1.ULTRASTRUCTURAL STUDIES ON THE SPERMATOGENESIS IN DIPLOID AND TRIPLOID TYPES OF PARAGONIMUS WESTERMANI

Diploid and triploid types of Paragonimus westermani were compared in the aspect of ultrastructure of spermatogenesis. The results showed that the process of spermatoge nesis in the diploid type was normal. The cells at every stage are well developed. A great number of morphologically and functionally normal spermatozoa could be formed. On the other hand, the process of spermatogenesis in the triploid type was abnormal, the cells at every stage developed abnormally. They were apparently degenerated or va-cuolated, lacking organelles. The cytophore was broken into small particles. No normal spermatozoa were found (Figs. 1-5).

Diploid and triploid types of Paragonimus westermani had similar cbromosomal karyotypes, however, they differed in chromosome c-band patterns. Two of three homologues of the triploid resembled those of the diploid in c-banding pattern, while the remaining chromosomes showed a different pattern. Heterogcneity in homologues was observed especially in chromosome Nos. 2, 3, 5, 6 and 7 (Fig. l, 2). In the diploid chromosome specimen, mitotic and meiosic phases were observed, while only the mitotic; phase was found in the triploid chromosome.
3.An investigation in the draw-down area of Three-Gorge reservior: Food consumption and dietary nutrition in the urban and rural population and the nutritional status in children

BACKGROUND: At present, there are fewer studies on the monitoring of nutritional status of the residents living in the draw-down area of ThreeGorge reservoir.OBJECTIVE: To investigate the nutritional status and dietary pattern of the residents before the formation of the draw-down area of Three-Gorge reservoir, so as to provide basic data for the continuous dynamic surveillance of food and nutrition.DESIGN: A cross-sectional study.SETTING: Department of Nutrition and Food Hygiene, School of Public Health, Chongqing University of Medical Sciences.PARTICIPANTS: The investigation was carried out between February and March 2004 in the draw-down area of Three-Gorge reservoir with denser population, larger area and greater influence, such as Kaixian and Yunyang counties, etc. The first surveillance of food nutritional status of the residents in the draw-down area was performed in 52 urban families and 50 rural families of Kaixian county, one family member was voluntarily selected from each family, whose main duty was to buy vegetables and food. Totally 102 subjects (42 males and 60 females) were investigated in this study. At the same time, the data of physical examination and detection of blood Hb in children from Kaixian county between May 1999 and November 2003 were analyzed.METHODS: The subjects were investigated in their house with questionnaires. Their general condition of family, food consumption and dietary nutrition were surveyed and analyzed. Nutrients intake was assessed by 24-hour recall, combined with food consuming frequency questionnaire, the Engel index, which was an important index for the level of family consumption, was detected in both rural and urban residents. Engel index=cost in food/total family income×100%.MAIN OUTCOME MEASURES: ① Amount of food consumption, intakes of energy and nutrients in residents of the draw-down area; ② Prevalence rate of low body mass and rate of growth retardation in children of the draw-down area.RESULTS: All the 102 residents of the draw-down area entered the analysis of results without deletion. ① Amount of food consumption in residents of the draw-down area: Among the urban residents of the draw-down area, the per capita food consumption was 243 g per day, and the consumptions of vegetable, fruit and fish were lower than the average national level, but that of grease food showed a higher level. Among rural residents of Kaixian county, the per capita food consumption was 268 g per day, and the consumption of foodstuff was obviously decreased, but those of meet and grease food showed an ascending trend. ② Energy intake in residents of the draw-down area: Among urban residents of Kaixian county, the per capita energy intake was 6 909 kJ per day, which was obviously lower than that in the urban residents of Chongqing city in 1998 and the average national urban level. The per capita energy intake in rural residents of Kaixian county was 7 577 kJ per day, which was 48.4% lower than that in the urban residents of Chongqing city in 1998 and 37% lower than the average national rural level. ③ Nutritional status in children of the draw-down area: The incidence rate of low body mass in children younger than 5 years in 1999 was close to the average national level (9.6%), it was obviously decreased from 2000, and then fluctuated between 1.0% and 4.8%. Since 1999, the incidence rate of growth retardation in children younger than 5 years of Kaixian county showed a gradually descending trend, but anemia and other common diseases still kept a higher incidence rate, the detection rate of anemia was 33.5% in 1999, and decreased to 20.8% in 2003.CONCLUSION: The current nutritional status of the Kaixian people living in draw-down area of Three-Gorge reservoir still needs further improvement, and dynamic food and nutrition surveillance in succession is necessary.
4.Verification of conserved domain peptides derived from human antimicrobial peptide hGlyrichin

Objective To verify the antibacterial activity of the conserved domain derived from the novel human antimicrobial peptide-hGlyrichin .Methods Bioinformative analysis was performed and two peptides derived from hGlyrichin were synthesized which contained the conserved domain .Results Analysis of antimicrobial activities showed that these two peptides exhibited strong antibacterial activity which was inversely proportional to the length of the peptide within an eligible range.Despite the effective inhibition and killing of bacteria , the synthetic peptide segments had no hemolytic effect on human red blood cells .Conclusion These results indicate that a conserved domain exists in hGlyrichin , and that the peptides which contain this domain have strong antibacterial activity but are not toxic to human somatic cells .
5.Clinical application of 3D printing guild plate in total knee arthroplasty

Objective To evaluate the efficacy and safety of total knee arthroplasty (TKA) with 3D printing guild plate by comparing with conventional TKA. Methods From May 2014 to September 2014, 40 patients suffered primary unilateral TKA were received, in which there were 11 males and 29 females, aged from 57 to 82 years with an average age of 68.5±6.3 years. The subjects were divided into two groups randomly. One group was treated with TKA with 3D printing guild plate while the other group was treated with the conventional TKA. The blood loss, operation time, post?operative Hospital for Special Surgery (HSS) score, range of motion (ROM), lower limb mechanical alignment and incidence of complication were compared with insignificant differences. Results The operation time in the 3D printing TKA group (103.4±11.7 min) was lower than that in the conventional TKA group (124.5±21.6 min), which was statistically significant (t=3.838, P=0.000). The blood loss in the 3D printing TKA group (370.2±96.0 ml) was lower than that in the conventional TKA group (510.0±235.9 ml), which was statistically significant (t=2.454, P=0.019). The post?operative ROM of knee in the 3D printing TKA group was 104.3° ± 15.5° and that in the conventional TKA group was 103.5° ± 12.5° (t=0.169, P=0.867). HSS scores in the 3D printing TKA group and in the conventional TKA group were 88.5±5.7 and 89.4±4.8, which was statistically insignificant (t=-0.633,P=0.530). Mechanical alignment in the 3D printing TKA group was 2.9°±1.1° and that in the conventional TKA group was 3.0°±0.9°, which was not statistically significant (t=-0.317, P=0.753). No obvious complication occurred in two groups. Conclusion TKA with 3D printing guild plate has similar results to conventional TKA in HSS score, mechanical alignment and ROM of knee, but it shortens operation time and decreases the blood loss.
6.The efficacy of microvascular decompression for hemifacial spasm caused by vertebral basilar artery compression

Objective To analyse the efficacy of microvascular decompression for hemifacial spasm (HFS) caused by vertebral basilar artery compression. Methods A total of 141 patients with HFS treated by microvascular decompression in our hospital were collected in this study. The improvement of the symptoms after operation was compared between patients with HFS caused by vertebral basilar artery compression (28 cases) and patients with HFS caused by non-vertebral basilar artery compression (113 cases). Results There was no significant difference in the effective rate between the two groups of HFS (96.43%vs. 98.23%,P=0.49) with mean following-up 13.81 ± 1.57 months. And there was no significant difference in the delayed cure rate after surgery between two groups (37.04%vs. 20.72%,χ2=1.38, P>0.05). Conclusion Microvascular decompression is a safe and effective method for the treatment of HFS caused by compressed vertebral basilar artery.
7.The surgical procedure and clinical results of Stanford A aortic dissection

Objective To evaluate the methods and consequences of surgical technique in the treatment of Stanford A aortic dissection.Methods 108 patients with type Standford A aortic dissection underwent surgery in our study,including urgent surgery in 53 and selective surgery in 55.The operation was performed under deep hypothennic circulatory arrest (DHCA) in 85 cases.Surgical procedures included ascending and semi arch replacement or total arch replacement (some cases combined with stented graft implanted into the descending aorta),"elephant trunk" procedure.Concomitant procedures included repair of intimal tear in arch or descending aorta,Bentall procedure,aortic valve replacement,Cabrol or modified Cabrol procedure,aortic valvuloplasty,mitral valvuloplasty or mitral valve replacement,tricuspid valvuloplasty and CABG.Results In-hospital mortality was 6.5% (7 of 108 patients).The mor- tality was 7.5% (4 of 53 patients) in urgent surgery group and in elective surgery group was 5.4% (3 of 55 patients).Ninety six percent survived patients were followed up for 1 month to 13.3 years [mean (3.2?1.3) years] and 2 deaths occurred during the fel- low-up period.3 patients underwent re-operatian.Conclusion The choice of surgical procedures depend on the location of intimal tear for Stanford A aortic dissection.The better operative effects can be expected with proper surgical indication,perfecting surgical technique,and enhancing postoperative treatment.
8.Using modular fluted tapered stem for management of periprosthetic femoral fracture in revision hip arthroplasty

Objective To evaluate the technique and clinical results of the modular fluted tapered stems for treating periprosthetic femoral fractures in revision hip arthroplasty.Methods From August 2007 to February 2014,fourteen patients (14hips) with periprosthetic femoral fractures underwent revision hip arthroplasty with modular fluted tapered stem.A retrospective analysis was performed involving all patients who were followed-up more than 2 years.The subjects consisted of 4 males and 10 females with mean age of 73.4±6.6 years (range,62-82 years) at the time of revision.All patients were unilateral with 8 left hips and 6 right hips.The status of primary arthroplasty was bipolar hemiarthroplasty in 3 patients and total hip arthroplasty in 11 patients.Nine cases were with cemented stems,5 cases with uncemented stems.The interval from primary hip arthroplasty to revision surgery ranged from 40 to 163 months,with an average of 120.9±31.9 months.Eight cases with Vancouver type B3 periprosthetic femoral fracture were found preoperatively,6 cases with aseptic loosening and bone deficiency occurred periprosthetic femoral fracture during the operation.Based on the Paprosky classification system for femoral bone deficiency:type ⅢA in 8 hips,type ⅢB in 5 hips,type Ⅳ in 1 hip.Both the femoral and the acetabular components were revised in 9 patients.All femoral fractures were treated with cable fixation,and the cortical allograft struts were used to augment femoral bone stock in 7 patients.The patients were followed up at 6 weeks,3 months,6 months,9 months,12 months and annually thereafter.Harris Hip Score System and radiographic examination was used to evaluate the clinical results,including fracture union,implant stability,bone stock,hip joint function and postoperative complications.Results The mean duration of follow-up was 62.1 ±22.2 months (range,30-96 months).The mean times of fracture union were 6.2±2.5 months (range,3-12 months).The Harris Hip Score improved from 29.6± 10.3 preoperatively to 86.3±4.2 postoperatively (t=-21.6,P=0.00).Thirteen cases (93%,13/14) had the stem subsidence of 4.9±2.5 mm (range,0-9 mm).Thirteen cases (93%,13/14) were presented with femoral bony restoration.Two cases showed incorporation of the allograft in 7 patients.One patient developed deep venous thrombosis of lower limbs,and one suffered from subcutaneous hematomas after surgery.There was no infection,aseptic loosening,dislocation or periprosthetic fracture complications at the followup.Conclusion The short-medium term results of the modular fluted tapered prosthesis applied in periprosthetic femoral fractures are encouraging in revision hip arthroplasty.The present technology can provide reliable primary stability and can tolerate minimal subsidence postoperatively.
9.Recent advances in directed evolution.

Screening is the bottleneck of directed evolution. In order to address this problem, a series of novel semi-rational designed strategies have been developed based on combinatorial active-site saturation test and iterative saturation mutagenesis, including single code saturation mutagenesis, double code saturation mutagenesis and triple code saturation mutagenesis. By creation of "small and smart" high qualified mutant libraries and combinatorial mutagenesis of specific sites, these new strategies have been successfully applied in multiparameter optimization, e.g. stereo/regioselectivity and activity. This review summarized recent advances in directed evolution and its applications in biocatalysis field.
10.Effect of Smoking on Lung Cancer Histology and Its Epidemiology in Chinese Male

Background and objective Studies found that there were changes in histological subtypes of lung cancer patients in China. This study investigated the effect of smoking on lung cancer histology and its trend in Chinese male. Methods Demographic, smoking history and histological information about male lung cancer patients diagnosed or treated from 2000 to 2012 was collected from Cancer Hospital, Chinese Academy of Medical Science. Trends of histological subtypes calculated with annual percentage change (APC).Results A total of 14,106 patients with lung cancer were enrolled, with smoking 11,750 cases and non-smoking 2,356 cases. The main histological type of smoking lung cancer was squamous cell car-cinoma (SCC)(39.38%), followed by adenocarcinoma (ADC)(29.85%). Among smokers, the proportion of SCC decreased from 44.19% to 35.50% (APC=-1.9%,P<0.001), however, the ADC increased from 15.25% to 41.85% (APC=6.8%,P<0.001). Adenosquamous carcinoma (ASC) was from 4.13% to 0.72% (APC=-14.9%,P<0.001). In non-smokers, the ADC was 53.86%, and SCC was 16.64%. ADC increased from 38.03% to 67.83% (APC=4.3%,P<0.001). Distributions of LCC and ASC were scattered.Conclusion Proportion of ADC increased significantly in smoking and non-smoking lung cancer patients, and the relationship between non-smoking factor exposure and lung cancer should be further studied.
