Objective To investigate the effect of alteration of CRELD1 gene expression on the related genes in the endocardial cushion development. Methods Over-expression and silence of CRELD1 gene were realized by the construction of lentiviral vector.Afterwards, the lentiviral vectors were used to infect the human fetal lung fibroblasts (HFL)-I.All of the cells were divided into the following 5 groups:the blank control group, the negative control group of interference, the interference group, the negative control group of over-expression,and the over-expression group.Western blot and real-time fluorescent quantitative polymerase chain reaction were applied to examine the mRNA and protein expression of CRELD1,Sox9,Aggrecan,Scleraxis and Tenascin-C.Results The DNA sequences of 2 recombinant plasmids pLV3-shRNA-CRELD1 and pLV4-CRELD1 matched very well with those which were designed according to the DNA sequence analysis.HFL-I was successfully infected with lentiviral vectors and displayed fluorescent green light under inverted fluorescence microscope. The results of real-time PCR detection and Western blot test were consistent:expressions of Sox9 and Aggrecan in the interference group were significantly higher than those in the negative control group of interference, while the expressions of the 2 genes in the over-expression group were significantly lower than those in the negative control group of over-expression.Expressions of Scleraxis increased in both the interference group and the over-expression group when compared with the negative control groups respectively.Compared to the corresponding negative control groups,Tenascin-C expression decreased markedly in the interference group, whereas it increased significantly in over-expression group. Conclusions CRELD1 gene has negative effect on the expression of the related genes Sox9 and Aggrecan in the endocardial cushion development, whereas it has positive effect on the Tenascin-C expression. It serves as a theoretical framework to illustrate the effect of CRELD1 gene on the atrioventricular septal defect.

Objective:To observe the efficacy and side effect of targeting and continuous interstitial chemotherapy in the treatment of tongue squamous cell carcinoma.Methods:Sustain-released cisplatin(20 mg/piece) was implanted into and around tongue squamous cell carcinoma in 10 cases,20 mg was used for the tumor at T1,40 mg for T2 and 60 mg for T3.7~10 days after implantation tumor resection was performed, the efficacy and side effect were observed. Results:The size of all tumor were reduced,50%-60% reduction was observed in 8 cases. Pathological examination of after-operation samples showed cancer cell necrosis in the center, and the degeneration and necrosis in the peripheral tissues where the drug had been implanted in.Necrosis and tissue degeneration could be seen in the metastasized lymph nodes. The side effects were local pain, hyperemia and edema in all the cases.Slight high temperature and nausea were observed in a few cases. Conclusion:The sustain-released cisplatin implanted into lesions is effective for the treatment of squamous cell carcinoma of the tongue with no severe side effects.
8.Impact of intraoperative hypotension and blood pressure fluctuations on early postoperative delirium after laparoscopic surgery
Journal of Chinese Physician 2016;18(7):1017-1020
Objective To investigate whether intraoperative hypotension was associated with postoperative delirium in older patients undergoing laparoscopic surgery with prospective cohort study. Methods Study subjects were patients,> 65 years of age, undergoing laparoscopic surgery, who were enrolled in an ongoing prospective observational study of the pathophysiology of postoperative delirium.Intraoperative blood pressure was measured, and predefined criteria were used to define hypotension.Delirium was measured by the Confusion Assessment Method on the first two postoperative days.Data were analyzed with t-test,two-sample proportion test, and ordered logistic regression muhivariable model, including correction for multiple comparison. Results Data from 213 patients with a mean age of 73.6 years (SD 6.1) were studied.Of these 63 (29.6%) developed delirium on day 1 and 61 (28.6%) on day 2.Relative hypotension (decreases by 20%,30%,or 40%) or absolute hypotension [mean arterial pressure (MAP) < 50mmHg] were not significantly associated with postoperative delirium, nor was the duration of hypotension (MAP < 50 mmHg).Conversely, intraoperative blood pressure variance was significantly associated with postoperative delirium. Conclusions These results showed that increased blood pressure fluctuation, not absolute or relative hypotension, was predictive of postoperative delirium.

The purpose of this study is to investigate the effects of multiple-trough sampling design and nonlinear mixed effect modeling (NONMEM) algorithm on the estimation of population and individual pharmacokinetic parameters. Oxcarbazepine and tacrolimus were used as one-compartment and two-compartment model drugs, respectively. Seven sampling designs were investigated using various number of trough concentrations per individual ranging from 1-4. Monte Carlo simulations were performed to produce state-steady trough concentrations. One-compartment model was used to fit simulated data from oxcarbazepine and tacrolimus. The accuracy and precision of the estimated parameters were evaluated using the median prediction error (PE), the median absolute PE and boxplot. The results indicated that trough concentrations could yield reliable estimates of apparent clearance (CL/F). For oxcarbazepine, as the number of trough concentrations per subject increased, the accuracy and precision of CL/F, between-subject variability (BSV) of CL/F and residual variability (RUV) tended to be improved. For tacrolimus, however, although no improvement were observed in the accuracy of CL/F and BSV of CL/F, the PE distribution ranges were significantly narrowed and the RUV estimates were less bias and imprecise. In terms of algorithm, Monte Carlo importance sampling (IMP) and IMP assisted by mode a posteriori estimation (IMPMAP) were consistently better than other methods. Additionally, the sampling design had no significant effects on the individual parameter estimates, which were only depended on the interaction between BSV and RUV in various algorithms. Decreased in BSV and RUV levels can improve the accuracy and precision of the estimation for both population and individual pharmacokinetic parameter estimates.
