2.A clinical analysis of Weixuening granule combined with prednisone for treatment of patients with primary immune thrombocytopenia

Objective To observe the clinical efficacy of Weixuening granule combined with prednisone for treatment of patients with primary immune thrombocytopenia (ITP) and its impact on platelet-associated antibodies(PAIgG, PAIgM, PAIgA). Methods Sixty patients with ITP were randomly divided into a observation group (30 cases, Weixuening granule combined with prednisone) and a control group (30 cases, prednisone therapy alone), 3 months constituting a therapeutic course. The clinical effects, platelet count (PLT) and PAIgG, PAIgM, PAIgA of two groups were observed after two courses of treatment. The time of withdrawing prednisone, relapse rate in 1 year and adverse reactions were also monitored. Results The total effective rate of the observation group was higher than that of the control group [90.00%(27/30) vs. 73.33%(22/30), P<0.05]. The time of withdrawing prednisone was significantly shorter in the observation group than that in the control group(days:39.8±7.8 vs. 55.3±6.5, P<0.05). The relapse rate in 1 year in the observation group was obviously lower than that of the control group [23.3%(7/30) vs. 50.0%(15/30), P < 0.05]. The PLT levels were markedly increased after treatment in the two groups, the levels of platelet-associated antibodies PAIgM and PAIgA after treatment in the two groups were lower than those before treatment, and the degree of improvement in observation group was superior to that of the control group [PLT (×109/L):113.25±15.47 vs. 84.79±20.37, PAIgM:(28.91±10.42)%vs. (30.75±9.37)%, PAIgA:(19.32±11.54)%vs. (21.15±15.16)%]. The level of PAIgG in the observation group was decreased obviously after treatment, while in the control group, it did not have significant change before and after treatment, and in the comparison of the level of PAIgG after treatment between the two groups, there was statistical significant difference [(15.29±9.02)%vs (32.41±9.34)%, P<0.05]. There were adverse reactions in the two groups, including centrality obesity, polytrichosis, acne, and hyperglycemia;the incidence of the above side effects in observation group was lower than that of the control group [73.3%(22/30) vs. 86.7%(26/30)], but the difference was not statistically significant (P>0.05). Conclusions The combined use of Weixuening granule and prednisone for treatment of patients with primary ITP is more effective than administration of prednisone alone;the adverse reactions are less in combined therapy group. Possibly, the better therapeutic effect of combined therapy is related to the decrease of platelet-associated antibodies and increase of the platelet generation.
3.A Cross-sectional Study for the Relationship Between Tea Drinking and Blood Lipids in Middle and Aged Population

Methods: A cross-sectional study was conducted based on the data cohort of China multicenter collaborative study of cardiovascular epidemiology in 2007-2008. A total of 7227 participants were enrolled including 3304 male and 3923 female at the mean age of (55.6±7.1) years. Tea drinking information was collected by questionnaire; participants were stratified by gender and grouped by regular tea drinking. Relationship between tea drinking and blood lipids, lipoprotein levels were assessed by covariance analysis. Results: There were 3012/7227 (41.7%) participants (male: 58.9% and female: 27.2%) regularly drunk tea. With adjusted age, urban and rural, education level, cigarette smoking, alcohol drinking, body mass index (BMI), daily red meat intake, physical work intensity, exercise intensity, histories of hypertension, diabetes and hypercholesterolemia, in male gender, compared with non-regular tea drinker, regular tea drinker had decreased blood level of low density lipoprotein cholesterol (LDL-C), the difference was -0.12 mmol/L, P=0.0001 and increased triglyceride (TG), the difference was 0.11 mmol/L, P=0.0001; in female gender, regular tea drinker showed increased high density lipoprotein cholesterol (HDL-C), the difference was 0.06 mmol/L, P<0.0001. Conclusion: In our research, regular tea drinking was negatively related to blood LDL-C level and positively related to TG in male gender, while it was positively related to HDL-C in female gender; the above correlations were independent from possible influencing factors. The impact of long term regular tea drinking on blood lipids and lipoprotein levels should be further prospectively investigated in community based middle and aged population.
4.Clinical effect of low-dose homoharringtonine and cytarbine with long-course in treating atypical chronic myeloid leukaemia

Objective To investigate the therapeutic effect of low-dose and long-course homoharringtonine and cytarbine (HA) in treating atypical chronic myeloid leukaemia (aCML).Methods Twenty-seven patients diagnosed atypical chronic myeloid leukaemia(aCML) from Oct.2003 to May 2014.in the Affiliated Hospital of Logistics University of People's Armed Police Force were divided into treatment and control groups.Fourteen cases in the treatment group were given the chemotherapy of HA (H:1.5 mg/(m2 · d),A:100 mg/(m2· d) for two weeks,and 13 cases in the control group were given the combined therapy of Hydroxycarbamide(1-3 g/d) and recombined human interferon α-2b(3×106U by intramuscular injection every other day) for four weeks.The haemoglobin (Hb) level,the numbers of white blood cell count (WBC) and platelet count(PLT) were measured.Moreover,the clinical effects were evaluated through measured hemogram and myelogram at 4 weeks after treatment.Results The levels of Hb,the numbers of WBC and PLT at before and 1 and 2 weeks in treatment group were as same as those in control group(P>0.05).While,the numbers of WBC and PLT at 2 weeks,as well as Hb and the numbers of WBC and PLT in 3 weeks,4 weeks in treatment group were significantly different from those in control group (P < 0.05).Furthermore,repeated measurement ANOVA showed that the levels of WBC,Hb,PLT between two groups were significant (F between groups =833.16,145.59,143.11;P<0.05),and there were significant differences at different time (F between groups =443.92,17.41,149.11;P < 0.05),meanwhile with interaction (F across group =69.77,43.26,75.25;P <0.05).In treatment group,there was 8 cases were complete remission,2 cases were partial remission and 4 cases were not released,and the total effective rate 71.4%.While in control group,there were no one was complete remission,2 cases were partial remission and 4 cases were not released,and the total effective rate was 15.4%.And the different was significant (x2 =11.25,P< 0.05).Conclusion The therapeutic effect of lowdose and long-course HA in treating atypical chronic myeloid leukaemia is more efficient and practical than traditional treatment.And its side effects are tolerable.
5.Clinical study on expression and methylation of SOX17 gene in chronic myeloid leukemia

Objective To investigate the expression level and promoter methylation of SOX17 gene and the clinical correlations in Chinese patients with chronic myeloid leukemia ( CML ) . Methods The levels of SOX17 expression and methylation were detected by RQ-PCR and RQ-MSP. Results SOX17 expression level was significantly lower in CML compared with 30 controls (P=0.001). The area under the receiver operating characteristic (ROC) curve (AUC) was 0.748 to differentiate CML from control (P=0.001). There was a trend of correlation between SOX17 expression and bcr/abl transcript (r = 0.439,P = 0.068) in CML patients. Hypermethylation of SOX17 promoter was detected in 3 (4%) CML patients, however, there was no difference as compared with 32 controls. In our study, SOX17 hypermethylation was not corrected with its expression. Conclusion Decreased SOX17 expression is a common molecular event in CML and may be considered as an available biomarker to diagnose CML. Dysregulated SOX17 is not caused by promoter hypermethylation in CML.

Objective:
To determine whether T1 mapping could monitor the dynamic changes of injury in myocardial infarction (MI) and be histologically validated.
Materials and Methods:
In 22 pigs, MI was induced by ligating the left anterior descending artery and they underwent serial cardiovascular magnetic resonance examinations with modified Look-Locker inversion T1 mapping and extracellular volume (ECV) computation in acute (within 24 hours, n = 22), subacute (7 days, n = 13), and chronic (3 months, n = 7) phases of MI. Masson’s trichrome staining was performed for histological ECV calculation. Myocardial native T1 and ECV were obtained by region of interest measurement in infarcted, peri-infarct, and remote myocardium.
Results:
Native T1 and ECV in peri-infarct myocardium differed from remote myocardium in acute (1181 ± 62 ms vs. 1113 ± 64 ms, p = 0.002; 24 ± 4% vs. 19 ± 4%, p = 0.031) and subacute phases (1264 ± 41 ms vs. 1171 ± 56 ms, p < 0.001; 27 ± 4% vs. 22 ± 2%, p = 0.009) but not in chronic phase (1157 ± 57 ms vs. 1120 ± 54 ms, p = 0.934; 23 ± 2% vs. 20 ± 1%, p = 0.109). From acute to chronic MI, infarcted native T1 peaked in subacute phase (1275 ± 63 ms vs. 1637 ± 123 ms vs. 1471 ± 98 ms, p < 0.001), while ECV progressively increased with time (35 ± 7% vs. 46 ± 6% vs. 52 ± 4%,p < 0.001). Native T1 correlated well with histological findings (R2 = 0.65 to 0.89, all p < 0.001) so did ECV (R2 = 0.73 to 0.94, all p < 0.001).
Conclusion
T1 mapping allows the quantitative assessment of injury in MI and the noninvasive monitoring of tissue injury evolution, which correlates well with histological findings.

9.Incidence of obesity and its modifiable risk factors in Chinese adults aged 35-74 years: a prospective cohort study.

OBJECTIVETo examine the incidence of obesity and its modifiable risk factors in Chinese adults aged 35-74 years.
METHODSA total of 27 020 participants aged 35 to 74 years from two prospective cohort studies in China were followed up in the years of 2007 and 2008. Obesity and overweight were defined as body mass index ≥ 28.0, and 24.0-27.9 kg/m(2), respectively. Relative risks of obesity for risk factors were computed by using logistic regression.
RESULTSThe annual incidence rates of obesity and overweight were 6.97 ‰ and 24.83 ‰ in Chinese adults aged 35-74 years, respectively. Women had a higher incidence of obesity than men (7.74 ‰ vs. 6.10 ‰). Participants in northern China had a higher incidence than those in southern (9.29 ‰ vs. 5.10 ‰) part of the country. Adults in rural had a higher incidence than those in urban (7.28 ‰ vs. 6.52 ‰). After adjusting for the baseline variables, such as gender, age, geographic region, degree of urbanization, the relative risk for obesity was 0.82 (95% CI:0.68-0.99) for participants with ≥ 12 years' education, compared with those <12 years. Participants with middle income, less physical activity at work/housework or being retirees, consuming more red meat and scented tea etc, had higher risk of incidence of obesity. Participants who consumed milk and moderate amount of fruits, would show a lower risk of obesity.
CONCLUSIONThe incidence of obesity was 6.97 ‰ in Chinese middle and older adults. Our results underscored that the promotion of healthy lifestyle which include issues as increasing physical activity, consuming moderate amount of fruits and milk but less red meat, drinking less scented tea etc, could play key roles in obesity prevention and control among the Chinese adults, especially among people with low education level or with middle income.
