1.Virtual screening the active ingredient and mechanism of Shenmai injection in treatment of novel coronavirus pneumonia based on network pharmacology and high throughput molecular docking

Objective:To virtual screen the active ingredient of Shenmai injection in treatment of Novel Coronavirus Pneumonia and discuss the potential mechanism based on network pharmacology and high throughput molecular docking. Methods:Based on network pharmacology and high-throughput molecular docking technology, the compounds and predicted targets of Shenmai injection were retrieved from TCMSP, BATMAN-TCM and Targetnet databases, and the composition target map was constructed. The genes related to coronavirus pneumonia were retrieved from OMIM and GeneCards databases, and the PPI network between target genes was constructed by searching the common parts of target genes; David 6.8 was used to analyze gene function and pathway enrichment, and PDB database was used to obtain protein crystal structure, and Autodock Vina and python scripts were used for high-throughput molecular docking. Results:A total of 27 compounds and 224 target genes were obtained. 15 core components and 15 core targets for the treatment of coronavirus pneumonia were identified: CASP3, NOS2, PARP1, CASP8, NOS3, BCL2, ADA, OPRM1, TGFB1, TLR9, ACHE, SLC29A1, BAX, ADK, and PNP. The enrichment analysis showed that the core targets acted on the signaling pathways such as Tuberculosis, Pathways in cancer, Hepatitis B and Apoptosis. The better components of Novel Coronavirus Pneumonia related targets were diosgenin, stigmasterol, beta-sitosterol and ginsenoside Rh1_qt obtained by virtual screening.Conclusion:This study screened out the active ingredient and tarket of Shenmai Injection in treatment of Novel Coronavirus Pneumonia. It laid a foundation for the further clinical application of Shenmai injection and development of novel coronavirus pneumonia drugs.

3.Early and late outcome of acute type A aortic dissection in dialysis dependent patients

Objective:To summarize acute type A aortic dissection(ATAAD) is relatively uncommon in dialysis patients, and repair outcomes are not fully understood.Methods:Between January 2014 and March 2020, 20 patients with ATAAD required dialysis for preoperative end-stage renal disease(ESRD) were treated by our group. There were 11 male and 9 female patients at mean age of(47.8±11.3) years. The mean duration of dialysis therapy in the total 20 patients before the onset of ATAAD was(4.5±3.9 )years, with 90%(18 cases) of these patients undergoing hemodialysis(rather than peritoneal dialysis). 17 patients were treated emergency surgically, surgical operation were performed under deep hypothermic circulatory arrest and perfused the cerebral selective cerebral perfusion, 5 cases with ascending aorta + arch fenestrated stent, 5 cases with ascending aorta+ hemi-arch replacement(2 cases with stent elephant trunk), 4 cases with ascending aorta+ arch replacement+ stent elephant trunk(1 case with coronary artery bypass grafting for left anterior descending coronary artery), 2 cases with aortic valvuloplasty + ascending aorta+ hemi-arch replacement, 1 case with Bentall+ arch fenestrated stent.Results:2 patients were died from aortic ruptured before operation, 1 patient treated medically was alive three months after admission. Cross-clamp, cardiopulmonary bypass, and circulatory arrest times of all the surgical patients were(233.8±84.4) min, (155.5±63.6)min and(28.2±10.8)min, respectively. The following complications occurred postoperative: 3 cases died in the hospital, 1 case of tracheotomy, 2 cases of cerebral infarction, 1 case of cerebral hemorrhage, 1 case of transient paraplegia, and 1 case of surgical site infection. After a mean follow-up of(11.6±14.5) months(rang, 3-61 months). the overall survival rate at 1 year and 5 years was 53% and 27% respectively.Conclusion:Dialysis patients with ATAAD should be operated on urgently and medical treatment carries high risks of aortic rupture, although in-hospital mortality is acceptable, long-term mortality is poor.

Long non-coding RNAs (lncRNAs) are a class of non-coding transcripts which are greater than 200 nucleotides in length and have a variety of biological functions. Studies have found that lncRNAs play an important role in the development of gastrointestinal cancers and can affect tumor cell growth, angiogenesis, metastasis and drug resistance. This paper has reviewed lncRNAs associated with gastrointestinal cancers and explored their roles in the occurrence, diagnosis and treatment of gastrointestinal cancers.

5. Treatment experience of cardiac rupture in patients with acute myocardial infarction

Objective:
To analyze the treatment results of cardiac rupture in patients with acute myocardial infarction (AMI) .
Method:
Clinical data of 6 with cardiac rupture after AMI, who were hospitalized in our hospital from June 2015 to June 2017, were retrospectively analyzed,and the clinical manifestations, methods of treatment and outcomes were investigated.
Results:
Cardiac function classification was Killip class Ⅱin all patients. There were 3 massive anterior wall myocardial infarction, 2 anterior wall myocardial infarction,and 1 inferior myocardial infarction. There were 4 patients with ventricular septal defect, 1 patient with rupture of papillary muscle,and 1 patient with left ventricular free wall rupture.All patients received continuous infusion of vasoactive medicines and treated with intra-aortic balloon pump(IABP), 2 patients (1 patient accepted operative treatment,and 1 patient received conservative treatment) were treated with extracorporeal membrane oxygenation (ECMO), mechanical ventilation,and continuous renal replacement therapy(CRRT).Three patients received surgical repair,1 case was supported by IABP, 1 case supported by ECMO,CRRT,and IABP,and 1 case did not use IABP or ECMO post operation. All 3 surgically treated patients recovered successfully and were discharged from hospital.Meanwhile, in the other 3 patients treated conservatively, 2 patients died in the hospital and 1 patient was discharged according to own will.
Conclusion
On the basis of vasoactive medicines and IABP, surgery repair is a feasible option for cardiac rupture patients secondary to AMI,and ECMO may improve the perioperative state in these patients.
7.Study on the active constituents and molecular mechanism of Calyx seu Fructus Physalis in treating lung cancer based on network pharmacology

Objective:With network pharmacology, this paper aims to explore the potential active constituents, related targets and signal pathways to elucidate the mechanism of Calyx seu Fructus Physalis in the treatment of lung cancer. Methods:By searching for Pharmacology Database of Traditional Chinese Medicine Systems and Analysis Platform (TCMSP) to obtain the active constituents of Calyx seu Fructus Physalis, and predict the target genes of active constituents by using SWISS and SEA. By collecting the corresponding targets approved by FDA and search for different database, including Therapeutic Target Database (TTD), Database of Gene-disease Associations (DisGeNET) and Online Mendelian Inheritance in OMIM to build a database of lung cancer related target genes; To get the target genes of herb-disease proteins by the intersection of the two databases and display the results through the network software of Cytoscape 3.7.2. Then to use the Database of Annotation, Visualization and Integrated Discovery (DAVID) to conduct key enrichment analysis of Kyoto Gene and Genome Encyclopedia (KEGG) pathway enrichment analysis. Combining related literature to analize the active ingredients and mechanism of Calyx seu Fructus Physalis in the prevention and treatment of lung cancer. Results:It was found that 11 active constituents of Calyx seu Fructus Physalis could play the role of anti-lung cancer by regulating 19 lung cancer related targets such as EGFR, ESR1, MDM2, MMP2 and MET. There were 15 signal pathways involved Proteoglycans in cancer, MicroRNAs in cancer, Pathways in cancer, Transcriptional misregulation in cancer, PI3K-Akt signaling pathway and other key signal pathways. Conclusion:The results show that the active constituents of anti-lung cancer effect of Calyx seu Fructus Physalis may be Oleic acid, Cycloartenol, Obturator, Graminesterol, β-sitosterol, etc. The mechanism of action may be related to multiple cancer-related signaling pathways such as Proteoglycans pathway, Transcriptional misregulation pathway, and PI3K-Akt signaling pathway.
8.Single lung transplantation assisted by extracorporeal membrane oxygenation technique duringperioperative period:a report of 6 cases

Objective To summarize the application experiences and curative efficacies of single lung transplantation assisted by extracorporeal circulation with coated lung ,centrifugal pump and coated pipe .Methods Retrospective analysis was conducted for clinical data of 6 adult patients with respiratory insufficiency undergoing single lung transplantation .The changes of hemodynamics and oxygenation before and after adjuvant treatment were observed ,the effects of adjuvant evaluated and the experiences of application summarized .Results The hemodynamic parameters post-assistance significantly improved as compared with that pre-assistance and pulmonary arterial pressure dropped from (56 ± 15 ) to (45 ± 13 ) mmHg with statistically significant differences . Arterial blood gas parameters significantly improved .PO2 spiked from (47 ± 12) to (68 ± 9) mmHg and PCO2 declined from (65 ± 14) to (55 ± 12)mmHg .And there were statistically significant differences .All patients were discharged successfully .Conclusions The simple extracorporeal membrane oxygenation system of coated lung , centrifugal pump and coated pipe during routine extracorporeal circulation may guarantee the operative safety of single lung transplantation and provide a new therapeutic option .
9.Influence of perioperative pulmonary rehabilitation system based on prehabilitation on early weaning during cardiac perioperative period

Objective:To evaluate the clinical effect of the pulmonary rehabilitation system based on the concept of prehabilitation for patients after cardiac surgery to wean tube and avoid related complications.Methods:From January 2018 to December 2019 in a single-center(third-class hospital in cardiac surgery intensive care unit), all adult patients hospitalized for 7 days before open-heart surgery were included. They were randomly divided into pulmonary rehabilitation group(198 cases) and control group(234 cases). To compare and analyze the clinical effects, the main observations were observed including overall outcome indicators(such as early extubation rate, ICU stay, hospitalization costs, advanced oxygen therapy support after extubation) and lung outcome related indicators(such as the occurrence of pulmonary complications, chest drainage, secondary intubation, tracheotomy, lung infection and chest tube drainage).Results:There was no statistical difference between groups in basic conditions and surgical conditions. The lung rehabilitation group significantly increased the rate of early extubation, reduced the number of advanced oxygen therapy after weaning, shortened the length of ICU stay, saved hospitalization cost, significantly reduced the occurrence of postoperative respiratory complications and improved postoperative respiratory function( P<0.05). Conclusion:During cardiac perioperation, pulmonary rehabilitation significantly can increase the rate of early extubation , shorten the length of mechanical ventilation, reduce the occurrence of secondary tracheal intubation and pulmonary complications. And it can also effectively promote the recovery of lung function and the overall recovery.
10.Responses of regional environmental factors to the reference value of neutrophil-lymphocyte ratio in healthy adults

【Objective】 This paper screened the factors that may influence the spatial differentiation of Neutrophil-to-lymphocyte ratio (NLR) reference values in healthy adults in China and explored the trend of NLR reference values in China. 【Methods】 For this research, we collected the NLR of 162 681 healthy adults from 62 cities in China. Spearman regression analysis was used to analyze the correlation between NLR and 25 geography secondary indexes. We extracted 9 indexes with significant correlation, built a random forest (RF) model, and predicted the country’s urban healthy adults’ NLR reference value. By using the disjunctive Kriging method, we obtained the geographical distribution of NLR reference value of healthy adults in China. 【Results】 The reference value of NLR of healthy adults in China was significantly correlated with the 9 secondary indexes, namely, altitude, sunshine duration, annual average temperature, annual average relative humidity, annual temperature range, annual average wind speed, content of organic matter in topsoil, cation exchange capacity in topsoil (clay), and total amount of CaSO
