1.A comparative study of the efficacy of ventricular septal defect in membrane position between interventional radiology and thoracotomy in children

Objective To compare surgical and interventional treatment in congenital ventricular septal defect in membrane position.Methods Among ventricular septal defect (VSD)in 384 cases in children,202 cases had surgical repair thoracotomy (tho-racotomy group),and 182 cases had interventional treatment (intervention group).The comparison items included operative time, success rate,intraoperative and postoperative blood transfusion,complication rate,postoperative recovery for several days after ICU monitoring time,CPB time,pericardial drainage time,duration of mechanical ventilation.Results Children in the intervention group did not need blood transfusions,ICU care,pericardial drainage,mechanical ventilation.In the thoracotomy group,the blood transfu-sion was (372.45±200.88)mL,postoperative ICU monitoring time was (3.21 ±2.1 7)days,CPB time was (71.09 ±34.92)mi-nutes,pericardial drainage time was (3.52 ± 1.22)days,mechanical ventilation time was (67.09 ±43.83)minutes.The operative time and postoperative recovery time in the intervention group was significantly shorter than thoracotomy group (P <0.05 ).The incidence of postoperative complications in the intervention group was significantly lower than thoracotomy group (P <0.05).Con-clusion Under the circumstances of the same indication,the interventional treatment is more beneficial to physical and mental health of children.
2.Serum inflammatory cytokine levels in periodontitis and its related with coronary heart disease

Objective:To explore the relationship between periodontitis and coronary heart disease (CHD)by meas-uring changes of serum inflammatory cytokine levels.Methods:A total of 197 patients from our hospital were en-rolled,including CHD + periodontitis group (123 cases),pure CHD group (CHD group,39 cases)and pure peri-odontitis group (periodontitis group,35 cases).Another 45 healthy subjects were enrolled as healthy control group. Concentrations of tumor necrosis factor-α(TNF-α),interleukin-6 (IL-6),serum amyloid-a (SAA),high sensitive C reactive protein (hsCRP)and plasma fibrinogen (Fg)were measured in all groups,and levels of above inflammatory cytokines were compared among four groups.Results:Compared with healthy control group,there were significant rise in levels of above inflammatory cytokines in CHD + periodontitis group (P<0.01 all),Com-pared with CHD group or periodontitis group,there were significant rise in levels of hsCRP [(6.52±2.03)mg/L, (4.34±1.76)mg/L vs.(9.21±2.65)mg/L],IL-6 [(65.32±21.01)ng/L,(65.12±12.34)ng/L vs.(85.76± 25.96)ng/L]and TNF-α[(9.75±3.74)ng/L,(9.34±4.55)ng/L vs.(13.46±8.12)ng/L],SAA [(2.25±1.2) pg/ml,(2.22 ±1.02)pg/ml vs. (2.85±1.45)pg/ml],P<0.05 all in CHD + periodontitis group;there were no significant difference in all serum inflammatory cytokine levels between CHD group and periodontitis group (P>0.05 all).Conclusion:Serum inflammatory cytokine levels significant rise in palients with periodontitis,it is related with coronary heart disease.
3.Management of complications after endovascular repair for De Bakey type Ⅲ aortic dissection

Objective To summarize the experience in endovascular repair of De Bakey type Ⅲ aortic dissection in recent years and summarize the prevention and management of the related perioperative complications.Methods From January 2009 to January 2011,49 cases of endovascular repair for De Bakey type Ⅲ aortic dissection were performed under general anesthesia in our department.There were 45 male and 4 female.The follow-up was performed in the outpatient department or by telephone.Results There was no inhospital death and no paraplegia events.Severe complication included:coma,2 cases ( 4.1% ) ; endoleak,2 cases (4.1% ) ; upper limb ischemia,2 cases (4.1% ).Recurrent proximal aortic dissection,1 case.Fever was occurred in most of those cases.Conclusion Endovascular repair of aortic dissection improves the outcome of aortic dissection patients.But more attention should be pay to prevent the severe complications,It will help to improve the prognosis and life quality by reducing the risk of retrograde dissection,acute brain ischemia and endoleak.
4.Influence of Porphyromonas gingivalis on vascular intima adhesion factors and metalloproteinases

Objective: To observe influence of porphyromonas gingivalis (Pg, a main pathogenic bacterium of periodontal disease) on vascular intima adhesion factors and metalloproteinases. Methods: A total of 60 rats were randomly and equally divided into blank control group, Pg low dose group (Pg 2ml) and Pg high dose group (Pg 5ml) according to number table. The latter two groups respectively received intramuscular injection of corresponding dose Pg every three days without antibiotic intervention for 12 weeks. Then their venous blood was taken to measure levels of intercellular adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1), matrix metalloproteinase (MMP)-3 and MMP-9 in three groups. Results: Compared with blank control group, there were significant increase in contents of ICAM-1 [(175.79±14.30)ng/ml vs.(182.62±15.07)ng/ml, (189.39±14.93)ng/ml]、VCAM-1 [(256.49±37.17)ng/ml vs.(271.58±32.85)ng/ml , (286.66±30.66)ng/ml] 、 MMP-3 [(3.23±0.69)ng/ml vs.(3.61±0.82)ng/ml, (3.97±0.83)ng/ml]及 MMP-9 [(1.30±0.39)mg/L vs.(1.48±0.39)mg/L, 1.67±0.45)mg/L ](P <0.05,or <0.01),Compared with Pg low dose group, there were significant increase in levels of above indexes in Pg high dose group (P<0.05). Conclusion: Porphyromonas gingivalis can significantly increase serum contents of vascular intima adhesion factors and metalloproteinases, aggravating pathological development of coronary heart disease.
5.Ginger-partition moxibustion combined with glucocorticoid for thyreoitis at subacute stage: a randomizd controlled trial.

OBJECTIVETo compare the clinical effects between ginger-partition moxibustion combined with glucocorticoid and simple oral glucocorticoid for thyreoitis at subacute stage.
METHODSEighty-one patients were randomly divided into an observation group (41 cases) and a control group (40 cases). In the observation group, ginger-partition moxibustion and hormone were applied. Moxa cones were used at local ashi points, Zusanli (ST 36), Guanyuan (CV 4) and Qihai (CV 6), six cones every point, once every other day and three times a week. Besides, 24 mg methylprednisolone tablets were adopted orally every day, and in two weeks the dose was 16 mg/d, in four weeks 8 mg/d, in six weeks 4 mg/d; all the patients were observed for 8 week. In the control group, simple methylprednisolone was prescribed orally, and the dose, the usage and treatment time were the same as those in the observation group. Erythrocyte sedimentation rate (ESR), triiodothyronine (T3), thyroxin (T4) and ultra-sensitive thyroid stimulating hormone (TSH) before and after treatment were observed in the two groups, as well as fever, the pain of thyroid gland, the regression time of swelling and adverse reaction.
RESULTSThe time of thyroid gland pain relieved of the observation group was earlier than that of the control group [(3.07 ± 0.78) days vs (3.62 ± 0.92) days, P < 0.05]. After treatment, T3, T4 and ESR were declined apparently (all P < 0.01), and TSH was obviously increased in the two groups (both P < 0.01). After 2-week treatment, ESR in the observation group was lower than that in the control group (P < 0.05). After 4-week treatment, T3, T4, TSH and ESR in the observation group were better than those in the control group (all P < 0.05). After 8 weeks, all indices in the observation group were superior to those in the control group, without statistical significance between the two groups (all P > 0.05). The effects of the observation group in 2 weeks, 4 weeks and 8 weeks were better than those in the control group (all P < 0.05). The cured course was shorter in the observation group than that in the control group (P < 0.05). The adverse reaction rate in the observation group was lower than that in the control group [4.9% (2/41) vs 22.5% (9/40), P < 0.05]. Three months later after treatment, the cured patients were followed. There was no recrudescence in the observation group and three patients caught the disease again in the control group, but there was no statistical significance between the two groups (P > 0.05).
CONCLUSIONGinger-partition moxibustion combined with glucocorticoid achieves better effect than simple oral glucocorticoid for thyreoitis at subacute stage, and the adverse reaction is less, which presents the clinical advantages of the integration of Chinese and western medicine.

6.Effects of electroacupuncture at different acupoints on apoptosis and the expression of miRNAs in myocardial cells in rats model of myocardial ischemia.

OBJECTIVETo observe the effects of electroacupuncture (EA) at different acupoints on apoptosis-related serum and expression of microRNA (miRNA) in rats with myocardial ischemia, so as to explore its mechanism of action.
METHODSA total of 48 male Wistar rats were randomly divided into a normal group, a model group, a Neiguan group and a acupoint compatibility group, 12 rats in each group. Isoprenaline hydrochloride (ISO) with a daily dose of 2 mg/kg was subcutaneously injected for 14 days to establish the myocardial ischemia model in the model group, Neiguan group and acupoint compatibility group. Rats in the normal group were subcutaneously injected with an equal volume of normal saline. After modeling, rats in the Neiguan group were treated with EA at "Neiguan" (PC 6), while rats in the acupoint compatibility group were treated with EA at "Guanyuan" (CV 4), "Zusanli" (ST 36) and "Neiguan" (PC 6). Rats in the normal group and model group were treated with immobilization, once day for 21 days. The contents of creatine kinase-MB (CK-MB), vascular cell adhesion molecule-1 (VCAM-1) and endothelin-1 (ET-1) in serum were detected by enzyme-linked immunosorbent assay (ELISA); apoptosis index (AI) of myocardial cells was detected by TUNEL method; the expressions of miRNA-1, miRNA-133, miRNA-208 and miRNA-499 were detected by real-time PCR method.
RESULTSCompared with the normal group, the serum CK-MB, VCAM-1 and ET-1 were significantly increased in the model group, Neiguan group and acupoint compatibility group (all P < 0.01), and the apoptosis index was significantly increased (all P < 0.01). The CK-MB, VCAM-1 and ET-1 in the Neiguan group and acupoint compatibility group were significantly lower than those in the model group (all P < 0.01); the AI was reduced, which was more significant in the acupoint compatibility group (P < 0.05). Compared with the normal group, the expression of miRNA-133 was reduced (P < 0.01) and those of miRNA-208, miRNA-1 and miRNA-499 were significantly increased in the model group (all P < 0.01). Compared with the model group, the expression of miRNA-133 was increased (both P < 0.01) and that of miRNA-208, miRNA-1 and miRNA-499 were significantly reduced (all P < 0.01) in the Neiguan group and acupoint compatibility group. Compared with the Neiguan group, the expression of miRNA-133 was increased (P < 0.01) and those of miRNA-208, miRNA-1 and miRNA-499 were significantly reduced in the acupoint compatibility group (P < 0.01, P < 0.05).
CONCLUSIONEA at acupoints, especially acupoint compatibility group, could effectively prevent and treat myocardial ischemia, and the protective effect is possibly correlated to the double regulation on increasing the expression of miRNA-133 and inhibiting the expression of miRNA-1, miRNA-208, miRNA-499.

7.An retrospective analysis of endovascular repair in 83 cases of staniord type B aortic dissection

Objective To summarize our experience in endovascular repair of 83 cases with type B aortic dissection.Methods A retrospective analysis was performed in 83 cases of type B aortic dissections who were treated in our hospital.Results The surgical procedure was successful for all the patients.Two patients died peri-operatively.One case encountered a coma,but no post-operative paraplegia occurred.Conclusion Endovascular repair for type B aortic dissection is a micro-invasive,safe and effective technique.Long-term follow-up is required to give a comprehensive evaluation.
8.Efficacy of transjugular intrahepatic portosystemic shunt with covered stents in the treatment of portal hypertension in 46 patients with primary hepatocellular carcinoma

Objective To investigate the efficacy of transjugular intrahepatic portosystemic shunt (TIPS) with covered stent in the treatment of portal hypertension in patients with hepatocellular carcinoma.Methods The clinical data of 46 patients with primary hepatocellular carcinoma and portal hypertension who received TIPS with stent treatment were retrospectively analyzed.After treatment,liver function,shunt patency,re-bleeding rate,hepatic encephalopathy,ascites,gastric intestinal symptoms, lifetime and causes of death were analyzed. Compare t test was performed for measurement data comparison among groups.The survival curves were used to analyse the cumulative stent shunt patency rate,the incidence of hepatic encephalopathy and the survival rate.Results After the operation,portal vein pressure significantly decreased compared with that before operation [(16.2 ± 4.6 )mmHg vs (28.3±5 .1)mmHg,1 mmHg=0.133 kPa],and the difference was statistically significant (t =21 .30, P <0.05).There was no significant difference in liver function before and after operation [(6.8 ±1 .8) score vs (6.9±1 .5)score,P >0.05].Cumulative stent shunt patency at 3,6,12,24,36 month after operation was 100.0%,95 .6%,93.5 %,91 .3% and 91 .3%,respectively.The cumulative incidence of hepatic encephalopathy was 6.5 %,8.7%,13.0%,17.4% and 26.1 %,respectively;cumulative survival rate was 95 .7%,82.6%,67.4%,43.5 % and 32.6%,respectively.In 10 cases of refractory ascites, ascites of all the patients reduced with different degree after operation and gastrointestinal uncomfortable symptoms were relieved.Conclusion In patients with primary hepatocellular carcinoma and portal hypertension,who have gastrointestinal bleeding or refractory ascites,treatment of TIPS with covered stents could effectively lower portal pressure,prevent re-bleeding and reduce the volume of ascites.

This study explored the differences in academic achievement and psychological/emotional well-being between criterion-referenced evaluation (CRE) and norm-referenced evaluation (NRE) at a medical school. The objective was to address concerns about declining academic performance and to propose evaluation methods that promote a more supportive learning environment for students. In total, 90 first-year medical students who completed the basic medical curriculum in 2022 and 2023 were surveyed and analyzed. The sample consisted of 45 students assessed by NRE in 2022 and 45 students assessed by CRE in 2023. Academic performance was measured using school grades in basic medical subjects and scores from the Basic Medical Education Evaluation. Psychological and emotional well-being were assessed through scales measuring academic burnout, academic motivation, coping efficacy, anxiety, depression, and quality of life. The results indicated that students assessed by CRE achieved significantly higher academic scores in certain subjects than those assessed by NRE. Additionally, the CRE group exhibited more positive psychological and emotional states, including lower levels of academic burnout, anxiety, and depression. These findings suggest that CRE may have a beneficial impact on students’ emotional well-being and overall academic experience. The study concludes that CRE plays a critical role in improving both academic performance and emotional health among medical students, and that transitioning to CRE may alleviate concerns about declining grades while promoting a more supportive learning environment.
