Objective To investigate the feasibility and safety of colorectal anastomosis using nickel titanium compression anastomosis ring (CAR).MethodsThe clinical data of 83 patients who received colorectal anastomosis at the First Affiliated Hospital of Fujian Medical University from May to December,2010 were prospectively analyzed. All patients were divided into CAR group (41 patients) and stapler group (42 patients) according to the random number table. The operation time, time to postoperative flatus and bowel movement, duration of hospital stay and the incidence of anastomosis-related complications of the 2 groups were observed. The time of extrusion of the rings and anastomotic healing of the CAR group were recorded. All data were analyzed by the t test or chi-square test.ResultsThe mean operation time of the CAR group and the stapler group were ( 122 ± 25) minutes and (128 ±30)minutes,respectively.The incidences of postoperative anastomotic fistula of the CAR group and the stapler group were 5% (2/41) and 7% (3/42),and the anastomotic fistula was cured by operation or conservative treatment. There were no significant differences in the mean operation time, time to postoperative flatus and bowel movement, duration of hospital stay and incidence of anastomotic fistula between the 2 groups (t =1.299,0.756,1.636,0.974,x2 =0.165,P > 0.05 ).The rings were expelled within 9-14 days. The incidences of anastomotic stenosis of the CAR group and stapler group were 0 (0/41) and 2% (1/42),with no significant difference between the 2 groups (x2 =0.988,P > 0.05).The anastomotic stoma was smooth and well bealed in the CAR group, while nail residues and inflammatory polyp (1 patient) were found at the anastomosis stoma in the stapler group.ConclusionNickel titanium CAR is safe and effective in colorectal anastomosis.
3. Combination of Serum Helicobacter pylori Antibody With Pepsinogens and OLGA/OLGIM Staging System for Risk Assessment of Gastric Precancerous Lesions
Chinese Journal of Gastroenterology 2019;24(2):71-75
Background: Serum pepsinogens (PGs), as an indicator of gastric mucosal atrophy, reflects the functional and morphological status of gastric mucosa. And OLGA/OLGIM, the staging system of gastritis integrating the severity and topography of gastric mucosal atrophy/intestinal metaplasia, has been gradually accepted and used in gastric cancer screening in recent years. Aims: To investigate the correlation between ABC method [combined assay for serum Helicobacter pylori (Hp) antibody and PGs] and OLGA/OLGIM staging system, as well as the role of PGs test in risk assessment of gastric precancerous lesions. Methods: A total of 331 patients undergoing gastroscopy for upper gastrointestinal symptoms from Jan. 2017 to Jan. 2018 at the First Hospital of Jiaxing were enrolled. According to the results of serum tests and biopsy pathology, these patients were divided into four groups by ABC method and five groups by OLGA/OLGIM staging system, respectively. Hp infection rate, serum levels of PG and PGⅡ, and PG/PGⅡ ratio (PGR) were compared between different OLGA/OLGIM groups, and the correlations between OLGA/OLGIM staging system and ABC method were analyzed. Results: For OLGA/OLGIM staging system, the Hp infection rate was significantly lower in stage-0 and significantly higher in stage- (P<0.05), whereas the PGR decreased with the rising of stage (P<0.05). Only in OLGA groups, the PG level decreased with the rising of stage (P<0.05). Gamma coefficient analysis showed significant correlation between OLGA/OLGIM staging system and ABC method (G=0.589, P<0.05; G=0.440, P<0.05). Conclusions: Serological ABC method and histological OLGA/OLGIM staging system are closely linked in risk assessment of gastric precancerous lesions. Serum PGs test could be applied for screening of gastric precancerous lesions in China, identifying high risk population for further gastroscopy.

The incidence of myopia among Chinese adolescents is progressively rising, indicating a distinct trend toward younger age onset. This paper aims to comprehensively review the impact of various visual performance on myopia and its progression, with a specific emphasis on accommodative function, convergence function, and ocular position. A meticulous exploration of accommodation function, encompassing accommodative amplitude, accommodative facility, accommodative response, positive relative accommodation, and negative relative accommodation, has been undertaken to elucidate its contributory role in myopia progression. Concurrently, an exhaustive analysis of convergence function has been conducted including esotropia and exotropia, convergence insufficiency and convergence excess, fusional function vergence, divergence insufficiency, and excess, providing a nuanced understanding of convergence's implications for myopia advancement. Furthermore, the influence of ocular position on myopia progression, along with other factors affecting perceptual ocular position and intermittent exotropia, is discussed. The primary objective of this article is to unveil the multifaceted visual performance influencing myopia and its progression, elucidating the paramount significance of accommodative function, convergence function, and ocular position in this context.

Journal of Zhejiang University. Medical sciences 2013;42(3):326-330
OBJECTIVETo investigate the protective effect of dexmedetomidine (Dex) preconditioning against ischemia/reperfusion (I/R) injuries in isolated rat hearts and its relation to mitochondrial permeability transition pore (mPTP) and mitochondrial ATP-sensitive K(+) channel (mitoKATP).
METHODSThe hearts of male SD rats were isolated to mount on the Langendorff apparatus and subjected to 30 min global ischemia followed by 120 min reperfusion. The isolated hearts were treated with Dex (10 nmol/L) before ischemia for 15 min. The left ventricular hemodynamic parameters, coronary flow (CF) and the lactate dehydrogenase (LDH) release in the coronary effluent at 5 min reperfusion were measured. The formazan content was assayed to determine the myocardial viability at the end of reperfusion.
RESULTSCompared with normal controls, I/R markedly decreased the left ventricular developed pressure and CF during the whole reperfusion period and the formazan content; while the left ventricular end diastolic pressure and LDH release were significantly increased. Dex preconditioning markedly improved the myocardial viability and cardiac function (P<0.01), which were reversed by the treatment with both atractyloside (20 μmol/L before ischemia), an opener of mPTP, and 5-hydroxydecanoate (100 μmol/L at the beginning of reperfusion), an inhibitor of mitoKATP, for 20 min.
CONCLUSIONDex has protective effect against I/R injuries in isolated rat hearts, which may be related to inhibiting the opening of mPTP at the beginning of reperfusion and activating mitoKATP before ischemia.
