Journal of Kunming Medical University 2013;(8):59-62
Objective To investigate the neuromuscular blocking effects and clinical pharmacology of different dosage of mivacurium chloride in infants during sevoflurane anesthesia. Methods Forty ASA I infants undergoing sevoflurane general anesthesia were randomly assigned to tow groups according to the dose level of mivacurium: group1: 200 μg/kg ( n =20) and group2: 250 μg/kg ( n =20) . TOFs were determined synchronously. The onset time of mivacurium, recovery time of spontaneous breathing and cardiovascular reactions were measured. Results The onset time was significantly shortened in group 2 compared with group 1 (P<0.05) . There was no significant difference in the recovery time of spontaneous breathing between the two groups. 2 minutes after mivacurium was injected, DBP in group 2 decreased significantly compared with baseline and group 1. 3 minutes after mivacurium was injected,SBP in group 2 decreased significantly compared with baseline and group 1. Conclusion In infants undergoing sevoflurane general anesthesia, the onset time of mivacurium can be shortened when 250μg/kg was administered, but the depression of cardiovascular system may occurr simultaneously.

Objective To investigate the clinical effects of different dosages of Shenfu injection for treatment of elderly patients with refractory chronic congestive heart failure (CHF).Methods A prospective study was conducted, 360 patients with CHF from Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine were enrolled, and they were randomly divided into low dose, middle dose and high dose Shenfu injection groups. In the three groups, the patients received the same conventional medicine therapy, and additionally they were treated by low, medium and high dose Shenfu injection (60, 80, 100 mL/d, respectively), once a day. The therapeutic course was 10 days in the three groups. After treatment, the clinic therapeutic effect, left ventricular diastolic end diameter (LVEDD), left ventricular ejection fraction (LVEF), stroke volume (SV), cardiac output (CO), cardiac index (CI) and plasma B type natriuretic peptide (BNP) were observed in the three groups. Results The total therapeutic effective rate in middle dose Shenfu injection group was significantly higher than that in low dose and high dose Shenfu injection groups [82.5% (99/120) vs. 54.2% (65/120), 60.0% (72/120), bothP < 0.05]. In high dose Shenfu injection group, increase of blood pressure occurred in 15 cases (12.5%), but no such phenomenon appeared in low and middle dose Shenfu injection groups. In the three groups, no cardiac arrhythmia, liver function abnormality, myocardial enzymogram abnormality, etc. adverse reactions occurred. Compared with those before treatment, after treatment in three groups the LVEDD and BNP were significantly decreased, while LVEF, SV, CO and CI were markedly increased. The changes of above index in middle dose Shenfu injection group were more significant [LVEDD (mm): 46.1±6.3 vs. 58.3±4.4; LVEF: 0.561±0.056 vs. 0.324±0.044, SV (mL): 58.1±6.3 vs. 35.7±5.4, CO (L/min): 5.78±0.60 vs. 4.21±0.78, CI (mL·s-1·m-2): 81.85±7.33 vs. 53.01±9.00, BNP (ng/L): 355.4±76.6 vs. 3 263.2±65.7, allP < 0.05]. Conclusion Shenfu injection 80 mL/d is the best effective dosage for treatment of elderly patients with refractory CHF and its incidence of adverse events is low.

Objective To explore the effect of electroacupuncture intervention on tumor growth and PCNA expression in HepG2 nude mice. Methods Thirty-two nude mice were randomized to electroacupuncture, electrostimulation, model and blank control groups, 8 mice each. A nude mouse model of subcutaneous tumor was made with HepG2. The electroacupuncture group received electroacupuncture at points Zusanli and Sanyinjiao on bilateral lower limbs and the electrostimulation group, direct electrical stimulation of the same points. The tumor volume, the tumor/weight ratio and the tumor growth inhibition rate were observed. PCNA expression in the tumor tissue was determined. Results There was no statistically significant pre-/post-intervention difference in the tumor volume in the electroacupuncture, electrostimulation and model groups (P>0.05). There was no statistically significant post-intervention difference in the tumor volume between the electroacupuncture or electrostimulation group and the model group (P>0.05), but tumor growth tended to slow in the electroacupuncture group. There was no statistically significant pre-/post-intervention difference in the nude mouse weight in every group (P>0.05). There was a statistically significant post-intervention difference in the tumor/weight ratio between the electroacupuncture group and the electrostimulation or model group (P<0.05), but no statistically significant post-intervention difference in the tumor/weight ratio between the electrostimulation and model groups (P>0.05). There was a statistically significantpost-intervention difference in the tumor growth inhibition rate between the electroacupuncture and electrostimulation groups (P<0.05). There was a statistically significant post-intervention difference in the PCNA expression level between the electroacupuncture or electrostimulation group and the model group (P<0.05), no statistically significant post-intervention difference in the PCNA expression level between the electroacupuncture and electrostimulation groups (P>0.05).Conclusion Electroacupuncture can delay HepG2 tumor growth to a certain extent.

Chinese Journal of Nephrology 1997;0(05):-
Objective To analyze type IV collagen a5 chain mRNA and to study the relationship between genotype and phenotype in X-linked Alport syndrome. Methods Total RNA was isolated from the cultured skin fibroblasts of 21 unrelated Chinese X-linked Alport syndrome patients (17 males and 4 females),then ?5 chain (Ⅳ) mRNA was analyzed by using reverse-transcription-polymerase reaction (RT-PCR) and direct sequencing. Meanwhile,by using PCR and direct sequencing, detection of COL4A5 gene mutations at genomic DNA level was carried out. Results Abnormal ?5 chain IV cDNA was detected in 21 X-linked Alport syndrome patients, and seventeen mutations detected in this study were novel mutations. In 15/21 of patients, identical COL4A5 mutations were detected both at mRNA level and genomic DNA level, and in 6/21 of patients with splicing-site mutations, changes in transcript structure differed from changes in genomic DNA level. 16/21 of patients belonged to X-linked juvenile kindreds, and 2/21 of patients to adult kindreds. Conclusion Different type of mutations in COL4A5 can lead to the severe form of X-linked Alport syndrome, and mRNA-based procedures can both directly detect mutations in the coding sequences,as well as changes in transcript level or structure, and can identify some abnormalities that would otherwise have been missed by DNA-based procedures.
