1.The H syndrome

A case of the H syndrome was reported.An 18-year-old man presented with an 8-year history of progressive cutaneous sclerosis,skin hyperpigmentation,and hypertrichosis on the trunk and extremities.In addition,he suffered from short stature,hearing loss,cardiac anomaly,hepatosplenomegaly,and scrotal masses.Laboratory examination revealed hypogonadism.Cutaneous histopathologic examination showed hyperpigmentation of the basal layer with acanthosis,widespread fibrosis (moderate in dermis and severe in subcutis) and an infiltrate composed of histiocytes and plasma cells.A perivascular lymphocyte infiltrate was also observed throughout the dermis and subcutaneous fat tissue.These findings were consistent with the H syndrome.Differential diagnosis must be made with Winchester syndrome and polyneuropathy, organomegaly,endocrinopathy,M-protein and skin change (POEMS) syndrome.
2.Effect of periodontal mechanical therapy on periodontal health and interleukin-6 levels in gingival crevicular fluid in type 2 diabetic patients with chronic periodontitis

Objective To evaluate the effectiveness of periodontal mechanical therapy on periodontal health and interleukin-6 (IL-6) levels in gingival crevicular fluid (GCF) in type 2 diabetic patients with chronic periodontitis. Methods GCF samples were obtained with filter papers in first molar sites from 37 type 2 diabetic patients with chronic periodontitis. The randomized controlled and blinded clinical trial with a duration of six months was designed to compare effects between three groups of professional mechanical tooth cleaning (PMTC), i.e. coronal scaling (group Ⅱ) with periodontal initial therapy (group Ⅰ) and without clinical therapy (control group). The clinical periodontal index such as probing depth (PD), attachment loss (AL), modified bleeding index (mBI) and bacteria plaque index (PLI) were obtained with a Williams type periodontal probe. Laboratory examination including GCF volume, concentrations and total amounts of IL-6 in GCF were detected with ELISA method. Results Significantly decreasing trends of PD, AL, mBI, GCF volume and total amounts of IL-6 were observed in group Ⅰin all test period. The group Ⅱ had a significant reduction of PD at the third month as compared with baseline (⊿=0.36 mm, P<0.05), and the other figures showed descending trends but didn′t present statistical significances. At the end of study, the group Ⅰand Ⅱ had significant reductions of GCF volume compared with increases of control group (0.96 μ1>0.03 μ1mm>-0.20 μ1, P<0.05). Conclusions The sequential periodontal supra- and sub- gingival scaling has definitive effects on periodontal health improvement and on reducing the IL-6 level in GCF in type 2 diabetic patients.
3.A case of asymptomatic process of hepatic artery thrombosis in the patients following orthotopic liver transplantation

Objective To investigate the possible causes for asymptomatic process of hepatic artery thrombosis in the patients following orthotopic liver transplantation (OLT). Methods Hepatic functional tests, color Doppler ultrasonic observation, hepatic arteriographic examinations were done during the course of the development of the hepatic arteriothrombosis. All possible factors leading to the asymptomatic process were comprehensively analyzed. Results At the 4th postoperative day, thrombosis started to form along the anastomosis line. At the 21st postoperative day, the artery became completely thromboses. During this course, serial liver biopsy revealed introhepatic cholestasis, hydropic degeneration of hepatocytes, atrophy of the biliary epithelium and fibrosis in the portal area. Serial monitoring of liver function gradually revealed normal with the exception of elevation of r-GT and ALP. At the 71st day after OLT, arteriography showed that hepatic artery remained completely occluded, while the collateral circulation and compensatory expansion of the portal vein were established. The patient didn't have any symptoms of arterial thrombosis. Conclusions In this case, it was the collateral circulation and the compensation of the portal vein that made the allograft survive and helped to prevent the patient from retransplantation after hepatic artery thrombosis. Color Doppler ultrasonic examinations within 2 weeks following OLT were helpful to the early diagnosis of hepatic arteriothrombosis.
4.The Causes of 60 Cases on Post Crown's Failure in Clinical

Objecive To evaluate the causes of 60 cases for clinical failure in post crown.Methods Sixty cases with failure in post crown cure were entered the study.The causes of failure were analyzed.Results Among the 60 cases with failure in post crown cure,38 cases were due to the crown'loosing or broken away,12 cases with the broken dental crown,10 cases with periapical periodontitis,6 cases with broken root.Conclusions It was the loosing or broken away of post crown,periapical periodontitis,dental crown and root broken consist of the main causes for clinical failure in post crown.
5.The effect of different concentrations of glucose with or without hyperlipidemic serum on the cytomorphology of arterial smooth muscle cells

The observations on cytomorphology and cytodynamics of cultured rabbit arterial smooth muscle cells(SMC)under concentrations of glucose 5.55,11.1,16.7,33.4 and 55.5 mmol/L with or without hyperlipidemic serum(HLS)were carried out by light microscopy and transmission election microscopy. The results showed that①The extent of disruption in SMC was positively correlated with concentrations of glucose,and negatively correlated with the multipication ralte of DNA,a parameter of cytodynamics.②There was no synergistic effect between glucose and HLS in the development of atherosclerosis.③In high glucose concentration groups(glucose 16.7mmol/L)a calcifies particle of high electronic density like substance,probably the core of atherosclerosis was found in SMC. Our results suggest that to keep lower levels of blood glucose and blood lipids may delay the development of atherosclerosis.
6.The effects of different concentrations of glucagon with or without hyperlipidemic serum on the cytomorphology of smooth muscle cells

A study of the effects of glucagon in concentrations of 100,1000 and 10000 ng/L with or without hyperlipidemic serum(HLS)on the cytomorphology and cytodynamics of cultured rabbit arterial smooth muscle cells(SMC) was carried out by light microscopy,transmission electron microscopy and isotope technique,The results were as follows:①No effect of glucagon in concentration of 100 ng/L on SMC in both cytomorphology and cytodynamics was found.②Glucagon in concentration of 1000 or 10000 ng/L caused slight damage of SMC,and tended to inhibit the multiplication rate of DNA,a parameter of cytodynamics.Furthermore,lipidic substance in SMC was found under such concentrations of glucagon without HLS,which was favorable for formation of foam cells,③There was positively synergistic effect between glucagon and HLS in the development of atherosclerosis.Our results suggest that to keep lower levels of plasma glucagon and lipids in diabetic patients may delay the development of atherosclerosis.
7.Study on relationship between changes of DNA content in human spleen nuclei and the postmortem interval by image analysis

Objective To study changes of DNA content in human spleen nuclei and seek an experimental method for estimation of the postmortem interval(PMI)using computerized image-analysis technique(CIAT).Methods Smear sections from spleen sampled were collected in 36 cadavers with known the accurate PMI respectively every hour within the first 36 hours after death,which were then fixed with cold Carony fixation.The smeared sections were stained by Feulgen-van's staining method.3 indices for spleen nucleic DNA including integral optical density(IOD),average optical density(AOD)and average gray(AG)were measured using the CIAT.Results IOD and AOD in the spleen nuclei declined regularly,whereas AG increased with the extension of PMIs in 36 hours.Conclusion There are definite relationships between the PMI and gray parameters(IOD、AOD and AG)representing the DAN content of nucleic DNA in the spleen in 36 hours after death,which may be used for estimation of PMI.
8.Preparation and rheological evaluation of ribavirin ophthalmic in situ-gel

BACKGROUND: Ribavirin is easily diluted by lacrimal fluid and has low bioavailability. In situ ophthalmic gel changes from liquid drug to gel phase upon exposure to physiological conditions, resulting in an increase in the precorneal residence time of drugs. OBJECTIVE: To prepare ribavirin in situ ophthalmic gel and valuate its theological feature. DESIGN, TIME AND SETTING: In vitro controlled observation, performed at the Engineering Research Center of Modem Chinese Medicine Discovery and Preparation Technique, Ministry of Education, Tianjin University of Traditional Chinese Medicine between September and October 2006. MATERIALS: Sodium alginate, Carbopol, and ribavirin. METHODS: Sodium alginate (1% and 2%) and Carbopol (0.3%, 0.6%, and 2%) were combined at different proportions to prepare in sire ophthalmic gel, then in which ribavirin was added. MAIN OUTCOME MEASURES: Viscosity determinations of prepared different concentrations of formulations were determined using a cone and plate viscometer. RESULTS: Ribavirin in situ ophthalmic gel prepared with 1% sodium alginate and 0.6% Carbopol had the largest viscosity. The viscosity change was not influenced by the addition of ribavirin. The in situ ophthalmic gel prepared with sodium alginate and Carbopol changed to the gel phase upon exposure to lacrimal fluid. CONCLUSION: 1% sodium alginate and 0.6% Carbopol produced the best rheological feature and could prolonge the precorneal residence time of drug than that of conventional ophthalmic solution.
9.Effects of growth stages and pH value on the expression of autolytic enzyme atIS gene of Streptococcus gordonii.

OBJECTIVEThis study aimed to detect the difference in the expression levels of autolysin atIS gene of Streptococcus gordonii (S. gordonii) at different growth stages and pH values, as well as to analyze the factors regulating atlS gene expression in S. gordonii.
METHODSS. gordonii wild strains (ATCC 35105) were collected at different growth stages (early exponential phase, mid-exponential phase, late exponential stage, and platform stage) and pH values (pH 7 and pH 5.5), and total RNA was extracted by using a conventional method. Fluorescence quantitative polymerase chain reaction (FQ-PCR) was used to measure the relative mRNA expression of atlS gene, with bacterial 16S rRNA as internal reference, for a comparison of the mRNA levels of atlS gene expression in S.gordonii at different growth stages and pH values.
RESULTSFQ-PCR results showed that atlS gene expression increased with gradually increasing growth stage under neutral conditions and was higher than that under acidic conditions (P < 0.05).
CONCLUSIONThe atlS gene expression in S. gordonii is influenced by growth stage and pH value factors.

10.Effects of simvastatin and cilazapril on expressions of transforming growth factor-?_1 and insulin-like growth factor-Ⅰ in cultured human glomerular mesangial cells

Objective To explore the effects of simvastatin and cilazapril on the expression of transforming growth factor-?_1 (TGF-?_1) and insulin-like growth factor-Ⅰ (IGF-Ⅰ) in the cultured human glomerular mesangial cells. Methods Human embryo glomerular mesangial cells were cultured in media with lower (5.6 mmol/L) or higher (30 mmol/L) glucose concentrations. Forty-eighthoursafteraddingsimvastatin(10 ?mol/L)and/or cilazapril (10 ?mol/L) to the cultured media, the concentrations of TGF-?_1, fibronection, laminin, type Ⅳ collagen proteins in the supernatant of cultured mesangial cells were determined by ELISA and radioimmunoassay and the expressions of TGF-?_1 and IGF-Ⅰ mRNA in cultured mesangial cells were also evaluated by RT-PCR. Results Compared with lower glucose control, the mesangial cells in the medium with higher glucose concentration showed excessive proliferation and higher expressions of TGF-?_1 and IGF-Ⅰ mRNA, and the levels of TGF-?_1, fibronection, laminin, and type Ⅳ collagen in the supernatant were also significantly increased. The expressions of TGF-?_1 and IGF-Ⅰ mRNA in the mesangial cells and the concentrations of TGF-?_1 and the extracellular matrix (ECM) proteins in the supernatant were all decreased after addition of simvastatin and cilazapril. Combination of simvastatin and cilazapril resulted in more profound suppressive effect on the expression of TGF-?_1 mRNA than either of them alone. Conclusion High concentration of glucose stimulates the cultured human mesangial cells excessively to express the TGF-?_1, IGF-Ⅰ and ECM proteins, and the high glucose-induced changes are suppressed by either simvastatin, cilazapril alone or combined treatment.
