1.The application effect of CT angiography features on qualitative and differential diagnosis of small nodules in the lung lesions

Objective To investigate the effect of CT angiography on the qualitative and differential diagnosis in small pulmonary nodules.Methods 120 patients with pulmonary small nodular lesions from September 2014 to October 2015 in our hospital were selected,and they were divided into the observation group and control group according to pathological type.the patients with malignant pathology were in the observation group,with totaling 64 cases;the patients with benign pathology were selected as a control group,with a total of 56 cases.Lesion type,sensitivity,vascular sign type,CT enhancement value and the ratio of enhancement value to aorta value in the two groups were compared and analyzed after the CT scan.Results In the observation group,positive CT angiography sign was 50 cases while negative ones was 14 cases,the sensitivity was 78.1%;In the control group,CT angiography sign was positive in 16 cases,negative in 40 cases,with the sensitivity of 28.6%.Comparing of two vascular symptoms sensitive case, there was a significant difference(χ2 =6.781,P =0.012).Vascular symptoms genotyping were compared in vascular symptoms positive patients,the results showed significant differences(χ2 =7.694,6.964,5.993,6.012,all P <0.05).Enhanced CT scan value of patients showed positive signs in the observation group was (40.07 ±3.72)Hu, with the control group of (10.54 ±4.83)Hu,the difference was statistically significant(t =11.783,P =0.019).The ratio of enhancement CT value to aorta value in patients with positive reaction showed 24.96% in the observation group,and 4.01% in the control group,the difference was statistically significant(χ2 =7.460,P =0.012).Conclusion
CT angiography features has played a very important role in qualitative and differential diagnosis of small nodular lesions in the lungs.It is of great significance in the clinical diagnosis and treatment,and is worth further promotion.
3.Effects of sevoflurane postconditioning on myocardial neutrophil infiltration in patients undergoing cardiac valve replacement under cardiopulmonary bypass

ObjectiveTo investigate the effects of sevoflurane postconditioning on myocardial neutrophil infiltration in patients undergoing cardiac valve replacement under cardiopulmonary bypass (CPB).Methods Twenty-four ASA Ⅱ or Ⅲ patients (NYHA Ⅱ or Ⅲ ) of both sexes aged 20-50 yr with BMI of 19-25 kg/m2 undergoing cardiac valve replacement under CPB were randomly divided into 2 groups ( n =12 each):group control (group C) and group sevoflurane postconditioning (group S).Anesthesia was induced with midazolam,fentanyl,etomidate and rocuronium and maintained with iv infusion of fentanyl,midazolam and vecuronium.The patients were intubated and mechanically ventilated (VT 8-10 ml/kg,RR 10-14 bpm,I∶E 1∶2,FiO2 100% ).PETCO2 was maintained at 35-40 mm Hg.In group S sevoflurane was inhaled immediately after aorta and vena cava were unclamped.The end-tidal sevoflurane concentration was maintained at 1.7% for 5 min.Arterial blood samples were collected before surgery and at 1,2 and 4 h after aortic unclamping for determination of plasma cardiac troponin T concentration.Myocardial specimens were obtained from right auricular appendage after opening of pericardium and at 1 h after aortic unclamping for microscopic examination and determination of myocardial myeloperoxidase expression.ResultsSevoflurane postconditioning significantly decreased plasma cardiac troponin T concentration and myocardial myeloperoxidase expression and ameliorated histo-pathological damage in group S as compared with group C.ConclusionMyocardial neutrophil infiltration is involved in the protective effect of sevoflurane postconditioning against myocardial injury in patients undergoing cardiac valve replacement under CPB.
4.Research in construction of core competence evaluation index system for nursing postgraduates

Objective To construct an objective,comprehensive core competence evaluation index system for nursing postgraduates,and to provide evidence for postgraduates nursing education.Methods According to literature review,expert interviews,focus group discussions,Delphi and other research methods,we conducted two rounds of 32 experts consultation in 18 cities from 10 provinces to establish the final core competence evaluation index system.Results The two experts consultation derived core competence evaluation index system including 7 level-1 indexes,20 level-2 indexes and 53 entries.Level-1 indexes:theoretical knowledge and skills,critical thinking ability,research ability,clinical care ability,teaching skills,interpersonal communication skills,professional development skills.Conclusions This program is rigorous and reliable,with some theoretical and practical significance of its results,which provide a reference for the training program,evaluation of the quality,assessment of postgraduate nursing education.
5.Detection of Sample Preparation about Sialy-LewisA/X on the Surface of Colon Carcinoma Cells by Flow Cytometry

Objective To investigate the effects of variable sample preparation about sialy LewisA/X on the surface of colon carcinoma cells.Methods Human colon carcinoma cell suspension was prepared by using EDTA,trypsin,cell scraper,trypsin-EDTA and trypsin-cell scraper.Cell counts and sialy-LewisA/X on the surface of colon carcinoma cells were detected by flow cytometry.Results Trypsin made fluorescence intensity of sialy-LewisA/X on the surface of colon carcinoma cells become weaken.Fluorescence intensity of sialy-LewisA/X had no significant deviation among EDTA,cell scraper and trypsin-cell scraper.Cell counts were multitude in EDTA group or trypsin-cell scraper group.Conclusion Trypsin-cell scraper is a available approach on preparing cell suspension of adherent cell.

Objective To evaluate the effects of different concentrations of dexmedetomidine on large-con-ductance Ca2+-activated K+ (BKca) channels in the rat mesenteric arterial smooth muscle (MASM) cells.Methods Sprague-Dawley rats of both sexes,weighing 180-220 g,were used in this study.Single MASM cell was freshly isolated from mesenteric arteries in two steps.Ten cells were chosen and studied.When holding potential was 40 mV and the concentration of free calcium ions was 10-7 mol/L,inside-out patch-clamp technique was used to record the single BKCa channel current before and after application of different concentrations of dexmedetomidine (0,10-9,10-8,10-7,10-6,10-5 mol/L).Total open probability (NPo),amplitude (Am),mean open time (To) and mean close time (To) of single BKca channel were observed and recorded.Results Compared with the baseline value,dexmedetomidine 10-7,10-6 and 10-5 mol/L increased NPo in a concentration-dependent man-ner,and dexmedetomidine 10-9,10-8,10-7,10-6,10-5mol/L shortened Tc (P <0.05 or 0.01).Compared with the value obtained when the concentration of dexmedetomidine was 10-9 mol/L,Tc was significantly shortened when the concentration of dexmedetomidine was 10-8 mol/L (P < 0.05),and no significant change was found in Am and To obtained when different concentrations of dexmedetomidine were applied (P > 0.05).Conclusion Dexmedetomidine 10-7,10-6 and 10-5 mol/L activate BKca channels in rat MASM cells in a concentration-depen-dent manner,which is one of the mechanisms of decrease in blood pressure by dexmedetomidine.
8.X-ray Diagnosis of Neonatal Respiratory Distress Syndrome(NRDS) (An Analysis of 20 Cases)

Objective To improve the knowledge and diagnostic ability of imagiology of NRDS.Methods The clinical data and X-ray features in 20 cases with NRDS who had complete materials were analysed.Results According to X-ray manifestations all cases were divided into four stages:Ⅰ stage(n=3),Ⅱ stage (n=5),Ⅲ stage (n=8) and Ⅳ stage (n=4).Of them,three cases were in company with aspiration pneumonia,four cases with infective pneumonia,five cases with air leak and two cases with intra-alveolar hemorrhage.Typical X-ray features included:the pulmonary lucency decreased,wide-ranging net and grain shadowes of high density,and in companing with a lot of air bronchus sign.Conclusion X-ray plain film,especially computed radiography is the most important imaging method in diagnosis of NRDS.
9.Diagnostic Value of CR Mammography Signs for Breast Cancer

Objective To explore the imaging features of breast cancer by CR mammography,in order to improve the diagnostic level. Methods The CR mamographic features of 37 patients with breast cancer confirmed surgically and pathologically were analysed retrospectively. Results In the 37 cases of breast cancer,the following abnormal signs were found in CR mammograms: mass(n=29),without mass(n=8),calcification(n=15),abnormal vascularity(n=7),inverted nipple(n=6),thickened skin(n=3),sign of tower's tine(n=3),sign of funnel(n=1),focal structural disorder and small focus of increased density(n=3). Conclusion The masses and calcificationsare the main radiological features in breast cancer,CR mammography may show the characteristics of breast cancer,it is one of the mosteffective means in diagnosis of breast cancer.
