1.Systematic review and meta analysis of in situ fenestration and pre-fenestration in the treatment of thoracic aortic diseases

Thoracic aortic disease, which involves some of the arch vessels, is challenging to treat because of its complex anatomical structure and variation. With the progress and development of endovascular repair technology, in situ fenestration and pre-fenestration have become important measures for the treatment of thoracic aortic diseases. In recent years, there have been a series of reports on the application of in situ fenestration or pre-fenestration in a few centers, and the preliminary results are satisfactory. However, there are some problems such as single operation and small sample size. In order to further compare and analyze the safety and efficacy between the two operations, meta-analysis was conducted by searching Pubmed, Embase, Cochrane, Web of Science, China National Knowledge Infrastructure, Wanfang data, VIP data and China Biology Medicine disc. There was no significant difference in technical success rate, 30-day mortality, endoleak rate and reintervention rate between the two methods. The application of surgical methods can be determined according to the patient's condition and the surgeon's clinical experience.
2.Clinical application and efficacy analysis of castor stent graft in the treatment of aortic diseases

Objective:To summarize the use of Castor stent graft in aortic diseases and to analyze their efficacy.Methods:The clinical data of patients with aortic diseases treated with Castor stent graft from November 2017 to August 2022 in Fujian Provincial Hospital were collected and divided into branched stent group and branched stent co-operative group according to the operation method, and the clinical data of both groups were summarized.Results:A total of 75 cases of aortic disease were treated with Castor stents, and finally 53 cases were classified as branched stent group and 22 cases as branched stent co-operative group. The operations in both groups were successful. The median operative time in the branched stent group was 120(100, 160)min, and the median postoperative hospital stay was 7.0(5.5, 10.5)days.There was no postoperative ischemic stroke, no spinal cord ischemia. One case of new aortic dissection occurred. During follow-up, there was one lost case and two deaths, and the rest did not have endoleak, branch stent stenosis, ischemic stroke or re-intervention. In the branched stent co-operative group, there was one postoperative ischemic stroke, one case of slight stenosis of the left subclavian artery stent during follow-up, the remaining cases had satisfactory postoperative outcomes.Conclusion:Castor stent graft is a safe and effective procedure in the treatment of aortic diseases. And Castor stent graft can be used in combination with other endovascular repair techniques in the treatment of complex aortic diseases, with safe and reliable postoperative outcomes.
3.Association between metal cobalt exposure and the risk of congenital heart defect occurrence in offspring: a multi-hospital case-control study.

BACKGROUND:
Many studies have investigated heavy metal exposure could increase the occurrence of congenital heart defects (CHDs). However, there are limited data regarding the relationship between cobalt exposure and CHD occurrence in offspring. The aim of this study was to analyze the association between cobalt exposure in mothers and the risk of CHDs in offspring.
MATERIALS AND METHODS:
In order to explore the association between cobalt exposure and occurrence of congenital heart defect (CHD), a case-control study with 490 controls and 399 cases with CHDs in China were developed. The concentrations of cobalt in hair of pregnant woman and fetal placental tissue were measured and processed by a logistic regression analysis to explore the relationship between cobalt exposure and risk of CHDs.
RESULTS:
The median concentration of hair cobalt in the control and case group was 0.023 ng/mg and 0.033 ng/mg (aOR, 1.837; 95% CI, 1.468-2.299; P < 0.001), respectively. And the median (5-95% range) fetal placental cobalt concentrations were 19.350 ng/g and 42.500 ng/g (aOR, 2.924; 95% CI, 2.211-3.868; P < 0.001) in the control and case groups, respectively. Significant differences in the middle level of cobalt in hair were found in the different CHD subtypes, including septal defects, conotruncal defects, right ventricular outflow tract obstruction, and left ventricular outflow tract obstruction (P < 0.001). Dramatically, different cobalt concentrations in fetal placental tissue were found in all subtypes of cases with CHDs (P < 0.01).
CONCLUSIONS
The finding suggested that the occurrence of CHDs may be associated with cobalt exposure.
