1.Application on improved small incision extracapsular cataract extraction in the sight-regaining program

AIM: To investigate the clinic efficacy and operation safety of improved small incision extracapsular cataract extraction in the sight-regaining program.
METHODS: Totally 82 cases ( 82 eyes ) included in the sight-regaining program of the cataract patients were underwent small incision extracapsular cataract extraction conbined with improved technology ( anterior capsule staining, continuous curvilinear capsulorhexis, auxiliary incision ) , postoperative visual recovery and operative complications were observed.
RESULTS: Postoperative visual acuity: uncorrected visual acuity when out of hospital: ≥0. 3 were 67 cases (82%);1wk after dismissed from hospital:≥0. 3 were 76 cases (93%), admist which ≥0. 8 were 45 cases (55%). There is no serious complication such as posterior capsule rupture, zonular dialysis, etc. in the operation.CONCLUSION: Combined use of improved technology can increase operation safety of small incision extracapsular cataract extraction.

6.Structure and function of a novel thermostable pullulanase.

Research on novel pullulanase has major significance on the domestic industrialization of pullulanase and the breakdown of foreign monopoly. A thermophilic bacteria LM 18-11 producing thermostable pullulanase was isolated from Lunma hot springs of Yunnan province. It was identified as Anoxybacillus sp. by 16S rDNA phylogenetic analysis. Full-length pullulanase gene was cloned from Anoxybacillus sp. LM18-11. The optimum temperature of the pullulanase was between 55 and 60 degrees C with a half-life as long as 48 h at 60 degrees C; and its optimum pH was between 5.6 and 6.4. V(max) and K(m) of the pullulanase was measured as 750 U/mg and 1.47 mg/mL, which is the highest specific activity reported so far. The pullulanase crystals structure showed a typical alpha-amylase family structure. The N-terminal has a special substrate binding domain. Activity and substrate binding were decreased when the domain was deleted, the V(max) and K(m) were 324 U/mg and 1.95 mg/mL, respectively. The pullulanase was highly heterologous expressed in Bacillus subtilis by P43 promoter. The extracellular enzyme activity was 42 U/mL, which increased more than 40 times compared to the initial strain. This pullulanase has good application prospects.

7.Therapeutic effect of indole-3-carbinol on pig serum-induced hepatic fibrosis in rats.

This study is to investigate the therapeutic effect and mechanism of indole-3-carbinol (I3C) on pig serum-induced liver fibrosis of rats. The liver fibrotic model of rats was induced by pig serum. After models were successfully established, rats in the treatment groups were administered with I3C through intraperitoneal injection or curcumin by intragastric administration, daily for 17 days. Hepatic hydroxyproline (Hyp) content was measured. The liver histology and immunohistochemistry with a-smooth muscle actin (alpha-SMA) were assayed. Hepatic stellate cells line, HSC-T6 was incubated with different concentrations of I3C (25, 50, and 100 micromol x L(-1)) for 24 h. The effect of I3C on cell apoptosis was identified by FITC-Annexin V/PI double labeled assay. And the mRNA expressions of Bax and Bcl-2 were measured by real time RT-PCR. The results showed that hepatic content of Hyp decreased by I3C treatment, as compared with the fibrotic model control. Histopathological changes, such as steatosis, necrosis, deposition of collagenous fiber reduced remarkably and the expression of alpha-SMA was significantly down-regulated in the I3C-treated groups (P < 0.01). Apoptosis analysis showed that I3C significantly increased HSC-T6 apoptosis rate and the expressional ratio of Bax to Bcl-2. The results indicated that I3C could effectively cure pig serum-induced liver fibrosis in vivo by inducing HSC apoptosis and promoting ECM degradation.
8.Effect of atorvastatin on TRPC5 expression in atherosclerosis of apoli-poprotein E-knockout mice

AIM: To observe the changes of transient receptor potential channel 5 (TRPC5) in vascular smooth muscle cells ( VSMCs) of apolipoprotein E-knockout ( ApoE-/-) mice and the effect of atorvastatin interference, and to investigate the mechanism of atorvastatin therapy.METHODS:Male ApoE-/-mice at 6 weeks of age were used to establish the atherosclerosis model by feeding with hyperlipidic diet.The mice were randomly divided into model group and atorvastatin group.The mice in atorvastatin group were lavaged with atorvastatin at 20 mg· kg-1 · d-1 , while the mice in model group received normal saline.The healthy C57BL/6J mice with the same age and the same genetic background, feeding with ordinary food, served as control group.At the time points of 14 and 24 weeks, the mice were sacrificed.The serum was collected for detecting the lipid levels.The aortic roots of the heart were taken to make paraffin sections with HE staining for measuring and comparing the relative atherosclerotic plaque area in each section.The expression of TRPC5 in VSMCs was examined with immunohistochemical staining.The mRNA levels of TRPC5 in the serum and the thoracoabdom-inal aorta were measured by real-time PCR.RESULTS: Compared with model group, blood lipids in atorvastatin group were significantly decreased, and the formation of plaque under aorta intima also decreased.The protein expression of TR-PC5 in atorvastatin group decreased significantly compared with model group.Compared with 20-week model group, TRPC5 in 30-week model group showed increasing tendency, but has no statistical significance.Compared with 20-week atorvasta-tin group, TRPC5 of 30-week atorvastatin group declined.CONCLUSION: Atorvastatin suppresses TRPC5 expression, thus attenuating atherosclerotic development in ApoE-/-mice.
9.Protective effects of terlipressin on perioperative renal function in patients undergoing liver transplantation

Objective To evaluate the effects of teriipressin on perioperative renal function in patients undergoing liver transplantation. Methods Forty ASA Ⅲ or Ⅳ patients (31 males and 9 females) aged 35-55 yr and weighing 46-81 kg were randomly divided into2 groups (n=20 each): terlipressin group and control group. The patients were premedicated with intramuscular midazolam 2- 3 mg and atropine 0.5 mg. Swan-ganz catheter was placed via the right internal jugular vein and the radial artery was cannulated. Electrocardiography (ECG), blood pressure (BP), heart rate (HR), central venous pressure (CVP) and pulmonary arterypressure (PAP) were monitored during general anesthesia. General anesthesia was induced with midazolam (0.1-0.2 mg/kg), fentanyl (5-10 μg/kg), propofol(1-2 mg/kg) and vecuronium (0.1 mg/kg) and maintained with 0.5%-1.5% isoflurane, propofol infusion at 2-5 mg·kg-1·h-1 and intermittent i.v. boluses of fentanyl and vecuronium. The patients were mechanically ventilated after tracheal intubation. In the terlipressin group, 2 mg of terlipressin was added to 50 ml of normal saline (NS) and was continuously infused at 10 ml/L from beginning of operation until the end of anhepatic phase, while in the control group, NS was infused only. Blood and urine samples were taken before operation(T0), at the end of anhepatic phase (T1), at the end of operation (T2), and on the 1st and 2nd day after operation (T3, T4)for determination of plasma angiotensin Ⅱ (AT- Ⅱ ), serumβ2-microglobulin (MG), blood urea nitrogen (BUN) and creatinine (Cr) concentrations and N-acetyl-βd-glucosaminidase (NAG) concentrations in the urine. Urine output was measured during pre-anhepatic, anhepatic and neo-hepatic phase and on the 1 st and2nd day after operation. Results The urinary NAG and serum β2-MG concentrations were significantly increased at T1 as compared with the baseline at T0in both groups. The urinary NAG, plasma AT-Ⅱ, serum β2-MG, BUN and Cr concentrations were significantly lower and theurinary output was significantly higher during T2-4 in the terlipressin group than in the control group. Conclusion Terlipressin has protective effects on renal function in patients undergoing orthotopic liver transplantation.
10.Protective Value of Low-dose CT Scanning in Temporal Bone of Children
