1.Diagnosis and treatment of hepatic trauma: a report of 55 cases

Objective To summarize the experiences of the diagnostic and therapeutiec approach of hepatic trauma.Methods Referencing to the literature of recent years,we retrospectively analyzed the clinieal data of 55 cases of liver trauma.Results Among the 55 cases.18 eases of stage Ⅰ~Ⅱ.1 5 cases of stage Ⅲ,17 eases of stage Ⅳ and 5 cases of stage V,Thirty-one cases received non-operative treatment.and 24 cases reeeived operative therapy.In non-operative treatment group,the cure rate was 100%.In operation Ireatmenl group the cure rate was 91.7%.Conclusions Focused abdominal sonography for trauma (FAST) and diagnostic peritoneal paraeentesis are the initial methods for diagnosis of liver trauma and are largely used nowadays.When the patient is hemodynamically stable,CT scan or simuhaneous contrast-enhanced scan are important to decide for conservative ur surgical treatment.Today.treatment of hepatic lesions takes in account mainly two aspects:hemodynamically stability and the mechanism for trauma(blunt or penetrating).Liver injmy score of patienlS is not as important as the hemodynamie status for determining conservative management.Nonoperative management under close continuous observation for the hemodynamically stable patients with stage Ⅰ and Ⅱ and some eases of stage Ⅲ blunt hepatic injury has become the stamlard of care.The decision for surgical intervention should be given in time according to the variance of hemodynamically stability and the presence of trauma (blunt or penetrating).Some cases of stage Ⅲ and all cases of stage Ⅳ to Ⅴ hepatic injury shouht preferably undergo surgieal treatment.The effective preventive measures to decrease mortality in patients with severe hepatic trauma include early remedy to henmrhagie shock.effective hemostasis,complete abdominal drainage and prevention of pnstoperative enmplieations.
2.Improvement of family therapy on the efficacy and social performance of adolescent depression

Objective To investigate the efficacy of family therapy in the treatment of adolescent depression and improvement of social performance.Methods 86 adolescent depressive patients were divided into drug therapy group(group A,n =43) and drug combined with family therapy group (group B,n =43) according to the order of inclusion.They were all treated for 12 weeks.Before and after the treatment,they were all evaluated with Hamilton Depression Rating Scale(HAMD-24),Personal and Social Performance scale (PSP) and Youth Self-Report scale(YSR).The changes of scores were compared with each other.Results The average scores of HAMD,PSP and YSR in group B before and after treatment was (27.30 ± 4.60) (81.47 ± 1.87) (73.28 ± 1.30) and (15.36 ±3.60) (12.61 ± 2.10) (9.46 ±2.80) (6.30 ± 2.20),(82.76 ± 1.09) (84.51 ± 1.32) (86.51 ±1.32) (92.05 ± 1.72),(71.42 ± 1.45) (69.09 ± 1.33) (67.47 ± 1.47) (64.23 ± 1.90) respectively.The average scores of HAMD,PSP and YSR in group A before and after treatment was (26.90 ± 5.10) (81.45 ± 1.85)(73.26 ± 1.28)and (19.42 ±2.90)(16.43 ±2.60)(13.50 ±3.80)(7.60±2.80),(82.00 ±1.14) (83.67 ±1.23) (85.88 ± 1.27) (87.98 ± 1.35),(72.84 ± 1.29) (70.07 ± 1.26) (68.84 ± 1.54) (66.02 ± 1.55) respectively.The difference of the scores after two-week treatment was significant statistically (P< 0.05).The difference of the deduction rate of the scores after four-week treatment was significant statistically (P < 0.05).Conclusion Family therapy has better and faster efficacy in the treatment of adolescent depression,and it can significantly improve their social performance.
3.Therapy of AF system inside and outside after joint vertebral bone fusion treatment on thoracolum-bar burst fracture

Objective To explore the clinical curative effect of AF system combined vertebra re-duction and posterolateral bone fusion treatment thoracolumbar burst fracture .Methods Seventeen pa-tients with thoracolumbar burst fracture using period stage , according to the system of AF clinical symptoms and image data line pressure decreased , and the pressure or not to hurt inside the vertebral pedicle autoge-nous bone grafts are grain filling and vertebral posterolateral line between the axon , lamina around small joints and bone , and postoperative follow-up X-ray inspection regularly , observe internal fixation and verte-bral vertebral height and adjacent intervertebral space changes , or after fixation and reset lost .Results following up 13 months, vertebral body height restore and maintain good , bolt looseness and broken and ver-tebral body height complications such as lost were no found in 17 cases.Conclusion Treatments of Poste-rior fixation combined short stage AF vertebral body and posterolateral bone fusion on thoracolumbar burst fracture are not only rebuild the vertebral height , but also make the vertebral obtained after three pillars in stable, reduce the internal fixation and compressed vertebral again such complications .

This study is based on the requirements of the construction of applied undergraduate colleges and development of excellent doctor education training pilot project of the ministry of education. The situation of higher medical education and the requirement of health career development are analyzed. The medical talents training mode of three oriented which is taking the post competency of medical students as the frame is constructed. This paper expounds the background proposed by training mode from the internal drive, external drive and project boosting. With the national education policy as the basis, the theoretical basis of the international advanced education teaching idea, theory and standard is explained. The connotation of the training mode is revealed by the humanistic quality education, professional quality training and physical and psychological quality cultivating. The evaluation strategy of clinical medical per-sonnel training standards, index system, the implementation approach and evaluation methods which have certain feasibility and timeliness is established.
5.Advances of FOXM1 and prospects in the research of tumor

FOXM1 (Forkhead box M1), one factor of the Forkhead family, has three subtypes (FOXM1a, FOXM1b, FOXM1c). The current study focuses on FOXM1b and FOXM1c. FOXM1 regulates transcription of prolifemtion-nssociated genes and plays a vital role in embryogenesis as well as reorgani-zation. Recent studies have shown that FOXM1 is closely related with tumor occurrence and development. Tumors with high expression of FOXM1 are often poorly differentiated, highly malignant, distantly metastat-ic, poorly predicted. FOXM1 has a influence on the tumor proliferation, invasion, metastasis and angiogene-sis through regulating its downstream tumor-related genes. At present, the synthesis and study of the anti-tumor chemicals targeting FOXM1 offer a possibility of the FOXM1 in clinical applications. More and more researchers attach importance to its tumor therapeutic value both at home and aboard. This paper will make a review about the lastest FOXM1 research in oncology.
6.The comparative study of the influence of comprehensive psychological intervention and cognitive therapy on cognitive function in schizophrenia

Objective To investigate the influence of comprehensive psychological intervention and cognitive therapy on cognitive function in schizophrenia.Methods A total of 80 patients with schizophrenia were randomly divided into the comprehensive psychological intervention group (group A,n=40)and the cognitive therapy group (group B,n=40).Before and after 8-week treatment,the influence on cognitive function were measured with WAIS-RC,WMS,WCST and TESS were compared between two groups.Results After eight weeks,the intelligence quotient,remembrance quotient ,sustained errors and random errors in group A (110.52±18.03,110.78±1.15.42,15.48±9.82,17.46±13.28) were higher than that in group B (96.64±18.26,102.23±13.4,20.35±11.47,24.92±13.71).The difference between the two groups was found in improving cognitive function in schizophrenia patients(P<0.05).Conclusion Cognitive therapy could improve cognitive function in schizophrenia,but comprehensive psychological intervention would do much better.
7.Supercritical CO_2 extraction of safflower essential oil

AIM:To obtain the fine safflower essential oil by supercritical CO_2 extraction(SFE-CO_2). METHODS: The effects of pressure, temperature and CO_2 flow rate on supercritical CO_2 extraction were studied and the safflower essential oil was analysed by GC-MS. Both yield and components of essential oil obtained by SFE-CO_2 were compared with that obtained from hydrodistillation and microwave-assisted extraction. RESULTS: The optimal conditions were determined: the pressure of 9 MPa, the temperature was at 40 ℃ and the CO_2 flow rate was 4 kg/h. The yield of essential oil by SFE-CO_2 was higher than that of hydrodistillation, the qualities of both kinds of essential oil were fine, as analysed by GC-MS. The yield of essential oil by microwave extraction was the highest, but its quality was worse than those obtained by using other methods. CONCLUSION: The SFE-CO_2 is a good method for extraction of safflower essential oils.
8.Comparison of curative effects from arthroscopic modified suture bridge and single-row modified Mason-Allen suture in repair of supraspinatus tendon tears

Objective To compare the early curative effects of arthroscopic modified suture bridge and single-row modified Mason-Allen suture in repair of supraspinatus tendon tears.Methods A retrospective case-control analysis was made on 28 patients with supraspinatus tendon tears admitted between June 2012 and June 2015.There were 16 males and 12 females,aged 43-63 years (mean,54.2 years).Thirteen patients (13 shoulders) were repaired using the arthroscopic modified suture bridge technique (Group A),and 15 patients (15 shoulders) were treated using the single-row modified Mason-Allen technique (Group B).Operation time and intraoperative blood loss were recorded.American shoulder and elbow surgeons (ASES) score,Constant score and visual analogue score (VAS) were used to evaluate the function and subjective outcomes preoperatively.Meanwhile,MRI was used for analysis of tendon integrity postoperatively.Results The operation time of Group A and B were (56.1 ± 23.2) minutes and (36.1 ± 15.6) minutes,respectively (P < 0.05).The intraoperative blood loss was (30.3 ± 20.5) ml and (28.5 ± 18.2) ml,respectively (P > 0.05).The average follow-up time for Groups A and B were 12 months and 12.6 months,respectively.The symptoms of 28 cases were alleviated after surgery,and the functions were obviously recovered.In Group A,thc VAS was decreased significantly from (7.0 ± 0.8) points preoperatively to (0.8 ± 0.8) points at final follow-up,ASES score was improved from (39.8 ± 3.1) points to (88.1 ± 4.8) points,and Constant score was improved from (54.8 ± 2.7) points to (88.2 ± 3.1) points (all P < 0.05).In Group B,the VAS was decreased significantly from (6.8 ± 0.8) points preoperatively to (0.9 ± 0.8) points at final follow-up,ASES score was improved from (40.7 ± 2.5) points to (89.5 ± 3.2) points,and Constant score was improved from (56.0 ± 4.5) points to (89.3 ± 3.4) points (all P < 0.05).There was no significant difference in the clinical outcomes between the two groups (P > 0.05).The retear rate in Group B was 20% (3/15),while no retear was presented in Group A (P < 0.05).Conclusion Arthroscopic modified suture bridge technique and single-row modified Mason-Allen technique are both clinically effective for function recovery and pain relief in patients with supraspinatus tendon tears,but the former associated with lower incidence of tendon re-tear is preferred for moderate to large rotator cuff injury or rotator cuff injury with large insertion avulsion.

10.Effects of recombinant human vascular endothelial growth factor on proliferation and differentiation of rat osteoblasts

Objective:To study the effects of recombinant human vascular endothelial growth factor on proliferation and differentiation of rat osteoblasts. Methods: Cell culturing, light microscop, MTT assay and PNPP assay were performed to observe the effects of rhVEGF on proliferation and differentiation of the cells. Results: rhVEGF could stimulate osteoblasts proliferation and differentiation. The maximal effect on proliferation was observed at a concentration of 3.125 ng/ml on the third day. ALP activities in osteoblasts were increased most significantly at a concentration of 12.5 ng/ml. Conclusion: rhVEGF may not only induce angiogenesis to facilitate bone formation, but also stimulate this process in a direct way by inducing proliferation and differentiation of osteoblasts.
