Objective To investigate the expressions and significances of micro RNA -192 and zinc finger transcription factor 1 in endometrial carcinoma.Methods 59 cases of endometrial cancer,42 cases of uterine fibroids and 42 cases of normal endometrial tissues were selected.The expressions of miR -192 and ZEB1 in different tissues were detected by using qRT -PCR.The expressions of ZEB1 proteins in difference tissues were tested by Western blot.Results The relative expressions of miR -192 in endometrial cancer tissues were (0.69 ±0.13),which were lower than the normal endometrial tissues and uterine fibroid tissues,which were (1.03 ±0.21)and (0.98 ±0.17), the differences were statistically significant (t =10.983 and 8.572,all P <0.05).The relative expressions of ZEB1 mRNA and the expressions of ZEB proteins in endometrial cancer tissues were (0.47 ±0.08)and (1.13 ±0.24), which were higher than the normal endometrial tissues and uterine fibroid tissues,which were (0.28 ±0.05),(0.89 ± 0.17),(0.31 ±0.07)and (0.87 ±0.21 ),the differences were statistically significant (t =6.834,7.053 and 5.983,6.751,all P <0.05).The expressions of miR -192 and ZEB1 in endometrial carcinoma tissues were associated with FIGO stage and lymph node metastasis (t =1.840 and 2.465,all P <0.05).Pearson correlation analysis showed that,the relative expressions of miR -192 in endometrial carcinoma tissues were negatively correlated with the expres-sions of ZEB1 (r =-0.402,P =0.002).Conclusion The expression of miR -192 in endometrial carcinoma tissues was low.It might participate in invasion and metastasis of endometrial carcinoma through acting on ZEB1.

Objective To evaluate the application value of circulating tumor cell ( CTC ) in the differential diagnosis of solitary pulmonary nodule ( SPN ) . Methods Peripheral blood samples were collected from 134 patients with solitary pulmonary nodule in Shanghai Chest Hospital from September 2013 to January 2015, including 80 patients with malignant nodule and 54 with benign nodule.CTC levels of the above subjects were detected by ligand-targeted polymerase chain reaction ( LT-PCR ) assay, and serum carcinoembryonic antigen ( CEA ) and cytokeratin 19 fragment ( CYFRA21-1 ) were detected by flow fluorescence assay.Results By Mann-Whitney U Test, the CTC levels of malignant SPN patients [11.06 (8.77-14.41)units/3 ml] were significantly higher than those of benign SPN patients [6.65(4.49 -7.84)units/3 ml] (Z=-6.217,P<0.001).The sensitivity and specificity of differential diagnosis of SPN for CTC were 80%(64/80) and 85%(46/54) respectively.According to the diameter of SPN, the patients were divided into three groups to evaluate the diagnostic value of CTC in SPN with different size .For SPN with diameter less than 8 mm, the sensitivity and specificity of CTC were 6/9 and 4/5 respectively .For SPN with diameter between 8 mm and 20 mm, the sensitivity and specificity of CTC were 83%(35/42) and 85%(29/34).For SPN with diameter greater than 20 mm, the sensitivity and specificity of CTC were 79%(23/29) and 13/15.Conclusion Comparing with the traditional tumor markers, CTC could provide more clinical value in the differential diagnosis of solitary pulmonary nodule .

AIM:To investigate the possible preventive mechanism of Tongfu Xiezhuo Decoction(Radix astragali,Radix et Rhizoma rhei,Rhizoma acori tatarinowii,Semen pharbitidis,Radix aconiti lateralis praeparata,Radix et Rhizoma ginseng, Rhizoma atractrylodis macrocephale,Radix et Rhizoma Salviae mltiorrhizae,Semen Vaccariae,Pericarpium citri reticulate) for the tubule interstitial fibrosis by influence on the expression of ?-SMA,LN,ColⅢ in rats. METHODS: Unilateral ureteral occlusion(UUO) rats were randomly divided into 6 groups,including Sham,UUO,Enalapril group and Tongfu Xiezhuo Decoction of high-dosage,mid-dosage and low-dosage groups,the same amount of physiological saline for intragastric administration was administered by gavage.Sera and the kidney tissues were collected from each group on the twelfth day,Scr and BUN were measured.HE,VG staining measurement of tubulo-interstitial damage index and collagen deposition level,immuneohistochemical studies localizing ?-smooth muscle,actin(?-SMA),laminin-(LN),collagenⅢ(ColⅢ)were carried out. Objective To investigate the value of vascular disease of lower extremity of 64 slice spiral CT angiography in patients with diabetic foot.Methods 48 patients with diabetic lower limb underwent arterial CT angi-ography,Using CT a variety of post -processing techniques and compared with the original image.Results 48 patients were successfully obtained CTA image,41 patients had peripheral artery disease,40 cases were bilateral,39 cases of unilateral or bilateral had severe stenosis or occlusion;25 cases had obvious collateral circulation.Lower extremity arterial segments CTA and DSA showed good agreement (Kappa Value =0.887).Positive predictive rate was 92.5%,the negative predictive value was 98.6%.After delayed scanning,CTA on the foot artery count develo-ping increased,checked with the DSA consistency.Conclusion 64 slice spiral CTA can accurately and comprehen-sive assessment of lower extremity arterial disease of diabetic foot.Diabetic lower limb vascular lesions can be used as the preferred method of examination.

Objective To estimate the diagnostic value of circulating tumor cell detection for non-small cell lung cancer.Methods A Non-intervention clinical study was conducted in this research.From October 2014 to April 2015, totally 162 NSCLC who presented at Thoracic Surgery Department, 119 benign pulmonary disease and 52 healthy individuals were collected from Shanghai Chest Hospital.Folate receptor ( FR) based polymerase chain reaction ( PCR) method was used to detect the circulating tumor cell ( CTC) level, CEA and CYFRA21-1 was detected by the flowcytometry fluorescence luminance method, SCC was detected with Chemiluminescent microparticle immunoassay.The differences among groups were analyzed by the Kruskal-Wallis test( multi group comparison) and the Mann-Whitney U test( two group comparison) , and the chi-square test was used in the positive rate comparison;the Receiver Operating Characteristics ( ROC) curve was established.Results The median level of CTC in NSCLC patients was 11.90 Units/3 ml, which was significantly higher than those of benign pulmonary disease ( 6.72 CTC Units/3 ml ) and healthy individuals (5.82 CTC Units/3 ml,χ2 =125.990, P<0.01).Areas Under Curve ( AUCs) of ROC curve for NSCLC was 0.853 2(95% CI: 0.809 5,0.896 9).The cut-off value for discriminating NSCLC with benign pulmonary disease/healthy people was 8.74 CTC Units/3 ml with sensitivity being 77.16% and specificity being 90.06%.The positive rate of CTC in Stage I NSCLC patients was 68.7%, which was much higher than that of the combination of tumor markers(χ2 =32.98,P<0.01).Conclusion With relatively high sensitivity and specificity, the detection of circulating tumor cell may has a clinical value of application and extension.

OBJECTIVE: To investigate the feasibility and clinical characteristics of small partial laryngectomy without tracheotomy for T1-2 stage glottic carcinoma. METHOD: Forty-five patients with laryngeal squamaous cell carcinoma in T1-2 stage received small partial laryngectomy without tracheotomy. RESULT: All patients were primarily healed and were hospitalized for an average of 11.5 days post-operatively. In all patients, the function of respiration and the reflection of cough were normal, and laryngeal obstruction did not happen. The only postoperative complication was subcutaneous emphysema noted in 29 patients. Among them, subcutaneous emphysema extincted after 4-6 days in 26 patiens, only 3 patiens suffered from delayed healing because the subcutaneous emphysema extincted after 2 weeks. Mild subcutaneous emphysema did not affect the function of respiration and deglutition, healing of wound, and psychology of patients. All patients had been followed-up for 1-13 years. Only 2 patients died of tumor recurrence or metastasis. The function of respiration and deglutition were normal in the living patients, and no implanting metastasis on surface of trachea were found. CONCLUSION The theoretical foundation of small partial laryngectomy without tracheotomy for T1-2 stage glottic carcinoma has been well established. This surgical technique is feasible, safe and effective. It can significantly improve clinical outcome of T1-2 stage glottic carcinoma with minimal invasiveness. Furthermore, it can obviously abate the surgical, physiological and psychological trauma on patients.

侵袭与转移是影响恶性肿瘤患者预后最重要的因素之一，也一直是预测肿瘤预后和改善患者生存的热点与难点。上 皮钙黏蛋白（E-cadherin）低表达是恶性肿瘤最常见的表型之一，在多种肿瘤的侵袭、转移中发挥重要作用。上调上皮钙黏蛋白表 达可以降低恶性肿瘤的侵袭与转移能力，甚至改善肿瘤患者的预后，为肿瘤患者的治疗提供有效措施。近年来， 以RNA诱导的 基因激活（RNAa）为代表的基因调控技术的发展为肿瘤精准治疗提供更多可能，为特异、有效地上调上皮钙黏蛋白表达提供新的 途径。本文就上皮钙黏蛋白与RNAa技术近年来的研究进展及小激活RNA（saRNA）上调上皮钙黏蛋白表达的分子机制与生物 学意义作一综述。

Objective To summarize the surgical experience in robotic-assisted laparoscopic partial nephrectomy,and to investigate the efficacy and safety of this surgery.Methods The clinical data of 12 patients who underwent robot-assisted laparoscopic partial nephrectomy in Changhai Hospital from March to July in 2012 were analyzed.All the patients were male and the age range was 43-66 years.In 4 cases the tumors were in the left kidney,and 8 in the right.In 7 cases the tumors were in the dorsal part of the kidney,and 2 in the ventral part.There were 3,5 and 4 cases in the upper,middle and lower pole of the kidney respectively.Preoperative GFR test was normal in all cases.Kidney CT scan showed the maximum diameters of the tumors were 2.0-5.8 cm,with an average of 3.3 cm.The pre-operative stages in all cases were T1N0M0.Results The surgery was successfully completed in all cases.The mean duration of the surgery was 160-310 min,with an average of 242 min.The blood loss was 30-300 ml,with an average of 135 ml,and the intraoperative blood transfusion was unnecessary.The warm ischemia time was 20-49 min,with an average of 31 min.There was no intraoperative morbidity,and no conversion to open surgery.The postoperative length of hospitalization was 9-31 d,with an average of 14 d.Gross hematuria arose in 1 patient at 1 week after the surgery.The post-operative pathology showed renal clear cell carcinoma with Furhman Grade Ⅱ in 11 cases,and renal angiomyolipoma in 1 case.The maximum diameters of the tumors were 2.0-5.0 cm,with an average of 3.5 cm.The tumor resection margin was negative in all cases.Conclusions Robot-assisted laparoscopic partial nephrectomy is safe and effective for local renal tumors.This surgery has significant advantage over traditional laparoscopic partial nephrectomy,in terms of the resection of the renal tumors and the reconstruction of the kidney.

Objective:To study the expression of adult and neonatal Nav1.5 isoforms in the dorsal root ganglia (DRG) neurons of rats with spared nerve injury (SNI) . Methods:The expression of adult and neonatal Nav1.5 isoforms in the DRG neurons of rats with SNI was detected by RT-PCR, DNA sequencing, restriction enzyme digestion, immunohistochemistry and immunofluo-rescence methods. The expression of PKC-γwas detected by Western blot. Results:Both adult and neonatal Nav1.5 isoforms were expressed in the DRG neurons, but their expression ratio was approximately 2.5∶1. In SNI rat models, the expression of both adult and neonatal Nav1.5 isoforms decreased by approximately a half in both mRNA and protein levels. In contrast, the expression of PKC-γincreased by approximately one-fold. Conclusions:Both adult and neonatal Nav1.5 isoforms expressed in the DRG neurons of rat,but their expression levels decrease in pain models. The up-regulation of PKC-γmay directly or indirectly down-regulate the expression of Nav1.5 isoforms in SNI rat models,which may further involve in the pathological process of neuropathic pain.

With the development of economy and medicine, chronic diseases have become a more and more prominent problem affecting children′s physical and mental development.Juvenile idiopathic arthritis(JIA)is a common rheumatic disease in children.As an important medium of immune response, cytokines play a role in systemic juvenile idiopathic arthritis(SJIA)and its complication such as macrophage activation syndrome(MAS), which has attracted more and more attention.Many studies have found that a variety of cytokines play an inflammatory or anti-inflammatory role in the occurrence and development of SJIA and MAS.Therefore, this paper summarizes the specific mechanism of action and clinical significance of related cytokines in SJIA and MAS, and reviews the progress of related drug therapy targeting cytokines.By exploring the pathogenesis of SJIA, we hope to provide theoretical basis for identifying new therapeutic targets.
