General anesthesia combined with epidural anesthesia and postoperative epidural analge-sia is capable of accelerating the recovery of gastrointestinal function,providing better postoperative pain re-lief,improving the patient's satisfaction as well as shortening hospitalization dates.The article summarized the mechanisms of epidural analgesia influencing the recovery of gastrointestinal function.

AIM Valproate (VPA) is widely used to treat epilepsy. Long-term treatment of women with VPA has been reported to be associated with a PCOS-like syndrome. The aim of the present study was to investigate the effects of clinically relevant concentrations of valproate on steroidogenesis and steroidogenesis acute regulatory protein (StAR),cholesterol side-chain cleavage enzyme (P450scc)and cytochrome P450 aromatase (P450arom) mRNA expression in human luteinized granulosa cells in vitro. Objective To investigate whether low level 45, X/46, XX mosaicism presents in women with premature ovarian failure (POF) and the sensitivity and specificity of fluorescent in situ hybridization (FISH) for detecting X chromosome mosaicism. Methods Karyotypes of peripheral lymphocyte in 18 patients with POF and 9 normal controls were analyzed and the orange signals during FISH using X chromosome enumeration probes (CEPX) were counted. Results Single signals of X chromosome (45, X) were found in 7.6% of the total counted cells, which was significantly greater than that in the controls, in spite of the normal 46, XX karyotypes by routine analyses in all POF patients. Conclusions This study indicated that some POF patients may attribute to low level 45, X/46, XX mosaicism. FISH is more sensitive than the routine chromosome analyses in detecting low level X chromosome monosomy.

Objective To evaluate the factors associated with clinical pregnancy rate of in-vitro fertilization (IVF) in endometriosis related infertility.Methods Total of 326 patients with endometriosis related infertility undergoing IVF between January 2007 and December 2011 were studied in Department of Reproductive Medicine,First Affiliated Hospital,Nanjing Medical University,retrospectively,which were divided into 141 cases in clinical pregnancy group and 185 cases in non-pregnancy group.Those factors including age,body mass index (BMI),basic FSH,antral follicle count (AFC),CA125 and CA199,endometriotic stage and history of surgery,stimulation scheme were analyzed by bivariate analysis and multivariable logistic regression.Results (1) Pregnancy rate:total of 141 pregnant cases and 185 nonpregnant cases treated by IVF were observed,pregnancy rate was 43.2％ (141/326).(2) Basic parameters:there was no statistical difference in age,BMI,basic FSH,AFC,CA125 and CA199 between clinical pregnancy group and non-pregnancy group (P ＞ 0.05).(3) Bivariate analysis:clinical pregnancy rate of 50.0％ (87/174) among patients with infertility year less than five years was significantly higher than 35.5％ (54/152) in patients with more than five years.Pregnancy rate of 56.1％ (46/82) in stage Ⅰ-Ⅱ was significantly higher than 42.5％ (79/186) in stage Ⅲ-Ⅳ.Pregnancy rate of 46.6％(125/268) with history of surgery was significantly higher than 27.6％ (16/58) with no history of surgery (P ＜ 0.05).Pregnancy rate of 48.2％ (79/164) in long-term scheme was higher than 38.3％ (62/162) in short-term scheme,but there was no significant difference (P =0.075).(4) Multivariable logistic regression:clinical pregnancy rate of infertility year with less than 5 years,stage Ⅰ-Ⅱ,history of surgery proved stage Ⅰ-Ⅱ and stage Ⅲ-Ⅳ was significantly higher compared with infertility year more than 5 years,stage Ⅲ-Ⅳ and no history of surgery respectively (adjusted OR and 95％ CI:2.003,1.263-3.175; 1.899,1.110-3.248; 3.769,1.802-7.887,P＜0.05).Conclusion Factors affecting clinical pregnancy rate of IVF in endometriosis related infertility were infertility year,stage and surgery.

Objective To investigate the reversal of the multidrug resistant gene mdr1 in vivo by antisense oligodeoxynucleotide (ASODN) on the basis of study in vitro. Methods The cultured drug-resistant human hepatocellular carcinoma cells were injected under the skin of axilla to establish the tumor model of nude mice. mdr1 ASODN accompanied by Lipofectamine were injected locally and ADM was injected intraperitoneally. Control 1 and control 2 were locally injected by Lipofectamine and normal saline separately, and ADM was also injected intraperitoneally. Results As time went on the tumor size increased and from the 5th day on alterations were marked, tumor size in different time phase showed marked difference to the prior time phase with significant difference (P 0.05). The results suggested that SODN and Lipofectamine showed no marked effect on tumor growth of nude mice and ASODN had marked inhibition effect on tumor growth.Conclusion mdr1 ASODN can also reverse multidrug resistance of drug-resistant human hepatocellular carcinoma cells in vivo. After the treatment the tumor’s growth in nude mice will slow down in a range of time.

Objective To explore the relation of the expressions of MDM2 and VEGF in osteosarcoma with the pathological parameters and prognosis of the tumor.Methods The expressions of MDM2 and VEGF were detected with immunohistochemical(SP) method in specimens from 56 cases of osteosarcoma.The correlation between the expressions of MDM2,VEGF and pathological grade,metastasis and prognosis was analyzed statistically.while 8 cases of fibrous dysplasia of bone were used as negative control group.Results The positive rates of MDM2 and VEGF in osteosarcoma were 64.3%(36/56) and 67.9%(38/56),respectively .MDM2 and VEGF didn't express in negative control group.The expression of MDM2 and VEGF were not significantly correlated to the pathological grades of the osteosarcoma,but which were significantly correlated with tumor metastasis and prognosis(P

Objective To study the expression of tight junction factors in human placental tissues derived from assisted reproductive technology (ART) and natural pregnancy and its role in placental barrier.Methods Ten placental samples were collected from the women who had undergone ART treatment and 11 placenta were collected from control group.Transmission electron microscope (TEM) examination was utilized to detect the morphology of placental tight junctions.The mRNA of claudin (CLDN) 1,CLDN4,CLDN5,CLDN8,zonula occudens (ZO) 1 was detected by real-time PCR and the protein of CLDN4,CLDN8 and occludin (OCLN) were measured by western blot.Results TEM microscopy results showed that placenta samples derived both ART and control placenta had normal microscopic histological features of tight junctions,localized in the apical part of the syncytium and also between the cell-cell contacts of fetal blood vessel endothelial.The expression level of CLDN4 mRNA were 0.87 ±0.17 in ART group and 1.18 ± 0.30 in control group,respectively.The expression level of CLDN8 mRNA were 3.25 ± 2.32 in ART group and 1.08±0.41 in control group,respectively.The mRNA level of CLDN4 and CLDN8 were significantly differentially expressed in ART derived placenta when compared with control groups.The expression level of CLDN1,CLDN5,OCLN and ZO1 mRNA were 0.49 ± 0.44,0.80 ± 0.20,0.92 ± 0.18 in ART group and 1.09±0.82,1.21 ±0.78,0.80± 0.27 in control group,respectively,in which there were no significant differences between two groups.Western Blot analysis showed the protein levels of tight junctions CLDN4,CLDN8 and OCLN did not differ between groups.Conclusions Tight junction factors were expressed in human placental tissues.Tight junction derived from ATR platenta might have mild dysfunction.

OBJECTIVETo recommend an improved method of prostate massage, which is safe, effective, simple and tolerable, and may increase the expressed prostatic secretin (EPS) success rate and promote the efficiency of the prostate massage.

METHODSTwo methods are described in this paper, one being the aside-bending and up-standing vs parallel and semi-squatting method to massage the prostate via the rectum, and the other being the traditional massage method. A total of 261 chronic prostatitis (CP) patients were randomly divided into two groups, and treated separately with the new method and the traditional method. Then, the two methods were compared by EPS quantity and the patients' tolerability.

RESULTSThe new method made it possible to collect larger quantity of EPS. The results of comparison showed that the new method was better received by the patients.

CONCLUSIONThe aside-bending and up-standing vs parallel and semi-squatting method to massage the prostate via the rectum may increase the EPS success rate of prostatic massage and hence help the clinical diagnosis and treatment of chronic prostatitis. This method is well worth recommending.
