2.The influence of peritoneal dialysate on peritoneal macrophage releasing TNF-?

To investigate the damage on macrophage of the commercial peritoneal dialysis solution(CDS). Methods Macrophages were seperated from peritoneal fluid remained overnight of seven CAPD patients and TNF-a level of supernatant was determined and compared with those macrophages from uremic patients not yet recieving peritoneal dialysis. Results TNF-a levels of different glucose concentration decreased obviously in experimental group compared with control group, especially lower in 2.5% and 4.25% group. Conclusion In vivo experiment confirms that CDS possesses a long time inhibition on macrophage and this inhibition varies with different glucose concentrations.
3.Re-evaluation of solute transport groups of peritoneal dialysis patients using peritoneal equilibration test

Objective To clarify whether the peritoneal equilibration test (PET)-determined solute transport groups defined by Twardowski fits patients in our medical center. Methods 158 initial standardized PET data since 1995 was selected and proportions of four transport groups were calculated according to Twardowski's criterion. Using the mean and standard deviations of 4-hour dialysis/plasma ratio of creatinine (D/Pcr), transport groups of our patients were re-determined. Patients were classified as follow: according to both two criteria, patients whose 4-hour D/Pcr in the range of high transport, low transport and average transport were classified as group H1, group L1 and group A, respectively; several average transport patients who changed to low transport after re-evaluation were classified as group L2; high transport patients who changed to average transport were classified as group H2. Every group was compared with clinical status in order to evaluate which criterion fit our patients. Results The 4-hour D/Pcr was 0. 70 ?0. 14 in our patients. The proportion of high, high-average, low-average and low transport were 21.5%, 44.9%, 17.8% and 5. 7% according to Twardowski's and 14. 6% , 33.5%, 33.5% and 18. 4% after re-evaluation. ObjectiveTo investigate the relationship between sepsis induced high mobility group 1 (HMG 1) gene expression and endotoxemia in rats. MethodsSepsis was induced by cecal ligation puncture (CLP). One hundred Wistar model rats were randomly divided into four groups: normal control group ( n =10), sham operation (SO) group ( n =10), CLP group (further subdivided into 2?h, 6?h, 12?h, 24?h, 48?h and 72?h post CLP, n =10 in each subgroup), and bactericidal/permeability increasing protein(rBPI 21 ) treatment group (further subdivided into 12?h, 24?h post CLP, n =10 in each subgroup). 5.Significance of fluctuated level of endogenous bactericidal/permeability-increasing protein in patients after major abdominal surgery

Objective To investigate the fluctuated pattern of endogenous bactericidal/permeability-increasing protein(BPI)levels,and to elucidate its significance in endotoxin response after major abdominal surgery.Methods Eleven patients undergoing major abdominal surgery were involved in present study.The levels of plasma BPI,lipopolysaccharide-binding protein(LBP)and interleukin-6(IL-6)were measured preoperatively and on postoperative days 1,3,5,7,and 14 by ELISA.Plasma endotoxin concentration was determined with the chromogenic limulus amebocyte lysate(LAL),which was modified by perchloric acid(PCA)pretreatment of samples.The polymorphonuclear neutrophil count was also done at each time point.Results In comparison with normal controls,plasma BPI/LBP ratios [(1.058?0.074)?10-3,P=0.004] were significantly increased on postoperative day 1,maintained at same level on day 3,and declined on day 5.No plasma IL-6 was detected from the blood of all the 11 patients prior to operation,but plasma IL-6 level rose rapidly on day 1 after operation(27.440?12.144pg/ml,P=0.04),maintaining a high level(11.530?5.312pg/ml)on postoperative day 3,and disappeared on day 5.Conclusions Plasma BPI/LBP ratios were markedly elevated during 3 days after a major abdominal surgery,and endogenous elevated BPI may play an important role in prevention from the development of endotoxemia in surgical patients.
6.Effects of high glucose and losartan on the expression of Smad in human peritoneal mesothelial cells

Objective To investigate if the mechanism of high glucose and losartan in mediating the expression of Smad in human peritoneal mesothelial cells (HPMC) and the possible management of HPMC. Methods Peritoneum was obtained from patients undergoing elective abdominal surgery. HPMCs were incubated in medium containing different concentrations of dextrose, mannitol (1. 5%, 2. 5% , 4. 25% ) and combination with dextrose and losartan. TGF-?1 in supernatant was detected by ELISA and HPMCs were collected to examine Smad family expression with RT-PCR and Western Blot. 7.Content Determination of Polysaccharides in Radix and Rootlet Ophiopogonis by the method of Phenylhydrate Sulfuric Acid

Objective To detect the content of polysaccharides in Radix and Rootlet ophiopogonins. Methods Phenylhydrate-sulfuric acid method was used. Results The maximum absorption wavelength was 485 nm. Linear range was 0.101~1.012 mg/L (A=0.826 7C+0.016 3, r=0.999 2). The polysaccharide content in Radix ophiopogonis was 0.301 6 mg/g, and in Rootlet ophiopogonis was 0.359 4 mg/g. The average recovery rate was 100.30% and RSD was 3.14%. Conclusion The polysaccharide content in Rootlet Ophiopogonis was not lower than in Radix Ophiopogonis. It deserves to be more developed and researched.
8.Analysis of clinical outcome in long-term peritoneal dialysis patients

Objective To analysis the clinical outcomes in long-term peritoneal dialysis (PD) patients. Methods The data of 58 PD patients survived more than 3 years from January 1994 to August 2003 in our hospital were reviewed. According to their different clinical outcomes, the patients were divided into four groups:continuous PD group, transplant group,hemodialysis (HD) group and death group. The recent nutritional index, such as serum albumin, and the recent dialysis adequacy index, including fluid removal and residual renal function, were evaluated. The "predeath values" of 1/2 year and 1 year prior to death in the death group were compared. 12-month PD indices in continuous PD patients were reviewed retrospectively and the same indices over a 12-month period of time were followed up. Objective To investigate the mecA gene expression lvevl in clinically isolated Staphylococcus aureus (SA) strains and its correlation with drug resistance .Methods Clinically isolated 186 SA strains were collected .The K-P method was adopted to conduct the durg sensitivity test .Then DNA of these strains was extracted and the mecA gene was amplified by using PCR .Results The detection rate of methicillin-resistant SA(MRSA) was 30 .65% (57/186) ,the positive mecA gene was detected in 56 strains of MRSA and 10 strains of methicillin susceptible S .aureus(MSSA) among 129 strains of MSSA ;except susceptible to vancomycin and linezolid ,the resistance rate of MRSA to ther antibacterial durgs were higher than that of MSSA ,the resistance rate to antibac-terial durgs in the strains carrying mecA gene was higher than that in the strains without carrying mecA gene ,ther difference be-tween them had statistical significance (P<0 .05) .Conclusion Clinically isolated SA strains carrying mecA gene are resistant to multiple antibacterial drugs ,which indicating that mecA gene play an important role in SA drug-resistance mechanism .
10.Association of serum soluble intercellular cell adhesion molecule-1, soluble vascular cell adhesion molecule-1 and hypersensitivity-CRP levels with peripheral vascular disease of lower limbs in patients with type 2 diabetes mellitus

Objective To investigate the association of serum levels of soluble intercellular cell adhesion molecule-1 (sICAM-1),soluble vascular cell adhesion molecule-1 (sVCAM-1) and high sensitivity C-reactive protein (hs-CRP) with peripheral vascular disease of lower limbs in patients with type 2 diabetes mellitus (T2DM).Methods One hundred and thirty T2DM patients admitted from October 2011 to October 2012,and 30 age/sex-matched healthy subjects were enrolled in the study.The serum levels of sICAM-1,sVCAM-1,hs-CRP and other clinical parameters were measured; the peripheral blood vessels of lower limbs were examined with color Doppler ultrasonography.Based on the extent of angiopathy of lower limbs T2DM patients were classified as normal vascular group (n =26),mild angiopathy group (n =45),moderate/severe angiopathy group (n =59).Results The serum levels of sICAM-1 and sVCAM-1 in moderate/ severe angiopathy group of T2DM patients were higher than those in mild angiopathy group,normal vascular group and healthy controls (t:4.15-8.93,all P <0.05) ; the serum levels of hs-CRP in moderate/severe angiopathy group were higher than those in mild angiopathy group,normal vascular group and healthy controls (t:2.18-4.27,all P < 0.05).The serum sICAM-1 level was positively correlated with total cholesterol (TC),low density lipoprotein cholesterol (LDL-C) and sVCAM-1.The serum sVCAM-1 level was positively correlated with course of disease,systolic blood pressure and CRP.Conclusions Serum levels of sICAM-1,sVCAM-1 and hs-CRP are correlated with the extent of angiopathy of lower limbs in T2DM patients,and the elevated sICAM-1 ; sVCAM-1 and hs-CRP levels are also associated with hyper blood pressure,dislipidemia and chronic inflammation.
