1.Exploration of the application of SECUE framework in assessment on doctor-patient communication skills of clinical internship

Objective To investigate doctor-patient communication skills status and the influencing factors so as to pu forward to develop the proposal.Methods We selected undergraduate students in fourth grade which participated in the 2012 Shanghai Jiaotong University School of Medicine OSCE examinations before practice of clinical medicine by cluster sampling 1 and we selected four western and chinese medicine hospitals randomly sample 2,which are three level of first-class hospitals in Shanghai.We design the questionnaire and observe sampling with the SEGUE framework.Application of SPSS 13.0 data for statistical analysis,descriptive statistics analysis,u test,t test,ANOVA,rank sum test and correlation analysis method.Results The test with SEGUE framework shows that the scores of samplingl were 12.54 points(out of 24),the score rate is low which was 52.25%,and in the score rate of the dimensions,the end of inquiry dimension is 4% which is the lowest.There are significant statistical differences between different groups of influencing factors such as Medical occupation interest score and so on.There is no statistically significant difference between the SEGUE score of traditional Chinese medicine and western medicine clinical Intern.The communication skills of western medical students are higher.Doctor-patient communication skills related factors such as recognition of the importance of doctor-patient communication skills and other groups are statistically significant,the correlation coefficients are very small which is between 0.1 and 0.3.Conclusion The overall level of doctor-patient communication skills on clinical internship are not high,through the guidance and training on the occupation interest of medical students,we can strengthen the recognition of the importance of doctor-patient communication skills,so that we will improve the doctor-patient communication skills for the medical students.SEGUE framework should be used to routine medical students communication skills evaluation.
2.Action of Tuina on Retro-positioning of Skull Spatial Offset in Patients with Cervical Vertigo

Objective:To investigate the onset mechanism of cervical vertigo from the proprioceptive sensation and provide clinical basis for its treatment.Methods:Among the 121 cases that conformed to the diagnostic criteria of cervical vertigo,70 cases who presented with negative neck-rotation test and cervical vertigo without obvious vascular factors by transcranial Doppler(TCD)were assigned to the observation group,while 51 cases who presented with positive neck-rotation test and cervical vertigo due to spasm of vertebral basal artery or insufficient blood supply by TCD were assigned to the control group.The cases in the two groups were treated once every day,5 days make up one treatment course and the resuIts were statistically analyrzed after one treatment course.Results:The skull triaxial spatial offset of the cases in the two groups were significantly reduced after the treatment (P<<0.01).However, there was no statistically significant difference between the skull triaxial spatial offset between the two groups (p>0.05).It is not conclusive that the therapeutic effect in the two groups was significantly different after one treatment course.Conclusion:Tuina manipulation therapy can improve the skull spatial ofrset repositioning ability of the Patients.
4.Effect of transitional intervention on compliance and life quality of patients after percutaneous coronary intervention

Objective To explore the effect of transitional intervention on compliance and life quality of patients after percutane-ous coronary intervention(PCI) in cardiovascular internal department ,and provide the basis for the compliance in patients after PCI operation .Methods From January 2010 to September 2012 ,80 patients after PCI infection who had coronary heart disease were di-vided into observation group and control group ,and 40 cases in each group .The control group used conventional methods to rehabil-itation guidance for patients after PCI while the observation group used continued intervention system .According to the exercise of selfcare agency scale(ESCA) and health promoting lifestyle profile(HPLP) to improve the evaluation ,the compliance and life quali-ty of the two groups patients who discharged from hospital after 6 months and 12 months were evaluated .Results In the 6 months and 12 months after discharge ,compliance and life quality of the observation group was better than the control group ,and the differ-ence was statistically significant (P<0 .05) .And the difference of ESCA and HPLP scores at different time points between groups were statistically significant (P<0 .05) .Conclusion Continued intervention system can significantly improve the compliance and life quality in infection patients after PCI .
5.Application of subjective and objective parameters for nutritional assessment in elderly leprosy patients

Objective To survey the prevalence of undemutrition in elderly leprosy patients and explore the feasibility of nutritional assessment. Methods The nutrition status of 60 elderly patients with advanced leprosy was analyzed using Subjective Global Assessment (SGA) and objective methods. Results Undemutrition was determined in 22 patients (36. 6% ), including 5 patients ( 8. 3% ) were at a status of severe undernutrition. Kap-pa value of SGA and the objective items were 0. 619, 0. 571, 0. 476, 0. 454, and 0. 419 for triceps skin-fold thickness (TSF), pre-albumin (PALB), mid arm muscle circumference (MAMC), hemoglobin (Hb), and ser-um albumin, respectively. Gender, admission duration, type of leprosy, or disability grade was not correlated to all nutritional parameters except body height and cholesteroL Age and extent of ulceration were significantly correlated with some parameters such as BMI, TSF, lib, ALB, and PALB ( P < O. 01 ). Sensitivity and specificity of SGA on objective parameters screened were different. TSF and ulcer had the hrgest impacts on SGA. Conclusions The prevalence of undernutrition is not high in elderly leprosy patients. Any single objective parameter is not feasible for the assessment of nutrition status in these patients. A comprehensive method using SGA is preferred.
6.Improving effects of Huangdi Anxiao Capsules on insulin resistance in type 2 diabetic GK rats

AIM To observe the effects of Huangdi Anxiao Capsules (Puerariae lobatae Radix,Eriobotryae Folium,Notoginseng Radix et Rhizoma,etc.) on blood glucose and insulin resistance in GK rats with type 2 diabetes mellitus (T2DM) and its possible mechanism of action.METHODS GK rats with T2DM included in the experiment were randomly divided into model group,rosiglitazone (1.44 mg/kg) group,Shenqi Jiangtang Granules (1.08 g/kg) group,Huangdi Anxiao Capsules high,medium and low (12,6,3 g/kg) group and normal control group of Wistar rats.After six weeks of consecutive administration,fasting blood glucose (FBG),serum insulin (FINS),fasting plasma glucose (FPG),and insulin resistance index (HOMA-IR) were measured in all groups.Serum biochemical indexes of (TG,TC,HDL-C,LDL-C),glycosylated hemoglobin (HbA1c),glucagon-like peptide (GLP-1) and insulin-like growth factor (IGF-1) were measured.The pancreatic tissue was obtained by routine paraffin embedding and HE staining to observe the pathological changes.RESULTS Compared with the normal group,FBG,FPG,FINS,HOMA-IR,TG,TC,HDL-C,LDL-C and HbA1c of the model group were significantly higher (P <0.01),and GLP-1 and IGF-1 expressions were markedly decreased (P < 0.01).Compared with the model group,the levels of FBG,FPG,FINS,HOMA-IR,TG,TC,HDL-C,LDL-C and HbA1 c in Huangdi Anxiao Capsules high-,medium-and low-dose group were significantly decreased (P < 0.05,P <0.01);GLP-1 and IGF-1 expressions were markedly increased (P <0.05,P <0.01),compared with the model group,the structure changes of pancreatic tissue in Huangdi Anxiao Capsules high-,medium-and low-dose groups largely reduced.CONCLUSION Huangdi Anxiao Capsules can reduce GK rats fasting blood glucose,insulin resistance,and its mechanism may be related to promoting the emergence and proliferation of pancreatic islet cells,improving the function of islet beta cells,and increasing insulin secretion.
7.Effect of Buyang Huanwutang on Keap1/Nrf2/HO-1 Antioxidant Signaling Pathway in Rats with Idiopathic Pulmonary Fibrosis

ObjectiveTo study the effect of Buyang Huanwutang on Kelch-like Ech-related protein 1 (Keap1)/nuclear factor E2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1) antioxidant signaling pathway in rats with idiopathic pulmonary fibrosis (IPF) and explore the mechanism of this prescription in the treatment of IPF. MethodForty SPF-grade male SD rats were assigned into a sham operation group, a model group, a Buyang Huanwutang group, and a nintedanib group according to random number table method, with 10 rats in each group. IPF rat model was established by intratracheal infusion of bleomycin (0.005 g·kg-1) in other groups except the sham operation group. Buyang Huanwutang group was administrated with Buyang Huanwutang (14.84 g·kg-1),intragastric administration of nitedanib suspension (0.1 g·kg-1),sham operation group and model group were given equal volume of normal saline, for 28 days. After lung function test, serum and lung tissue samples were collected. Hematoxylin-eosin (HE) staining and Masson trichrome staining were employed to observe the pathological changes of the lung tissue. The content of hydroxyproline (HYP) in lung tissue was detected. The levels of malondialdehyde (MDA) in serum and lung tissue, and the activities of superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and catalase (CAT) were determined. The mRNA and protein levels of Keap1, Nrf2, and HO-1 was determined by Real-time fluorescent quantitative polymerase chain reaction, immunohistochemical staining, and Western blot. ResultCompared with the sham operation group, the modeling increased the resistance and elasticity and decreased the compliance of respiratory system (P<0.01), elevated the lung index, pathological score, and HYP content in lung tissue (P<0.01), and enriched MDA in serum and lung tissue, while it decreased the activities of SOD, GSH-Px, and CAT (P<0.01). Furthermore, the modeling down-regulated the mRNA and protein levels of Keap1 and up-regulated those of Nrf2 and HO-1 in lung tissue (P<0.01). Compared with the model group, Buyang Huanwutang decreased the resistance and elasticity and increased the compliance of respiratory system (P<0.01), lowered the lung index, pathological score, and HYP content in lung tissue (P<0.01), and reduced MDA in serum and lung tissue, while it increased the activities of SOD, GSH-Px, and CAT (P<0.01). Additionally, Buyang Huanwutang down-regulated the expression of Keap1 and up-regulated that of Nrf2 and HO-1 in lung tissue (P<0.05, P<0.01). ConclusionBuyang Huanwutang can activate Keap1/Nrf2/HO-1 signaling pathway to enhance the antioxidant capacity and slow down the pathological process of IPF in rats.

OBJECTIVESince 1986, the reference of birth weight for gestational age has not been updated. The aim of this study was to set up Chinese neonatal network to investigate the current situation of birth weight in China, especially preterm birth weight, to develop the new reference for birth weight for gestational age and birth weight curve.
METHODA nationwide neonatology network was established in China. This survey was carried out in 63 hospitals of 23 provinces, municipalities and autonomous regions. We continuously collected the information of live births in participating hospitals during the study period of 2011-2014. Data describing birth weight and gestational age were collected prospectively. Newborn's birth weight was measured by electronic scale within 2 hours after birth when baby was undressed. The evaluation of gestational age was based on the combination of mother's last menstrual period, ultrasound in first trimester and gestational age estimation by gestational age scoring system.
STATISTICAL ANALYSISthe growth curve was drawn by using LMSP method, which was conducted in GAMLSS 1.9-4 software package in R software 2.11.1.
RESULTA total of 159 334 newborn infants were enrolled in this study. There were 84 447 male and 74 907 female. The mean birth weight was (3 232 ± 555) g, the mean birth weight of male newborn was (3 271 ± 576) g, the mean weight of female newborn was (3 188 ± 528) g. The test of the variables' distribution suggested that the distribution of gestational age and birth weight did not fit the normal distribution, the optimal distribution for them was BCT distribution. The Q-Q plot test and worm plot test suggested that this curve fitted the distribution optimally. The male and female neonatal birth weight curve was developed using the same method.
CONCLUSIONUsing GAMLSS method to establish nationwide neonatal birth weight curve, and the first time to update the birth weight reference in recent 28 years.

9.Application of nanoparticles in preventing enamel demineralization during orthodontics

Enamel demineralization is one of the most common adverse reactions to orthodontic treatment. The existence of orthodontic appliances affects oral hygiene maintenance, which easily leads to plaque accumulation and oral flora dysbiosis, and cariogenic bacteria produce acid to cause enamel demineralization. It not only affects aesthetics but may develop into caries and endanger oral health. Therefore, enamel demineralization has become an urgent problem. Nanoparticles generally refer to solid particles with diameters of 1 to 100 nm and have unique physicochemical properties that provide a new strategy for preventing enamel demineralization during orthodontics. Reviewing the relevant literature, nanoparticles used for the prevention of enamel demineralization in orthodontics may be classified into antibacterial, remineralization and carrier-type nanoparticles according to their functions. Most research was performed on the application of nanoparticles to modify orthodontic adhesives for enhancement of antibacterial or remineralization properties, but some studies also focused on the modification of orthodontic appliances with nanoparticles for surface coating or overall doping to provide antimicrobial properties. The advantage of these two approaches is that they are not dependent on patient compliance. Nanoparticle-modified fluoride varnishes and nanocarriers loaded with antimicrobial or remineralization agents may be used to promote oral health care in orthodontic patients, which have a sustained preventive effect but depend on the cooperation of the patient. It was indicated that the small size effect of nanoparticles provides better performance, but there may be certain safety issues, and there is still some influence on the physicochemical properties of the modified materials themselves. These issues must be further explored. Although there are some limitations in the current studies, nanoparticles are expected to play an important role in the prevention of enamel demineralization during orthodontics in the future.
10.Research progress on the relationship between orthodontic treatment and the oral microbiome

With the deepening of research in oral microbiomics, an important relationship between changes in the oral microbiome and orthodontic treatment has been found. Orthodontic treatment will have an impact on the oral and systemic microbiome. The presence of oral appliances can change the quantity and quality of the oral microbiometo and increase the risk of oral and even systemic diseases in patients undergoing orthodontic treatment. Compared with fixed orthodontic treatment, clear aligners will not have a harmful impact on the structure of the oral microbiome, which is more conducive to maintain oral health during the orthodontic treatment process. In addition, different bracket types and materials can lead to different changes in the oral microbiome, and the occurrence and development of orthodontic-related diseases, such as white spot lesions, dental caries, gingivitis and periodontitis, are also related to changes in the oral microbiome. At present, the role of the oral microbiome in the process of orthodontic treatment needs to be further studied. Whether a change in the oral microbiome caused by orthodontic treatment can be restored after orthodontic treatment is still uncertain and needs more research. This paper reviews the research progress on the application of microbiomics in orthodontics, including the impact of fixed appliances and clear aligners on the microbiome and the relationship between orthodontic-related diseases and the oral microbiome.
