1.The early diagnostic value of serum Cys-C、β~2-MG、RBP in patients with diabetic nephropathy

Objective To evaluate the Cys-C、β~2-MG、RBP as marker三of early diagnosis in patients with diabetic nephropathy Ⅱ(DN).Methods ninety patients with DN were divided into three groups according to urinary albumin excretion rate (UAER),that is normal,micro and mass group.Serum Cys-C、β~2-MG、RBP、BUN,Crea were detected and parallelly contrasted in the three groups and control group with thirty healthy people.Results compared with the control group,the serum Cys-C、β~2-MG、RBP of micro and mass group both significantly increased and the normal group had no significant difference.The positive rate of Cys-C、β~2-MG,RBP in mass group was respectively 100%、100%、96.1%;the positive rate in micro group as follows:Cys C (92.3%)>B2-MG(84.6%)>RBP(66.6%)>Crea(46.1%)>BUN(28.2%).Conclusion serum Cys-C、β~2-MG、RBP have high positive rates in patients with diabetic nephropathy Ⅱ(DN).They can be used as early diagnostic marker of diabetic nephropathy Ⅱespecially Cys-C.
2.Pedicle screw paraspinal muscle approach versus posterior median approach fixation for thoracolumbar fractures:comparison of the stability

BACKGROUND:Spinal posterior surgery is the most common treatment method for thoracolumbar fracture. During exposure of conventional posterior surgery, a wide-range stripping and pul ing of paraspinal muscles easily induced failure syndrome of lumbar surgery.
OBJECTIVE:To compare the reset conditions and stability of thoracolumbar fractures after treatment with pedicle screw paraspinal muscle approach and conventional posterior median approach fixation.
METHODA total of 62 patients with thoracolumbar fractures without nerve injury were retrospectively analyzed. 22 patients were treated with paraspinal muscle approach and general spine system. 21 patients were treated with conventional median approach and general spine system. 19 patients were treated with conventional median approach and AF internal fixation system. The therapeutic effects of the three kinds of fixation methods were compared by comparing clinical indexes in patients of the three groups, including operation time, intraoperative blood loss, postoperative drainage, dead space volume, scores of the Visual Analogue Scale of back pain, wound complications, height of injured vertebrae and the Cobb angle.
RESULTS AND CONCLUSION:Operation time, intraoperative blood loss, postoperative drainage and dead space volume were better in the paraspinal muscle approach and general spine system group than in the conventional median approach and general spine system group and conventional median approach and AF internal fixation system group (P<0.05). No significant difference in height of injured vertebrae and the Cobb angle was detectable among the three groups at 3 days after fixation (P>0.05). No significant difference in scores of the Visual Analogue Scale of back pain was visible among the three groups at 1 week after fixation (P>0.05). The scores of the Visual Analogue Scale of back pain were apparently lower in the paraspinal muscle approach and general spine system group than in the conventional median approach and general spine system group and conventional median approach and AF internal fixation system group at 3 and 6 months after fixation. No incision infection was observed in patients of the three groups. These results suggested that compared with conventional posterior median approach, paraspinal muscle approach has some advantages, such as smal trauma, less bleeding, postoperative rapid recovery, and high degree of satisfaction. The effects of general spine system and AF internal fixation system in the repair of thoracolumbar fractures on internal fixation are similar, but general spine system has some advantages such as simple to be operated, save time, less bleeding, stable fixation and good reduction. General spine system combined with paraspinal muscle approach is a good method to repair thoracolumbar spine fracture.
3.Meta-analysis on relationship between Helicobacter pylori infection and esophagogastric junction adenocarcinoma

Objective To systematically evaluate the etiological association between esophagogastric junction adenocarcinoma and Helicobacter pylori (Hp) infection and the pathogenicity of Cag pathogenic island in Hp infection cases.Methods Literatures about Hp infection and esophagogastric junction adenocarcinoma published from January 1980 to April 2015 were retrieved from CNKI,Wanfang data,VIP,the Cochrane Library,PubMed,EMBase databases.The literatures which met the inclusion criteria were collected and evaluated by using Newcastle-Ottawa Scale,then the heterogeneity was analyzed with Review Manager 5.3,and the pooled OR value and 95% confidence interval were calculated.Results A total of 5 547 study subjects were recruited in 13 studies,including 1 446 esophagogastric junction adenocarcinoma cases and 4 101 controls.The combined OR for Hp infection was 0.95 (95%CI:0.66-1.36),P=0.71;The OR in high risk areas was 1.66 (95%CI:1.33-2.08,P<0.001),higher than that in low-risk areas (0.68,95%CI:0.49-0.94,P=0.020).In addition,six studies found that the combined detection rates of Cag pathogenic island in esophagogastric junction adenocarcinoma cases and controls were 80.50% and 79.80%,respectively.There was no significant difference between two group,the combined OR was 1.24 (95%CI:0.96-1.60).Conclusion The association between Hp infection and esophagogastric junction adenocarcinoma was not significant,however,the significant difference was observed between high risk area and low risk area;the detection rate of Gag pathogenic island in Hp infection cases was high,but the pathogenicity for esophagogastric junction adenocarcinoma needs further study.
4.Biomechanical properties and measurement advances in spinal cord research

The spinal cord, encompassed by the dura mater, arachnoid membrane, pia mater, dentate ligament, and cerebrospinal fluid, consists of both gray and white matter. This study delves into the biomechanical properties of the spinal cord and its adjacent structures, revealing its nature as a nonlinear viscoelastic medium. Notably, both gray and white matter exhibit hyperelastic characteristics, displaying distinct mechanical responses during uniaxial tensile and mechanical compression experiments. Furthermore, it is noteworthy that the human spinal cord does not maintain uniform length, while the dura mater exhibits pronounced anisotropy, with its elastic modulus gradually decreasing from the cervical to the lumbar region. While research on the biomechanical behavior of the arachnoid membrane is limited, its potential to enhance predictive accuracy in spinal finite element models is evident. Unfortunately, there is a lack of documented studies exploring the biomechanics of the human pia mater. Crucially, the spinal cord is immersed in cerebrospinal fluid, which acts as a cushion against spinal cord vibrations. Therefore, the significance of cerebrospinal fluid should not be underestimated in examining the biomechanical dynamics of the spinal cord, as changes in cerebrospinal fluid pressure correspondingly affect spinal cord stress levels. Additionally, the strength of the dentate ligament decreases progressively from superior to inferior regions. Due to the inherent softness of spinal cord tissue, it often undergoes complex mechanical alterations such as stretching, compression, and torsion when subjected to injury. Various measurement techniques, including magnetic resonance elastography (MRE), atomic force microscopy, microindentation, and myelography, are employed for spinal cord assessment. MRE, in particular, offers distinct advantages in scrutinizing spinal cord morphology. Accurately quantifying the mechanical parameters of spinal cord deformation injuries remains a challenge. Advanced imaging technologies are employed to monitor the dynamic pathological transformations of the spinal cord, providing valuable insights for clinical prevention and treatment strategies. Finite element analysis plays a pivotal role in the study of spinal cord injuries. However, existing modeling methodologies often oversimplify the spinal cord, portraying it as a homogenous material. Further experimental validation is required to confirm its accuracy. An exhaustive exploration of spinal cord biomechanics and measurement techniques is essential to gain a deeper understanding of the mechanisms underlying spinal cord injuries. This knowledge can serve as crucial theoretical guidance and support for the treatment and prevention of such injuries.
5.Deficiency of succinic dehydrogenase or succinyl-coA synthetase enhances the production of 5-aminolevulinic acid in recombinant Escherichia coli.

5-aminolevulinic acid (ALA), a precursor for biosynthesis of pyrrole compounds in living organisms, has been widely used in agriculture and medical photodynamics therapy and is regarded as a promising value-added bio-based chemical. In the previous investigations on ALA production with recombinant Escherichia coli expressing heterogenous C4 pathway gene, LB media supplemented with glucose and ALA precursors succinate and glycine is widely used, leading to high production cost. Succinate participates in ALA biosynthesis in a form of succinyl-CoA. In this study, genes involved in succinyl-CoA consumption, sdhAB (encoding succinic dehydrogenase) or sucCD (encoding succinyl-CoA synthetase) of E. coli MG1655 was knocked out and tested for ALA accumulation. In comparison with the recombinant E. coli strain expressing heterogenous ALA synthetase, the sdhAB- or sucCD-deficient strain accumulate 25.59% and 12.40%, respectively, more ALA in a 5 L fermentor using a defined synthetic medium with glucose as main carbon source and without supplementation of succinate, providing a novel cost-effective approach for industrial production of ALA.

6.Association between -590C/T polymorphisms of interleukin-4 gene and periodontitis: A meta-analysis

Objective:
To evaluate the relationship between-590C/T polymorphisms of interleukin-4 gene and periodontitis by meta-analysis.
Methods:
Databases including PubMed, EMbase, Web of Science, CBM, CNKI, Wanfang and VIP, were searched from their establishment to December 31, 2016. Only case-control studies on the association between -590C/ T polymorphisms of interleukin-4 gene and periodontitis were selected, and the references of those selected studies were searched by hand. Assessment and data extraction were conducted by 2 reviewers independently, and the meta-analysis was performed with the R 3.3.1 software.
Results :
The meta-analysis showed that there was no association between the IL-4-590C/T polymorphism and periodontitis susceptibility (T vs C: OR=1.19, 95%CI=0.81~1.76, P =0.37). Subgroup analysis showed that the IL-4 -590 TT was associated with periodontitis in Caucasians (TT vs CT: OR=1.75, 95%CI=1.10 ~ 2.78, P = 0.02), but no significant association in Asians (P =0.63). Meanwhile, IL-4 gene polymorphisms were not significantly associated with the susceptibility of chronic periodontitis (CP) or aggressive periodontitis (AgP) ( e. g. T vs C, CP: P=0.45; AgP: P =0.45).
Conclusion
Based on current evidence, the polymorphisms of IL-4-590C/T are associated with the increased risk of periodontitis in Caucasians.
