1.The comparative study of the influence of comprehensive psychological intervention and cognitive therapy on cognitive function in schizophrenia

Objective To investigate the influence of comprehensive psychological intervention and cognitive therapy on cognitive function in schizophrenia.Methods A total of 80 patients with schizophrenia were randomly divided into the comprehensive psychological intervention group (group A,n=40)and the cognitive therapy group (group B,n=40).Before and after 8-week treatment,the influence on cognitive function were measured with WAIS-RC,WMS,WCST and TESS were compared between two groups.Results After eight weeks,the intelligence quotient,remembrance quotient ,sustained errors and random errors in group A (110.52±18.03,110.78±1.15.42,15.48±9.82,17.46±13.28) were higher than that in group B (96.64±18.26,102.23±13.4,20.35±11.47,24.92±13.71).The difference between the two groups was found in improving cognitive function in schizophrenia patients(P<0.05).Conclusion Cognitive therapy could improve cognitive function in schizophrenia,but comprehensive psychological intervention would do much better.
2.One new galloyl glycoside from fresh leaves of Psidium guajava L.

To investigate the chemical constituents of Psidium Guajava L, the EtOH/H2O extract of the fresh leaves was subjected to various chromatography. Five constituents with galloyl moiety were isolated and elucidated as 1-O-(1, 2-propanediol)-6-O-galloyl-beta-D-glucopyranoside (1), gallic acid (2), ellagic acid (3), ellagic acid-4-O-beta-D-glucopyranoside (4) and quercetin-3-O-(6"-galloyl) beta-D-galactopyranoside (5) by spectroscopic methods, including 2D NMR and HR-ESI-MS spectrometry as well as by comparison with published data. Compounds 4 and 5 were obtained from P. guajava for the first time, and compound 1 is a new polyhydroxyl compound.
3.Application of multislice CT scan combined with 99mTC -MIBI -SPECT in the diagnosis of isolation pulmonary nodule lesions

Objective To discuss the applied value of multislice CT scan in combination with 99 MTC -MI-BI -SPECT for the diagnosis of isolated pulmonary nodule lesions.Methods 60 patients with solitary pulmonary nodules shadow in the X -ray film from July 2014 to August 2015 in our hospital were selected and detected by spiral CT scanning and 99mTC -MIBI -SPECT imaging.Scan results and pathological examination or surgery were com-pared,specificity,sensitivity and accuracy of pulmonary nodules were concluded about the method in the differential diagnosis isolation.Results The specificity,sensitivity and accuracy of multislice spiral CT scanning alone were 73.6%,78.2%,79.3% respectively;The specificity,sensitivity and accuracy of 99mTC -MIBI -SPECT imaging alone,were 82.6%,78.2%,79.3% respectively;The specificity,sensitivity and accuracy of multislice CT scan com-bined with 99mTC -MIBI -SPECT imaging were 93.3%,93.2%,93.8% respectively.The accuracy of combined detection was significantly higher than that of 99mTC -MIBI -SPECT examination(χ2 =6.783,P =0.010)or CT scan (χ2 =8.012,P =0.008).Conclusion Multislice CT scanning combined with 99mTC -MIBI -SPECT imaging has better effect in diagnosing pulmonary nodule lesions,which is superior to the method alone.The combination meth-od can obviously improve the detection rate of lung nodule lesions,which is,worthy of further promotion.
4.A research on measuring health status for residents in rural areas

Objective To study the possibility of measuring health status for residents in rural areas by the Chinese SF-36 scale. Methods Health status for 207 residents in rural areas of Fuyang city who were 14 years old and over was measured by the Chinese SF-36 scale. The reliability and validity were analyzed. Results The internal consistency reliability coefficients (Cronbach'α) exceeded 0.50 for all scales except SF and RE scales. Inter-scale correlation coefficients were lower than Cronbach'α coefficients for all scales except SF scale. Factor analysis identified three principal components, which could be used to explain 64.6% of the total variance. The differences of scale scores between the young group and the middle age group weren't significant. There weren't significant differences of scale scores between sexes except RP scale. The differences of scale scores between smokers and no-smokers, drinkers and no-drinkers weren't significant. Nor were the differences between various educational levels. Conclusion Whether the Chinese SF-36 scale is suitable for measurement of health status for residents in rural areas needs further studies to give more evidences.
5.HIGH-LEVEL EXPRESSION OF RECOMBINANT TRICHOSANTHIN IN ESCHERICHIA COLI AND EVALUATION OF ITS IMMUNOGENICITY

Objective The TCS gene without N-and C-terminal sequences was cloned into pET-29b in order to get the recombinant trichosanthin(rTCS) at the relatively high level in E.coli,and the rTCS immunogenicity was compared with natural TCS(nTCS). Methods TCS gene without the N-and C-terminal coding sequences was amplified by RT-PCR,and then inserted into pET-29b.The soluble rTCS was induced with IPTG and purified through metal chelate affinity chromatography,the purity and content were showed by the thin-layer scan analysis and bradford assay.After the rabbits were immunized with purified rTCS and nTCS,respectively,the titer and specificity of polyclonal antibody were detected by indirect ELISA assay and Western blotting. Results 1.The vector pET-29b-TCS was constructed successfully after it was confirmed with the same restriction enzymes.2.The rTCS attained the maximum in the presence of 1 mmol/L IPTG for 4?h at 30℃.3.The soluble rTCS was expressed at relatively high level(36% of total protein in E.coli) and the yield of rTCS with a purity of 95% was 1.2?g in 1 L bacterium.4.The polyclonal antisera titer of nTCS was significantly higher than that of rTCS(1∶40?960 and 1∶5?120 respectively).5.The specific reaction between rTCS and anti-nTCS antibody was detected by Western blotting.Conclusion The high level expressing vector pET-29b-TCS,carrying TCS gene withoug the N-and C-terminal coding sequences,was constructed successfully.The immunogenicity of rTCS was remarkably lower than that of nTCS,which indicates that glycosylation of nTCS has a relationship with the antigenicity.
6.Retrospective analysis of plasma exchange for chronic severe hepatitis

Objective To retrospectively analyse the efficacy and the influencing factors of plasma exchange(PE)in patients with chronic severe hepatitis.Methods Sixty patients of chronic severe hepatitis were included.Results The effective rate was 61.7%.The efficacy of PE was closely related to age,clinical stage,pre-treatment serum bilirubin values,prothrombin activity and incidence of complications(P0.05).Conclusion PE can significantly improve the survival of patients with chronic severe hepatitis.Especially for the patients under 60 years old,those with low serum TBIL level and high PTA,and those in the early clinical stage,PE treatment would achieve a better efficacy.But the improvement of prognosis of severe hepatitis still depends on the early specific treatment and active prevention of complications.

8.Optimization of the Formulation of Film Coating of Metformin Hydrochloride Sustained-release Pellets in Compound Glipizde Extended-release Capsules

OBJECTIVE: To optimize the formulation of metformin hydrochloride sustained-release pellets prepared using film coating technology. METHODS: Taking release rate in vitro as index, the formulation of the film-coat was prelimilarily selected using one facotr sreening method and optimzed using orthogonal test. The release rate of the prepared metformin hydrochloride sustained-release pellets was determined and compared with that metformin hydrochloride pellets. RESULTS: The optimized formulation was as follows: the film material of Eudragit NE 30D and Eudragit L 30D55 at a ratio of 10∶1 were added with 40% talcum powder and 0.25% sodium dodecyl sulfate. The release rate of the metformin hydrochloride sustained-release pellets was higher than that of metformin hydrochloride pellets. CONCLUSION: The metformin hydrochloride sustained-release pellets were up to the requirements of sustained release and characterized by 24h continuous drug release.
9.An ultrastructural observation of corneal collagen matrix of rabbit in the different time after death

Objective To study the relationship between the ultrastructural changes of corneal matrix collagen fibrils diameter and the postmortem interval.Methods Twenty-eight rabbits were killed by the method of air embolism and the samples of cornea were separately cut down in 0,6,12,18,24,48 and 72h after death.The ultrastructure of the collagen diameter was observed by transmission electro microscopy(TEM)and the 4 parameters of area(Y1),perimeter(Y2),average-diameter(Y3)and mean-diameter(Y4)were measured by a computer image technique and analyzed by Excel 2000 and SPSS 10.0 software.Results The 4 parameters of Y1,Y2,Y3 and Y4 were respectively increased from 1 131?53nm to 1 628?26nm,from 132.8?23nm to167.5nm,from 38nm to 45nm and from 37.71?6nm to 44.89?5nm within 72 hours after the death.Conclusion The corneal matrix collagen diameter increase regularly with the postmortem interval in 72h after death,and the parameters of Y1,Y2,Y3 and Y4 could be used as a new reference mark for timing the postmortem interval after death.
10.Risk factors for progression of advanced chronic kidney disease——A monocentric cohort study from a specific CKD outpatient management clinic

Objective To identify the risk factors for progression of advanced chronic kidney disease(CKD) patients who were cared by nephrologists in a specific CKD outpatient management clinic.Methods A prospective monocentric cohort study was performed.CKD patients of stage 3, 4 and 5 without renal replacement treatment were followed up regularly by nephrologists in this specific CKD management clinic.Patients with established atherosclerotic renal artery stenosis(ARAS) and chronic tubulointerstitial nephritis, and those who had not been followed-up for at least 12 months before Jun.30, 2010 were excluded.Clinical and laboratory data including blood pressure (BP), proteinuria, hemoglobulin (Hb), calcium phosphate product (Ca×P) and serum creatinine were consecutively collected.The treatment regimen was also recorded.Estimated glomerular filtration rate(eGFR) was calculated with the formula modified for Chinese to evaluate the change of renal function.The progression of kidney disease was defined as initiation of renal replacement therapy, the annual decrease of eGFR>4 ml·min-1·(1.73 m2)-1, and/or death associated with renal disease.Results A total of 138 patients were enrolled in the final analysis with 84 patients of CKD stage 3, 36 of CKD stage 4 and 18 of CKD stage 5, respectively.At the time of enrollment, patients had an average age of (56.5:±:16.7) years old with an average eGFR of (32.3±13.4) ml·min-1·(1.73 m2)-1.During a mean follow-up interval of (27.1±12.1) months, the patients were well-controlled with an average blood pressure of (126.5±12.4)/(76.4±7.9) mm Hg in 50.7%(70/138), less than or equal to 130/80 mm Hg, an average Hb of(123.8±17.1) g/L in 73.9%(102/138), above or equal to 110 g/L and an average Ca×P of (45.2±7.7) mg2/dl2 in 89.1%(123/138), less than or equal to 55 mg2/dl2.Sixty-two patients (44.9%) had progression of kidney disease. On univariate analysis, factors predicting progression were low eGFR at referral, high systolic pressure, low Hb level, high Ca×P and proteinuria during follow-up, and renin-angiotensin system inhibitors treatment did not affect the progression.After the adjustment, multivariate analysis revealed proteinuria and low Hb level were independent factors for the progression of kidney disease.Conclusions The co-morbidities of advanced CKD patients can be managed efficiently in specific CKD outpatient management clinic.Control of proteinuria and correction of anemia may be beneficial to prevent the progression of advanced CKD.
