2.Establishing a 29 Y-STR Loci Multiplex PCR System.

OBJECTIVE:
To establish a 29 Y-STR loci multiplex PCR system for investigating the genetic polymorphisms and to assess its application value in forensic science.
METHODS:
A multiplex PCR system was established using a five color fluorescence labeling 29 Y-STR loci (DYS456, DYS389 I , DYS437, DYS447, DYS389 11, DYS438, DYS522, DYS460, DYS458, DYS622, DYS390, DYS392, DYS448, DYS449, DYS391, Y-GA TA-H4, DYS388, DYS19, DYS385a/b, DYS527a/b, DYS393, DYS459a/b, DYS635, DYS439, DYS570 and DYS627) for multiple amplification and capillary electrophoresis. And its applicability was validated with genetic polymorphism data of 29 Y-STR of unrelated 2,000 male samples in Shandong Han population.
RESULTS:
A total of 1,981 different haplotypes of 2,000 individuals showed genotype diver- sity between 0.370 0 and 0.965 4. The system provided stable and accurate typing with high sensitivity of 0.05 ng. It satisfied the needs of variety of routine biological samples.
CONCLUSION
The 29 Y-STR loci multiplex PCR system could be applied for actual cases and establishment of Y-STR database. In addition, it has great significance in forensic science practices and related research.

3.The expression of platelet activity in hypertensive patients complicated with ischemic cardiovascular and cerebrovascular disease

Objective To observe the change of platelet activity marker in hypertensive patients complicated with ischemic cardiovascular and cerebrovascular disease ,and assess it s value in the happening of thrombotic disease. Method The platelet activity marker (CD62p,CD63) were measured by flow cytometry in 40 hypertensive patients complicated with ischemic cardiovascular and cerebrovascular disease (group C ),46 hypertensive patients not complicated with ischemic cardiovascular and cerebrovascular disease (group B),and 30 cases of healthy control (group A). 5.Therapeutic effects of blood purification in 160 cases with liver failure

Objective: To evaluate the clinical efficacy of various methods of blood purification in the treatment of patients with liver failure.Methods: Two hundred and seventy-three patients suffering from liver failure were randomly divided into two groups.On the base of medical routine treatment,160 patients were additionally treated with various methods of blood purification treatment group,including plasma exchange,plasma exchange associated with continuously venovenous hemofiltration,plasma exchange associated with hemodiafiltration,plasma exchange associated with plasma absorption,molecular absorbents recycling(system) and so on.While the other 113 patients received medical routine treatment only(control group).The efficacy of treatment and prognosis in the two groups were then observed and compared.Results: In the treatment group,the total bilirubin(TB),albumin(ALB),total bile acid(TBA),alanine aminotransferase(ALT),pre-albumin(PALB),cholinesterase(CHE) and prothrombin activity(PTA) after the treatment were better compared with those before the treatment(all P
6.Dynamic changes and clinical value of blood perfusion in perfusion weighted image-diffusion weighted image mismatched regions after stroke

Objective To investigate the changes and the mechanism of blood perfusion in perfusion weighted image ( PWI)-diffusion weighted image ( DWI) mismatched regions after infarction and to assess the effect on prognosis .Methods We assessed 84 patients with acute cerebral infarction within 72 hours of onset in the territory of anterior circulation , who admitted to Affiliated Drum Tower Hospital , Nanjing University Medical School between January 2009 and June 2011.All patients were imaged with 3.0 T MRI on day one.The patients were divided into mismatched group (31 patients) and matched group (53 patients) according to the basis of PWI-DWI.The two groups received the same treatment including anti-platelet, oxygen free radical scavenger and lowing cholesterol therapy .Barthel index was used to evaluate neurological deficit on day 0, day 12 and three months later.We also compared the perfusion parameters of day 0 with day 12 by PWI from mismatched group.Results On day 12, the value of relative cerebral blood flow increased by 7.37%(t=-2.076, P<0.05) and the value of relative mean transit time decreased by 11.06%(t=2.972,P<0.01) compared with that on day 0 from mismatched group.However, there was no significant difference in Activity of Daily Living scale ( ADL) score between the two groups after 12 days′treatment.After three months, the patients of mismatched group had higher ADL score (75.48 ±3.26) than that of matched group (66.23 ±3.04; t=2.079, P<0.05).Conclusion The patients of mismatched group had better neurofunction than matched group , possibly related to improvement of perfusion in mismatched area.
7.Evaluation on tolerability and safety of long-term administration with shexiang baoxin pill in patients with coronary heart disease of stable angina pectoris.

OBJECTIVETo evaluate the safety and tolerability of long-term (6 months) administration with Shexiang Baoxin Pill (SBP) in patients with coronary heart disease (CHD) of stable angina pectoris.
METHODSAdopting randomized non-blinded and parallel controlled trial, 200 patients with CHD were randomly and equally assigned to the SBP group and the control group. Both received basic therapy for CHD, including anti-platelet, lipid regulating and anti-ischemia with additional SBP 2 pills, taken orally three times per day in the SBP group. They were followed up for 6 months. The drug tolerability and adverse drug reactions occurred in the observation period were recorded, and the laboratory indexes involving blood routine, liver function, renal function, blood glucose and blood lipids were detected before and after treatment.
RESULTSThe trial was completed in 92% of the patients, 5 patients withdrew in the SBP group and 11 patients in the control group; but none for the intolerable therapy. There were 1 case of adverse reaction related to SBP. No obvious change was found in blood glucose and blood lipids in the two groups before and after treatment. No serious adverse reaction and injury of liver and renal function or others happened.
CONCLUSIONLong-term administration of SBP has favorable clinical tolerability and safety for CHD patients.

8.Application of real-time three-dimensional color doppler transthoracic echocardiography on preoperative assessment of patients with structural heart disease

Objective To investigate the diagnostic value of preoperative real-time three-dimensional color Doppler transthoracic echocardiography on assessment of patients with structural heart disease (SHD). Methods A total of 111 pa?tients were assessed preoperatively using real-time three-dimensional color Doppler transthoracic echocardiography (RT-3D-CDTTE), which include 31 SHD patients and 80 patients without SHD that were designed as negative control. Conven?tional two-dimensional color Doppler transthoracic echocardiography (2D-CDTTE) were used to compared with RT-3D-CDTTE while cardiovascular angiography and intraoperative findings were used as“Golden Standard”simultaneously. First of all, preoperative echocardiographic examination were performed and diagnosis was given. Angiography and intraoperative findings were hired to verify the accuracy of echocardiographic diagnosis before operation. Results (1) The preoperative RT-3D-CDTTE displayed three-dimensional structure and hemodynamic status of SHD cardiac lesions clearly, which were consistent with cardiovascular angiography and intraoperative findings. (2) P value of McNemar test between 2D-CDTTE and“Golden Standard”was greater than 0.05, Kappa=0.654 (P<0.001). P value of McNemar test between RT-3D-CDTTE and“Golden Standard”was greater than 0.05, Kappa=0.932 (P<0.001). Conclusion RT-3D-CDTTE can provides essen?tial information for preoperative evaluation which is important for decision of SHD management.
9.Effects of Tamm-Horsfall protein on kidney stone formation

Objective Tamm-Horsfall protein (THP) may play a role in kidney stone formation.The article aimed to conduct a preliminary study on the role of THP in kidney stone formation by investigating the changes of THP in rat urine, pathological changes of renal tissue and the formation of calcium salt crystals after establishing CNPs rat model of kidney stones.Methods Stone samples of 40 patients from February to June 2015 in our department were collected to establish the model of CNPs-induced kidney stone in rats and prepare CNPs suspension.48 SD rats were randomly divided into experimental group (group A) and blank control group (group B).Group A were injected with CNPs and the same amount of sterile saline injection in the group B.The urine of rats was collected after injection at 3h, 6h, 12h, 24h, 1w, 2w, 4w and 8w.ELISA were applied to detect THP levels in the urine.Then the rats were killed to take the kidney tissue.HE staining was used to investigate the pathological changes of the cells and evaluate the formation of the calcium salt crystals.Results THP levels in group A at 24h, 1w, 2w, 4w and 8w ([166.03±3.02], [173.50±1.78], [174.55±2.05], [176.54±2.45], [177.11±1.76]pg/mL) were significantly higher than that at 3h(165.89±2.23pg/mL)(P<0.05), which was the same case in comparison with those of group B ([157.65±2.22], [156.54±1.43], [159.45±3.21], [158.63±2.98], [157.33±2.05]pg/mL).Compared with the calcium salt crystal score at 6h (1 point), the scores at 3,6,12,24h (average score 2 points) increased.At 2w the score increased significantly to 3 points and reached the top score(6.7points) at 8w, which was of significant difference.The score of calcium salt crystals was in positive correlation with THP content (r=0.843,P<0.05).Conclusion THP in urine may contribute to the aggregation of calcium salt crystals and the formation of kidney stones.
