Objective To analyze the prognostic factors and evaluate the effect of stereotactic radiosurgery for patients with multiple brain metastases. Methods Comparison was made in 53 such patients treated by stereotactic radiosurgery plus radiotherapy and 53 treated by radiotherapy alone. Patients were matched paired according to the following criteria: age, Karnofsky performance scale(KPS) before treatment, extent of systemic cancer and number of brain metastasis.Fourty patients had stereotactic radiosurgery, 13 patients stereotactic fractionated radiosurgery. In the stereotactic radiosurgery group, the patients were given a mean marginal dose of 20?Gy. Methods of stereotactic fractionated radiosurgery was 4 12?Gy per fraction, twice a week to a total dose of 15 30?Gy. Whole brain radiotherapy was given immediately after stereotactic radiosurgery. For patients treated by radiotherapy alone, the entire brain was treated by 30 40?Gy in 3 4 weeks. Results The median survival was 11.6 months in stereotactic radiosurgery plus radiotherapy and 6.7 months in radiotherapy alone. The one year survival rate and one year local control rate were 44.3%,17.1% and 50.9%,13.2%. Those with KPS increased after treatment gave 1 year survivals of 69.8% and 30.2%, respectively. The validity rates in CT or MRI three months after treatment were 82.0% and 55.0%.The difference in the two groups was found to be statistically significant (P

The methyl group plays an important role in the rational drug design. Introducing methyl into small molecules has become an important strategy of lead compound optimization. The application of methyl in drug design is reviewed in this paper. Methyl can modulate the physicochemical, pharmacodynamic, and pharmacokinetic properties by ortho effect, inductive effect, and conformational effect. It also improves the metabolic stability as a soft metabolic point. In addition, introducing methyl into drug molecules can also be applied as a strategy in new uses of old drugs and generate me-too drugs.

Objective To observe the effects of hyperbaric oxygen (HBO) on the functional with hemiplegic patients of traumatic brain injury(TBI).Methods 186 TBI cases were allocated to the HBO group (94 cases) and control group (92 cases). The control group was subjected to rehabilitation treatment, while the HBO group received HBO in addition to the rehabilitation treatment. Results Fugl-Meyer Assessment (FMA) and Modified Barthel Index (MBI) scores in both groups were significantly increased after treatment(P<0.05). FMA and MBI scores in the HBO group were significantly better than those in the control group(P<0.05).Conclusion HBO is effective to improve the motor function and activities of daily living with hemiplegic patients of traumatic brain injury and can improve the curative effects.

Objective To study the effect of balloon dilatation therapy on dysphagia caused by cricopharyn- geal achalasia. Methods Ten cases of dysphagia were diagnosed as cricopharyngeal achalasia by videofluoroscopic swallowing study(VFSS).A 14~* urethral catheter was inserted into the esophagus and an amount of water was injec- ted into the balloon of the urethral catheter to make it turgid. Then the catheter was pulled upwards and passed through the stricture of esophagus to dilatate the cricopbarygeus muscle.Meanwhile, low frequency electrical stimula- tion was used and combined with functional training of the organs related to deglutition and ingestion. The results be- fore and after the treatment were evaluated. Results After 19.7 times of dilatation therapy, the content of water in- jected into the balloon was increased from 2.65?0.91 ml to 8.20?0.92 ml.Cricopharyngeal achalasia was alle- viated significantly(P
