1.Update on anti-inflammation mechanism of ketogenic diet

The ketogenic diet (KD) is an effective treatment for refractory epilepsy,including some inflammation-induced epileptic encephalopathies,and the mechanism of it is complicated.KD,through multiple targets,plays the role to inhibit pathogens,control inflammation and regulate immune function.In experiments,KD provides an anti-inflammatory effect in some rat models of fever.It has been shown that,KD decreases peripheral edema,attenuates thermal nociception and reduce arachidomic acid level in circulation.KD blocks Nod-like receptor protein 3 (NLRP3),nuclear factor-kappa B (NF-KB),oxygen free radicals,and improves Peroxisome proliferator-actived Receptor(PPAR) γlevel,to reach the effect of anti-inflammatory.This review focuses on ketogenic diet anti-inflammatory mechanism and the recent advances in clinical application.
2.Trichostatin A promotes mouse bone marrow mesenchymal stem cells to differentiate into insulin-secreting cells

AIM:To investigate whether trichostatin A ( TSA) , a new revulsant ,can induce mouse mesenchy-mal stem cells to differentiate into insulin-secreting cells and to explore the appropriate concentration of TSA .METHODS:The mesenchymal stem cell line from C57BL/6 mice was cultured in vitro and divided into 5 groups before treated with dif-ferent concentrations of TSA , ( group A:DMSO;group B~E:treated with 25 nmol/L, 50 nmol/L, 100 nmol/L and 200 nmol/L of TSA, respectively).After exposed to different cultured media for 10 d during the 2 stages, the cells were detec-ted by the following methods:the insulin-secreting cells in each group were identified by dithizone staining and the results were calculated with immunohistochemical half quantitative analysis .The insulin secreted by insulin-secreting cells in each group was identified by immunofluorescence , and the mean fluorescence intensity of insulin was compared .The content of insulin in each group was quantified by ELISA .The appropriate concentration of TSA was determined according to the above results .RESULTS:TSA treatment for 10 d promoted the mouse bone marrow mesenchymal stem cells to differenti-ate into insulin-secreting cells which produced insulin .The immunohistochemistry and immunofluorescence imaging analysis of insulin-secreting cells showed that the insulin staining positive area , positive ratio , total density of insulin expression and mean fluorescence intensity of insulin in group B were significantly higher than those in the other TSA -treated groups .When the concentrations of TSA gradually increased , the content of insulin reduced accordingly .The content of insulin in group B was significantly higher than that in the other TSA-treated groups .CONCLUSION:TSA treatment for 10 d promotes bone marrow mesenchymal stem cells from C57BL/6 mice to differentiate into insulin-secreting cells and the appropriate concen-tration of TSA is 25 nmol/L.
3.Analysis of treatment effect of chronomodulated chemotherapy plus concurrent intensity-modulated radiotherapy in patients with nasopharyngeal carcinoma (NPC)

Objective To evaluate the therapeutic effects,acute and late side-effects of cisplatin plus 5-fluorouracil (PF) schemed chronomodulated chemotherapy combined with concurrent intensitymodulated radiotherapy(IMRT) for nasopharyngeal carcinoma.Methods A total of 48 nasopharyngeal carcinoma patients treated with chronomodulated chemotherapy plus concurrent IMRT were retrospectively studied.The radiation doses prescribed to the gross target volume GTVnx of nasopharyngeal,to the positive neck lymphnodes GTVnd,to the clinical target volume CTV1,and to CTV2 were 73.92-77.88,69.96,60.06-66.00 and 50.96-56.00 Gy respectively.The Kaplan-Meier survival analysis was used to calculate the local-regional progression-free rate.The acute and late side-effects were graded according to RTOG radiation morbidity scoring criteria.Results The complete remission (CR) rate was 41.6%(20/48) ; the partial remission (PR) rate was 47.9% (23/48) ; the stable disease (SD) rate was 4.2%(2/48) ; and the local tumor control rate was 89.6%.Most of the patients had grade 1 to 2 acute side effects and grade 0 to 1 late side-effects.No grade 4 acute and late toxicity was observed.1,2,4-year survival rates were 93.8%,79.2% and 64.5%,respectively.The DVH analysis showed that IMRT improved the total dose and dose per-fraction to the target volume with considerable dose reduction of the normal structures at the same time.Conclusions PF chronomodulated chemotherapy plus concurrent intensity-modulated radiotherapy for NPC could improve therapeutic effects with satisfactory tolerance of the side effects.
4.Clinical efficacy of fluid resuscitation with 75 g/L NaCi solution in traumatic hypovolemic shock:a Meta-analysis of randomized controlled trials

Objective To investigate the efficacy and safety of fluid resuscitation with 75 g/L NaCl solution in traumatic hypovolemic shock using Meta-analysis. Methods Based on the inclusion criteria,the randomized controlled trials (RCTs) were identified by retrieving the databases including PUBMED,EMBASE,OVID,Cochrane library and EBSCO.The quality of studies was also evaluated.Indices including systolic blood pressure,hemoglobin,serum Na level and mortality were extracted from the enrolled RTCs and were comparatively analyzed between the 75 g/L NaCl group and the isotonic saline group for evaluating the safety and efficacy of 75 g/L NaCI solution.The extracted data were analyzed by using the Review Manager 5.0.25. Results Six RCTs were eligible for Meta-analysis.The 75 g/L NaCl group displayed remarkable increase in systolic blood pressure and decrease in hemoglobin level,compared with the isotonic saline group ( MD =6.23,95% CI 2.78-9.69,P < 0.01 ; MD =-6.11,95%CI -8.25-3.96,P < 0.01,respectively).Short-term hyperosmolar state in 75 g/L NaCl group was increased significantly but was considered acceptable ( MD =7.97,95% CI 7.55-8.38,P < 0.01 ).No significant difference in mortality was found between 75 g/L NaCl group and isotonic saline group ( RR =0.96,95% CI 0.84-1.10,P > 0.05). Conclusion Fluid resuscitation with 75 g/L NaCl solution is safe and effective for traumatic hypovolemic shock.
5.Logistic Regression Analysis on Risk Factors of Northwest Dryness Syndrome Among Patients of Metabolic Syndrome in Xinjiang Uygur Autonomous Region

Objective To analyze risk factors of northwest dryness syndrome (NDS) among the patients of metabolic syndrome in Xinjiang Uygur Autonomous Region.Methods Totally 321 cases of metabolic syndrome in Kashgar, Hami and Urumqi were collected to filter 15 variables to calculate OR value and 95% CI, and evaluated by single factor and multi-factor analysis by applying conditional Logistic regression analysis.Results Single factor conditional Logistic regression analysis showed that high fat diet, addiction to hot diet, salty diet, smoking history and hypertension were the risk factors of NDS. The OR values of them were 0.387, 0.071, 0.033, 2.614 and 0.440, respectively (P<0.05). Multi-factor conditional Logistic regression analysis showed that smoking history, alcohol drinking history, high fat diet and hypertension were all entered into regression model. The OR values of them were 3.945, 4.334, 0.370 and 2.142, respectively (P<0.01).Conclusion Patients of metabolic syndrome with a high fat, salty and hot diet are suggested to adjust their diet habit by adding sour food, giving up smoking and drinking, and controlling their blood pressure. It will be helpful for them to adapt local climate and living environment, and reduce the attack rate of NDS.
6.Study on delineation of tumor volume of primary locally advanced nasopharyngeal carcinoma after induction chemotherapy

ObjectiveTo investigate the delineation of gross tumor volume (GTV) in locally advanced nasopharyngeal carcinoma (LANC) according to imageological changes before and after induction chemotherapy (IC) in order to decrease high dose area and protect normal tissue better.MethodsBetween Mar 2010 to Jan 2011,11 patients with LANC were enrolled and treated with TPF regimen followed by intensity-modulated radiotherapy (IMRT) with concurrent chemotherapy,target volumes were delineated based on fused CT imaging before and after IC following project determination.Tumor target volumes after and before IC were respectively delineated according to imaging tumor residues and were overlaid by CTVnx in order to ensure radical doses for the imaging tumor volume before IC,the resulting differences of tumor target volumes of IC before and after were measured and analyzed by paired t-test.ResultsBefore and after IC,the average volumes of GTVnx were respectively 44.72 cm3 and 28.87 ( t =3.89,P =0.003 ),the average volumes of GTVnd were respectively 32.76 cm3 and 19.82 cm3 ( t =2.47,P =0.033 ),the volumes of maximum dose area in brainstem and spinal cord as well as eyeball decreased ( t =2.93-4.59,all P <0.05).ConclusionsLANC treated by 3 cycle TPF regimen followed by IMRT with concurrent chemotherapy showes significant shrinkage of tumor volume.The volume of high dose region which caused by normally recovered tissues were decreased by re-delineation of target volume in brainstem and spinal cord as well as eyeball of CT images after IC.
7.Effect of the new carbon fiber bed board of Elekta Precise linear accelerator on the radiation dose

Objective To investigate the dosimetric influence of pure carbon fiber treatment tabletop of Elekta Precise new linear accelerator in radiotherapy.Methods Surface-axis distance (SAD) technology was employed for the measurement.Two groups of fields were set and both of them were SAD opposed portals ( one of them went through the tabletop,while the other did not).A PTW electrometer and a 0.6 cm3 Farmer ionization chamber were utilized for comparison measurement.Then dose attenuation of the main table board,extended body board,the extended board for head,neck and shoulders,and the joints of these boards were calculated.Results Under the energy of 6 MV,the dose attenuations of the following locations were:1.4% - 7.2% at the main treatment table board; 2.8% - 38.7%,1.4% -30.1%,1.5% -20.8% and 1.4% - 11.2%,respectively at distances of 1,4,7 and 8 cm from the joint of the main table board ;0.5% - 5.0% at the extended body board; 4.7% - 15.4% at distance of 1cm from the joint of the extended body board; 0.5% -3.3% at the neck position of the extended board for head,neck and shoulders; 5.3% - 16.7% at the shoulder positions; and 6.8% -30.4% at the joint between the extended boards and the main table board.Conclusions The dose attenuations of the new linear accelerator pure carbon fiber treatment tabletop vary at different locations. Considerable higher attenuations are observed at the table board joints than other locations.
8.Value of contrast-enhanced ultrasound in preoperative evaluation for hilar cholangiocarcinoma

Objective To evaluate the value of contrast-enhanced ultrasound (CEUS) in preoperative classification for hilar cholangiocarcinoma. Methods Forty-six patients with 46 hilar cholangiocarcinoma were diagnosed by surgical pathology in Zhongshan Hospital of Fudan University from January 2007 to April 2013. The echogenicity difference on conventinal ultrasound and CEUS were compared with chi-square test. The accuracy of conventinal ultrasound and CEUS for evaluating invaded bile duct, detective rates for portal vein invasion and displaying rate of metastatic hilar lymph nodes were compared with chi-square test or Fisher’s Exact test according to the golden standard of operative exploration. Results On CEUS, 82.6%(38/46) and 91.3%(42/46) hilar cholangiocarcinoma were hypoechoic in portal vein phase and delayed phase respectively, while 63.0%(29/46) hilar cholangiocarcinoma were isoechoic on conventinal ultrasound with vague margin. The clearly displaying rates were 37.0%(17/46), 84.8%(39/46) and 91.3%(42/46) in conventinal ultrasound, portal vein and delayed phase of CEUS and the echogenicity was signiifcantly different. The evaluation accuracy of hilar cholangiocarcinoma invading bile duct was improved from 80.4%(37/46, conventinal ultrasound) to 100%(46/46, CEUS) significantly (χ2=7.882,P=0.005). Portal vein invasion were found in 9 cases during operative exploration and the detective rates on conventinal ultrasound and CEUS were 78%(7/9) and 89%(8/9) without signiifcant difference (P=1.000). Metastatic hilar lymph nodes were found in 8 cases and the displaying rates on conventinal ultrasound and CEUS were the same (75%, 6/8) without signiifcant difference (P=1.000). Conclusions CEUS could signiifcantly improve the clearly displaying rate of hilar cholangiocarcinoma and improve the evaluation accuracy for invaded bile duct comparing with conventinal ultrasound.

Objective To investigate the role of endothelial bioreacter device in sepsis porcine model.Method Sepsis porcine model was induced gy established endotoxin (LPS,0.25 mg·kg~(-1)) in healthy hybrid swines. The animals were randomly divided(random number) into endothelial bioreactor device group(EBR group) and sham circulation group( Sham group)( n = 6, respectively). After the infusion of endotoxin, extracorporeal circulation was started with the blood flow of 30 mL/min. The blood went through the endothelial bioreactor, then went back to the body via internal jugular vein in the EBR group. The bioreactor with the same size and without endothelial cells(ECs) was used in the sham group. Hemodynamic variables, blood biochemistry, inflammatory markers, Endothelin-1(ET-11) and yon Willebrand Factor(vWF) were examined just before and every hour after the injection. When the survival time of the animals was recorded,the animals were sacrificed to calculate the lung injury score. The time-dependent hemodynamics and cytokine data were compared between groups by repeated measurement ANOVA .Student's t -test was used to analyze the survival time. Results The mean artetial blood pressure (MAP) remarkably decreased in both groups after LPS injection, while the decreasing rate in EBR group was significantly lower than that in control group after 2 hours( P < 0.05). The ET- 1 level in EBR group increased after a slight decrease at the beginning, while that in the sham group went on increasing(P<0.01). The vWF levels increased first, then returned to the baseline in the sixth hour in both groups, while the change in EBR group was significantly less than that in the sham group(P<0.05). The Lung Injury Score in EBR-treated group was significantly lower than that in the sham group(6.1 ± 0.9 vs. 8.2 ± 1. 0, P < 0.05). These physiologic and biochemical alterations were associated with a significant advantage to the survivals in the EBR group when compared with the control sham group(6.7 ± 1.32 vs. 5.2 ± 0.61 h, P < 0.01 ). Conclusions Timely intervention in endotoxin shock with EC therapy by using tissue-engineered bioreactor may improve cardiovascular performance and alter the natural course of this disease process, probably via modulating ioflammation and coagulation cascades.
