1.Investigation and Analysis of 3425 Prescriptions of Traditional Chinese Medicines in Our Hospital

OBJECTIVE:To offer guidance for pharmacy work and provide references for rational use of traditional Chinese medicines. METHODS: 3 425 outpatient prescriptions of traditional Chinese medicines in our hospital in 2006 were randomly collected for the analysis of the drug use pattern by tabulation method. RESULTS & CONCLUSION: The use of traditional Chinese medicines prescribed in our hospital was rational on the whole, but the prescriptions of traditional Chinese medicines for diseases of respiratory system were on the high side, and the diagnoses in the prescriptions were more like the diagnoses of west medicines but lacked the characteristics of traditional Chinese medicine, so that the international classification coding of disease was not suitable for traditional Chinese medicine. Medication frequency of traditional Chinese medicines serves as guidance for the dispensary of traditional Chinese medicine. The hospital information system is far from perfect.

3.Analysis of early complications after surgery for portal hypertension

Objective To investigate the relevant factors for development of early postoperative complications in patients with portal hypertension due to advanced schistosomiasis, and to offer a theoretical basis for preventing the occurrence of postoperative complications. Methods The cases of advanced schistosomiasis with portal (hypertension) operated upon over the past 40 years were retrospectively reviewed. Results In 8 240 cases, the complication rate was 19.66% at one month after operation, and the mortality rate was 15.31%. The main causes of death were MDOS, massive hemorrhage into abdominal cavity, hepatorenal syndrome and (recurrence) of bleeding of upper alimentary tract. Conclusions The relevant factors for development of early complications after operation in patients with portal hypertension is related to the cause of portal hypertension, whether or not there is a history of ascites, the timing of operation and the stage of liver function(Child′s classification).
4.Management of invasive cervical cancer in pregnancy: clinical analysis of 13 cases

Objective To analyze the clinical characteristics and assess the outcome of treatment for cervical cancer during pregnancy.Methods A cohort of 13 patients with cervical cancer diagnosed during pregnancy from January 2001 to September 2011 in Peking Union Medical College Hospital (PUMCH) was retrospectively studied.Clinical information,gestational age at diagnosis,treatment options and maternal and child outcomes were collected and analyzed.Results Thirteen patients out of 2030 cases of invasive cervical cancer were diagnosed during pregnancy with an incidence of 0.64% (13/2030).The Mean gestational age at diagnosis of 13 patients is 21+6 weeks.Two cases were diagnosed during the first trimester,8 cases at second trimester and 3 cases at third trimester respectively.Vaginal bleeding during the pregnancy was main clinical manifestation presented in 8 patients and all thirteen cases were diagnosed by biopsy with pathological types of squamous cell carcinoma in 10 cases.The International Federation of Gynecology and Obstetrics (FIGO) stage was Ⅰ in eleven cases and stage Ⅱ in two cases.Six patients of them received treatment promptly after diagnosis.The other 7 patients had delayed treatment with mean diagnosis-treatment interval time of 65 days due to fertility reasons,who ended pregnancy by cesarean section at mean gestational age of 34+6 weeks,two of them received chemotherapy with cisplatin + fiuorouracil (PF)or cisplatin respectively before the end of the pregnancy,while the one with PF chemotherapy experienced neonatal death.The rest 6 neonatal outcomes were good.As follow-up of 13 cases:11 cases in stage Ⅰ received surgical treatment,and two of which had recurrence respectively,15 months and 7 months post surgery,and one case had died.One case of Stage Ⅱ patients died and one had recurrence after 53 months after radiotherapy.The recurrence rate in 13 cases was 3/13 and the mortality rate was 2/13.Conclusions Most cases of cervical cancer diagnosed during pregnancy were in early FIGO stage.For those patients diagnosed in late pregnancy with strong fertility demand,considering delayed treatment according to FIGO stage of the disease and fetus maturity is appropriate.Chemotherapy during pregnancy may cause neonatal complications.
5.Transcranial Doppler ultrasound for assessment of dynamic cerebral autoregulation in patients with acute ischemic stroke

Transcranial Doppler (TCD) is a method for measuring cerebral artery blood flow velocity, which has the advantages of low cost, non-invasive, high temporal resolution. Dynamic cerebral autoregulation (dCA) is to study the instantaneous change of cerebral blood flow (CBF) at the moment of arterial blood pressure or intracranial pressure change, that is, the ability to recover CBF in the face of sudden change of perfusion pressure. After the onset of acute ischemic stroke (AIS), effective dCA can maintain the stability of cerebral hemodynamics and avoid excessive or insufficient perfusion at the injured site. Therefore, it is of great significance to evaluate dCA in patients with AIS. However, at present, there is no recognized optimal evaluation method for dCA. This article mainly summarizes the advantages of TCD in evaluating dCA compared with other methods and the application of TCD combined with transfer function analysis (TFA) in evaluating dCA in AIS.
6.Study on TCM Syndromes of Advanced Gastric Cancer Based on Principal Component Analysis and Clustering Analysis

Objective To analyze the characteristics and distribution of TCM syndromes of advanced gastric cancer;To provide reference for the standardization and clinical research of TCM syndromes of advanced gastric cancer.Methods The four diagnosis information with advanced gastric cancer was retrospectively collected at Dongzhimen Hospital of Beijing University of Chinese Medicine from January 2010 to December 2020.And the investigation results were analyzed by combining principal component analysis and clustering analysis,so as to explore the distribution pattern of TCM syndromes of advanced gastric cancer.Results Totally 164 patients were included,involving 601 case-times.10 principal components were obtained through principal component analysis on 29 items of four diagnosis information.The four diagnosis information with factor coefficient>0.4 were selected and allocated to the 10 principal components.Then,based on the results of principal component analysis,clustering analysis was conducted to obtain the distribution proportion of the three types of TCM syndromes.According to the syndrome differentiation by professional clinicians,the results were followed by the frequency distribution as cold coagulation and blood stasis(356,59.28%),stomach yin deficiency(145,24.17%)and phlegm-heat accumulation(100,16.55%).Conclusion There are three basic TCM syndromes of advanced gastric cancer,which are cold coagulation and blood stasis,stomach yin deficiency and phlegm-heat accumulation.Cold coagulation and blood stasis occupies the largest proportion,and the treatment should be based on warming the middle to dissipate cold and promoting blood circulation to remove blood stasis.

Long non-coding RNAs (lncRNAs) are a class of non-coding transcripts which are greater than 200 nucleotides in length and have a variety of biological functions. Studies have found that lncRNAs play an important role in the development of gastrointestinal cancers and can affect tumor cell growth, angiogenesis, metastasis and drug resistance. This paper has reviewed lncRNAs associated with gastrointestinal cancers and explored their roles in the occurrence, diagnosis and treatment of gastrointestinal cancers.

8.Study on the anti-RSV activity and mechanism of Mentha haplocalyx Briq.

Objective:To explore the inhibitory effects of Mentha haplocalyx Briq. With different extraction methods on respiratory syncytial virus (RSV) in vitro and antiviral mechanism of it seffective parts. Methods:By using the virus infection model in vitro to detect the cytopathic effect (CPE) of the anti-RSV effect of Mentha haplocalyx Briq. in vitro. Make the anti-RSV therapeutic index (TI) in vitroas the index to screen the extraction method of mint and the macro-porous adsorption resin, so as to determine the best extraction method, and the best macroporous adsorption resin was used for separation and purification. Elution with different concentrations of eluent was used to determine the effective parts of Mentha haplocalyx Briq. resisting RSV. The antiviral mechanism was analyzed by different administration methods. Results:The supernatant of Mint after water extraction and alcohol precipitation had the strongest inhibitory effect on RSV, with TI value of 37.58; HPD100 resin had the best separation and purification effect on the supernatant, and 25% ethanol elution site had the best effect, and the TI value was 57.8. The effective substances were determined to be polyphenols by physicochemical identification reaction. After the effective parts are mixed with the virus, the antiviral experiment is the best.Conclusions:The supernatant of mint after water extraction and alcohol precipitation was adsorbed by HPD100, and 25% ethanol eluent had the best anti-RSV effect, with preventive and therapeutic effects.
9.Intervention ef fect of clinical nursing pathway on the contrast induced nephropathy in percutaneous coronary intervention patients

Objective:To investigate the effect of clinical nursing pathway in prevention of contrast induced nephropathy in percutaneous coronary intervention patients.Methods:A total of 118 cases of coronary heart disease patients who had undergo percutaneous coronary intervention in hospital from May 2018 to May 2019 were randomly divided into the intervention group ( n=59) and the control group ( n=59). Participants in the control group received routine nursing, while the intervention group carried out clinical nursing pathway nursing method. The renal index such as serum creatinine, blood urea nitrogen, urine β 2 microglobulin, N-acetyl-beta-D-glucosidase (NAG) were compared before and after intervention between two groups, the incidence of CIN were also compared. Meanwhile, the psychology status and sleep quality was assessed by self-rating anxiety scale (SAS), self-rating depression scale (SDS) and Pittsburgh sleep quality index (PSQI), respectively. Results:The 3 rd day after the operation, serum creatinine, serum urea nitrogen, urine β 2 microglobulin, NAG in the intervention group and control group were (81.06±15.60) μmol/L, (9.43 ± 2.73) mmol/L, (256.87 ± 18.99) μg/L, (19.56 ± 2.44) U/L and (87.87 ± 19.60) μmol/L, (10.55 ± 2.18) mmol/L, (270.45 ± 40.85) μg/L, (20.60 ± 2.13) U/L, respectively. The levels of serum creatinine, serum urea nitrogen, urine β 2 microglobulin, NAG were significantly increased in the intervention group compared to the control group ( t=2.087-2.464, P<0.05). The incidence of CIN in the intervention group were 3.4% (2/59) and 13.6% (8/59) in the control group, the differences had statistical significance ( χ2=3.933, P<0.05). In addition, the scores of SAS, SDS and sleep quality, sleep time, sleep duration, daytime function and total PSQI score were (44.71 ± 8.20), (41.36 ± 6.52), (0.78 ± 0.11), (1.02 ± 0.15), (1.20 ± 0.19), (0.97 ± 0.27), (6.42 ± 0.54), those index were (48.85 ± 6.52), (46.49 ± 8.29), (1.03 ± 0.21), (1.23 ± 0.28), (1.44 ± 0.30), (1.30 ± 0.28), (7.79 ± 0.69), the differences had statistical significance ( t=3.033-12.016, P<0.05). Conclusion:Clinical nursing pathway can improve renal function, reduce the incidence of contrast-induced nephropathy, and improve psychological status and sleep quality of percutaneous coronary intervention patients.
10.Expression of four-jointed box kinase 1 gene in colorectal cancer and its clinical significance

Objective:To investigate the differential expression of four-jointed box kinase 1 (FJX1) gene in colorectal cancer and its relationship with prognosis and the related mechanisms.Methods:On July 16, 2021, the transcriptome data and clinical data of colorectal cancer were downloaded from The Cancer Genome Atlas (TCGA) database to analyze the expressions of FJX1 mRNA in colorectal cancer tissues and paracancerous tissues, and the relationship between FJX1 mRNA and clinicopathological characteristics and prognosis of patients. Receiver operating characteristic (ROC) curve was drawn to evaluate the value of FJX1 mRNA in predicting the survival of patients with colorectal cancer. Cox proportional hazards model was used to evaluate whether FJX1 mRNA was an independent influencing factor for prognosis of colorectal cancer. The overall survival (OS) time and survival status of colorectal cancer patients were downloaded from the Gene Expression Omnibus (GEO) database, and the relationship between FJX1 mRNA and prognosis of patients was analyzed. The methylation data of colorectal cancer was downloaded from the University of California, Santa Cruz (UCSC xena) database to determine the degree of methylation at each site of FJX1 mRNA and the correlation between the expression of FJX1 mRNA and the degree of methylation at each site. Signaling pathways associated with FJX1 mRNA in colorectal cancer were analyzed by using the Gene Set Enrichment Analysis (GSEA) (4.1.0). The correlation between FJX1 mRNA and tumor-infiltrating immune cells was investigated by using the Tumor Immunity Evaluation Resource (TIMER) database. Spearman analysis and small molecule/drug sensitivity analysis were used to explore the correlation between FJX1 mRNA expression and drug sensitivity.Results:In the transcriptome data of 612 colorectal cancer cases in TCGA database, the expression of FJX1 mRNA in colorectal cancer tissues was higher than that in the paracancerous tissues ( P < 0.001). In 549 colorectal cancer patients with complete data, FJX1 mRNA expression was correlated with M stage ( P = 0.007), pathological stage (stage Ⅳ vs. stage Ⅰ, P = 0.016; stage Ⅳ vs. stage Ⅱ, P = 0.03; stage Ⅳ vs. stage Ⅲ, P = 0.012), but it was not correlated with age, gender, T stage and N stage (all P > 0.05). In TCGA database and GEO database, the patients were divided into high expression group and low expression group according to the median expression of FJX1 mRNA. The OS in FJX1 mRNA high expression group was worse than that in low expression group (all P<0.05). The ROC curve of FJX1 mRNA expression on the 1-, 3-, and 5-year OS rates of colorectal cancer patients was drawn by using the data in TCGA database, and the areas under the curve (AUC) were 0.595, 0.625 and 0.764, respectively. Multivariate Cox regression analysis showed that age ( HR = 1.050, 95% CI 1.028-1.073, P < 0.001), T stage ( HR = 1.787, 95% CI 1.090-2.927, P = 0.021) and high FJX1 mRNA expression ( HR = 1.160, 95% CI 1.049-1.282, P = 0.004) were independent influencing factors for poor OS in colorectal cancer. The gene set enrichment analysis found that FJX1 mRNA was related to colorectal cancer, TGF-β signaling pathway, VEGF signaling pathway, Wnt signaling pathway, etc. The expression of FJX1 mRNA in colon cancer was negatively correlated with the degree of methylation of FJX1 mRNA ( r = -0.16, P < 0.001), and the expression of FJX1 mRNA in rectal cancer was positively correlated with the degree of methylation of FJX1 mRNA ( r = 0.33, P < 0.001). The expression of FJX1 mRNA was related to the infiltration of resting memory CD4 + T cells, M0 macrophages and resting dendritic cells. FJX1 mRNA was significantly associated with the resistance of various chemotherapeutic drugs and tumor-targeted drugs such as methotrexate, 5-fluorouracil, gefitinib, etc. Conclusions:FJX1 mRNA may be a potential biomarker of colorectal cancer and is associated with the infiltration of immune cells.
