Leptin and its functional receptors play a crucial role in hepatic fibrogenesis. However, the precise mechanisms underlying the profibrogenic action of leptin in the liver has not been well elucidated. There are two different mechanisms at present.Immunity system and some cytokines may act on it.The reseach on the mechanism of leptin offer a new way for the clinical treatment of hepatic fibrosis.

Objective To evaluate the effect of health education on preventing birth defects in community in terms of knowledge, attitude and practice (KAP) for married women in reproductive age. Method A self-designed general information form and a questionnaire on KAP of married women′s preventing birth defects were used to investigate the KAP among 300 married nulliparous women. Result The average rate of cognitive knowledge of birth defects was 60.8%, the average rate of cognitive behaviors was 59.2%, and the average rate of cognitive attitude was 67.4%. Conclusion In order to reduce the rate of birth defects and improve the quality of births, we should implement three-level prevention education system, enrich the health education methods to enhance the knowledge, attitude and behavior of married women.

Objective To detect the effect of a new bioactive bone screws composed of nanohydroxyapatite/polyamid 66/glass fiber (n-HA/PA66/GF) biomaterial on biological behavior of osteoblasts with a view to a later clinical application of the screws. Methods Mice osteoblasts were co-cultured with n-HA/PA66/GF biomaterial or its extract.Cell growth was detected under a microscope, cell proliferation using MTT assay, apoptosis by flow cytometry, osteocalcin by ELISA method, cell migration using Transwell invasion assay, cell adhesion and growth by scanning electron microscope, and cytoskeleton and actin filament distribution using immunofluorescene. Results Direct contact test demonstrated n-HA/PA66/GF material had no obvious cytotoxicity to osteoblasts.Extract of n-HA/PA66/GF material stimulated osteoblast proliferative activity over time with absorbance value of 0.96 ± 0.14,1.54 ± 0.15,and 2.39 ±0.27 respectively after 2,4,and 6 hours of coculture (P < 0.05).The n-HA/PA66/GF material induced more osteoblasts to entering S period without obvious influence on apoptosis rate and promoted osteocalcin secretion.Migrated cells in medium supplied with n-HA/PA66/GF material or FBS was 8.73 ± 3.26 and 9.47 ± 3.29 in each visual field, but there was no significant difference (P > 0.05).Osteoblasts had a regular form on the surface of n-HA/PA66/GF material, closely adhered to the surface, and reproduced and aggregated with each other to form stratified cell layers.However,nHA/PA66/GF material exhibited no obvious influence on distribution of cytoskeleton and actin filament. Conclusion The new n-HA/PA66/GF screws has excellent cytocompatibility with positive regulatory effect on cell growth, proliferation, secretion, adhesion, cycle, and osteocalcin secretion.

Objective To evaluate the sensitive indicator of left ventricular rotation/torsion assessed by two-dimensional speckle tracking imaging (STI) and Logistic regression analysis, and to investigate the clinical value of the sensitive indicator for assessment of left ventricular dysfunction in patients with chronic cor pulmonale (CCP).Methods 36 patients with CCP (CCP group) and 38 healthy controls (control group) were included in this study.Imaging in parasternal short-axis view (in basal and apical level) were selected.Parasternal short-axis views at mitral valve and apical levels were collected.Basal peak rotation, apical peak rotation, peak torsion, basal end-systolic rotation, apical end-systolic rotation and end systolic torsion were measured with Echo PAC software.Relevant indicators of left ventricular rotation/torsion were selected by using logistic regression analysis and the regression equation was established.Optimal values of specific parameters (Peak torsion and end-systolic torsion) were calculated with receiver-operating characteristic (ROC) curve. Results Specific parameters of rotation/torsion were significantly reduced in patients with CCP as compared with controls (all P<0.01).Logistic regression analysis showed that end-systolic torsion and peak torsion were correlated with CCP (OR=0.473 and 0.706,P=0.007 and 0.011).Cut-off value of peak torsion for predicting left ventricular dysfunction was 12.070°,the area under the ROC curve (AUC) was 0.819 (95% CI:0.683-0.956),the sensitivity was 84.6%,and the specificity was 73.9 %.Cut-off value of end-systolic torsion for predicting left ventricular dysfunction was 10.680°,AUC was 0.875(95%CI:0.744 1.000),the sensitivity was 84.6%,and the specificity was 91.3%.Conclusions Two-dimensional speckle tracking imaging can sensitively assess left ventricular torsion and evaluate the left ventricular dysfunction in patients with CCP.
9.Cardiac Rehabilitation in Patients with Coronary Artery Disease.
Hanyang Medical Reviews 2006;26(2):75-79
Cardiovascular diseases including ischemic heart disease have been emerging as one of the most important public health problems in Korea. Although secondary prevention and rehabilitation are essential components in the management of patients with ischemic heart disease, its program has not been provided to all the patients. There are several barriers, such as availability and accessibility, low awareness of both patients and physicians in implementing cardiac rehabilitation program. Home-based and community-based programs under the direction of medical supervision are needed to further widen its availability and accessibility, as well as hospital-based programs. Cardiac rehabilitation programs emphasize risk factor control and life style change, besides exercise training. Health education, diet, counseling, psychosocial support, and exercise are all important in restoring the patient's quality of life. For this purpose, cardiac rehabilitation should be well integrated with preventive cardiology, and a multidisciplinary approach is mandatory. Since Korean society is very rapidly aging, a big impact on future trend of ischemic heart disease, preventive cardiology and cardiac rehabilitation will play a very important role to overcome the approaching wave of cardiovascular epidemic.
