1.Performance and Interventions of Mind Wandering in Sustained Attention Response Task

Objective To investigate the impact of mind wandering in sustained attention response task, and offer interventions to cope with mind wandering. Methods In December, 2016, a total of 37 athletes were tested with sustained attention response task. Their technical index, reaction time variation and accurate rate were calculated. Results The statistics of 25 athletes showed that, the technical index was lower in complex task than in simple task (t(35)=-8.019, P<0.001). No significant difference was found in the reaction time variation between two tasks (t(23)=-1.575, P=0.129), the main effect of mind wandering was not significant (F(1,23)=0.488, P=0.492), as well as the interaction of mind wandering and task difficulty (F(1,23)=0.054, P=0.262). No significant difference was found in the accurate rate between two tasks (t(23)=-1.046, P=0.307), the main effect of mind wandering was not significant (F(1,23)=0.119, P=0.733), and the interaction of mind wander-ing and task difficulty was marginal significance (F(1,23)=3.608, P=0.070). Conclusion Athlete's mind wandering with awareness would influ-ence the performance in complex sustained attention response task. Mindfulness training and tasks engagement could effectively cope with mind wandering.
2.Sports Post-traumatic Stress Disorder and It's Intervention (review)

Due to the influence of potential psychological trauma, athletes in the competition may repetitively suffer poor performances, such as losing his usual form, slump, choking, and so on. These under-performing problems actually are symptoms of sports post-traumatic stress disorder (SPTSD). SPTSD refers to the pathological physical and mental after-effects of a trauma previously suffered by the athlete on that athlete's subsequent performances. This is a very common physical and emotional problem in athletics that impairs athletes' performance, affecting their competition results. By peak performance eye movement desensitization and reprocessing therapy, therapists can deal with this problem effectively and help athletes eliminate symptoms of sports post-traumatic stress disorder, improving the competition performance.
3.Comparison of ultrasonography and magnetic resonance imaging in diagnosis of breast cancer

Objective To compare the clinical diagnostic value of ultrasound and magnetic resonance imaging (MRI) in breast cancer.Methods Ultrasound and MRI were performed in patients with breast disease.The postoperative pathological examination results were seemed as diagnostic criteria.The diagnostic accuracy was analyzed in two kinds of imaging examination.Results The diagnostic accuracy of MRI in breast cancer(86.9%) was significantly higher than the accuracy of ultrasonic diagnosis(39.3%),the sensitivity and specificity of MRI were significantly higher than those in ultrasonic diagnosis,the rate of missed diagnosis and misdiagnosis rate of MRI were lower than those of ultrasonic diagnosis,the differences were statistically significant (x2 =6.583,12.573,4.892,23.175,8.649,all P < 0.05).Conclusion Ultrasound and MRI have high ability in diagnosis of breast cancer.MRI diagnostic efficacy has more advantages,but the ultrasound is convenient to operate and repeate inspection,can be used in screening.
4.Clinical observation on Governor Vessel-unblocking and brain-refreshing scalp acupuncture for cerebral palsy complicated with intellectual disabilities

Objective:To observe the clinical efficacy of Governor Vessel-unblocking and brain-refreshing scalp acupuncture for cerebral palsy (CP) complicated with intellectual disabilities. Methods:A total of 300 CP cases aged between 1 and 5 years were randomly allocated into a treatment group (n=150) and a control group (n=150). Patients in the treatment group were treated with Governor Vessel-unblocking and brain- refreshing scalp acupuncture combined with rehabilitation training, whereas patients in the control group received rehabilitation training alone. The Beijing Gesell developmental (Gesell) scale and gross motor function measure (GMFM) scale were used to assess the motor and intellectual development before and 3 months after the treatment. In addition, the head CT/MRI examination was applied to assess the brain nerve repair before and after the treatment. Results:After the treatment, the total effective rate in the treatment group was 78.0%, versus 42.7% in the control group, showing a statistical significance (P<0.05). As for scores of five subscales in the Gesell scale, there were significant intra-group differences in the treatment group (allP<0.05); and the scores in the treatment group were significantly better than those in the control group (allP<0.05). As for scores of five dimensions in GMFM scale, there were significant intra-group differences in the treatment group (allP<0.05); and the scores in the treatment group were significantly better than those in the control group (allP<0.05). According to the head CT/MRI findings, the total effective rate was 73.3% in the treatment group, versus 62.0% in the control group, showing a statistical significance (P<0.05). Conclusion:When used in combination with rehabilitative training, Governor Vessel-unblocking and brain-refreshing scalp acupuncture can significantly improve the clinical efficacy for cerebral palsy complicated with intellectual disabilities. It can also improve the patients' motor function, intelligence and language ability.
5.Effect of pulmonary surfactant on the incidence of bronchopulmonary dysplasia

Objective To explore, the effect of different dosage of pulmonary surfactant (PS) on the inci-dence of bronchopulmanary dysplasia. Method Four hundred and three premature infants with hyaline membrane disease were divided into 3 groups according to the dose of PS: low-dose group (L-PS group, ≤ 100 mg/kg, n =188) ,high-dose group(H-PS group, > 100 mg/kg, n = 94) and no-PS group (N-PS group, n = 121). The frac-tional inspired oxygen(FiO2) and ptlmonary oxygenating function before and after 6 hours treatment were observed and the durations of oxygen therapy and mechanical ventilation, frequency of repeated intubafion, length of hospi-talization and the incidence of BPD were compared among the three groups. Results After 6 hours PS administra-tion, the FiO2,oxygen index and duration of oxygen therapy and mechanical ventilation were significantly decreased (P <0.05), while PO2 and the arterio-alveolar partial pressure of oxygen were significantly increased (P <0.05)in the H-PS and L-PS groups, compared with the N-PS group. Compared with the L-PS and N-PS groups,the H-PS group showed a decreased incidence of BPD. Conclusions PS administration could improve the pul-monary oxygenation and prevent the development of BPD, especially in high-dose.
6.Diagnosis and management of adverse reaction due to botulinum toxin type A

Objective To analyze the clinical manifestation caused by local injections of botulinum toxin type A for cosmetic reason and the effect of antitoxin for adverse reaction.Methods Diagnosis and treatment process of 40 patients were retrospectively analyzed for adverse reactions due to botulinum toxin injections for beauty.Severe adverse reaction was assessed according to the patients clinical manifestation and the nervous system of physical assessment.The patients were given symptomatic treatment,observation or antitoxin therapy.Results Adverse reactions developed in 40 patients were mainly the nervous system abnormalities.Dyspnea developed in four cases.34 cases (85 %) were given symptomatic treatment.4 cases (10 %) were given botox treatment,and the patients were followed up for 1-3 months.The patients fully recovered to normal and did not leave any neurological sequelae.Conclusions The management of type A botulinum toxin injections should be strengthened.In case of poisoning,antitoxin therapy should be given as early as possible.
7.Effects of Four Interior-warming Drugs on the Tension of Ileum Smooth Muscle and Ca2+-ATPase in Rabbits

Objective To investigate the effects of four interior-warming drugs( galangal,cinnamon,evodia rutaecarpa,and dried ginger)on the tension of ileum smooth muscle and Ca2+-ATPase on the cell membrane in rabbits. Methods The effects of galangal,cinnamon,evodia rutaecarpa,and dried ginger were examined on normal ileum smooth muscle,in vitro intestinal muscle contraction caused by acetylcholine(ACh),barium chloride(BaCl2 )and histamine(His), and ACh-induced calcium release by using BL-420E+ biological signal collection and processing system.The average tension was measured within 1 min before delivery and within 3 minutes after the treatment,and the inhibition rate was calculated according to the average tension value.The effects of sera containing galangal,cinnamon,evodia rutaecarpa,and dried ginger on Ca2+-ATPase activity on the cell membrane of the intestinal smooth muscle were examined by phosphorus method. Results Galangal,cinnamon,evodia rutaecarpa,and dried ginger at high concentrations could restrain in vitro intestinal contraction in normal circumstances(P<0.05 or P<0.01).Significant inhibitory effects on intestinal contraction caused by ACh,His and BaCl2 were found in low,medium and high concentration groups(P<0.01).There was a dose-effectiveness relationship between the inhibition rate and final drug concentrations.The ACh-induced intracellular and extracellular calcium dependent contraction were significantly inhibited by the four interior-warming drugs( P < 0. 05 or P < 0. 01). The Ca2+-ATPase activities were( 0. 384 ± 0.070),(0.302±0.016),(0.307±0.016),(0.296±0.016),(0.313±0.003)U·mg-1 ,respectively,in intestinal smooth muscle in normal control group and high concentration groups of galangal,cinnamon,evodia rutaecarpa,and dried ginger(P<0.01). Conclusion Interior-warming drugs may relax intestinal smooth muscle by reducing the intracellular calcium release and the extracellular calcium inflow via receptor-controlled calcium channels,and inhibiting the Ca2+-ATPase activity in smooth muscle.
8.Clinical study of propacetamol and dezocine for the prevention and treatment of vascular crisis in preemptive analgesia after digit replantation

Objective To explore the clinical effect of propacetamol and dezocine for the prevention and treatment of vascular crisis in the preemptive analgesia after digit replantation.Methods 85 cases of patients treated with digit replantation were selected for the study and divided into the propacetamol group (n =45) and the dezocine group (n =40) according to the different postoperative preemptive analgesic method.The two groups were given with propacetamol or dezocine and fentanyl and ondansetron patient-controlled intravenous analgesia (PCIA) respectively.The analgesic and sedative effects were observed between the two groups,and the occurrence of vascular embolism and vascular crisis and the survival rate of rcplantation were counted.Results There was no significant difference between the two groups in the visual analogue pain score (VAS) at 3 hours after operation (P <0.05).The VAS scores of the two groups were significantly increased at 6 h and 24 h after operation while the VAS score was significantly decreased at 48 h after operation (P < 0.05),and the VAS scores were significantly lower in the propacetamol group than those in the dezocine group at 6 h,24h and 48 h after operation (P < 0.05).The difference of the Ramsay sedation scores distribution at 3 hours after operation was statistically significant between the two groups (P < 0.05),and the sedative effect was more satisfactory in the propacetamol group.Within 48 h after operation,PCI effective times/actual sub values of the propacetamol group were significantly higher than the dezocine group (P <0.05).The incidence rates of postoperative vascular thrombosis and arterial or venous crisis were 4.44%,6.67% and 6.67% respectively in the propacetamol group,which were significantly lower than those in the dezocine group with 20.00%,22.50% and 27.50% (P < 0.05).The survival rate of digit replantation,digit replantation at 6 months after treatment were 97.78%,85.00% which was higher than that of the dezocine group with 91.11%,75.00% (P < 0.05).Conclusions Compared with dezocine,propacetamol can attain the ideal analgesic and sedative effects and improve the survival rate of replantation,which has a significant role in the prevention and treatment of vascular crisis in the adjuvant treatment of postoperative analgesia in digit replantation.
9.Effect of the humanized nursing care on type 1 diabetes management outside the hospital

Objective To investigate the effect of the humanized care on the blood glucose, blood lipid and diabetic ketoacidosis control of type 1 diabetes patients outside the hospital. Methods Fifty type 1 diabetes patients were enrolled in this study, aged from 18 to 30 years old, outside the hospital. The duration of this study was from Mar 2009 to Jan 2016.The clinical and laboratory data of patients,including HbA1c,LDL-C,HDL-C,TG and diabetic ketoacidosis were collected and analyzed. Results About 60.0% and 80.0% of type 1 diabetes pa-tients achieved the goals of contorlling glycemia (HbA1c < 6.5%) and dyslipidemia (LDL-C < 2.6 mmol/L) respec-tively, after receiving the humanized care outside the hospital, while only 32.0% and 22.0% of type 1 diabetes pa-tients achieved these goals before.Moreover,after receiving the humanized care outside the hospital,only 16.0% of type 1 diabetes patients suffered from diabetic ketoacidosis which was 40.0% before.Conclusions The humanized care outside the hospital can help patients to acheive the goals of contorlling glycemia and dyslipidemia among the type 1 diabetes.Besides,it contributes to reduce the incidence rate of the diabetic ketoacidosis.
10. Application of bronchoalveolar lavage fluid in patients with pulmonary complications after allogeneic hematopoietic stem cell transplantation

Objective:
To evaluate the diagnostic value of bronchoalveolar lavage (BAL) for pulmonary complications in patients after allogeneic hematopoietic stem cell transplantation (allo-HSCT) and its safety.
Methods:
Patients with pulmonary complications after allo-HSCT underwent BAL. Microbiological smears, culture, PCR of CMV-DNA, EBV-DNA and TB-DNA, macro genomes new generation sequencing (mNGS) techniques were performed to detect pathogens in BAL fluid (BALF) .
Results:
A total of 73 allo-HSCT patients with 86 times of pulmonary complications enrolled this prospective study. They underwent 132 times of BAL procedures. The clinical diagnoses of 88.4% cases were made based on BALF analysis. Of them, 67 cases (77.9%) had infectious pulmonary complications, including 29 cases (33.7%) of fungal infection, 18 cases (20.9%) of mixed infection, 11 cases (12.8%) of viral infection and 9 cases (10.5%) of bacterial infection. The other 9 cases (10.5%) of non-infectious pulmonary complications included 8 cases (9.3%) of idiopathic pneumonia syndrome (IPS) and 1 case (1.2%) of pulmonary infiltration of lymphoma. The diagnoses of the remaining 10 cases (11.6%) were not determined. The platelet counts of 33 patients were less than 50×109/L before BAL. None of them developed severe bleeding complications during or after BAL. Transient fever occurred in 10 patients after BAL. Blood cultures showed staphylococcal bacteremia in them and anti-infection therapies were effective. No life-threatening complications occurred in all of the patients during or after BAL.
Conclusion
BALF analysis was informative for the diagnosis of pulmonary complication and safe for patients with pulmonary complications after allo-HSCT.
