Objective:To study microwave-assisted extraction technique of flavonoids from Ophiopogon japonicus. Methods:The effect of the parameters,i.e.ratio of solid to liquid, microwave treatment time, microwave power and volume ratio of ethanol on extraction amount of flavonoid were analyzed by single factor experiment. Results:Optimum processing data were determined as follows:90.0% ethanol as extracting solvent, stock ratio 1:14(g/ml) ,microwave extracting for 4 minutes at the power of MED. The extraction rate was 39.7% higher than that of non-microwave's.Conclusion:Extraction of flavonoids by microwave from Ophiopogon japonicus is an eficient method.

Effective cost control and structure optimization of medical consumables in public hospitals can facilitate a shift from extensive cost control to scientific and refine management. On the premise of ensuring medical quality, reducing the burden of patients'diagnosis and treatment and meeting the actual needs of hospital management, this approach aims to realize valuable healthcare outcomes. This study was conducted in a tertiary hospital, which balanced both the medical and economic val-ue of medical consumables.Using an integrated approach to specialty capacity building and disease structure optimization, the hospital restructured the use of medical consumables in Transcatheter Arterial Embolization(TAE)procedures. It developed standardized pathways and usage protocols tailored to specific diseases and surgical requirements.A targeted consumable usage policy framework was introduced, comprising"one department, one policy;one surgery type, one policy;and one consumable, one policy."This included initiatives such as validating the use of drug-loaded embolic microspheres, conducting multi-depart-mental review meetings, strictly regulating indications for these microspheres, limiting personnel involvement, and negotiating re-duced pricing on imported microspheres.Following implementation, the average case-mix index(CMI)for discharged patients undergoing TAE increased from 2.23 to 2.34(P<0.001),while the average per-case cost of consumables decreased from 19 600 to 15 600(P<0.001).These measures offer valuable decision-making and operational reference for hospitals nation-wide,supporting efficient,quality-focused consumables management.

This study was aimed to observe the effect of Bai-Zhu Huang-Qi (BZHQ) decoction ethyl acetate extract on NOD like receptor family, pyrin domain-containing 3 (NLRP3) inflammasome in macrophages. The U937 cells were pretreated with phorbol 12-myristate 13-acetate (PMA,10 ng· mL-1) for 48 hours to induce macrophages.Effects on cell viability by different doses of BZHQ decoction ethyl acetate extract (0,3.125,6.25,12.5,25,50,100 μg· mL-1) were observed to select the appropriate concentration.Contents of NLRP3 and caspase-1 in cells were detected by real-time PCR and western blot. The concentration of interleukin-1β (IL-1β) in cell supernatant was detected by enzyme linked immunosorbent assay (ELISA).The cell counting kit-8 (CCK-8) assay showed that when the drug concentration was lower than 25 μg· mL-1,there was no impact on cell viability;when the drug concentration was higher than 50 μg· mL-1,there was inhibition on cell viability (P < 0.05).The concentration of 25 μg· mL-1 was used to conduct the following experiment.Compared to the blank group, the expression of NLRP3 and caspase-l in cells of the LPS group were significantly increased (P < 0.01).The concentration of IL-1β in cell supernatant was also significantly increased (P < 0.01).After treated with BZHQ decoction ethyl acetate extract, levels of NLRP3,caspase-1 and IL-1β were significantly decreased (P < 0.05).It was concluded that BZHQ decoction ethyl acetate extract can inhibit the production of NLRP3 inflammasome in LPS-stimulated macrophages.

The biological clock(also known as the circadian rhythm)is the fundamental reliance for all organisms on Earth to adapt and survive in the Earth's rotation environment.Circadian rhythm is the most basic regulatory mechanism of life activities, and plays a key role in maintaining normal physiological and biochemical homeostasis, disease occurrence and treatment.Recent studies have shown that the biologi-cal clock plays an important role in the development of oral tissues and in the occurrence and treatment of oral diseases.Since there is cur-rently no guiding literature on the research methods of biological clock in stomatology, researchers mainly conduct research based on pub-lished references, which has led to controversy about the research methods of biological clock in stomatology, and there are many confusions about how to rationally apply the research methods of circadia rhythms. In view of this, this expert consensus summarizes the characteristics of the biological clock and analyzes the shortcomings of the current biological clock research in stomatology, and organizes relevant experts to summarize and recommend 10 principles as a reference for the rational implementation of the biological clock in stomatology research.

BACKGROUND:
Studies have shown that the incidence and severity of corona virus disease 2019 (COVID-19) in patients with lung cancer are higher than those in healthy people. At present, the main anti-tumor treatments for lung cancer include surgery, immunotherapy, chemotherapy, radiotherapy, targeted therapy and anti-angiogenesis therapy. While the effects of different anti-tumor treatments on the occurrence and severity of COVID-19 pneumonia are not uniform. Therefore, we aimed to describe clinical characteristics and antitumor therapy of patients with lung cancer and COVID-19 pneumonia, and examined risk factors for severity in this population.
METHODS:
From December 1, 2022 to February 15, 2023, a retrospective study was conducted in 217 patients diagnosed with COVID-19 and pathologically confirmed lung cancer in the Jinling Hospital. We collected data about patients' clinical features, antitumor treatment regimen within 6 months, and the diagnosis and treatment of COVID-19. Risk factors for occurrence and severity of COVID-19 pneumonia were identified by univariable and multivariable Logistic regression models.
RESULTS:
(1) Among the 217 patients included, 51 (23.5%) developed COVID-19 pneumonia, of which 42 (82.4%) were classified as medium and 9 (17.6%) were classified as severe; (2) Univariate and multivariate analysis revealed overweight (OR=2.405, 95%CI: 1.095-5.286) and intrapulmonary focal radiotherapy (OR=2.977, 95%CI: 1.071-8.274) are risk factors for increasing occurrence of COVID-19 pneumonia, while other therapies are not; (3) Chronic obstructive pulmonary disease (COPD) history (OR=7.600, 95%CI: 1.430-40.387) was more likely to develop severe pneumonia and anti-tumor therapies such as intrapulmonary focal radiotherapy, chemotherapy, targeted therapy and immunotherapy did not increase severity.
CONCLUSIONS
Intrapulmonary focal radiation therapy within 6 months increased the incidence of COVID-19 pneumonia, but did not increase the severity. However, there was no safety concern for chemotherapy, targeted therapy, surgery and immunotherapy.
