1.Application of monoclonal antibody in the natural medicine research

After the monoclonal antibody drug come out,it has played an important role in the clinical treatment,especially in tumor therapy.With the further understanding of monoclonal antibody,it has been applicated in the food, transgenic animal and plants,plant virology aspect. However,rare research of natural medicine have been reported in China.The monoclonal antibody-based enzyme-linked immunosorbent assay method, which is sensitivity,accuracy,fastness and simpler,can be used in quantitative analysis of bioactive compound in the natural products. It also can be used in isolation and purify effective component of the natural medicine, the pharmacokintic study,the quanlity control, screening candidacate drug and expanding the resources of medicinal plant It is important to exploit and utilizate the natural medicine,and promote the progess of modern chinese medicine.
2.Primary results of the 3-in-1 technique of Disc-FX system for the discogenic low back pain

ObjectiveTo investigate the clinical efficacy of the 3-in-1 technique of Disc-FX system,namely:discectomy,radiofrequency ablation and annuloplasty for discogenic low back pain in the early stage.MethodsFrom February 2010 to February 2011,40 patients with the discogenic low back pain underwent discectomy,radiofrequency ablation and annuloplasty using Disc-FX system were retrospectively analyzed,including 22 males and 18 females with an average age of 38.7 years(range,32-58 years).The visual analogue scale (VAS) scoring was applied to evaluate the back and low limb pain at preoperative,postoperative 1week,3 months,6 months,and last follow-up.The Macnab score was also evaluated at last follow-up.Results All the patients were followed up for average 13.8 months (range,6-18 months).The operative time averaged 26 min(range,20-40 min).No surgical complication was found during the follow-up.The VAS of preoperative low back pain was 6.60±1.47,VAS score of low back pain at postoperative 1 week,3 months,6 months,and final follow-up were 1.05±0.68,1.15±0.70,1.00±0.62,0.95±0.63,respectively.The postoperative VAS of low back pain decreased significantly compared with that preoperative.The VAS of preoperative limb pain was 3.05±1.23,VAS score of limb pain at postoperative 1 week,3 months,6 months,and final follow-up after operation were 1.10±0.74,1.15±0.70,1.10±0.72,0.95±0.54,respectively.The postoperative VAS of limb pain decreased significantly compared with that preoperative.The evaluation of Macnab score were excellent in 20 patients,good in 17,fair in 2,and poor in 1,suggesting an effective rate of 92.5% (37/40).Conclusion Disc-FX system,which combined three surgical technique in one procedure,provides a new choice for orthopedics to treat discogenic low back pain.It can give satisfactory clinical outcomes during a short-term followup,but the long term clinical outcomes remains unknown.
3. Progress in targeted therapy for high-risk diffuse large B cell lymphoma

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of adult lymphoma and is a group of invasive and heterogeneous diseases. Although rituximab in combination with CHOP regimen (R-CHOP) for DLBCL is better, one third of patients have relapsed/refractory conditions. DLBCL is divided into many subtypes due to its high heterogeneity. Different histological types have different response to treatment. High-risk DLBCL has little effect on R-CHOP treatment. How to further improve the first-line cure rate of high-risk DLBCL has become an important challenge in the field of lymphoma treatment. Currently in the era of precision medicine, in recent years, many new targeted drugs, such as immunosuppressive agents, mammalian target of rapamycin (mTOR) receptor inhibitors and Bruton tyrosine kinase (BTK) inhibitors, have been developed for DLBCL-related pathways and molecular targets, provide more new possibilities for the treatment of DLBCL.
4.Research progress on decision aids for diagnosis and treatment of colorectal cancer

5.Analysis of the incentive effect of the patent linkage system on the application of generic drugs in China and suggestions for countermeasures

OBJECTIVE To optimize the system of patent linkage system and provide a reference for encouraging generic drugs to apply for listing. METHODS On the basis of literature research, combined with the author’s work experience, the situation of generic drug application and patent challenge were analyzed comparatively after the implementation of the patent linkage system in China. Based on the patent linkage system, the reasons for the insufficient incentive for the challenge of generic drug patents in China were analyzed; corresponding countermeasures and suggestions were proposed. RESULTS & CONCLUSIONS The patent linkage system in China was implemented in 2021, but due to the implementation of the system being still in its initial stage, the relevant documents for practical guidance are not yet complete, and the supporting incentive mechanism is not yet sound, resulting in a weak willingness of generic pharmaceutical enterprises to initiate patent challenges. It is suggested that China can further improve the patent linkage system by improving the patent information registration platform of generic drugs, developing professional drug patent agencies, and establishing the patent common challenges platform of the first generic drug, in order to incentivize Chinese generic drug enterprises to increase innovation motivation, challenge original research drug patents, and apply for listing, achieving the effect of suppressing drug prices and benefiting the people through full competition.
6.Effect of improving water on endoplasmic reticulum stress in fluoride exposed rats

Objective To analyze the changes of endoplasmic reticulum stress (ERS) in the spleen of water-improving fluorosis rat,to explore the mechanism of fluoride-induced immune system damage,and to provide a scientific basis for prevention and control of endemic fluorosis.Methods Forty-eight male Wistar rats of SPF grade were randomly divided into control group and low,medium and high fluoride dose groups according to body mass (120-140 g),12 rats in each group.The sodium fluoride (NaF) content was 0,50,100 and 150 mg/L,respectively.The animals were allowed free access to water and food.After 12 weeks of fluoride exposure,6 rats in each group were selected to isolate the spleen;the remaining rats in each group were changed to drink distilled water containing no NaF,and the spleen was separated after 12 weeks of feeding.The levels of mRNA of glucoseregulated protein (GRP78),spliced X-box binding protein 1 (XBP1-s),activating transcription factor 4 (ATF4),homologous protein (CHOP) and cysteine containing aspartate specific protease 12 (Caspase-12) in spleen were determined by quantitative real-time PCR (qRT-PCR).Results Before the water-improving,the expressions of GRP78 (1.00 ± 0.09,1.69 ± 0.35,1.39 ± 0.29,1.19 ± 0.19),XBP1-s (1.00 ± 0.12,1.40 ± 0.23,1.24 ± 0.26,1.38 ± 0.11),ATF4 (1.00 ± 0.17,1.86 ± 0.56,2.33 ± 0.55,1.95 ± 0.74),CHOP (1.00 ± 0.53,2.84 ± 0.68,3.06 ± 1.29,2.50 ± 0.35) and Caspase-12(1.00 ± 0.12,1.90 ± 0.29,1.56 ± 0.35,1.76 ± 0.23) mRNA in the control group and low,medium and high fluoride dose groups were statistically significant (F =8.45,5.38,6.38,8.21,11.31,P < 0.05).Except for the GRP78 in high fluoride dose group,the above indicators in fluoride groups were higher than the control group (P < 0.05).After the water-improving,the expressions of GRP78 (1.00 ± 0.36,0.75 ± 0.13,0.98 ± 0.41,0.47 ± 0.19),XBP1-s (1.00 ± 0.25,0.70 ± 0.06,0.74 ± 0.17,0.65 ± 0.21),ATF4 (1.00 ± 0.51,0.66 ± 0.09,0.91 ± 0.34,0.81 ± 0.29),CHOP (1.00 ± 0.36,0.92 ± 0.12,0.84 ± 0.16,0.67 ± 0.20) and Caspase-12 (1.00 ± 0.45,0.65 ± 0.11,0.65 ± 0.25,0.51 ± 0.27) mRNA in the control group and low,medium and high fluoride dose groups were not statistically significant (P > 0.05).Before and after the water-improving,the expressions of XBP1-s,ATF4,CHOP and Caspase-12 mRNA were statistically significant in fluoride groups (P < 0.05),and the GRP78 only had a statistically significant difference in the low fluoride dose group (P < 0.05).Conclusions Fluoride exposure causes ERS response in rat spleen,up-regulation of ERS-related gene expression,which is decreased after water-improving,and the ERS response is weakened.The water-improving may contribute to the recovery of fluoride-induced immune function damage.
