1.Preliminary experience of Bacillus Calmette-Guerin in the treatment of bladder cancer secondary to kidney transplantation

Objective:To summarize the experience of Bacillus Calmette-Guerin(BCG) in the treatment of bladder cancer secondary to renal transplantation.Methods:The clinical data of 5 patients who underwent BCG bladder irrigation after secondary bladder cancer after kidney transplantation in Tianjin First Central Hospital from January 2015 to December 2019 were analyzed. There were 1 male and 4 female cases. During the period of immunosuppression after transplantation, 1 case developed secondary high-level non-muscular invasive bladder cancer (NMIBC), 3 cases developed secondary low-grade NMIBC, and 1 case developed secondary glandular cystitis (4 cases). The mean age of the 5 patients with secondary bladder cancer was 59.7±4.0 years. Case one with high level NMIBC was treated with transurethral resection of bladder tumor (TURBT) and postoperative irrigation of epirubicin. Case 3 and 5 with low-level NMIBC accepted regular postoperative irrigation of gemcitabine. No irrigative therapy was performed in case 2. Bladder cancer recurred in case 1, 2, 3 and 5 after 20.1±9.7 months. TURBT was observed in all the 4 patients, among which 3 were of high grade NMIBC and 1 was of low grade NMIBC. Four patients were irrigated with BCG 2 weeks after operation. Postoperative pathology indicated low-level NMIBC in case 4, and BCG was irrigated 2 weeks after the operation. During perfusion therapy, immunosuppressive agents were continued.Results:During BCG perfusion, 4 of the 5 cases showed BCG related local inflammation, among which 2 cases presented symptoms of bladder irritation, 1 case presented hematuria, and 1 case presented hematuria with low fever. Patients with frequent urination, pain in urine, hematuria and other symptoms improved after drinking plenty of water, taking bed rest and taking levofloxacin (0.5g/ day ×7 days). Patients with low fever were treated with antipyretic treatment. No antituberculous agents were used prophylactically during BCG perfusion. There were no symptoms of tuberculosis infection or sepsis. The function of transplantated kidney was normal and no tendency of rejection. The 5 patients were followed up for 7-24 months, 1 patient was lost to follow-up after 7 months of BCG bladder perfusion, and no tumor recurrence or metastasis was found in 5 patients during the follow-up.Conclusions:The use of immunosuppressive agents does not reduce the biological activity of BCG, and BCG does not increase the risk of systemic toxicity or affect the function of transplanted kidneys in immunocompromised patients. BCG is a treatment option for bladder cancer secondary to renal transplantation.
2.Prognostic value analysis of TOP2A gene expression for bladder cancer

Objective To investigate the relationship between DNA topoisomerase Ⅱ α (TOP2A) gene expression and clinicopathological characteristics and its significance of prognostic evaluation for patients with bladder cancer.Methods Bladder cancer gene expression profile GSE13507 (n =165) and GSE31189 (n =52) were obtained.The expression profile and clinical information of patients with bladder cancer were retrospectively analyzed,and the survival analysis was made.Gene set enrichment analysis (GSEA) was conducted to explore the related pathways which were regulated by TOP2A.Results Compared with normal bladder tissues,TOP2A was upregnlated in bladder cancer tissues (5.823 ± 1.079 vs.4.820 ± 1.129),with a statistically significant difference (t =4.336,P < 0.001).The TOP2A gene expression in patients with bladder cancer was correlated with the age of patients (x2 =5.926,P =0.015),sex (x2 =6.046,P =0.014),T staging (x2 =19.484,P < 0.001),N staging (x2 =9.178,P =0.002),M staging (x2 =21.142,P < 0.001),tumor grade (x2 =47.005,P < 0.001),and progression (x2 =11.735,P =0.001),but it was not correlated with recurrence (x2 =0.808,P =0.369).Survival analysis showed that the specific survival rate in the 100 months of TOP2A gene high expression group and low expression group had a statistically significant difference (66.59% vs.87.95%,x2 =15.820,P < 0.001).The median overall survival time of TOP2A gene high expression group and low expression group were 51.77 months and 134.97 months respectively,with a statistically significant difference (x2 =11.280,P =0.008).The results of GSEA indicated that TOP2A could regulate gene sets related with several pathways like MYC-V1 signaling (P =0.035,FDR =0.132),MYC-V2 signaling (P =0.012,FDR =0.058),E2F signaling (P < 0.001,FDR =0.006) and G2M checkpoint (P =0.006,FDR =0.044).Conclusion The TOP2A gene expression is closely related with clinicopathological characteristics of patients with bladder cancer.TOP2A may function as a potential marker of prognosis for patients with bladder cancer.
3.Mortality and years of life lost of lung cancer among residents in Pudong New Area of Shanghai from 1995 to 2021

ObjectiveTo investigate the epidemiological traits and potential years of life lost associated with lung cancer mortality among inhabitants of Shanghai's Pudong New Area from 1995 to 2021, in order to serve as a reference for developing intervention approaches. MethodsThe death surveillance system was used to gather statistics on lung cancer deaths. Crude mortality rate (CMR), standardized mortality rate (SMR), potential years of life lost (PYLL), average years of life lost (AYLL), annual percent change (APC) of the lung cancer deaths were analyzed. The impact of age-structural and non-age-structural factors on changes in lung cancer mortality was quantified using difference decomposition. ResultsThe CMR and SMR of lung cancer among residents in Pudong New Area between 1995 and 2021 were 58.21/105 and 26.75/105, respectively. The CMR of lung cancer increased over the years (APC=1.91%, 95%CI=1.60%‒2.30%; Z=11.487, P<0.001), and the SMR of lung cancer declined over the years (APC=-1.50%, 95%CI=-1.80%‒-1.20%; Z=-9.006, P<0.001). Age structure factors and gender factors contributed to the increase of lung cancer mortality, while non-population age structure factors overall appeared to play a protective role which might be related to the improvements in factors such as tobacco control and environmental management. The PYLL of lung cancer was 160 296 person years, the PYLL rate was 2.24‰, and the AYLL was 3.86 years per person. ConclusionAge structure factors are a significant contributor to the disease burden and result in the increase in the crude lung cancer mortality rate of Pudong residents of shanghai. Comprehensive monitoring, preventive, and control methods should be implemented.
4.TCM constitution distribution and clinical features of patients with hepatitis B cirrhosis and dysplastic nodules

ObjectiveTo investigate the characteristics of TCM constitution distribution in hepatitis B cirrhosis patients with dysplastic nodules (DN), and to provide a basis for the prevention and treatment of precancerous lesions of liver cancer. MethodsThis study was conducted among 113 hepatitis B cirrhosis patients with DN, 105 hepatitis B cirrhosis patients with regenerative nodules (RN), and 70 hepatitis B cirrhosis patients with small hepatocellular carcinoma (sHCC) who were hospitalized in Guangdong Provincial Hospital of Traditional Chinese Medicine from May 2015 to March 2023. Related data were collected, including age, sex, liver function Child-Pugh class, TCM constitution type, and laboratory markers. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups; the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups; the chi-square test was used for comparison of categorical data between groups, and the Bonferroni correction method was used for further comparison between two groups. ResultsThe main TCM constitution types of hepatitis B cirrhosis patients with DN were Qi-deficiency constitution (27 patients, 23.89%), blood-stasis constitution (26 patients, 23.01%), and phlegm-dampness constitution (23 patients, 20.35%). There were significant differences between the three groups in the proportion of patients with phlegm-dampness constitution or damp-heat constitution (χ2=6.822 and 6.383, both P<0.05); the hepatitis B cirrhosis patients with RN had the highest proportion of patients with phlegm-dampness constitution (30.48%), followed by those with DN (20.35%) and those with sHCC (14.29%), while the hepatitis B cirrhosis patients with sHCC had the highest proportion of patients with damp-heat constitution (27.14%), followed by those with DN (16.81%) and those with RN (12.38%). There were significant differences between the hepatitis B cirrhosis DN patients with different TCM constitution types in sex, age, Child-Pugh class, prealbumin, albumin (Alb), aspartate aminotransferase, total bilirubin (TBil), total bile acid, and alpha-fetoprotein (all P<0.05). Compared with the male hepatitis B cirrhosis DN patients, female patients showed a significantly higher proportion of patients with Qi-deficiency constitution (χ2=4.895, P=0.027). Among the patients with Qi-deficiency constitution, the patients with Child-Pugh class A liver function accounted for a significantly lower proportion than those with Child-Pugh class B liver function (χ2=6.380, P=0.012), while among the patients with phlegm-dampness constitution, the patients with Child-Pugh class A liver function accounted for a significantly higher proportion than those with Child-Pugh class B liver function (χ2=8.515, P=0.004). The patients with phlegm-dampness constitution had significantly higher levels of prealbumin and Alb than those with the other four constitutions (all P<0.05), as well as significantly lower levels of TBil and total bile acid than those with damp-heat constitution (P<0.05); the patients with Yin-deficiency constitution had a significantly lower level of Alb than those with qi-deficiency constitution, blood-stasis constitution, or phlegm-dampness constitution (all P<0.05); the patients with Yin-deficiency constitution had a significantly lower proportion of patients with abnormal alpha-fetoprotein than those with non-Yin-deficiency constitutions (χ2=4.448, P=0.035). ConclusionHepatitis B cirrhosis patients with DN mainly have the TCM constitution types of Qi deficiency, blood stasis, and phlegm dampness. The patients with phlegm-dampness constitution seem to have a low probability of carcinogenesis, while those with damp-heat constitution and Yin-deficiency constitution have a relatively high risk of carcinogenesis.
5.Influence of gestational weight gain and preconception body mass index on overweight and obesity of school-age children

【Objective】 To understand the prevalence of overweight/obesity among school-age children in Pudong New Area of Shanghai, and to explore the influence of gestational weight gain and pre-pregnancy body mass index (BMI) on weight status of school-age children. 【Methods】 From November to December 2020,a stratified cluster sampling method was adopted to select first-grade students from 13 primary schools in Pudong New Area of Shanghai.After matching with the birth monitoring database, 755 students with complete birth information were selected as the study subjects.The relevant information of mothers before and during pregnancy was retrospectively collected, and the effects of pregnancy weight gain combined with pre-pregnancy BMI on overweight/obesity in school-age children were analyzed. 【Results】 1) The prevalence rates of overweight and obesity of first-grade children were 15.89% and 18.41%, respectively.2) Maternal excessive weight gain during pregnancy (OR=1.678) and overweight/obesity before pregnancy (OR=2.315,2.412) were risk factors for overweight/obesity of the offspring at school age(P<0.05).3) For mothers who were underweight before pregnancy, excessive weight gain during pregnancy was associated with overweight/obesity in school-age children in their offspring (OR=7.436, 95%CI: 1.489 - 37.143,P<0.05).4) Excessive weight gain during pregnancy combined with overweight/obesity before pregnancy significantly increased the risk of overweight/obesity in offspring (OR=3.606, 95%CI: 2.030 - 6.405, P<0.05). Mothers who gained a moderate amount of weight during pregnancy and were emaciated before pregnancy had a significantly lower risk of overweight/obesity in their school-age children (OR=0.217, 95%CI: 0.049 - 0.967, P<0.05). 【Conclusion】 Excessive weight gain during pregnancy increases the risk of overweight/obesity in school-age children in their offspring, strengthening pregnancy health education and perinatal care to help pregnant women maintain appropriate weight gain during pregnancy may be an important and novel strategy to prevent childhood obesity.
