1. The expression levels of Reg4 and Ki67 in superficial elevated mucosa lesions of atrophic gastritis with intestinal metaplasia are higher than those in superficial depressed mucosa lesions

Objective: To infer which morphologic change of gastric mucosa has more probability to develop into dysplasia or cancer by detecting the expressions of regenerating gene 4 (Reg 4) and Ki67 in different morphologies of intestinal metaplasia in gastric antrum. Methods: Immunohistochemistry and semiquantitative reverse transcriptase-polymerase chain reaction (RT-PCR) were used to examine the expression levels of Reg4 and Ki67 proteins and mRNAs in mucosal biopsy specimens of 385 cases of different types of gastric disease. Results: In the gastric mucosal biopsy specimens each from nonatrophic gastritis (NAG), atrophic gastritis (AG), atrophic gastritis with intestinal metaplasia displaying superficial depressed mucosa lesions (DIM), atrophic gastritis with intestinal metaplasia displaying superficial elevated mucosa lesions (EIM) and gastric cancer (GC), the positive rates of Reg4 protein was 12.3%, 26.7%, 43.6%, 93.6% and 31.5%, respectively; the positive rate of Reg4 protein of EIM was significantly different from those of NAG, AG, DIM and GC (P <0.05). The positive rates of Ki67 protein were 14.8%,18.7%, 23.1%, 69.2% and 71.2% in NAG, AG, DIM, EIM and GC, respectively; the positive rate of Ki67 protein in EIM was significantly different from those of NAG, AG and DIM (P <0.05) but not different from that of GC (P >0.05). The expressions of Reg4 and Ki67 mRNAs in EIM were also significantly higher than those in DIM. Conclusion: The expression levels of Reg4 and Ki67 in EIM were higher than those in DIM, which suggests that EIM may have more probability to develop into dysplasia or cancer than DIM, and DIMmay become a more valuable specimen for gastric biopsy. 3.Research progress on the measurement of human lens thickness in vivo

The precise measurement in lens thickness in vivo, provides great application value for intraocular accommodation and ametropia development mechanism research.And it has great clinical significance for the diagnosis and treatment of glaucoma and cataract.Currently, many ultrasonic methods and optical methods are used in measuring lens thickness.The measurement principles, advantages, disadvantages and the accuracy of the instruments are summarized in this paper.Among these methods, Orbscan II, Pentacam, Lenstar and AS-OCT can be used to measure lens thickness instead of A-scan.More important is the fact that UL-OCT can dynamically monitor the change of the lens thickness with intraocular accommodation.Choosing an instrument with higher measuring accuracy to examine the lens thickness, can provide more accurate and convincing lens thickness data for clinical and scientific research.

5.Analysis of correlation between dry eye and diabetic retinopathy in type 2 diabetic patients

?AIM:To investigate the correlation between dry eye and different degrees of diabetic retinopathy ( DR ) in type 2 diabetic patients.?METHODS: In the cross-sectional study, 340 patients (340 eyes) with type 2 diabetes were enrolled. Tear film function tests including tear meniscus height, tear film breakup time ( BUT ) , fluorescein staining, Schirmer Ⅰtest were performed followed by surveying questionnaires about dry eye. Retinal status was evaluated by retinal color photography and indirect ophthalmoscopy exam with dilated pupils to evaluate DR and whether companied by macular edema.?RESULTS:The prevalence of dry eye was 49. 41%. The mean duration of diabetes in patients with dry eye was 11.15±7.07a, while 6.92±5.45a without dry eye(P<0.01). Dry eye had the positive correlation to the development of DR. The incidence of dry eye in people with mild nonproliferative diabetic retinopathy ( NPDR ) , moderate NPDR, severe NPDR and proliferative diabetic retinopathy (PDR) was 1. 097 times, 1. 724 times, 2. 86 times and 5. 43 times respectively, compared with people without DR. The occurrence of dry eye in people with macular edema increased by 3. 697 times compared with people without macular edema.?CONCLUSION: Dry eye was more prevalent in people with type 2 diabetes. The incidence of dry eye increased gradually with the occurrence and development of diabetic retinopathy.
6.Immune protection and mechanism of plasmid DNA encoding Gglycoprotein of respiratory syncytial virus(RSV)

Objective To construct a plasmid DNA encoding G glycoprotein of respiratory syncytial virus(RSV) and investigate the protective immune response against RSV infection. Methods Recombinant plasmid DNA of pcDNA3.1~G was constructed by standard RT-PCR based cloning procedure. The immunogenicity of recombinant G protein transiently expressed in HEK293 cells was detected by Western blot. BABL/c mice were intramuscularly immunized with pcDNA3.1~G. Samples of lung, sera, bronchoalveolar lavage fluid(BALF) were collected before and after RSV challenge; virus titer in lung was detected by viral titration; sections of paraffin embedding lung tissues were stained by haematoxylin and eosin(HE) for histological analyses; sera anti-RSV IgG levels were examined by ELISA; Th1/Th2 cytokine were detected by ELISA kit, the T lymphocyte subsets of BALF was determined by immunefluorescence staining followed by flow cytometry. Results Plasmid DNA of pcDNA3.1~G was successfully constructed. The expressed target protein possesses immunogenicity. After challenge, pcDNA3.1~G immunized mice presented relieved pathological changes in lung as well as reduced lung viral titers. The RSV specific IgG was detected in sera of immunized mice. There was significantly increased number of CD25~+CD4~+ T cells in mice BALF. Conclusion We constructed a pcDNA3.1~G plasmid DNA vaccination which can induce evident protective cellular immunity against RSV infection in mice with the increased number of CD25~+CD4~+ T cell subpopulation.
7.The role of C2-7 angle in the development of dysphagia after anterior cervical spine surgery

Objective To analyze the relationship between cervical lordosis and the development of dysphagia after anterior cervical spine surgery.Methods From June 2007 to May 2010,data of 172 successive patients who had undergone ACDF operation in our hospital were reviewed in this study.The presence and duration of postoperative dysphagia were recorded via face-to-face questioning or telephone interview at least one year after the procedure.Plain cervical radiographs before and after surgery were collected.The C2-7 angle was measured.The change of C2-7 angle was defined as dC2-7 angle=postoperative C2-7-preoperative C2-7 angle.The correlation between postoperative dysphagia and dC2-7 angle was studied.Results There were 22 patients in dysphagia group,including 17 males and 5 females.Their age ranged from 25 to 70 years old,and average was 47.7±5.4.The average of BMI was 25.0±2.9 kg/m2.150 patients were in non-dysphagia group,including 101 males and 49 females.Their age ranged from 18 to 72 years old,and average age was 49.2±4.8.The average of BMI was 24.4±3.4 kg/m2.There was no statistical difference in gender,age,and BMI between two groups.The dC2-7 angle of dysphagia group ranged from-1 °-20.5°,and average was 8.6°±4.0°.The dC2-7 angle of non-dysphagia group ranged from-13°-28.5°,and average was 5.0°±4.3°.There was significant difference in dC2 7 angle between dysphagia and non-dysphagia group.Spearman Analysis revealed that there was strong correlativity between dC2-7 angle and postoperative dysphagia.When dC2-7 angle was greater than 5°,the chance of developing postoperative dysphagia significantly increased (19.3% [17/88] vs 6.0% [5/84]).What's more,Spearman Analysis also revealed that there was no correlativity between dC2 7 angle and degree of operative dysphagia.There was no significant difference in gender,age,and BMI between dysphagia and non-dysphagia group.There was no statistical difference in operative time,blood loss revision surgery,revision surgery ratio,most cephalic operative level and number of operative levels between dysphagia with non-dysphagia group.Logistic regression model showed that an increased likelihood of postoperative dysphagia persists with increasing dC2-7 angle,but had no relationship with operative time,blood loss,revision surgery,most cephalic operative level and number of operative levels.Conclusion dC2-7 angle may play an important role in the development of postoperative dysphagia.We found no statistical difference in operative time,blood loss revision surgery,revision surgery ratio,most cephalic operative level and number of operative levels between dysphagia and non-dysphagia group.Intraoperative measurement of the dC2-7 angle is practical and essential for reducing the postoperative dysphagia.
8.Cytapheresis for Induction of Response,Remission and Maintenance of Remission in Ulcerative Colitis:A Meta-analysis

Background:Cytapheresis has been used gradually in the treatment of ulcerative colitis( UC),however,its efficacy is still controversial. Aims:To systematically evaluate the efficacy and safety of cytapheresis on UC. Methods:PubMed, Embase,Web of Science,Cochrane Library,CNKI,Wanfang and VIP database were retrieved to collect the randomized controlled trials( RCTs)using cytapheresis for UC. Article selection,data extraction and quality evaluation were conducted independently by two reviewers. Meta-analysis was conducted by using RevMan 5. 3 software. Results:A total of 20 RCTs involving 1 354 UC patients were included. The results of the meta-analysis showed that cytapheresis was superior to conventional therapy or sham procedure in inducing clinical response(RR=1. 36,95% CI:1. 20-1. 55,P﹤0. 000 01), clinical remission(RR=1. 38,95% CI:1. 15-1. 66,P=0. 000 5)and maintenance of remission(RR=2. 92,95% CI:1. 40-6. 08,P=0. 004 ),and no differences in adverse events were found( RR =0. 37,95% CI:0. 11-1. 24,P =0. 110). Conclusions:Cytapheresis has porfound effect on induction of clinical response, clinical remission and maintenance of remission,and is safe as well. However,the conclusion should be verified by more large-scale and multi-center RCTs.
