Objective To explore the clinical significance of changes of cell immune function in patients with oral cancer before and after operation.Methods 20 healthy volunteers(control group) and 20 patients with oral squamous cell carcinoma who pathologically confirmed after operation (observation group) were included in this study.The peripheral blood T lymphocyte subsets and the levels of cytokines in Th1 cells were detected before and after surgery.Results Compared with the control group,the percentage of preoperative serum CD3+,CD4+,CD8+,CD5+ T lymphocyte and CD4/CD8 ratio in the observation group were decreased significantly (t =7.908,7.282,2.428,4.360,4.329,P ＜ O.05).In the observation group,the percentage of CD3+,CD4+,CDs+,CD5+ T lymphocyte and CD4/CD8 ratio after operation were significantly improved than before surgery (t =3.994,6.159,2.401,2.446,2.102,all P ＜0.05),but CD4+ T lymphocytes and the ratio of CD4/CD8 were still significantly lower than the control group(t =2.741,3.133,all P ＜ 0.05).In the observation group,the preoperative levels of cytokines secreted by T lymphocytes were significantly lower than those in the control group (t =3.175,2.747,43.000,3.137,16.160,46.750,all P ＜ 0.05).After operation,the levels of cytokines secreted by T lymphocytes were improved.Conclusion Patients with oral cancer have immune suppression,which manifest with the decrease of Th cells,the increase of suppressor T cells and the decrease of CD4/CD8 ratio.The immune function of the patients is improved after operation.

Objective To discuss oral cavity nursing in senile diabetic patients with oral disease.Methods 80 senile diabetic patients were offered diet control and treatment for lowering blood glucose with simultaneous oral nursing for 4 weeks.The blood glucose,blood lipid before and after nursing were observed.Results The total effective rate reached 91.3% after oral nursing,blood glucose and blood lipid greatly lowered after nursing.Conclusions Prevention of oral disease is important for preventing diabetic complications,appropriate oral nursing contributes to prevention and treatment of diabetes mellitus complicated with oral disease.

Objective To observe the efifcacy and safety of XELOX plus bevacizumab as a ifrst-ine treatment for Chinese patients with metastatic colorectal cancer. Methods In this study, we retrospectively reviewed cases in which XELOX plus bevacizumab were administered in order to evaluate its efifcacy and safety in clinical practice.In total, 40 patients with mCRC who presented at Fuchu Hospital received XELOX plus bevacizumab as a ifrst?line treatment from September, 2009 to April, 2012. Eligible patients had histologically conifrmed mCRC. XELOX consisted of a 2-h intravenous infusion of oxaliplatin 130 mg/m 2 on day 1 plus oral capecitabine 1,000 mg/m 2 twice daily for two weeks of athree?week cycle. Overall survival (OS) and survival benefit were analyzed when patients continued with XELOX plus bevacizumab beyond disease progression. Results The median progression?free survival (PFS) was 290 days [95%conif-dence interval (CI):222?409 days] and the median OS was 816 days (95%CI:490?842 days). The response rate (RR:complete plus partial response) was 67.5%, and the disease control rate (RR plus stable disease) was 90%. Conclusion XELOX plus bevacizumab may be considered a routine ifrst?line treatment option for patients with mCRC.

Objective To compare curative effects between laparoscopic and open surgery in the treatment of pediatric indirect inguinal hernia.Methods A group of 410 children with indirect inguinal hernia underwent micro-laparoscopic operation(Laparoscopic Group) from April 2001 to January 2006.This group was compared with another group of 408 children with indirect inguinal hernia,treated with open high ligation of the hernial sac(Open Group) from September 2004 to January 2006.In the Laparoscopic Group,a 5-mm trocar was inserted at the umbilicus for the exploration of bilateral internal rings.Another 5-mm trocar was inserted at the site slightly superior to the McBurney's point for introducing a needle holder.At the level of the neck of the hernial sac,the orifice of the vaginal process was circumferentially sutured and ligated.In the Open Group,high ligation of the hernial sac was conducted through a transversal incision at the external ring.Results The operations in the Laparoscopic Group were smoothly completed.The operative time for one side of hernia was significantly shorter in the Laparoscopic Group(33.4?9.6 min) than in the Open Group(38.0?7.6 min)(t=-7.596,P=0.000).In the Laparoscopic Group,no edema of the scrotum,subcutaneous hydrops,or infection occurred.The patients were followed for 1~55 months(mean,23.2 months).Short-term recurrence was observed in 3 children,who required a re-operation of laparoscopic repair.No testicular atrophy,iatrogenic retained testis,intestinal adhesion,or other long-term complications happened.In the Open Group,follow-up observations for 1~18 months(mean,10 months) revealed recurrence in 18 children,8 of whom were then cured with laparoscopic surgery and 10 of whom with open surgery.Iatrogenic retained testis happened in 9 children and a re-operation of open surgery was carried out.Conclusions Two-port laparoscopic high ligation of hernial sac for pediatric indirect inguinal hernia is minimally invasive,safe,and effective,but not suitable for children with cardiac insufficiency or newborn incarcerated hernia.

Objective To predict and identify T cell epitopes of major group 3 allergen derived from Dermatophagoides fari-na(Der f 3). Methods The T cell epitopes of Der f 3 were analyzed through the sequence analysis by using the bioinformatics online tools. The five predicted peptides of T-cell epitopes were artificially synthesized. The spleen lymphocytes were co-cultured with the five T cell epitopes by using the modified MTT method and the levels of IL-2,IFN-γ,IL-4 and IL-5 in the supernatant of the cultures were detected by ELISA. Results Five T cell epitopes of Der f 3 were predicted and three of which could pro-mote the proliferation of the mouse spleen lymphocytes. The secretions of IL-2 and IFN-γwere significantly induced and the se-cretions of IL-4 and IL-5 were significantly decreased by three of five prediction epitopes of Der f 3:37GDCPYQISLQSSSHFC-GG54,98IYQHENYDSMTIDNDVALIKLKTPMT123 and 164SELQRVDIDVVSREQCDQLYS184. Objective To investigate the effect and clinical significance of neoadjuvant radiotherapy for the expression of CD44v6 in lung squamous cell carcinoma tissues. Methods Fifty cases lung squamous cell carcinoma patients confirmed by aspiration biopsy from May 2013 to January 2015 were collected in Yangquan Coal Mine Group Genernal Hospital,including 20 cases of patients with stageⅢA were treated with neoadjuvant radiotherapy before surgery,and then performed surgery after neoadjuvant therapy. The expression of CD44v6 was detected by immunohistochemistry. The correlation between CD44v6 and clinicopathological features was analyzed by chi?square test. Results Immunohistochemical staining of CD44v6 was performed in tumor tissue of puncture biopsy, the positive rate expression of CD44v6 was 72%( 36/50 ) , it was associated with lymphatic metastasis(χ2 =3. 964, P=0. 046 ) and advanced TNM stage (Ⅲ+Ⅳstage ) (χ2 =4. 276, P=0. 039 ) . The positive expression of CD44v6 protein in tumor tissue was significantly decreased in 20 patients with neoadjuvant radiotherapy compared with before radiotherapy(7. 23±1. 45 vs. 11. 42±1. 31,t=2. 524,P=0. 025). Conclusion Positive expression of CD44v6 in human lung squamous cell carcinoma is related to the malignant clinicopathological features. Neoadjuvant radiotherapy before operation may improve prognosis via down?regulating CD44v6 expression.

BACKGROUND:It is crucial for us to find an objective and sensitive means for the assessment ofthe severitv and development of dementia,so as to make instant diagnosis and examination of therapeutic effect on patientswith vascular dementias(VD).Event related potential endogenetic eomponent P300 is proved closely correlated with the cognitive management of examinees but not with stimulus property,thereby considered as one of the most valuable and objective index for brain superior psychological activity.OBJECTIVE:To study the role and significance of events related potential P300,providing objective basis for clinical diagnosis and therapeutic assessment for patients with VD.DESIGN:Non-randomized simultaneous controlled explorative study based on patients. SETTING:Neurological department and electromyography department in aprovincial hospital and neurological department in a medical college hospital.PARTICIPANTS:Between February 2000 and June 2001,64 patients were of VD at neurological clinic and ward ofthe Second Affiliated Hospital of Chongqing Medical College,including 38 males and 26 females with theaverage age of(66.0±4.6)years(VD group).Culturallevel:19 cases re-ceiyed over senior school education,31 cases of junior school education and14 cases of illiteracy.Meanwhile 60 inpatients without VD were enrolled(non-VD group)including 35 males and 25 females withthe average age of(68.0±3.7)years.Culturallevel:senior school or above,15 cases,junior school, 29 cases and illiteracy,16 cases.METHODS:Mental sate of both VD and non-VD groups were assessed with mini-mental state examination(MMSE),and tested with P300 potential observing the variability of latency and amplitude of P300 in patients.Both methods were compared between VD group and non-VD group, moreover relationship between P300 latency and MMSE scores was also explored.MAIN OUTCOME MEASURES:Variability of P300 latency and amplitude Relationship between P300 latency and MMSE scores.RESULTS:P300 latency was found prolonged in 74% and 7% of VD group and non-VD group respectively,with obvious significance between them,meanwhile prolonged P300 latency was proved closely correlated with the MMSE scores(r=0.609 6,P＜0.05).CONCLUSION:P300 latency can be considered as an obiective predictor for the earlier diagnosis and the assessment of therapeutic effects in patients with VD.
