Objective To investigate the antibiotic resistance rate of Helicobacter pylori,and to provide reliable evidence of choosing optimal drugs for clinical therapy.Methods Overall 3304 patients were enrolled from February 2006 to March 2007 who accepted gastroscopy followed by Helicobacter pylori culture of the gastric mucosa from biopsy.The antimicrobial susceptibilities of Helicobacter pylori strains for six antibiotics including metronidazole,amoxicilin,gentamycin,levofloxacin,furazolidone,clarithromycin were observed by Kirby-Bauer disk diffusion test.Results 1171(35.4%)biopsy clinical specimens were proved to be Helicobacter pylori positive when cultured in vitro and identified by conventional assays.The metronidazole resistance rate(99.32%)of Helicobacter pylori was significantly higher than that of any of the other 5 antibiotics,and the levofloxacin resistance rate(0.51%)was the lowest.Furthermore there were no significantly change in the tendency of resistance rate for the two above antibiotics from February 2006 to March 2007.However,Helicobacter pylori was resistant to amoxicilin,gentamycin,furazolidone,clarithromycin,and the resistance rate was 61.06%、4.78%、21.35% and 39.45% respectively.Meanwhile,the resistance rates corresponding to four antibiotics were different obviously.Conclusions With the application of antibiotics for Helicobacter pylori eradication,the antibiotic resistance rate increased perceptibly.The rate of resistance to metronidazole appeared to be the highest among six antibiotics applied to clinical treatment.Drug susceptibility testing was valuable to eradicate Helicobacter pylori in clinical therapy.

The automatic nervous system(ANS) has 2 main branches:the sympathetic nervous system and the parasympathetic nervous system.The ANS controls mainly automatic bodily functions that are engaged in homeostasis.Autonomic dysfunction lead to many diseases,for example,orthostatic intolerance etc.The relationship between vitamin D and cardiovascular disease has becomes the focus of study gradually in recent years.1,25-dihydroxy vitamin D participates in the regulation of renin-angiotensin axis,vascular effects.Vitamin D deficiency triggers secondary hyperparathyroidism,promotes the development of hypertension,diabetes,dyslipidemia which can influence the incidence and prognosis of cardiovascular disease as well.The study confirmed that vitamin D deficiency is one of the risk factors of cardiac autonomic dysfunction diseases such as orthostatic intolerance.The mechanism is still not very clear.Supplement of vitamin D can offer an effective method to decrease cardiovascular disease risk in populations with low vitamin D status.

Objective:To evaluate the clinical effects of clindamycin injection on respiratory tract infections. 　Methods:Clindamycin injection was used in 50 respiratary tract infections patients aged over 18 years by intravenous drip with a dosage of 1.2 g once a day for a consecutive of 10 days. The symptoms and signs of the patients were observed, and the laboratory tests made.　Results: The numbers of patients who were cured, remarkably improved, improved and without any improvement were 25, 19, 5 and 1, respectively. The total effectiveness rate was 98.0%.　Conclusion: Clindamycin injection is significantly effective for treating the mild to moderate respiratory tract infections, especially for those caused by haemolytic streptococcus, streptococcus pneumoniae and lamellar bacillus. It has very little and mild side effects and is suitable for those allergic to penicillin.

Objective To evaluate methods and effect of the endovascular embolization of intracranial aneurysms. Methods From 2004,3 to 2004,8,14 patients with intracranial aneurysms, underwent intravascular embolization was reviewed. Clinic presentation: 6 cases males, 8 cases females, age range 45-78 years. 1 case ICA aneurysms, 8 cases PCA aneurysms, 5 cases ACA aneurysms . All patients were treated with DCS/MATRIX embolization. Results 11 cases aneurysms were 100% occluded, 2 cases aneurysms were 70-90% occluded,1 case aneurysm was 50% occluded; 6 patients found cerebral vasospasm.Conclusions In the Cerebral vasospasm time, endovascular embolization of intracranial aneurysms is a safe, minimal traumatic and effective method.

Oxyfedrine (Oxy ) exhibited inhibitory effect on washed rabbit platelet aggregation and MDA production induced by thrombin (4 U /ml) . The concentration of Oxy causing 50% inhibition of platelet aggregation induced by thrombin was 254 ?mol/L . Up to 2 nig/ml, acetysalicylic acid (ASA) could not antagonize platelet aggregation induced by thrombin 。 However, ASA 50 ?g/ml reduced the generation of MDA markedly. These results suggest that anti-aggregation of Oxy may be related to inhibition of PAF formation and the effect of Oxy on MDA generation may be due to the inhibition of arachi-donic acid release.

ObjectiveTo investigate the clinical value of right lateral position with real-time monitor for capsule endoscopy.MethodsA total of 80 patients were randomly divided into two groups.The observation group assumed the right lateral position,while the control group was in upright,standing or sitting position.The capsule endoscope was monitored real time.The patients were allowed to move after the capsule endoscope passed pylorus.Gastric transit time,small bowel transit time,small bowel examination completion rate and positive detection rate were compared between the two groups.ResultsThe mean gastric transit time of the observation group was (31.7 ± 29.8)min,which was significantly shorter than that in the control group (62.6 ± 55.9) min ( U =559.000,P =0.020).The mean transit time of small bowel was (221.3 ±78.8) min in the observation group,which was not significantly different from that of the control group ( t =0.511,P =0.611 ).The examination completion rate of whole small bowel was 95.0％ (38/40)in the observation group,which was significantly higher than that of the control group (x2 =5.165,P =0.023).The positive detection rates were 75.0％ (30/40) and 65.0％ (26/40),respectively,which were not significantly different (x2 =0.952,P =0.329).ConclusionThe right lateral position with real-time monitor during capsule endoscopy is of better clinical value.

5 cases diagnosed with Dandy-Walker Syndrome from May 2006 to November 2009 were investigated and relevant literature reviewed. Dandy-Walker Syndrome was characterized by retarded motordevelopment and cerebellar signs. The latter was observed in 3 cases in our series. The disorder, mostly influencing childhood, typically causes the fourth ventricle enlarged, and the cauda cerebelli atrophic or even absent. MRI is the most optimal radiological method to diagnose it.

Purpose To evaluate the carotid artery structure and diastolic function of the brachial artery in children with coarctation of aorta (CoA) using high-frequency ultrasound. Materials and Methods Twenty-four cases of children with CoA were selected, among them seven cases were simple CoA and 17 cases were combined with other cardiac malformations, and 20 cases of simple ventricular septal defect (VSD) children and 20 healthy children over the same period were selected as the control groups. Carotid artery intima-media thickness (IMT) and brachial artery flow-mediated vasodilation (FMD) in all subjects were evaluated using high-frequency ultrasound, vascular structure and functional changes were compared before CoA correction and VSD repair treatment. Results Carotid artery IMT of CoA group was (0.47±0.12) mm, which was significantly larger than the VSD group (0.41±0.10) mm and the control group (0.40±0.09) mm, and the difference was statistically significant (F=4.275, P<0.05); brachial artery FMD [(5.48±1.66)%] of CoA group was significantly lower than that of the VSD group [(6.69± 1.45)%] and the control group [(6.59±1.84)%], and the difference was also statistically significant (F=4.318, P<0.05), but there was no significant difference (q=0.400, 0.191;P>0.05) between the VSD group and the control group for carotid artery IMT and brachial artery FMD. Conclusion In children with CoA, besides the structure abnormalities of the narrowing vascular, there is also the combination of structural and fuctional changes such as carotid artery intima-media complex thickening and brachial artery flow-mediated dilation function decrease.

The tuberous sclerosis complex (TSC) in children is also named Bourneville disease,the 3 main symptoms namely epilepsy,mental retardation and facial angiofibromas.It is an autosomal dominant disease.It is an important cause of epilepsy,skin disease,and renal and pulmonary disease in children and adults.The appropriate therapy and prognosis for TSC patients are often different than that for individuals with epilepsy,renal tumors,or interstitial lung disease from other causes.In recent years,certain progress has been made in management of tuberous sclerosis,inhibitors of the mammalian target of rapamycin (mTOR) have demonstrated regression of astrocytomas,angiofibromas,and angiomyoliomas,as well as improved pulmonary function in persons with TSC.This article reviews the current therapeutic recommendations for medical and surgical management of neurologic,renal,and pulmonary manifestations of TSC.
