1.A pharmacological study of chitosan hemostatic sponge

Objective: To investigate the extravasate sponging and hemostatic property of chitosan hemostatic sponge on the wound of otic vein of rabbit, and its healing effect on the scald of rabbit skin, for clinical application. Methods: Lacerating the otic vein of rabbit with bistoury and promptly pressing the wound with chitosan hemostatic sponge, gelatin sponge or gauze, respectively, the hemostatic effect was observed by uncovering each 30 seconds. Spreading chitosan hemostatic sponge, gelatin sponge or gauze, respectively, on the scalds of rabbit skin, the decrustation (pulling off scab) was observed within 10 days. Objective To determine the factors contributing to the development of deep vein thrombosis (DVT) in the lower extremity in patients after hip or knee arthroplasty and hip fracture internal fixation.Methods One hundred and forty-seven consecutive patients receiving hip or knee arthroplasty and hip fracture internal fixation from 2004 to 2005 were included in this study. Their age ranged from 33 to 92 years. Duplex color ultrasonic inspection was performed on veins of the bilateral lower extremities before operation and 2 weeks after operation for detection of DVT. The patients were divided into a DVT group and a DVT-free group based on the development of DVT after operation. Detailed perioperative clinical information about the patients, surgery and anesthesia was collected.Results Lower extremity DVT was found in 42.2% of the patients after operation, while the incidence of proximal DVT was 2.7%. Compared with the DVT-free group, the usage rate and dosage of ephedrine increased significantly, the duration of anesthesia was significantly longer, and the white blood cell count (WBC) on the 1st postoperative day and the highest WBC count were significantly higher in the DVT group(P<0.05). Logistic regression analysis indicated that the above factors were closely related to DVT.Conclusion Duration of anesthesia > 3 h, ephedrine administration and a marked increase in WBC count after operation are the risk factors for DVT in the lower extremities in patients after hip or knee arthroplasty and hip fracture internal fixation.

Objective To study the clinical characteristics, diagnosis, treatment and prognosis of primary pulmonary peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS). Methods Two cases of primary pulmonary PTCL-NOS were studied and relevant literature were reviewed. Results Case 1 diagnosed as primary pulmonary PTCL-NOS was a 44 years old woman and disease progressed after GLD (Gem+L-OHP+ DXM) chemotherapy regimen. At last, the patient died of respiratory failure after one month. Case 2 diagnosed as primary pulmonary PTCL-NOS was a 46 years old man and reach partial response after CHOP regimen, and still alive now.Conclusion Primary pulmonary PTCL-NOS is very rare. It is easy to be misdiagnosed due to non-specific clinical and imaging manifestations. Acquiring enough tissue specimens for pathologic examination is the key to a definitive diagnosis. At present, there is no standard chemotherapy regimen for these patients, the prognosis is relatively poor.
5.Study on the trend of vital signs in patients with granulomatous infection of malignant hematological diseases

Objective:To investigate the death of patients with granulocytopenia complicated with infection after chemotherapy and the changes of vital signs before emergency treatment, and to analyze the prognosis of different vital signs on patients' prognosis and emergency treatment.Methods:This study used a case-control study method to select 211 patients with hematologic malignancies who met the inclusion criteria in two tertiary hospitals in Weifang City. The vital signs of patients were collected and the vital signs were analyzed using SPSS 17.0 software. And the statistical significance and predictive value in the emergency response group.Results:The heart rate, respiratory rate, systolic blood pressure, blood oxygen saturation and urine volume were significantly different between the survival group (112 cases) and the death group (99 cases)( t values were 11.038-177.102, P<0.01). The area under the receiver operating characteristic curve of body temperature, heart rate, respiratory rate, systolic blood pressure, and blood oxygenation saturation and urine volume were 0.547, 0.495, 0.294, 0.899, 0.988, and 0.827, respectively. The highest predictive efficiency (higher level) was observed with the change of blood oxygen saturation, and the corresponding optimal cutoff point. 0.91; between the emergency treatment group (103 cases) and the non-emergency treatment group(108 cases), the difference in heart rate, respiratory rate and oxygen saturation between the two groups was statistically significant ( t values were 5.247, 8.001, 9.066, P<0.01). The area under the receiver operating characteristic curve of body temperature, heart rate, respiratory rate, systolic blood pressure, blood oxygen saturation and urine volume were 0.581, 0.732, 0.813, 0.346, 0.102, and 0.543, respectively. Among them, the predicted value of respiratory frequency change was the highest (medium level), which was the best corresponding. The cutoff point was 27.5. Conclusions:Patients with granulocytic infection after malignant hematologic disease will have abnormal changes in vital signs before death and emergency treatment. However, different vital signs have different effects on predicting disease changes, and should focus on respiratory rate and oxygen saturation. Changes, when the respiratory rate exceeds 27 beats/min, the probability that the patient needs to implement emergency treatment such as rescue will increase. If the condition is not effectively controlled, the blood oxygen saturation is lower than 0.91, the risk of death of the patient is greatly increased.
6.Effects of Lycium barbarum polysaccharides on blood indexes andliver tissue morphology in rats with intrahepatic cholestasis

Objective:
To study the effects of Lycium barbarum polysaccharides ( LBP ) on blood indexes and liver tissue morphology in rats with intrahepatic cholestasis.
Methods:
Sprague-Dawley rats were randomly divided into the control group, the model group, and LBP low, medium and high dose group. The rats in the model group and LBP dose groups were given 60 mg/kg alpha-naphthylisothiocyanate ( ANIT ) by gavage every three days of the experiment, and the rats in the control group were given salad oil instead of ANIT. From the third day, the rats in each dose group were given 40, 150 and 600 mg/kg LBP, and the rats in the model group were given distilled water. After four weeks, the blood and urine indexes were measured, and the morphological changes of liver tissue were observed.
Results:
From the third day of the experiment, the activity of rats in the model group and LBP dose groups decreased, and the color of urine changed to dark yellow. There was no abnormality in the group. In the model group, the levels of serum total bilirubin, direct bilirubin, total bile acid ( TBA ), alkaline phosphatase ( ALP ), γ-glutamyltransferase(γ-GGT), cholesterol, alanine aminotransferase ( ALT ), aspartate aminotransferase ( AST ), white blood cell ( WBC ), percentage of granulocyte, urinary bilirubin, urinary bile acid, liver mass and liver to body ratio were higher than those in the control group, while red blood cell and percentage of lymphocyte were lower than those in the control group ( all P<0.05 ). Pathological changes of liver tissue were observed. The levels of serum TBA, ALP, γ-GGT, ALT, AST, WBC and liver to body ratio in LBP high dose group were lower than those in the model group ( all P<0.05 ). The infiltration of inflammatory cells, proliferation and expansion of bile duct, degeneration and necrosis of liver cells were alleviated.
Conclusions
LBP can improve the blood indexes and pathological changes of liver tissue in rats with intrahepatic cholestasis at the dosage of 600 mg/kg. Inhibition of inflammatory response and reduction of oxidative stress injury may be the mechanism for alleviating cholestatic liver injury.
7.Skin damage caused by sunlight ultraviolet in mice

Objective :
To study the damage effect of sunlight ultraviolet exposure on skin.
Methods :
No exposure group, low exposure group and high exposure group were set up with 10 mice in each. The exposure doses of sunlight ultraviolet were 0, 10 J/cm2 and 20 J/cm2, respectively. The skin of mice was irradiated by a sunlight ultraviolet simulator for 5 days a week, 13 weeks. At the end of the experiment, the skin appearance of mice was examined; the skin moisture, oil content, texture density, hydroxyproline ( HYP ), hyaluronic acid (HA), malondialdehyde ( MDA ), glutathione ( GSH ) and superoxide dismutase ( SOD ) activities were detected; and the skin tissue morphology, collagen fiber morphology and elastic fiber morphology were observed.
Results :
The skin appearance of mice in the no exposure group was normal; in the low exposure group, only one mouse had mild skin desquamation; in the high exposure group, the skin was loose and wrinkled, dry and desquamated, local thickening and erythema formation. Compared with the no exposure group, the contents of skin moisture, HYP, HA and SOD activity were lower, texture density, MDA content, morphological scores of skin tissue, collagen fiber tissue and elastic fiber tissue were higher in the high exposure group ( all P<0.05 ). Compared with the low exposure group, the HA content and SOD activity were lower, the skin texture density, MDA content, and histomorphological scores of skin tissue and collagen fibers were higher in the high exposure group ( all P<0.05 ).
Conclusion
Exposure to 20 J/cm2 sunlight ultraviolet can significantly lead to abnormal skin appearance and function in mice.
8.Quality of Life in Obstructive Sleep Apnoea: A Role for Oxygen Desaturation Indices?

INTRODUCTIONThis study aimed to determine the impact of obstructive sleep apnoea (OSA) on quality of life (QOL) and evaluate the utility of polysomnographic parameters in reflecting QOL.
MATERIALS AND METHODSEighty-eight patients who underwent polysomnography (PSG) between December 2010 and November 2012 consecutively were recruited and they completed the 36-Item Short-Form Health Survey (SF-36) and Epworth Sleepiness Scale (ESS) questionnaires. Based on the apnoea-hypopnoea index (AHI), patients were classified as primary snorers (AHI <5), suffering from mild (5 ≤15), moderate (15 ≤30) or severe OSA (≥30).
RESULTSSeventy-nine male and 9 female patients with a mean age of 41 years were recruited. OSA patients scored significantly lower on 7 domains of SF-36 compared to the population. As AHI increased, only Physical Function (PF) and Physical Component Summary (PCS) but not ESS scores significantly worsened. PSG parameters correlated poorly with all QOL measures except PF, PCS and ESS. After adjusting for age, sex and body mass index (BMI), multiple linear regression revealed that only the oxygen desaturation parameters, but not sleep architecture indices or AHI were significant predictors of PF and ESS. For every fall in the lowest oxygen saturation (LSAT) by 1%, there was a decrease in PF by 0.59 points, and an increase in ESS by 0.13 points.
CONCLUSIONOSA patients have a poor QOL compared to the population. The amount of physical impairment and daytime sleepiness they experience is better predicted by severity and duration of hypoxia and not AHI.

9.Analysis of screening for colorectal cancer high-risk groups in Harbin, China 2012-2015.

OBJECTIVETo evaluate the result of colorectal cancer screening for high-risk groups in Harbin, China from 2012 to 2015.
METHODSAccording to the Project of National Colorectal Cancer Screening in Urban Area, epidemiological investigation and cancer risk evaluation established by National Cancer Center were performed among 40-69 years old residents in Harbin. Questionnaires were issued to assess high-risk groups for clinical screening, including colonoscope examination. The results of colorectal screening were evaluated in detail.
RESULTSProject of Colorectal Cancer Screening in Urban Area was completed successfully from 2012 to 2015 in various districts of Harbin. The accomplishment of high-risk assessment included 3 017 people, while 2 996 people received the clinical screening, including 1 376 males and 1 629 females, from 40 to 69 (53.3±7.1) years old. Among 2 996 people, 1 158 cases of colorectal polyps(38.7%) were screened, including 36.6%(901/2 465) cases of age<60 and 48.4%(257/531) cases of age>60 years old with significant difference (χ=19.19, P<0.01), and 47.6% (651/1 367) cases of male and 31.1%(507/1 629) cases of female with significant difference (χ=85.33, P<0.01). Of 1 158 polyps patients, 141 received the pathological examination and 11 patients were diagnosed as colorectal cancer, accounting for 0.4% of overall screening people (11/2 996). All these 11 patients underwent operation by advice and the postoperative pathology results all indicated early adenocarcinoma. Furthermore, among 141 polyps patients, 97 cases of tubular adenoma were found, which were distributed as follows: 2 cases of cecum (2.1%), 14 cases of ascending colon(14.4%), 12 cases of transverse colon (12.4%), 15 cases of descending colon (15.5%), 35 cases of sigmoid colon (36.1%) and 19 cases of rectum(19.6%).
CONCLUSIONWith the increase in age, the risk of colorectal polyps is significantly elevated. The males have higher incidence of colorectal polyps than the females. Adenoma mainly locates in the distal colon and rectum, especially in the sigmoid colon. Early screening for high-risk group can find out colorectal precancerosis and cancer, so the patients can receive early treatment.

10.Relationship of age with carotid artery plaques and luminal stenosis

Objective To study the relationship of age with carotid artery plaques and luminal stenosis.Methods One hundred and sixty-five patients who underwent carotid artery ultrasonogra phy in our hospital were divided into ischemic stroke (IS) group (n=100) and IS-free group (n=65) according to their clinical symptoms and ultrasonographic findings.Risk factors for their IS were analyzed.The patients were further divided into ≤59 years old group (n=36),60-79 years old group (n=98) and ≥80 years old group (n=31).Their carotid artery stenosis (CAS) and plaques were analyzed.Results The age was significantly older,the incidence of hypertension,CAS and vulnerable plaques was significantly higher in IS group than in IS-free group (70.6±11.2 years vs 65.1±12.8 years,P=0.004;79.0% vs 60.0%,P=0.008;40.0% vs 24.6%,P=0.041;20.0% vs 6.2%,P=0.014).The detection rate of stable and vulnerable plaques was significantly higher in 60-79 years old group and ≥80 years old group than in ≤59 years old group (P<0.01).Multivariate logistic regression analysis showed that age was a risk factor for IS,CAS and vulnerablelaques(P=0.000,P=0.023).Conclusion Age is a risk factor for IS.The number of carotid artery vulnerable plaques is greater and the incidence of moderate and severe carotid artery luminal stenosis is higher in IS patients than IS-free patients.The number of carotid artery vulnerable plaques is greater in ≥60 years old patients than in ≤59 years old patients.The incidence of CAS is the highest in ≥80 years old patients who therefore have a higher risk of IS.
