1.Human papillomavirus related head and neck squamous cell carcinomas
Journal of International Oncology 2010;37(6):438-441
Accumulating evidence shows that human papillomavirus (HPV) may play a significant role in head and neck squamous cell carcinomas (HNSCC) initiation and development, however, the detailed mechanism remains unclear. It is possible that the malicious transformation of infected cells by HPV E6 and E7might contribute to development of HPV positive HNSCC. In order to give a full description of HPV positive HNSCC, this review will focus on HPV infection epidemiology, carcinogenesis mechanism and feasibility of HPV vaccine in cancer prevention/treatment.

Objective To investigate the relationship between the acute phase blood glucose and the severity and prognosis of stroke in patients with stroke. Methods One hundred seventy-three consecutive patients with acute stroke were randomly allocated into either a hyperglycemia group (n =72) or a normal group (n = 101) according to whether their random blood glucose levels were > 7.8 mmol/L or not. The average time from stroke onset to admis-sion was 6.5 ± 5.1 hours. The patients were evaluated by the National Institutes of Health Stroke Scale (NIHSS) and Glasgow Coma Stroke (GCS) scores within 20 minutes after clinical reception, The plasma blood glucose detection and bead CT scan were completed in 40 minutes. The patients with cerebral infarction were classified according to the Oxfordshire Community Stroke Project (OCSP) criteria, and the survivors were evaluated by the modified Rankin scale (mRS) after 3 months. Results There were no significant differences between the two groups in age, gender, vascular risk factors, stroke property and treatment principle. There were significant differences between the NIHSS and GCS at the first admission and the mRS scores at 3 months (P<0.01). The proportion of the patients with the history of diabetic mellitus in the random hyperglycemia group was significantly higher than that in the normal group (P<0.01).The random blood glucose levels were not correlated with the severity of stroke between the 2 groups of patients with the history of diabetes mellitus; the increased random blood glucose levels were significantly correlated with the severity and prognosis of stroke between the 2 groups of patients without the history of diabetes mellitus (P<0.01). Conclusions The increased blood glucose in patients with stroke without the history of diabetes mellitus in acute phase is an independent risk factor for the severity and prognosis. The increased random blood glucose in patients with stroke in acute phase may also suggest having the history of diabetes mellitus.

The relationship between abnormal circulating GH-insulin-like growth factor (IGF) axis and hypoglycaemia was explored in 2 cases of non-islet cell tumor associated hypoglycaemia (NICTH). Serum level of IGF-Ⅱ was increased, the levels of GH, IGF-Ⅰ, IGFBP-3 were decreased to some extent, and IGF-Ⅱ/IGF-Ⅰ (molar ratio) was significantly increased in these two cases. In one case, circulating GH-IGF axis returned to normal after complete removal of the tumor. In the diagnosis of NICTH, the abnormal circulating GH-IGF axis is quite valuable and IGF-Ⅱ/IGF-Ⅰ appears to be a more sensitive parameter.

Objective To analyze the association of elevated carotid intima-media thickness(CIMT)with low-grade albuminuria in a community-based population. Methods A total of 10 375 participants aged 40 years or older were recruited using cluster sampling from Jiading district, Shanghai. Standardized questionnaires were adopted to collect information on health status and lifestyles. Anthropometric measurements including weight, height, and blood pressure, and biochemical tests for blood and urine were performed. CIMT was detected by B-mode ultrasound examinations. 9 523 subjects were included for final analysis. All these subjects were divided into 4 groups according to sex-specific urinary albumin to creatinine ratio ( UACR ) quartiles. CIMT≥upper decile of the population was defined as elevated CIMT and low-grade albuminuria was defined as UACR≥sex-specific upper quartile. The relationship between UACR and CIMT was investigated by multiple logistic regression analysis. Results The prevalence of elevated CIMT in this population aged 40 years or older with albuminuria in normal range was 21. 44%. Comparing to participants without elevated CIMT, those with elevated CIMT showed higher level of UACR (4. 79 vs 4. 51 mg/g, P<0. 01). The metabolic status was deteriorated with the increment of UACR level. The prevalences of elevated CIMT were 18. 99%, 19. 24%, 20. 20%, and 27. 33% from the lowest to highest UACR quartile, respectively(Ptrend<0. 01). After adjusting for some conventional cardiovascular risk factors, multiple logistic regression revealed that compared with the participants in the lowest quartile of UACR, those in the highest quartile had a 1. 25-fold risk of elevated CIMT. Conclusions This study shows that low-grade albuminuria is positively associated with elevated CIMT, independent of conventional cardiovascular risk factors in middle-aged and elderly adults.

OBJECTIVETo study the effect of compound Danshen dripping pills and atorvastatin on restenosis after abdominal aorta angioplasty in rabbits.
METHODSRabbit models of abdominal aorta restenosis after angioplasty were established and treated with saline (group A), compound Danshen dripping pills (group B), atorvastatin (group C), or compound Danshen dripping pills plus atorvastatin (group D). HE staining was used to determine the thickness of arterial intimal hyperplasia and assess the morphological changes of the narrowed artery. Immunohistochemistry was employed to detect the expression of nuclear factor-κB (NF-κB) and monocyte chemoattractant protein-1 (MCP-1).
RESULTSCompared with group A, the 3 treatment groups showed significant increased vascular cavity area and reduced intimal area and percentage of intimal hyperplasia (P<0.05). The vascular cavity area, intimal area and percentage of intimal hyperplasia levels differed significantly between group D and groups B and C (P<0.05). Immunohistochemistry showed a significant reduction of the expression rate of NF-κB and MCP-1 in the 3 treatment groups compared with group A (P<0.05), and the reduction was especially obvious in group D (P<0.05).
CONCLUTIONSCompound danshen dripping pills combined with atorvastatin produces better effects than the drugs used alone in inhibiting vascular smooth muscle cell proliferation in rabbits after abdominal aorta angioplasty possibly due to a decreased expression of MCP-1 as a result of NF-κB inhibition.
