1.Evaluation of SYNTAX score in predicting prognosis of patients with unprotected left main coronary artery disease undergoing percutaneous coronary intervention

Objective To evaluate the long-term prognostic capacity of the SYNTAX score Ⅱ(SxScore Ⅱ) and SYNTAX score (SxScore) in patients undergoing left main percutaneous coronary intervention (LM-PCI). Methods A total of 209 patients undergoing unprotected LM-PCI in the Cardiology Department of the Affiliated Hospital of PAP of Logistic College were prospectively collected. Follow up was carried out by telephone or outpatient or rehospitalization. The clinical endpoint focused on MACCE after PCI including composite death, nonfatal myocardial infarction, target vessel revascularization and stroke. The secondary endpoint included cardiac death and stent thrombosis. The SxScore and SxScoreⅡ were retrospectively calculated according to results of coronary angiography and clinical features of patients. Patients were stratified according to tertiles of low (≤22), intermediate (23-32), and high (≥33). The clinical data were comparedbetween three groups. The predictive ability of two scoring systems to MACCE after PCI was compared by COX regression evaluation. Results In 209 patients, 12 patients were lost to follow-up (5.7%), and the median follow-up was 30.2 months, 56 cases (28.4%) were observed to suffer from MACCE. The incidence rates of MACCE were 19.0%, 28.6%and 44.4%in SxScore low, intermediate and high groups respectively. The incidence rates of MACCE were 12.8%, 23.8%and 45.5%in SxScoreⅡlow, intermediate and high groups respectively. Single factor analysis showed that SxScore, SxScoreⅡ, age, diabetes and left ventricular ejection fraction (LVEF) were the independent predictors of MACCE. Multivariate analysis showed that SxScore and SxScoreⅡwere still risk independent predictors for MACCE. Conclusion Both SxScore and SxScoreⅡare independent risk predictors for MACCE in patients with unprotected left main coronary artery disease undergoing PCI treatment.

Objective To screen and analysis the virulent genes and pathways in golden hamster cheek pouch mucosa precan-cerous lesions and squamous cell carcinomas.Methods The experimental models of golden hamster cheek pouch mucosa precancer-ous lesions and squamous cell carcinomas were induced by DMBA.The total RNA of precancerous lesions and squamous cell carci-nomas of golden hamster cheek pouch was extracted and the cRNA was labeled by Cy3.Then gene chip was used to screen the dif-ferentially expressed genes.At last,the Gene Ontology and pathway was used to analysis the biology function of important virulent genes.Meanwhile,we confirmed the correctness of the results by using the RT-PCR.Results A total of 1 981 differentially ex-pressed genes were detected during the process from precancerous lesions to squamous cell carcinomas (120 genes remained known).One thousand and thirty-seven genes were up-regulated and 944 genes down-regulated.GO analysis showed that these dif-ferentially expressed genes mainly related to the macromolecular metabolism,signal transduction and so on.Pathway analysis showed that 9 pathways were significant changes.14 genes were enriched in above 9 change pathways.Conclusion There were 1 981 differentially expressed genes and 9 abnormal changes pathways during the process from precancerous lesions to squamous cell carcinomas,in which 14 differentially expressed genes led to changes in cellular pathways.These genes might be likely to have the important pathogenic genes in the process of transformation.
6.Correlation of job burnout and security among nurses working in operating room

Objective To explore the relationship between job burnout and sense of security among nurses in operating room in order to provide a basis for managers to improve nurses′ sense of security and reduce their job burnout. Methods 127 nurses working in operating room were investigated by Maslach Job Burnout Scale in Chinese version and Security Questionnaire. Results The scores of emotional exhaustion, personification and personal satisfaction in Maslach Job Burnout Scale were respectively (28.92±6.44) points,(13.20±4.97) points and (20.72±5.73) points. The positive detection rate of each domain was 57.5% (73/127),89.8% (114/127) and 79.5% (101/127).The scores of interpersonal security and perceived control in Security questionnaire were respectively (27.13±4.79) points and (25.69± 5.64) points. The personification was negatively correlated to the interpersonal security (r =-0.243, P <0.01) and perceived control (r=-0.436, P < 0.01). Personal accomplishment was positively correlated to interpersonal security (r=0.195, P<0.05). Conclusions The nurses in operating room have serious job burnout and moderate and high level of psychological security. Nursing managers should take some measures to reduce the positive detection rate of job burnout and improve the level of psychological security.
7.Orbital implantation of hydroxyapatite: An analysis of 23 cases

This study was designed following eyeball extirpation to evaluate the therapeutic effects of hydroxyapatite and to explore the causes and preventing from exposure during orbital implantation. A total of 23 cases with autologou sclera and muscular suture selected from Department of Ophthalmology, Affiliated Hospital of Luzhou Medical College between January 2005 and December 2007 were retrospectively summarized and analyzed. Seventeen cases were implanted initially and the other 6 cases received secondary implantation. For the implantation with scleral-wrap, a 360? peritomy, limbic sclerotomy and full keratectomy were performed with corneoscleral scissors. Uveal tissue was completely removed. Posterior sclera was radially incised, and the hydroxyapatite orbital implant was implanted. Anterior sclera was sutured completely. Other implantations without using sclera-wrap were performed with the hydroxyapatite orbital implant in muscle cone and four rectal muscles were sutured to the hole of the hydroxyapatite orbital implant. Mattress suture of fascia tissue with double staggered position was performed in all the implantations. Conjunctiva was sutured discontinuously. This implant provided satisfactory functional motility and stability. Two cases developed implant exposure, suggesting that hydroxyapatite was the implant of choice for cosmesis following surgical removal of the eyeball, or enucleation. The complication rate is low and implant exposure, if it occurs, can be easily managed.
8.The significance of RASSF2A promoter methylation in the early diagnosis of epithelial ovarian cancer

Objective To evaluate the methylation status of RASSF2A in epithelial ovarian cancer (EOC) tissues and plasma, and to explore the correlations in two kinds of samples with clinicopathological characteristics. Methods The frequency of methylation of the RASSF2A gene in tissues and corresponding plasma samples from 60 EOC patients, 20 patients with benign ovarian tumors and 10 patients with normal ovarian tissues were detected respectively. The methylation-specific PCR was used to determine the methylation status. Results The frequency of aberrant methylation of RASSF2A was 58.3% (35/60) in EOC tissues and 43.3%(26/60) in corresponding plasma samples respectively, however, such hypermethylation was not detected in the benign ovarian tumors and normal ovarian samples, and there was significant difference between them (P<0.05). Besides, RASSF2A methylation levels in serum of EOC patients correlated reasonably well with methylation status in tumor samples (r=0.739, P=0.000). No significant differences were observed between the promoter hypermethylated status of the RASSF2A and clinicopathological characteristics (P>0.05). Conclusions The promoter methylation of RASSF2A gene is an early and frequent epigenetic event in EOC. Detection of promoter hypermethylation in plasma specimens can help the early diagnosis of EOC.
9.Development and biomechanical evaluation of interlocking expanding compressive screws for femoral neck fracture

Objective To explore the methods of internal fixation for femoral neck fractures and evaluate the biomechanical characteristics of the interlocking expanding compressive screw (IECS)of Type HZS. Methods Based on the special biomechanical characteristics of the femoral neck fracture, the device of internal fixation was designed to consist of main screw, bifurcate screw, boost screw cap, washer, compressive screw cap, oblique interlocking screw and main screw cap. Its biomechanical performances were evaluated through comparison with those of the dynamic compressive screw (DCS), twin compressive screw (2CS) and three cannulated screws (3CS) respectively. Results The mechanical performances of the femoral neck fracture fixed with the interlocking expanding compressive screw were superior to those of the others in displacement ,axial and level stiffness, torsional strength and stiffness and terminal loading. Conclusion Interlocking expanding compressive screw can efficiently withstand the splitting, torsional, shearing, tensional and compressive forces in the fracture, and is simple, safe and easy to use.
