1.Relationship between REG Iα gene expression and the clinicopathologic parameters in the patients with primary gastric carcinoma

Objective To investigate the relationship between expression of REG Iα gene and its clinicopathologic parameters, the survival rate in the patients with primary gastric cancer. Methods Using RT-PCR after extracting RNA from paraffin-embedded materials and immunohistochemical techniques, REG Iα gene expression was investigated in 235 samples. And the relation among their results with the clinicopathologic parameters of primary gastric carcinoma was discussed in experiment by SPSS 13.0 statistic software. Results The positive REG Iα mRNA was 78% (183/235) of primary gastric carcinoma and the positive rate of REG Iα protein was 31.1% (73/235) in 235 patients with primary gastric carcinoma. REG Iα gene expression in infiltrating tumors was found to be significantly higher compared with localized tumors ( P <0.05). REG Iα gene was closely linked to the infiltrative growth pattern, signet ring cell and poorly differentitated adencarcinoma. The incidence of venous invasion of REG Iα gene-positive differentiated adenocarcinoma was significantly higher than that of REG Iα gene-negative tumors. Moreover, the patients with REG Iα gene-positive differentiated adenocarcinoma were found to have a significantly poorer prognosis as compared with REG Iα gene-negative tumor patients. Conclusions The results of the experiment demonstrated that the expression of the REG Iα gene is closely related to the infiltrating property of primary gastric carcinoma. Signet ring cells and poorly differentiated adencarcinoma significantly enhance the risk of venous invasion of REG Iα gene-positive differentiated adenocarcinoma and might be as a prognostic indicator of differentiated adenocarcinoma of the stomach in clinic diagnosis.

Objective To explore the clinical curative effect of AF system combined vertebra re-duction and posterolateral bone fusion treatment thoracolumbar burst fracture .Methods Seventeen pa-tients with thoracolumbar burst fracture using period stage , according to the system of AF clinical symptoms and image data line pressure decreased , and the pressure or not to hurt inside the vertebral pedicle autoge-nous bone grafts are grain filling and vertebral posterolateral line between the axon , lamina around small joints and bone , and postoperative follow-up X-ray inspection regularly , observe internal fixation and verte-bral vertebral height and adjacent intervertebral space changes , or after fixation and reset lost .Results following up 13 months, vertebral body height restore and maintain good , bolt looseness and broken and ver-tebral body height complications such as lost were no found in 17 cases.Conclusion Treatments of Poste-rior fixation combined short stage AF vertebral body and posterolateral bone fusion on thoracolumbar burst fracture are not only rebuild the vertebral height , but also make the vertebral obtained after three pillars in stable, reduce the internal fixation and compressed vertebral again such complications .
3.Synergistic Immune Actions of Polysaccharides Compound

Objective To study the component and dose of polysaccharides compound and its inhibitive effect on the growth of S180 tumor.Methods Proliferative activity of splenolymphocytes was observed to study the component and dose of polysaccharides compound with MTT method. Mice bearing in-vivo transplanted tumor were used to evaluate the antitumor effect of polysaccharides compound.Results Polysaccharides compound, composed of lentinan100 mg/kg, agruecus blazei murill 200mg/kg and pachman 50 mg/kg,improved the activity of splenolymphocytes. Polysaccharides compound had an inhibitory effect on the growth of S180 tumor.Conclusion Polysaccharides compound with appropriate component in proper dose has synergistic immune actions and can inhibit the growth of S180 tumor.

5.Optimization of the Formulation of Film Coating of Metformin Hydrochloride Sustained-release Pellets in Compound Glipizde Extended-release Capsules

OBJECTIVE: To optimize the formulation of metformin hydrochloride sustained-release pellets prepared using film coating technology. METHODS: Taking release rate in vitro as index, the formulation of the film-coat was prelimilarily selected using one facotr sreening method and optimzed using orthogonal test. The release rate of the prepared metformin hydrochloride sustained-release pellets was determined and compared with that metformin hydrochloride pellets. RESULTS: The optimized formulation was as follows: the film material of Eudragit NE 30D and Eudragit L 30D55 at a ratio of 10∶1 were added with 40% talcum powder and 0.25% sodium dodecyl sulfate. The release rate of the metformin hydrochloride sustained-release pellets was higher than that of metformin hydrochloride pellets. CONCLUSION: The metformin hydrochloride sustained-release pellets were up to the requirements of sustained release and characterized by 24h continuous drug release.

This study is based on the requirements of the construction of applied undergraduate colleges and development of excellent doctor education training pilot project of the ministry of education. The situation of higher medical education and the requirement of health career development are analyzed. The medical talents training mode of three oriented which is taking the post competency of medical students as the frame is constructed. This paper expounds the background proposed by training mode from the internal drive, external drive and project boosting. With the national education policy as the basis, the theoretical basis of the international advanced education teaching idea, theory and standard is explained. The connotation of the training mode is revealed by the humanistic quality education, professional quality training and physical and psychological quality cultivating. The evaluation strategy of clinical medical per-sonnel training standards, index system, the implementation approach and evaluation methods which have certain feasibility and timeliness is established.
7.Immunity-enhancing effect of polysaccharide ABP-AW1 isolated from Agaricus blazei Murill on immunosuppressive mice

OBJECTIVE To explore the immunity-enhancing effect of ABP-AW1, a low-molecular-mass polysaccharides isolated from Agaricus blazei Murill, on immunosuppressive mice. METHODS ICR mice were ip injected cyclophosphamide 80 mg · kg-1, once daily for 3 d, to establish an immuno?suppressive mouse model. Then, ABP-AW1125, 250 and 500 mg · kg-1 were ig given to the immuno?suppressive mice,respectively, once daily for 7 d. The mouse thymus index and spleen index were calcu?lated, and the phagocytic function of phagocytes was determined using carbon clearance test. Splenic lym?phocyte proliferation was measured by MTT method. The interleukin 2 (IL-2) and interferon γ (IFN-γ) production from splenic lymphocytes was examined by ELISA. The splenic lymphocyte CD4+/CD8+ratio was determined by flow cytometric analysis. RESULTS Compared with normal control group, the thymus index, spleen index and phagocytic index of phagocytes in the immunosuppressive model mice declined (P<0.05). ABP-AW1250 and 500 mg·kg-1 treatment significantly increased the thymus index, spleen index and phagocytic index in immunosuppressive mice (P<0.05). Compared with normal control group, concanavalin A (Con A) and lipopolysaccharide (LPS) induced T and B lymphocyte proliferation, respectively, and IL-2 and IFN-γproduction from splenic lymphocytes in the immunosuppressive model mice was lower (P<0.05). Compared with model group, ABP-AW1250 and 500 mg·kg-1 promoted Con A and LPS induced T and B lymphocyte proliferation (P<0.05) , and elevated IL-2 and IFN-γ production from splenic lymphocytes (P<0.05). In addition, ABP-AW1250 and 500 mg·kg-1 reversed the decreased splenocyte CD4+/CD8+ratio in immunosuppressive model mice (P<0.05). CONCLUSION ABP-AW1 has immuneity-enhancing effect on cyclophosphamide-induced immunosuppressive mice.
8.Plasma adenomedullin, calcitonin gene-related peptide and c-type natriuretic peptide in rats with hypoxic pulmonary hypertension

Objective To explore the effects of hypoxia on the syntheses and secretion of adrenomedullin (AM), calcitonin gene related peptide (CGRP) and c-type natriuretic peptide (CNP) and the relationship between these peptides. Methods Rat models were established with hypoxia for 10, 20 and 30 d respectively and rats under normal altitude were served as control. Pulmonary artery pressure and the maximum increasing speed of right ventricle (RVdp/dtmax) were measured in every group. The dynamic changes of AM, CGRP and CNP concentrations in plasma were studied with radioimmunoassay. Results During hypoxia, pulmonary artery pressure and RVdp/dtmax were enhanced. Plasma AM and CNP concentrations were increased while CGRP was decreased significantly. The plasma level of AM had positive correlation with that of CNP, but negatively correlated with that of CGRP. Conclusion Results indicate that hypoxia may cause pulmonary artery pressure change and right ventricle has compensatory reaction to hypoxic pulmonary hypertension. Dynamic changes of plasma AM, CGRP and CNP concentrations can be regarded as indexes for condition of illness.
9.Preparation of metformin hydrochloride pellets with centrifugal granulation technique

Objective To prepare metformin hydrochloride pellets with centrifugal granulator by powder layering technique. Methods Metformin hydrochloride and MCC pellets were prepared by means of powder layering with the centrifugal granulation equipment and evaluated by yield percentage,angle of repose,bulk density,and friability. Results The preparation process parameters were optimized as follows: the ratio of metformin hydrochloride and lactose and MCC was 100∶5∶3,150 r/min rotating speed of the centrifugal granulation,20 r/min spraying serosity speed,25 r/min purveying powder speed,and the time of spheronization was 6 min.Metformin hydrochlorid pellets were prepared under the condition. Conclution The metformin hydrochloride pellets had good appearance,uniform granulation diameter,standard rigidity,good fluidity and did not have the structure of cage by means of detecting dissolubility.
10.Blood pressure rhythm and its association with clinicopathological indices in patients with IgA nephropathy

Objective To investigate the blood pressure circadian rhythm in patients with IgA nephropathy by ambulatory blood pressure monitoring and explore its role in the disease progression. Methods A cross sectional study was carried out.Blood pressure rhythm was studied by ambulatory 24-hour monitoring with a portable oscillometric recorder in selected patients with primary IgA nephropathy.The term dipper was described as blood pressure during night dropped at least 10% below daytime blood pressure.The term non-dipper referred to those in whom the nocturnal decline in blood pressure was less than 10%.Clinicopathological indices between dipper and non-dipper groups were compared. Results Ninety-three patients completed ambulatory blood pressure monitoring among whom 68 (73%) patients were non-dipper.The frequency of non-dipper was 70%,70% and 81% in the patients at chronic kidney disease stage 1,2 and 3 or more.The frequency did not differ among these three group patients (P=-0.587).77% of patients with hypertension and 69% of patients with normotension were non-dipper (P=0.373).The disappearance of blood pressure circadian rhythm in IgA nephropathy was not influenced by age,gender,blood pressure,proteinuria,renal function and renal pathology lesions.Among the patients who were followed up regularly for more than 12 months (n=54),patients in the dipper group had a trend of slower eGFR decline rate than those in non-dipper group albeit the difference was not significant (P=0.329).Subgroup analysis revealed that in patients with hypertension and non-dipper (n=29),the eGFR decline rate was much faster than that in dipper group[(-6.79±11.58 )vs (-0.34±1.74) ml ·min-1 ·(1.73 m2)-1·year-1,P=0.019]. Conclusions Most patients with IgA nephropathy present disappearance of blood pressure circadian rhythm,even among those at an early stage or without hypertension.The loss of blood pressure rhythm may be associated with a rapid renal function decline rate in those with hypertension.
