1. Changes in serum thyroid hormone levels in psychiatric patients after risperidone monotherapy or polytherapy treatment

AIM: To investigate changes in serum thyroid hormone levels in psychiatric patients after risperidone monotherapy or polytherapy treatment. METHODS: Twenty-nine in-patients who received risperidone monotherapy treatment and 25 in-patients who received polytherapy treatment of risperidone and other second-generation antipsychotics were included. Changes in thyroid hormone levels after treatment were retrospectively analyzed. RESULTS: After risperidone monotherapy treatment, serum levels of free thyroxine (FT4) and total thyroxine (TT4) significantly decreased, and serum TSH levels significantly increased (changes from baseline: FT4: median -2.52 pmol/L, P<0.001; TT4: -21.17 nmol/L, P=0.012; TSH: median 0.49 mIU/L, P=0.001). After risperidone polytherapy treatment, serum levels of free triiodothyronine (FT3), total triiodothyronine (TT3) and FT4 significantly decreased, and serum TSH levels significantly increased (changes from baseline: FT3: median -0.37 pmol/L, P=0.001; TT3: median -0.17 nmol/L, P=0.008; FT4: median -2.25 pmol/L, P<0.001; TSH: median 0.97 mIU/L, P=0.029). After risperidone monotherapy treatment, moderately positive relation was found between changes in serum TT4 levels and average daily dose of risperidone. No statistical difference was found on the incidence of subclinical thyroid dysfunction between the two therapeutic regimen (6.9% vs. 12.0%, P=0.862). CONCLUSION: To avoid other disease caused by abnormal thyroid hormone levels, changes in serum thyroid hormone levels should be noticed after risperidone treatment.
2.Study on correlation between plasma paliperidone concentration and serum prolactin level

Objective To investigate the correlation between plasma paliperidone concentration and serum prolactin level .Meth‐ods The plasma paliperidone concentration and serum prolactin level at the ends of 0 ,1 ,2 ,4 ,6 ,8 weeks after taking paliperidone were separately detected in 49 patients with schizophrene (25 males and 24 females) .Then the statistical analysis was performed .Results The serum prolactin level in the schizophrenic patients had statistically significant difference before and after taking paliperidone(P<0 .05) .The increase of plasma paliperidone concentration could cause the increase of serum prolactin level .Conclusion Inthe application of paliper‐idone for treating schizophrene ,plasma paliperidone concentration and serum prolactin level should be monitored for improving the medication safety and decreasing the adverse drug reactions .
3.Pregnancy outcome of frozen-thawed embryo transfers in different endometrial progesterone preparation time

Objective To investigate the pregnancy outcome of frozen-thawed embryos transfer in different endometrial progesterone preparation time.Methods From January to December 2012,pregnant outcome of 1 103 frozen-thawed embryo transfer cycles using artificial periodic endometrial preparation were studied retrospectively in Reproductive Medical Center of Henan Provincial People's Hospital.It was divided into 4 groups:group 3-3 (n =543,3 days after endometrial progesterone preparation and transfer D3 embryo),group 4-3(n =330,4 days after endometrial progesterone preparation and transfer D3 embryo),group 5-5 (n =150,5 days after endometrial progesterone preparation and transfer D5 blastula),group 6-5 (n =80,6 days after endometrial progesterone preparation and transfer D5 blastula).The rate of implantation,pregnancy,ectopic pregnancy,multiple pregnancy,and first trimester abortion were compared among those groups.Results (1) With the different endometrial progesterone preparation methods and transfer D3 embryos,implantation rate [group 3-3:39.9% (429/1 074) ; group 4-3:44.1% (286/648)],pregnancy rate [group 3-3:56.0% (304/543) ; group 4-3:59.4% (196/330)],ectopic pregnancy rate [group 3-3:3.3% (10/304) ; group 4-3:2.6% (5/196)],multiple pregnancy rate [group 3-3:38.5% (117/304) ; group 4-3:43.4% (85/196)]and early abortion rate[group 3-3:13.5% (41/304); group 4-3:13.3% (26/196)] had no significant differences between group 3-3 and group 4-3 (all P > 0.05).(2) With the different endometrial progesterone preparation methods and transfer D5 blastocysts,implantation rate [group 5-5:64.7% (191/295) ; group 6-5:69.4% (100/144)],pregnancy rate [group 5-5:80.7% (121/150) ; group 6-5:78.8% (63/80)],ectopic pregnancy rate [group 5-5:2.5% (3/121) ; group 6-5:0],multiple pregnancy rate[group 5-5:55.4% (67/121) ; group 6-5:46.3% (37/80)] and early abortion rate[group 5-5:5.8% (7/121); group 6-5:7.9% (5/63)].However,there were no significantly differences between group 5-5 and group 6-5 (all P > 0.05).Conclusions The two different progesterone transformed endometrial methods can obtain satisfactory clinical outcome with D3 embryo or D5 blastocysts transfor.It is convenient to clinical and laboratory work arrangements.
4.Role of endometrial and subendometrial blood flow index assessment in predicting pregnancy outcome of freezing embryo transfer cycles

Objective To evaluate the role of transvaginal color Doppler ultrasound measurements of endometrial thickness,endometrium and subendometrial blood distribution patterns and flow index in predicting endometrial receptivity and pregnancy outcome of freezing embryo transfer(FET).Methods 468 cycles FET were analysed retrospectively.Endometrial thickness,endometrium and subendometrial blood distribution patterns,pulsatility index (PI),resistance index (RI),systolic /diastolic ratio (S/D) of endometrial and subendometrial blood flows with transvaginal ultrasonography were measured on the day of progesterone initiation.Based on the clinical pregnancy or not,patients were divided into two groups.The comparison of these indicators were carried out between pregnant group and non pregnant group.Results The endometrium and subendometrial blood distribution patterns had statistical difference between the two groups(P <0.05),the clinical pregnancy rate of Ⅰ,Ⅱ,Ⅲ blood distribution patterns were 31.0%,62.5%,79.0% respectively.In the pregnancy group,the S/D,PI,and RI of endometrium were significantly lower than those in the nonpregnant group (P < 0.05),and there was no significant difference between the pregnant group and nonpregnant group in endometrial thickness(P >0.05).Conclusions Endometrium and subendometrial blood distribution patterns and flow index measured with transvaginal color Doppler ultrasound could assess endometrial receptivity and guide the time selecting of FET transplantation.
5.External Quality Assessment of Clozapine for Therapeutic Drug Monitoring in Guangdong Province

Objective:To implement the external quality assessment ( EQA) of clozapine for therapeutic drug monitoring ( TDM) among psychiatric hospitals in Guangdong. Methods:The parameters of HPLC used by the hospitals were understood through question-naires. The EQA of clozapine for TDM was implemented by distributing the quality control serum. Results:The qualified rate of cloza-pine in the quality control serum at low, medium and high concentrations for TDM was 66. 67%, 66. 67% and 83. 33%, respectively. Conclusion:It is necessary to implement EQA of clozapine for TDM in Guangdong.
6.Osteotomy of mandible ascending ramus by “a cover of box” manner provides a surgical approach to large tumors in the parapharyngeal space

Objective:To explore an improved osteotomy of mandible ascending ramus for the surgical approach to large tumors in parapharyngeal space.Methods:According to the operation of parotid gland, masseter muscle was cut near the border of mandibular angle till subperiosteum dissection to mandibular notch, vertical osteotomy outside mandibular foramen to 1.0 cm under mandibular notch, vertical to posterior border of ascending ramus, tumor was exposed and removed, bone plate was repositioned and fixed with titanium. Results:Tumors were completely removed in this way in 3 patients without complications. Conclusion:This surgical approach is suitable for the surgical removal of parapharyngeal interstitial giantism tumor.
7.Effects of interaction between dengue virus type 2-infected HUVECs and human CD4+T cells on the expression of adhesion molecules and immunosuppressive factors

Objective To investigate the effects of interaction between human umbilical vein endothelial cells (HUVECs) which were infected with dengue virus type 2 (DENV-2) and CD4+T cells on the expression of ICAM-1 (intercellular adhesion molecule 1),VCAM-1 (vascular cell adhesion molecule 1),IL-10 and TGF-β1 at mRNA level for further understanding the immunological mechanism of DENV infection.Methods HUVECs were treated with CYM-5442,a selective agonist for sphingosine-1-phosphate receptor 1 (S1P1),for 24 hours and then infected with 103 TCID50 (50% tissue culture infective dose) of DENV-2 before co-culturing with CD4+T cells.Changes in the expression of NS1 (DENV-2 nonstructural protein),SPHK1 (sphingosine kinase 1,phosphorylating sphingosine to S1P),ICAM-1,VCAM-1,IL-10 and TGF-β1 at mRNA level were detected by real-time PCR after 4,8,12,24,48 and 72 hours of co-culturing.Results There was a certain timeliness in the expression of NS1 at mRNA level after infecting HUVECs with DENV-2 and the expression reached a peak at 24 h.Treating HUVECs with or without CYM-5442 had no significant influence on the expression of DENV-2 NS1 at mRNA level.The expression of SPHK1 at mRNA level was significantly increased after treating HUVECs with CYM-5442 and DENV-2 (P<0.05).Compared with DENV-2-infected or untreated HUVECs,Co-culturing DENV-2-infected HUVECs with CD4+T cells increased the expression of ICAM-1 and VCAM-1 in HUVECs at mRNA level (P<0.01) as well as the expression of IL-10 in CD4+T cells at mRNA level (P<0.05),but had no significant influence on the expression of TGF-β1 in CD4+T cells at mRNA level.Conclusion This study shows that DENV-2 can replicate and proliferate in HUVECs,but CD4+T cells inhibit the replication and proliferation.CD4+T cells play an important role in promoting the expression of VCAM-1 and ICAM-1 in DENV-2-infected HUVECs at mRNA level,activating HUVECs and increasing inflammation,which may be associated with increased vascular permeability induced by DENV-2 infection.Co-culturing CD4+T cells with DENV-2-infected HUVECs promotes the expression of IL-10 in CD4+T cells at mRNA level,but has no significant effect on TGF-β1.
8.Application of amide proton transfer imaging in differentiating glioma from treatment effect

Objective To explore the application of amide proton transfer (APT) imaging in differentiating glioma from treatment effect and to evaluate the diagnostic efficiency of the quantitative APT-related parameters.Methods A total of 23 patients (15 males, 8 females, age: 13-80 years) with 27 lesions who had underwent APT imaging in Tongji Hospital(Wuhan, China) from October 2014 to June 2015 were enrolled in this prospective study.The scan protocols were MRI normal plain scanning, diffusion WI, contrast-enhancement T1WI and APT imaging.Both the magnetization transfer ratio (MTR) and the relative MTR (rMTR) of lesions were manually measured by drawing ROI in the functional post-processing workstation.The results were compared with those of pathologic examinations and radiographic follow-up (≥3 months).Mann-Whitney u test was used to analyze the data.Results Compared with contralateral white matter, the primary gliomas (n=12) and recurrent gliomas (n=8) manifested hyper-intensity, while the treatment induced injuries (n=7) showed iso-or hypo-intensity.The difference of MTR between tumors and treatment effects was significant (102.78(101.93,103.84) vs 100.17(99.94, 100.63);z=-3.76, P<0.01), so was the difference of rMTR between tumors and treatment effects (3.92%(2.69%,4.67%) vs 0.47%(-0.79%,1.11%);z=-3.43, P<0.01).Both those two quantitative parameters exhibited excellent diagnostic performance with the AUC of 0.986 and 0.943.The sensitivity, specificity and accuracy of MTR were 100%(20/20), 6/7 and 96.3%(26/27) in the threshold of 100.68, while those of rMTR were 95.0%(19/20), 6/7 and 92.6%(25/27) in the threshold of 1.66%.Conclusions Combined with the routine MRI images, APT imaging can provide excellent qualitative and quantitative information in differentiating glioma from treatment effect.Both MTR and rMTR are helpful for the differentiation with high sensitivity and specificity and can be used as non-invasive imaging biomarkers in evaluating treatment effect of glioma.
9. Different cosmetic incision selection in parotidectomy

Objective:
Based on the nature, location and size of parotid tumor, different incisions were chosen and method of selecting the best cosmetic incision for parotid gland tumor operation would be discussed.
Methods:
33 cases of parotid benign tumor patients that received by Department of Oral and Maxillofacial surgery of First Affiliated Hospital of Nanchang University during 2015-2017 were included in this study. According to the size of the tumor location, different incisions, mainly pretragal vertical, pretragal crutch and rhytidectomy incision were selected. The facial nerve and the great auricular nerve were protected during the operation. In 1-month and 3-month follow-up, clinical examination of temporary facial paralysis, salivary fistula, especially the cosmetic outcome after the surgery were evaluated.
Results:
The satisfaction rate of parotid cosmetic incision was significantly higher than that of the traditional group, the incidence of transient facial paralysis, salivary fistula was not statistically significant compared with the traditional incision.
Conclusions
According to the location of parotid tumor, we should choose the proper cosmetic incision approach, which is better than the traditional surgical incision.
10.Percentile-Based Analysis of Non-Gaussian Diffusion Parameters for Improved Glioma Grading

The purpose of this study is to systematically determine an optimal percentile cutoff in histogram analysis for calculating the mean parameters obtained from a nonGaussian continuous-time random-walk (CTRW) diffusion model for differentiating individual glioma grades. This retrospective study included 90 patients with histopathologically proven gliomas (42 grade II, 19 grade III, and 29 grade IV). We performed diffusion-weighted imaging using 17 b-values (0-4000 s/mm²) at 3T, and analyzed the images with the CTRW model to produce an anomalous diffusion coefficient (D m ) along with temporal (α) and spatial (β) diffusion heterogeneity parameters. Given the tumor ROIs, we created a histogram of each parameter; computed the P-values (using a Student’s t-test) for the statistical differences in the mean D m , α, or β for differentiating grade II vs. grade III gliomas and grade III vs. grade IV gliomas at different percentiles (1% to 100%); and selected the highest percentile with P < 0.05 as the optimal percentile. We used the mean parameter values calculated from the optimal percentile cut-offs to do a receiver operating characteristic (ROC) analysis based on individual parameters or their combinations.We compared the results with those obtained by averaging data over the entire region of interest (i.e., 100th percentile). We found the optimal percentiles for D m , α, and β to be 68%, 75%, and 100% for differentiating grade II vs. III and 58%, 19%, and 100% for differentiating grade III vs. IV gliomas, respectively. The optimal percentile cut-offs outperformed the entire-ROI-based analysis in sensitivity (0.761 vs. 0.690), specificity (0.578 vs. 0.526), accuracy (0.704 vs. 0.639), and AUC (0.671 vs.0.599) for grade II vs. III differentiations and in sensitivity (0.789 vs. 0.578) and AUC (0.637 vs. 0.620) for grade III vs. IV differentiations, respectively. Percentile-based histogram analysis, coupled with the multi-parametric approach enabled by the CTRW diffusion model using high b-values, can improve glioma grading.
