1.Recent research progress on anti-addiction drugs

Recent discoveries about the effects of drugs of abuse on the brain and the mechanisms of their addictions, and new actions of available medications are offering many opportunities for the discovery and development of novel medications to treat addictive disorders.This article reviews the current medications that have shown promising results for treating opioid, cocaine, methamphetamine, cannabis, alcohol and nicotine addictions.
2.The evaluation of OMOM capsule endoscopy in diagnosis of gastrointestinal disease in elderly patients

Objective To explore the value of OMOM capsule endoscopy in the diagnosis of gastrointestinal diseases in the elderly patients.Methods A retrospective analysis of the clinical data on 86 elderly patients completed OMOM capsule endoscopy during May 2008 to April 2013 were performed.Results All the patients were well tolerated,and capsules were successfully excreted.The mean detention time in gastrointestinal was 50.64 min,the mean run time was 315.86 min.Thirty-eight (44.19%,38/86) patients gained positive diagnosis.In 45 patients with unknown reason,28 patients (62.22%,28/45) gained positive diagnosis.In 28 patients with abdominal pain and diarrhea,5 patients gained positive diagnosis.In 4 patients with other reason,no patients gained positive diagnosis.Conclusions The manipulation of OMOM capsule endoscopy is simple,safe and effective.And the elderly patients can tolerate well.Obscure gastrointestinal bleeding is the main reason to check,and has a higher diagnostic yield.
3.Application value of polymerase chain reaction combined with probe-based in the diagnosis of pediatric Mycoplasma pneumoniae pneumonia and drug resistance mutation

Objective To evaluate the value of polymerase chain reaction (PCR) combined with probe detection method in diagnosis of Mycoplasma pneumonia (MP) pneumonia (MPP) in children and to analyze the factors influencing the diagnostic accuracy,and to identify the rate of MP mutation for drug resistance and the involving factors.Methods Two hundred and twenty-five children with MPP hospitalized in the Department of Respiratory Medicine,Beijing Children's Hospital,Capital Medical University between June 2015 and March 2016 were enrolled in this study.Nasopharyngeal swab samples from the participants within 24 hours of admission were detected by using PCR combined with fluorescence probes for MP-DNA and macrolide-resistant mutations.The information of age,sex,clinical symptoms,course of disease,duration by admission,the history of macrolide treatment and the increase or decrease of quadruple or more serum MP antibody titer were extracted from medical records within 4 weeks of treatment,which received further correlation analysis with the detection rate of MP-DNA and the drug resistance mutation.Results The sensitivity of the MPP by using the method of PCR combined with fluorescence probes was 80.4% (181/225 cases),while the specificity was 98.0% (99/101 cases).The MP-DNA positive rate for patients with double MP antibody 4 times increased during treatment was 88.8% (71/80 cases),which was significantly higher than that of patients with antibody titer ≥1 ∶ 160 [75.9% (110/145 cases)],and the difference was sigmficant(x =5.443,P =0.020).The positive rate of MP-DNA of patients had no obvious association with gender,age,and disease duration and macrolide treatment history before admission.Macrolide-resistant mutation rate of MP-DNA was 85.1% (154/181 cases),macrolide-resistant mutation rate of MP for patients finishing one course of macrolide treatment when admission(89.6%)was higher than that of the patients without using macrolide and the patients treated with macrolide but not finishing one course of treatment (71.9% and 86.6%),and the significant difference among the three groups was observed(x2 =4.454,P =0.035).Conclusions PCR combined with fluorescence probe for MP-DNA detection has a high accuracy for the diagnosis of MPP,and the overall mutation rate is high,suggesting that the clinical treatment of MPP needs to be adjusted according to drug resistance in children.
4.The application of biomarkers in early identification and risk stratification of acute chest pain

Chest pain is among the most frequent cause of emergence department (ED) encounters.The etiological factors of chest pain are various and complex,which make early diagnosis remains a clinical challenge.Even worse,on account of the difficulty in rapid risk stratification,some high-risk patients didnt get optimal therapy,whereas low-risk patients got unnecessary hospitalization and extensive evaluation.With the quick development of biomarkers,early diagnosis and risk stratification becomes possible.In this paper,a systemic review of these biomarkers was performed.
5.Reliability and validity of Chinese version of the Spirituality and Spiritual Care Rating Scale

Objective:To translate the Spirituality and Spiritual Care Rating Scale (SSCRS) into Chinese and test its reliability and validity.Methods:After obtaining authorization from the original author, we carried out translation, back translation and cross-cultural adaptation based on the Brislin principle, and formed the Chinese version of the SSCRS. From April to June 2019, we conducted the questionnaire survey among 189 nurses at a Class Ⅲ Grade A hospital in Shiyan by convenience sampling to evaluate the reliability and validity of the Chinese version of SSCRS.Results:The Chinese version of SSCRS had 17 items. Exploratory factor analysis extracted a total of 4 common factors, with a cumulative contribution rate of 62.83%. The Cronbach's α coefficient of the entire scale was 0.840, the split-half reliability was 0.842, and the test-retest reliability was 0.860. The scale-level content validity index of all items was 0.91, and the item-level content validity index of each item ranged from 0.81 to 1.00.Conclusions:The Chinese version of SSCRS has a good reliability and validity, which can be used as an effective tool to evaluate nurses' cognition of spirituality and spiritual care in China.
6.Application of anti-HCV and HCV RNA detection in intravenous drug users

Objective:To explore the diagnostic value of anti-HCV and HCV RNA so as to provide an accurate and efficient detection strategy for the diagnosis of HCV in intravenous drug users.Methods:527 plasma samples from intravenous drug users were collected, and preliminary anti-HCV ELISA screening test was performed. A recombinant immunoblot assay (RIBA) was used as confirmatory assay for reactive antibody samples. All samples were tested for HCV RNA, followed by analysis of anti-HCV screening test, RIBA and HCV nucleic acid test results.Results:Anti-HCV ELISA results were reactive in 386 out of 527 intravenous drug users and non-reactive in 141. Among the 386 reactive antibody samples detected by RIBA, 370 cases were anti-HCV positive, 6 cases were anti-HCV indeterminate and 10 cases were anti-HCV negative. Anti-HCV ELISA and RIBA positive coincidence detection rate was 95.85% (370/386), and 70.21% (370/527) among intravenous drug users. HCV RNA was negative in all 10 anti-HCV RIBA non-reactive samples. 376 anti-HCV RIBA-positive and indeterminate samples were tested for HCV RNA, of which 56.93% (300/527) were current HCV infection, and 14.42% (76/527) were past HCV infection. Among 141 anti-HCV ELISA negative samples, the residual risk by anti-HCV ELISA screening for HCV RNA was 1.52% (8/527). HCV viral load distribution among intravenous drug users showed that the high viral load value (>10 7 IU/ml) and low viral load values (< 10 2 IU/ml) accounted for 1.95% and 2.27%, respectively, while the samples with viral load value of 1×10 2 ~ 1×10 7 IU/ mL accounted for 95.78% (295/308), and were mainly distributed in 1×10 5 ~ 1×10 6 IU/ml (37.99%). ELISA + RIBA + NAT assay detection strategies had differentiated 300 cases of current HCV infection, 76 cases of past HCV infection and 10 cases of false positive anti-HCV results, while ELISA+NAT assay detection strategies had only detected 300 cases of current HCV infection. However, of the 386 positive subjects screened for antibodies, 10 (2.59%) were undifferentiated false positives. Conclusion:Intravenous drug users are the high-risk population of HCV infection with high prevalence and high viral load. Anti-HCV screening for intravenous drug users will have a certain degree of residual risk. Therefore, anti-HCV ELISA screening and nucleic acid detection strategy can accurately diagnose the current infected patients; however, it cannot distinguish the false positive results of antibody screening.
7.Epidemiological characteristics and macrolide-resistance of children hospitalized with Mycoplasma pneumo-niae infection in Beijing from 2016 to 2019

Objective:To investigate epidemiological characteristics and macrolide-resistance of hospitalized children with Mycoplasma pneumoniae (MP) infections in Beijing from 2016 to 2019, so as to provide basis for the prevention and treatment of pediatric Mycoplasma pneumoniae pneumonia (MPP).Methods:The clinical data were analyzed retrospectively from 8 691 children hospitalized with community acquired pneumonia in Beijing Children′s Hospital between January 2016 and September 2019.MP RNA was detected by simultaneous amplification and testing (SAT), and macrolide resistance of MP was examined by MP and macrolide-resistant isolate diagnostic kit (PCR with fluorescence probes). Chi- square test was used for categorical analysis. Results:Among 8 691 cases detected by SAT, the overall detection rate of MP was 28.10% (2 442/8 691 cases). The detection rates of MP from 2016 to 2019 were 26.23%, 31.36%, 27.84 % and 26.57%, respectively.The detection rate of MP in 2017 was significantly higher than that in other years ( χ2=16.11, P<0.05). The detection rate of MP in females was 29.65%(1 107/3 733 cases), which was evidently higher than that in males 26.93%(1 335/4 958 cases) ( χ2=7.85, P<0.05). The positive rates of MP in summer[32.21% (726/2 254 cases)] and autumn[39.76%(852/2 143 cases)] were significantly higher than those in spring[17.00% (327/1 924 cases)] and winter[22.66%(537/2 370 cases)] ( χ2=315.15, P<0.001). The percentages of MP were 35.06%(732/2 088 cases) in preschoolers and 37.71%(1 160/3 076 cases) in school-age children, which were significantly higher than 11.20%(232/2 072 cases) in infants and 22.01% (318/1 445 cases) in toddlers ( χ2=509.89, P<0.001). Macrolide resistance detection was conducted in 1 524 patients by fluorescent PCR.Among them, 1 386 patients were positive for drug resistance, and the positive rate was 90.94%.The prevalence of macrolide-resistant MP from 2016 to 2019 were 88.19%, 90.93%, 90.56% and 92.90%, respectively.Macrolide-resistant rates were not related with gender, age and season. Conclusions:MP can be detected in all seasons, but most prevalently in summer and autumn.Girls are more prone to MP infections than boys.The detection rate of MP increases with age, and the positive rate is higher in preschoolers and school-age children.During the 4-year study period, the drug resistant rate of MP remain high.
8.Analysis of the frequency of therapy-oriented oral radiation in Nanping, China

Objective To investigate the frequency of therapy-oriented oral radiation in Nanping, China and its distribution, and to provide a basis for the rational application of therapy-oriented oral radiation and the effective allocation of resources in Nanping. Methods A questionnaire was designed to investigate the frequency of therapy-oriented oral radiation in all oral radiation diagnosis and treatment institutions in Nanping. Results In 2021, there were 54 oral radiation diagnosis and treatment institutions and 79 oral radiation machines in Nanping. The total frequency of therapy-oriented oral radiation was 61593 visits and the radiation frequency was 19.54 visits per thousand patients. The average annual frequency of medical institutions at all levels was 721.87 to 3713.25 visits per institution; the male-to-female composition ratio of frequency of therapy-oriented oral radiation in December 2021 was 50.5%:49.5%. The proportion of radiation frequency of different devices was as follows: 38.7% (intraoral dental film), 46.5% (oral panorama), 10.3% (oral computed tomography [CT]), and 4.5% (cranial photography). The proportion of radiation frequency in patients of different ages was as follows: 17.1% (0−15 years), 48.2% (15−40 years), and 34.7% (over 40 years). The frequency of therapy-oriented oral radiation grew by 77.43%, 35.18%, and 8.16% every two years from 2015 to 2021, respectively. Conclusion The frequency level of therapy-oriented oral radiation in Nanping is at the level of Class II health care. The distribution of therapy-oriented oral radiation is highly unbalanced and is related to the level of economic development. Private healthcare institutions are growing rapidly, and public healthcare institutions of grade two and above occupy the main healthcare resources. The oral panorama accounts for the most, cranial photography accounts for the least, and oral CT is the fastest-growing portion. Therapy-oriented oral radiation is predominantly performed in the young and middle-aged populations, regardless of sex. Except for intraoral dental films, the general trend is upward.
