1.Correlation of serum concentrations of IL-17A and CXCL12 with illness severity, formation of lymph follicles in patients with COPD

Objective To investigate the change of serum concentrations of IL‐17A and CXCL12 in patients with chronic ob‐structive pulmonary disease(COPD) and its correlation with illness severity and formation of lymph follicles .Methods Totally 88 cases of patients with COPD in acute phase in our hospital from 1 January 2014 to 1 January 2015 were selected as the research sub‐jects ,and were divided into mild group(n= 23) ,moderate group(n= 42) and severe group(n= 23) according to the severity of COPD ,and were divided into lymph follicles group(n=21) and non lymph follicles group(n=67) according to lymphoid follicles formation .Enzyme‐linked immunosorbent method was applied to detect the serum IL‐17A and CXCL12 concentrations of these groups in acute aggravating period and stability period ,and parameters of pulmonary function in the same period were also detected . The correlations of serum concentrations of IL‐17A and CXCL12 with illness severity ,formation of lymph follicles and lung function in patients with COPD were analysed .Results Compared with mild group ,serum concentrations of IL‐17A and CXCL12 of moder‐ate group and severe group in acute aggravating period and stable period were increased ,while FEV1 and FVC of medium group and severe group in the same period were decreased;compared with the moderate group ,serum concentrations of IL‐17A and CXCL12 of severe group in acute aggravating period and stable period were increased ,while FEV1 and FVC of severe group in the same period were decreased (P<0 .05) .Compared with non lymph follicles group ,serum concentrations of IL‐17A and CXCL12 of lymph folli‐cles group in acute aggravating period and stable period were increased ,while FEV1 and FVC of lymph follicles group in the same period were decreased (P<0 .05) .The results of correlation analysis showed that serum concentrations of IL‐17A and CXCL12 in patients with COPD were positively correlated to the illness severity and lymph follicle formation rate(IL‐17A :r=0 .728 ,0 .762 ;CXCL12 :r=0 .752 ,0 .776 ,P<0 .05) ,while were negatively correlated to FEV1 and FVC(IL‐17A :r= -0 .756 ,-0 .783 ;CXCL12 :r= -0 .743 ,-0 .767 ,P<0 .05) .Conclusion Serum concentrations of IL‐17A and CXCL12 in patients with COPD were associated with its illness severity ,pulmonary function and lymphoid follicles formation ,which may serve as a reference indexes for the assess‐ment of the illness severity ,pulmonary function and lymph follicle formation .
2.Progress in the pathogenesis of Henoch-Schonlein purpura

Henoch-Schonlein purpura(HSP) is one of the most common blood capillary allergic disorder in children, and its pathogenesis has not been well explained. The studies show that the main mechanism of HSP is the abnormality of humoral immunity, and the role of IgA is emphaszed. Meanwhile, many agents such as changes in T cell function,participation of cytokine and mediators of intlammation, suscepti-bility gene and other factors also play an important role in the incidence of HSP. The pathogenesis of HSP can be better understood through extensive analysis of the immunology factors,genetic factors,and so on.
3.Analysis of the effect of Doula accompany combined with labor analgesia on the natural delivery of primipara

Objective:To analyze the effect of Doula delivery combined with labor analgesia on the delivery process and delivery outcomes of primiparas.Methods:Retrospectively selected 200 primiparas treated in Beijing Friendship Hospital, Capital Medical University from January 1, 2019 to September 30, 2019 as the research objects.All patients underwent labor analgesia, and were divided into 100 cases in the Doula group and 100 cases in the control group (without Doula delivery) according to whether or not Doula delivery was performed.T test of two independent samples and chi square test were used to compare the labor process and delivery outcome of the two groups.Results:The natural delivery rate in the Doula group was 76%(76/100), which was significantly higher than the control group (61%(61/100)), and the difference was statistically significant (χ 2=5.214, P=0.022). The time of the first, second and third stage of labor in Doula group was (8.35±3.59), (1.07±0.26), (0.54±0.19) h, and that in the control group was (10.94±4.76), (1.86±0.63), (0.78±0.21) h, respectively.There were significant differences between the two groups (t value were 10.354, 5.312 and 8.169, respectively; P value were 0.016, 0.042 and 0.039, respectively). The neonatal asphyxia rates of Doula group and control group were 1% (1/100) and 3% (3/100) respectively, and there was no significant difference between the two groups ( P=0.621). The incidence of postpartum hemorrhage in the Doula group and the control group was 3% (3/100) and 10% (10/100) respectively, and the incidence of postpartum urinary retention was 0 (0/100) and 6% (6/100), respectively.The success rate of early sucking was 98% (98/100) and 90% (90/100) respectively, and the difference between the two groups was statistically significant (P value were 0.045, 0.029 and 0.017, respectively). The hospitalization time of Doula group was (3.17±0.85) d, which was significantly shorter than that of the control group (5.64±1.29) d)( t=6.359, P=0.031). The visual analogue pain score of 2 h postpartum in Doula group was 3.49±0.98, which was significantly lower than the control group (5.82±1.06)( t=9.327, P<0.001). Conclusion:Doula delivery combined with labor analgesia can effectively reduce the pain of uterine contraction, promote natural delivery, reduce the rate of cesarean section, help to improve the quality of obstetric medical service, improve patient satisfaction, it is worth promoting in the condition of the hospital.
4.Application of internal fixation technique in eyebrow plastic surgery

Objective To discuss the application of internal fixation technique in eyebrow lift and infrabrow excision blepharoplasty.Methods With the incision within eyebrow or infrabrow excision incision,fixation of the eyebrow to the brow periosteum using 4-0 silk thread and then suturing the upper and lower border of the incision could lift the eyebrow and correct the ptosis of upper eyelid skin and fishtail lines.Results All the 138 cases got good results,showing that the eyebrow and upper eyelid skin ptosis and lateral fishtail lines were all corrected.Conclusions Internal fixation technique can get better results in eyebrow lift and infrabrow exicion blepheraplasty surgery.
6.Effects of DNaseⅠ-treated streptococcal antigens on peripheral blood mononudear cell proliferation in patients with psoriasis vulgaris

Objective To investigate the effects of DNaseⅠ-treated streptococcal antigens on peripheral blood mononuclear cells(PBMC)proliferation in psoriasis patients.Methods PBMC from 15 patients with psoriasis and 10 healthy controls were stimulated with three concentrations(2,10,25?g/mL) of streptococcal antigens(SA), nd DNaseⅠ-treated streptococcal antigens(DNaseⅠ-SA).After 72 h of culture,cellular proliferation was determined by ~3H thymidine incorporation.Results The DNaseⅠ-SA,at all the concentrations tested,was less potent than the corresponding concentrations of SA(P0.05),however,the lower concentrations(2,10?g/mL) of DNaseⅠ-SA were less potent than those of SA(P0.05).Also,the extent of proliferation response to SA and DNaseⅠ-SA was higher in the patients than in the controls(P
7. Effectiveness comparison between Scarf osteotomy combined with Akin osteotomy fixed by absorbable screws and fixed by metal screws for the treatment of moderate to severe hallux valgus

Objective: To evaluate the effectiveness of Scarf osteotomy combined with Akin osteotomy in the treatment of moderate to severe hallux valgus with absorbable screws or metal screws fixation by clinical and radiological data. Methods: Between March 2014 and May 2016, the Scarf osteotomy combined with Akin osteotomy was used to treat 62 patients (83 feet) with moderate to severe hallux valgus. Twenty-five patients (35 feet) were fixed by absorbable screws (group A) and 37 patients (48 feet) were fixed by metal screws (group B). The difference in gender, age, affected side, etiology, severity, disease duration, preoperative hallux valgus angle (HVA) and first-second intermetatarsal angle (1-2IMA) measured in weight-bearing anteroposterior X-ray film, preoperative American Orthopaedic Foot and Ankle Society (AOFAS) scores and visual analogue scale (VAS) scores between 2 groups ( P>0.05). The complications and healing time of 2 groups were recorded and compared. At last follow-up, the AOFAS and VAS scores, and HVA, 1-2IMA measured by weight-bearing anteroposterior X-ray film were used to evaluate the effectiveness. Results: All incisions healed by first intention and no incision-related complication occurred in the two groups after operation. Both groups were followed up, the follow-up time in group A was 12-36 months (mean, 24.4 months) and in group B was 14-38 months (mean, 25.7 months). In group A, 1 foot was complicated with hallux varus, 2 feet had mild stiffness, 1 foot appeared metastatic metatarsal pain, and in group B was 2, 3, 2 feet, respectively. There was no recurrence of hallux valgus in both groups. The difference in the incidence of complications between the two groups after the first operation was not significant ( χ2=0.275, P=0.843). The removal rate of internal fixator in group B was 89.2% (33 cases) during the second operation, among which 3 cases had screw slip and 1 case had screw fracture. And 10.8% (4 cases) refused the second removal operation due to their age. Postoperative X-ray films showed that both groups had good healing at the osteotomy site, and there was no significant difference in healing time between the two groups ( t=1.633, P=0.285). At last follow-up, the AOFAS score, VAS score, HVA, and 1-2IMA were significantly improved in the two groups when compared with preoperative ones ( P<0.05); but no significant difference was found between the two groups ( P>0.05). Conclusion: The effectiveness of Scarf osteotomy combined with Akin osteotomy in the treatment of moderate to severe hallux valgus is significant, with few complications. Compared with being fixed by metal screws, being fixed by absorbable screws has the same effectiveness, but can avoid the risk of second operation to remove the internal fixator.
9.Diagnosis and Treatment of the Severe Complications After Endoscopic Sphincterotomy

Objective To investigate and analyze the severe complications and their surgical managements after endoscopic sphincterotomy(EST).Method 36 patients with complications after EST such as severe hemorrhage, perforation and severe pancreatitis were analyzed,of them there were 16 cases of hemorrhage, 11 case of perforation, 6 cases of hemorrhage combined with perforation and 3 cases of severe pancreatitis combined with perforation. Results 14 cases of merely hemorrhage patients, 11 cases of merely perforation patients, 1 case of perforation combined with severe pancreatitis patient and 3 case of perforation combined with hemorrhage patient were survived while 2 case of merely hemorrhage patient, 3 cases of perforation combined with hemorrhage patients and 2 case of perforation combined with severe pancreatitis patients died.Conclusions Hemorrhage is the most common complication of EST and most complications can be healed well through expectant treatment. Once severe complications occurred, prompt operations are the keys to lower the mortality.
10.THE HISTOLOGICAL OBSERVATION OF THE STEPWISE DEVELOPMENT OF NEONATAL MOUSE TESTIS IN IMMUNODEFICIENT MICE

Objective To investigate the development of neonatal mouse testis in castrated immunodeficient mice by monitoring the graft survival and weight and observing the structure of seminiferous epithelium and the composition of spermatogenic cells in grafts. Methods Neonatal Kun-ming mouse testis were grafted under the skin of castrated nude mice(7-12week-old).Grafts were then taken out at different time intervals(namely 16 time points: 3 days,1-11 weeks respectively and 3-6 months respectively).The survival rate of grafts was calculated and the wet weight was measured.Histological analysis was performed for the observation of the structure of seminiferous epithelium and the composition of spermatogenic cells in grafts. Results Four hundrcd and five grafts recovered out of 450 testis grafted, resulting in a recovery rate of 90.0%.The graft weight increased more than 40 times.The developmental pattern of seminiferous tubules and the appearance time of various spermatogenic cells in grafts were similar as seen in intact mice.Eight weeks after the grafrting,an increasing degradation of seminiferous epithelium was observed.Conclusion When neonatal mouse testis were grafted into nude mice,the developmental course was similar as that in normal donors.The optimal retrieval time of round spermatids and sperms was around the end of the first spermatogenesis wave,about 5-7weeks after the grafting procedure.
