzymatic ultrasonic cleaning combined with hypothermia sterilization. The cleaning effects of two groups were compared using three different methods: visual method, protein clean & testing stick, and pharynx swab culture.Results All the endoscopic instruments in the observation group has a higher qualified rate than those in the control group, with statistically significant differences between two groups (P<0.05). Both groups have a qualified rate of 100%.Conclusions The enzymatic ultrasonic cleaning combined with ethylene oxide or hydrogen peroxide hypothermia sterilization for endoscopic instruments guarantes the quality of cleaning and sterilization and is worthy to be promoted.
8.Efficacy of percutaneous endoscopic gastrostomy in pediatric patients.

OBJECTIVETo analyze the efficacy of percutaneous endoscopic gastrostomy (PEG) in pediatric patients.
METHODFrom October 2011 to October 2014, children in the gastrointestinal ward of Guangzhou Women and Children's Medical Center received PEG or jejunal tube PEG(JET-PEG). The success rate, operation time were recorded. The changes of their weight, enteral nutrition calories and the incidence of pneumonia before and after the first 6 months of operation were compared. Follow-up was conducted until October 2014, the recent and long term complications, the length of indwelling time, the replacement or removal of the tube were recorded, the patients swallowing function or the primary disease's outcomes were observed.
RESULTOf the 13 cases, 10 were male, 3 were female, their average age was 2 years (range 1.8 months-9 years). We performed PEG for 12 of the patients who had congenital craniofacial problems that led to feeding difficulties or recurrent cough and pneumonia (6/12), or neurological disorders (6/12) with inability to swallow, and in one case JET-PEG was performed, this child suffered from chronic intestinal pseudo-obstruction with vomiting and abdominal distension. The gastrostomy was successful in all the patients through one operation, the average operation time of PEG was (25 ± 3) minutes, JET-PEG was 60 minutes. One local skin infection was noted, no long-term complication occurred. In the first 6 months after operation, all the patients gained weight((5.5-30.5) kg postoperation vs. (3.0-30.0) kg preoperation), and 12 cases' enteral nutrition calories increased (from (209-502) to(272-543) kJ/(kg·d)), the incidence of pneumonia decreased in the children who had recurrent pneumonia before the operation (from (0-1.5) to (0-0.16) per month). Until October 2014, their average length of gastric tube indwelling time was 17.8 months (range 4-36 months). In 4 cases PEG tube was removed when they could eat completely independently, the other 9 needed enteral vein nutrition via PEG tube or jejunal tube, in 3 of them balloon type gastric fistula tube was applied. Two of the 13 cases who had cleft palate received stomatological operations when their weight grew to meet the standard.
CONCLUSIONPEG and JET-PEG are safe and effective method for enteric nutrition feeding in pediatrics, the technique causes minimal trauma and has rapid postoperative recovery, few complications, good aesthetic appearances and simple nursing, it can significantly improve their nutritional status and quality of life.

9.Detection of Rare Mutations in EGFR-ARMS-PCR-Negative Lung Adenocarcinoma by Sanger Sequencing

PURPOSE: This study aimed to identify potential epidermal growth factor receptor (EGFR) gene mutations in non-small cell lung cancer that went undetected by amplification refractory mutation system-Scorpion real-time PCR (ARMS-PCR). MATERIALS AND METHODS: A total of 200 specimens were obtained from the First Affiliated Hospital of Guangzhou Medical University from August 2014 to August 2015. In total, 100 ARMS-negative and 100 ARMS-positive specimens were evaluated for EGFR gene mutations by Sanger sequencing. The methodology and sensitivity of each method and the outcomes of EGFR-tyrosine kinase inhibitor (TKI) therapy were analyzed. RESULTS: Among the 100 ARMS-PCR-positive samples, 90 were positive by Sanger sequencing, while 10 cases were considered negative, because the mutation abundance was less than 10%. Among the 100 negative cases, three were positive for a rare EGFR mutation by Sanger sequencing. In the curative effect analysis of EGFR-TKIs, the progression-free survival (PFS) analysis based on ARMS and Sanger sequencing results showed no difference. However, the PFS of patients with a high abundance of EGFR mutation was 12.4 months [95% confidence interval (CI), 11.6−12.4 months], which was significantly higher than that of patients with a low abundance of mutations detected by Sanger sequencing (95% CI, 10.7−11.3 months) (p < 0.001). CONCLUSION: The ARMS method demonstrated higher sensitivity than Sanger sequencing, but was prone to missing mutations due to primer design. Sanger sequencing was able to detect rare EGFR mutations and deemed applicable for confirming EGFR status. A clinical trial evaluating the efficacy of EGFR-TKIs in patients with rare EGFR mutations is needed.

10.Consensus on technological standards for non-invasive prenatal screening of pathogenic copy number variations by high-throughput sequencing of maternal plasma cell-free DNA.

Genomic disorders caused by pathogenic copy number variation (pCNV) have proven to underlie a significant proportion of birth defects. With technological advance, improvement of bioinformatics analysis procedure, and accumulation of clinical data, non-invasive prenatal screening of pCNV (NIPS-pCNV) by high-throughput sequencing of maternal plasma cell-free DNA has been put to use in clinical settings. Specialized standards for clinical application of NIPS-pCNV are required. Based on the discussion, 10 pCNV-associated diseases with well-defined conditions and 5 common chromosomal aneuploidy syndromes are recommended as the target of screening in this consensus. Meanwhile, a standardized procedure for NIPS-pCNV is also provided, which may facilitate propagation of this technique in clinical settings.
