1.Changes of platelet activation markers and platelet ultra-microstructure in the patients with acute myocardial infarction

Objective To investigate the expressions of platelet activation-dependent granule membrane protein and platelet-derived growth factor receptor-αB, and the ultra-microstructure changes of platelets in patients with acute ST-segment elevation myocardial infarction(STEMI). Method The expressions of platelet activationdependent granule of glycoprotein (CD62P)and platelet derived growth factor receptor αβ subtype (PDGFR-αβ)of platelets in peripheral blood in 36 patients with acute ST-segment elevation myocardial infarction(STEMI) hospitalized and another 34 healthy subjects over the same period (control group) were investigated by flow cytometry and data were analyzed. The changes of ultra microstructure and activity of blood platelets in those patients and control group were observed under the scanning electron microscope. Results The expressions of CD62P and PDGFR-αβin patients with STEMI group before treatment were (3.65 ± 1.87) % and (0.43 ± 0.39) %, respectively, and those after treatment were (0.96 ± 0.79) % and (0.28 ± 0. 24) %, respectively, whereas those in control group were (0.67 ± 0.35) % and (0.27 ± 0.22) %, respectively, which were much lower in control than those in patients with STEMI before treatment (P < 0.01 or P < 0.05) respectively. There were statistically significant differences in the expressions of CD62P and PDGFR-αβ in patients group between pre-treatment and posttreatment (P <0.01 or P <0.05), respectively. Obvious ultra-microstructure changes of platelet surface in patients with STEMI group were observed. Conclusions Due to platelet activation in AMI, the expressions of CD62P can be used as effective indicators for monitoring coronary heart disease, and the PDGFR-αβ can be used as a reference indicator. The platelet surface ultra-microstructure changes during platelet activation in patients with AMI can be found by scanning electron microscopy.
2. Immediate nipple reconstruction with a local mastectomy flap following secondary DIEP flap breast reconstruction

Objective:
To investigate the clinical effects of immediate nipple reconstruction with a local mastectomy flap in secondary breast reconstruction by deep inferior epigastric artery perforator (DIEP) flap.
Methods:
A free bipedicle DIEP flap was raised and folded upward to form the breast. Meanwhile, a deepithelialized lower mastectomy flap with a distant skin paddle was elevated and pulled throughout the reconstructed breast. The skin paddle was carefully sutured to the position of future nipple. Three weeks later, the pedicle of mastectomy flap was divided, and the paddle was modeled to form the new nipple.
Results:
From February 2011 to June 2016, 42 patients who had previously undergone unilateral modified mastectomies underwent the breast reconstructions and immediate nipple reconstructions. All flaps survived postoperatively. The average projection of the reconstructed nipple was measured as (16.3±2.4) mm immediately after the operation and gradually decreased to (8.4±1.3) mm by one year follow up. 35 patients ranked the aesthetic appearance of the reconstructed nipple and breast very good or good.
Conclusions
The immediate nipple reconstruction with a local mastectomy flap and breast reconstruction by DIEP flap could achieve a good breast shape and maintain a long term residual nipple projection, which results in considerable patient satisfaction.
3.Trans-umbilical three-dimensional single-incision laparoscopic cholecystectomy: report of two cases.

OBJECTIVETo explore the feasibility of single-incision laparoscopic cholecystectomy using a three-dimensional (3D) laparoscopic system.
METHODSTwo patients with benign gallbladder disease with a history of recurrent abdominal pain were selected to undergo the surgery. Gallstones were diagnosed by B ultrasound examination. All the operations were performed through the umbilical incision with the 3D laparoscopic system.
RESULTSThe 2 operations were completed successfully with a operative time of 35 min and 50 min. Both of the patients were ambulatory 8 h after the surgery, began to have a normal diet 1 day after operation, and were discharged 2 days postoperatively without any clinical symptoms.
CONCLUSIONS3D single-incision laparoscopic operation can well reveal the tissue anatomy in the operative field especially for some important structures such as the Calot's triangle with an improved safety compared to conventional laparoscopic operations.

4.Quality study of pharmaceutical excipient polysorbate 80

To improve the current quality standards of polysorbate 80 and provide reference recommendations for the revision of the quality standards of polysorbate 80 in the fourth part of China Pharmacopoeia(2015 Edition). A total of 16 batches of polysorbate 80 samples from 6 domestic and foreign production companies were studied, optimize the detection method for some impurities. Adjust the split ratio of the ethylene oxide and dioxane check items, and appropriately increase the concentration of the reference solution solution and change the solvent. In the ethylene glycol and diethylene glycol items, the concentration of the reference solution and the internal standard solution were appropriately increased. The infrared identification and the triethylene glycol check items were added to the quality standards, and we carry out corresponding methodological investigation on the improved method. The results showed that the improved methods had good specificity, precision, linearity and recovery rate. The improved quality standard is more suitable for the detection of polysorbate 80, and can increase the quality standards of polysorbate 80 from safety and standardization.
5.Effect and mechanism of transplantation of human amniotic epithelial cells on endometrium of uterine scar model rats

Objective To investigate the effect of human amniotic epithelial cell(hAEC)transplantation on endometrium improvement and matrix metalloproteinase 8(MMP-8)and vascular endothelial growth factor(VEGF)expression in a rat model of uterine scaring.Methods The uterine scar model was established in rats that were randomly divided into model and transplantation groups with 18 rats in each group.The other 18 rats were used as the sham operation group.Rats in the transplantation group were injected with hAECs in the uterine scar,and rats in model and sham operation groups were administered the same amount of PBS.After 4 weeks,the uterine tissues of eight rats in each group were collected.Histomorphological changes and endometria fibrosis were observed by HE staining and Masson staining respectively,and the endometrial thickness and number of glands were measured.Endometrial growth and receptivity were evaluated by immunohistochemical staining of cytokeratin and integrin β3,respectively.mRNA expression of MMP-8 and VEGFA in endometrial tissues was measured by RT-qPCR.Western blot was used to measure MMP-8 and VEGFA protein expression.After 8 weeks,the remaining 10 rats in each group were used to assess gestational ability.Results The endometrial thickness,gland number,IOD value of keratin and integrin β3,relative mRNA and protein expression levels of MMP-8 and VEGFA,pregnancy rate and number of uterine embryos in model and transplantation groups were lower than those in sham operation group(P<0.05).The endometrial thickness,gland number,IOD value of keratin and integrin β3,relative mRNA and protein expression of MMP-8 and VEGFA,pregnancy rate and number of uterine embryos were higher than those in model group(P<0.05).Additionally,hAEC transplantation improved the pathological morphology of endometrial tissue in rats with uterine scaring and reduced the degree of endometrial fibrosis.Conclusions hAEC transplantation improves endometrial injury,reduces scar formation,improves endometrial receptivity,and enhances pregnancy function in model rats,which may be related to promotion of MMP-8 and VEGFA expression.

7.Surgical treatment of gynecomastia by liposuction and vacuum-assisted biopsy device

Objective:This study intends to explore the clinical outcomes of liposuction combined with a vacuum-assisted biopsy device in the treatment of Simon grade Ⅰ or Ⅱ gynecomastia (GYN) caused by glandular proliferation and excrescent fat deposition.Methods:The clinical data of Simon grade Ⅰ or Ⅱ GYN caused by glandular proliferation and excrescent fat deposition treated by liposuction and vacuum-assisted biopsy device in the Department of Plastic and Reconstructive Surgery of the Ninth People’s Hospital Affiliated to Medical College of Shanghai Jiao Tong University from June 2016 to June 2019 were analyzed retrospectively. Surgical procedures: A 5-mm skin incision was made at the lower outer margin of the breast, through which the excrescent fat deposition was removed by liposuction. In addition, a vacuum-assisted biopsy device was applied to remove the residual glandular tissue. The operation time, liposuction volume, removed glandular tissue weight, blood loss, drainage volume, drainage duration, patient satisfaction, and complications were observed and recorded. The patients were followed up from June to December 2020. Patients were asked to grade the cosmetic outcome between 0 and 5 (0 was extremely dissatisfied, 5 was extremely satisfied). The data were sorted out and analyzed by SPSS 19.0, and the data were expressed as Mean±SD or M ( P25, P75). Results:A total of 27 patients with 51 breasts were enrolled in this study. Unilateral operation time, liposuction volume, removed glandular tissue weight, blood loss, drainage volume, drainage duration were (45.3±11.1) min, (206.0±66.7) ml, (28.9±9.5) g, (10.0±4.3) ml, (78.8±33.9) ml and (2.3±0.4) d respectively. Bruising occurred in 9 breasts. No other complications were observed. The postoperative follow-up time was 21.0 (17.0, 28.0) months, and the score of patient satisfaction was 4.98±0.14.Conclusions:Liposuction combined with vacuum-assisted biopsy device can be used as a feasible and minimally invasive approach to treat the Simon grade Ⅰ or Ⅱ GYN caused by glandular proliferation and excrescent fat deposition, which is safe and reliable with few complications, excellent cosmetic results, and high patient satisfaction. However, it has some shortcomings, such as hemostasis under the indirect vision.

10.Spatiotemporal distribution of oncomelania hupensis snail habitats in the Middle and Lower Reaches of the Yangtze River

Schistosomiasis is one of the major communicable diseases, which seriously impacts human health, Oncomelania hupensis snail is unique intermediate host of Schistosoma japonicum as well as posts significant influence on schistosomiasis transmission. The long-term serial data of oncomelania snail area in the Middle and Lower Reaches of the Yangtze River in the Schistosomiasis situation in People's Republic of China ,were collected from 1999 to 2018. The spatial and temporal variation characteristics of oncomelania hupensis area in five provinces were analyzed by Manner-Kendall test. In the spatial change, the area composition ratio of lake-and-marshland regions in Hunan increased, while that in Hubei and Jiangxi showed a slight decline. The lake-and-marshland snails in Anhui showed a significant downward trend, while Jiangsu showed no stable trend. In the spatial change, the area of lake-and-marshland snails in Jiangsu, Anhui, and Hubei showed a downward trend, among which the downward trend in Hubei was not obvious, while the upward trend in Jiangxi and Hunan had an upward trend but the trend was not obvious. the year of the sudden change in the area of the lake and marshes in in Jiangsu and Hubei were 2010 and 2017 ; There was no significant mutation in Anhui Province; the mutation point of the lake-and-marshland snails in Hunan was between 2001 and 2002; the mutation year of the lake-and-marshland snails in Jiangxi was 2002.This paper studies the temporal and spatial changes of the snail breeding area in the middle and lower Yangtze River, and the results can provide a basis for the elimination of snails in different types of distribution areas.
