1.Interventional treatment of primary liver cancer with spontaneous bleeding (3 cases report)

Objective To evaluate the efficiency of interventional treatment for primary liver cancer with spontaneous rupture. Methods Three cases of spontaneous rupture of primary liver cancer were undertaken hepatic arterial chemoembolization with prior chemo-infusion and followed by gelfoam partical and stripes for embolization Results 3 cases of massive type of primary liver carcinoma with spontaneous rupture were all successfully once embolized, including one with additional super liquefied lipiodal as embolic agent who has been survived for more than 20 months. Conclusions Hepatic arterial chemo-embolization shows prominent hemostasis during emergency for spontaneous rupture of primary hepatic carcinoma providing double effectiveness of hemostasis and treatment.
2.Research on a tool for organizational culture assessment of public general hospitals in China

Objective A scientific evaluation of hospital culture with the Dimension organizational culture model, in view of features of China's general public hospitals.Methods Based on Denison model, according to the characteristics of the public general hospitals in China, the authors developed a tool for organizational culture assessment (TOCA) by using the survey data from 87 hospitals in three provinces from the East, Central, and West areas in China.Results This tool, an evaluation scale, comprises the four cultural characteristics of direction, consistency, participation, and adaptability, as well as 13 cultural dimensions of social responsibility and competitive consciousness. The tool is tested as having good internal reliability and validity.Conclusion The TOCA provides hospital administrators with a tool for hospital culture evaluation, diagnosis and improvement.
3.Large-diameter balloon sphincteroplasty for removal of bile duct stones: a prospective and randomized study from two endoscopy centers

Objective To investigate the efficiency and safety of endoscopic large balloon dilation following small sphincterotomy (ESBD) for treatment of common bile duct (CBD) stones, and to prospectively compare it with traditional sphincterotomy (ES). Methods Patients with single or multiple calculi in extra-hepatic bile ducts and with the diameter of CBD larger than 13 mm, were randomized into 2 groups to receive maximal sphincterotomy (ES group), or partial sphincterotomy plus sphincteroplasty with 15mm-balloon (ESBD group), respectively, followed by conventional stones removal and/or lithotripsy. The success rate of stone clearance, operation time and related complications were observed. Results A total of 86 patients were enrolled in 2 endoscopy centers, with 2 excluded because of papillary pre-cut in 1 and non-compliance with the protocol in the other, and were randomly divided into groups ES and ESBD, with 42 patients in each. The clinical characteristics, including average diameter of CBD, size and quantity of calculi, and incidences of peri-ampullary diverticulum, of patients in both groups were all similar. Stones clearance with single session in 2 groups were 88% and 93%, respectively (P =0. 531 ). The operation time were (25.76 ± 12. 74) min and (26. 38 ± 12. 86) min ( P = 0. 825 ). The rate of mechanical lithotripsy was 36% and 25%, respectively (P=0. 363). Complications occurred in 5 cases in ES group (3 mild, 2 medium), and 2 mild complications occurred in ESBD group (no statistical difference). No death happened in any group.Conclusion ESBD is an effective method for clearance of extra-hepatic bile duct stones with the similar safety, compared with traditional sphincterotomy. It could be an optimal alternative for those with large stones or difficult sphincterotomy.
4.Over-expression of suppressor of cytokine signaling 3 inhibits the proliferation of human mesangial cells stimulated by aggregated IgA1 from IgA nephropathy patients

Objective To investigate the effect of suppressor of cytokine signaling 3 (SOCS3)on the proliferation of human mesangial cells stimulated by aggregated IgA1 (aIgA1) from patients with IgA nephropathy(IgAN),and explore its possible mechanism.Methods Serum monomeric IgA1 was isolated with jacalin affinity and Sephacryl S-200 HR chromatography from IgAN patients,and then heated to aggregated form (aIgA1).Human glomerular mesangial cells(HMC) were transfected with AdvSOCS3-IRES2-EGFP for 48 hours,and incubated with aIgA1 for 12-48 h.The cells were divided into blank control group,IgA1 group,IgA1 +Adv-EGFP group and IgA 1 +Adv-SOCS3-IRES2-EGFP group.The mesangial cell proliferation was observed through MTT,and the levels of SOCS3,TLR4,TGF-β1 protein and mRNA were detected through Western blotting and real-time PCR.Results HMC proliferation was promoted significantly after IgA1 stimulated at 24 h.Compared with control group,the protein and mRNA expression of SOCS3,TLR4,TGF-β1 were significantly increased in IgA1 group (P < 0.05).Compared with IgA1 group and IgA1 +Adv-EGFP group,MTT absorbency was obviously reduced after incubation with aIgA1 for 24 h and 48 h in IgA+Adv-SOCS3-IRES2-EGFP group,and the protein and mRNA expression of TLR4 and TGF-β1 were significantly decreased in IgA1 +AdvSOCS3-EGFP group (P< 0.05).Conclusion Over-expression of SOCS3 may inhibit the proliferation of HMC stimulated by aIgA1,partly through down-regulating the expression of TLR4 and TGF-β1.
5.Study on the cerebral protection of fasudil for hypoxic-ischemic cerebral damage of neonatal rats

Objective To investigate the effect of the cerebral protection and possible mechanism of fasudil for hypoxic-ischemic cerebral damage (HIBD) in neonatal rats. Methods The HBID model was established, then the mice were randomly divided into different groups. The expressions ofα-SMA and ROCK-2 were detected in the newborn rats with ischemia. Results Compared with the model group, expressions of α-SMA, ROCK-2 decreased in each treatment group with significant differences (P < 0.05 or P < 0.01). Following with the increases of administration dose and the administration time, expressions of α-SMA, ROCK-2 decreased gradually with significant differences (P<0.05 or P<0.01). Conclusion Fasudil can reduce the expressions of α-SMA, ROCK-2 in the newborn mice with hypoxic-ischemic brain damage to attenuate the brain tissue hypoxic-ischemic injury. The protective effect on brain is significant by giving high-dose fasudil in the early neonatal rat HIBD (0 h).
6.Establishment of a HL-60 cell line with stable RYBP silencing

Objective To establish stable HL-60 cell line with stable RYBP gene silencing using lentivirus-mediated RNA interference. Methods Five special shRNAs for RYBP gene were cloned to lentivirus vector.Recombinate lentivirus vectors were packed into lentivirus,which were used to infect HL-60 cells, and took empty vector and non-specific shRNA as control groups. Stable infected cells were selected with puromycin in 8 μg/ml concentration.The expression levels of RYBP were analyzed by Western blot,and confirmed the most effective RYBP shRNA.Then the level of mRNA was analyzed by real-time PCR.Results Stable infected cells were selected by puromycin successfully.Comparing to control groups,the expression of RYBP were reduced at different degrees (P < 0.01). And RYBP shRNA2 took the most silencing effect, the RYBP mRNA was decreased by more than 95 % (P < 0.05).Conclusion The shRNA2 targeting RYBP gene can effectively inhibit the expression of RYBP. HL-60 cell line with stable RYBP gene silencing were constructed successfully,which had provided experiment fundament for further studying the function of RYBP.
7.A novel mechanism of hepatitis B virus mutation in hepatitis B e antigen negative chronic hepatitis B infection

Objective To investigate mutation patterns in core promoter(CP)region of hepatitis B virus(HBV).Methods HBV DNA was extracted from sera of patients with chronic HBV infection.The CP sequence was amplified by polymerase chain reaction(PCR)and cloned into pMD19 T vector.The positive clones were then sequenced.The sequences were compared with known HBV genome in GenBank to identify the mutation sites and patterns of patients with chronic HBV infection.Results There were 74 clones from 21 patients with chronic HBV infection which were sequenced.The sequence comparisons showed that there was a 234-nucleotide deletion in CP region of HBV genome in 54 clones and a 245-nucleotide deletion in one clone.These deletion regions included CP,HBeAg initiation codon and direct repeat sequence(DR)Ⅰ regions,which named CP deletion(CPD).A1585T replacement mutation was also found in HBV strain with CPD,which indicated that there was linkage between these two mutations.Conclusions A novel mechanism of HBeAg negative chronic hepatitis B is observed,which includes deletions of CP and HBeAg initiation codon.Meanwhile,a simple and useful PCR method is developed to detect CPD.
8.Research progress of long-wavelength light-triggered photodynamic therapy for malignant tumor treatment

Malignant tumor is one of the most deadly diseases in the world. Researches focus on finding tumor therapy with better therapeutic efficiency and fewer side effects. Improving the therapeutic effect and reducing the side effects are the two hot topics in the field of malignant tumors treatment. As one of the new methods for non-invasive treatment of malignant tumors, photodynamic therapy (PDT) has the advantage of low cytoxicity and low drug resistance. PDT induces reactive oxygen species (ROS) production by irradiating light to specific sites, causing tumor cell apoptosis and necrosis, and achieving therapeutic purposes. Compared with the traditional PDT, long-wavelength light-triggered photodynamic therapy has deep tissue penetration and less cytoxicity. In this paper, the technical development of the long-wavelength light-triggered PDT was summarized including photosensitizers, two-photon activated PDT and upconversion PDT. The research progress of this therapeutic method in the treatment of malignant tumors was also reviewed.
9.Aplasia cutis congenita: clinical analysis of 258 cases reported in China

Objective To summarize the pathogenesis,diagnosis and treatment of aplasia cutis congenita (ACC) in China.Methods We reviewed and summarized all published ACC cases in China in the past four decades from China National Knowledge Internet(CNKI),Wan-Fang Medical Database and VIP database.Results Totally,258 cases were included,consisting of 144 males,95 females and 19 of unknown gender.Fifteen cases reported family history,16 had maternal morbidity during pregnancy,17 exposed to maternal medication or toxic material.Skin lesions of ACC occurred on the scalp (39 cases,15.1%),trunk (8 cases,3.1%) or limbs (107 cases,41.5%) and sometimes were manifested as multiples skin defects (104 cases,40.3%).Eighty-nine cases (38.4%) were presented with isolated skin defect,and 169 (61.6%) were complicated by other abnormalities or congenital defects.Totally,235(91.1%) cases were treated conservatively,15 cases with dural defect,skull defect or defect area > 10% underwent dermatoplasty,and the other eight cases refused any treatment.Among the 248 cases being followed up,233 (93.95%) healed after treatment for one week to four months,three had scar contracture,four developed joint contracture,and eight (3.2%) died of hemorrhage or infection.Conclusions ACC is a congenital skin defect which might involve skin,bone and multiple parts of the body,often complicated with other abnormalities.Superficial and small skin defect maybe managed conservatively,while for patients with bone exposure or defect,large area skin defect,surgical management is recommended.Appropriate management can ensure good outcomes in most cases.
10.Significance of serum adenosine deaminase level in type 1 diabetes patients with acute or chronic diabetes complications

Objective To investigate the significance of serum adenosine deaminase level in type 1 diabe-tes patients(T1DM)with acute or chronic diabetes complications. Methods One hundred and eighty-four cases of type 1 diabetes patients were divided into three groups:T1DM without any complications(n = 55),T1DM with acute diabetes complications(n = 47)and T1DM with chronic diabetes complications(n = 82). We also enrolled 108 healthy persons as the control group.The clinical and biochemical data were collected and the influencing fac-tors of serum ADA were analyzed. Resultes(1)Serum ADA level of T1DM patients was higher than that of the healthy control people.T1DM patients with acute diabetes complications had lower serum ADA level compared with T1DM patients with chronic complications.(2)Results of Pearson correlations analysis showed that serum ADA level had positive correlation with fasting blood glucose.(3)Logistic regression analysis showed that the increase of serum ADA level was an independent risk factor in patients with chronic diabetes complications(OR:1.159,95% Cl:1.044~1.286),while it was a protective factor in patients with acute diabetes complications(OR:0.857,95% Cl:0.748~0.981). Conclusions High serum ADA level was observed in type 1 diabetes patients. The serum ADA level was an independent risk factor for T1DM patients with chronic diabetes complications,while it was a protective factor for T1DM patients with acute diabetes complications.
