Objective To establish a method for the determination of 17 kinds of free amino acid in serum by the ultra perform-ance liquid chromatography(UPLC) with AQC pre-column derivatization .Methods After proteins precipitation in serum sample by acetonitrile ,free amino acids with aminoquinoline-N-butyl hydroxy diimide and carbamate(AQC) were reacted to generate the struc-tural stable derivative products with ultraviolet absorption .These products were separated by reversed-phase C18 column ,detected with UV detector and quantitated by the external standard .Results The within-run precision of 17 kinds of free amino acid was 2 . 81% -7 .35% and the between-run precision was 7 .44% -15 .40% .The linear relationship was good in the range of 2 .5-250μmol/L ,the linear correlation coefficient was 0 .9960 -1 .0000 ,RSD<3% .Conclusion Using this method for determining 17 kinds of amino acid in serum ,the complete separation is obtained within 10 min ,the results are accurate ,stable and highly efficient .

There are many causes of postpartum hemorrhage in lying-in women.The authors retrospectively analyzed the clinical data of 40 cases of uterine inertia-induced postpartum hemorrhage,investigated its causes,and proposed that for different cases midwives should take different preventive measures,such as psychological persuasion,childbirth guidance,anticipation,prediction,and pretreatment,so as to reduce the incidence of uterine inertia-induced postpartum hemorrhage.

Pulmonary edema frequently occurs after severe burn. It not only impairs pulmonary function directly, but also can induce or exacerbate other pulmonary complications such as lung infection, acute lung injury ( ALI), and ARDS. Extravascular lung water (EVLW) is closely related to the pulmonary edema. Dynamical monitor of EVLW has been used to predict and quantify the degree of pulmonary edema clinically. This review focuses on the recent progresses at home and abroad on the formation mechanism, monitoring approach, and prevention and treatment of EVLW after severe burn injury.

ObjectiveTo summarize the clinical characteristics of pt osis,one of complications after double eyelid blepharoplasty, and to explore i ts causes and attempt for the resolutions. MethodsThe clinica l symptoms were divided into three types based on the degrees of the ptosis afte r double eyelid blepharoplasty. The causations in 25 cases of different degree o f ptosis after double eyelid blepharoplasty were investigated and analyzed durin g operations. The local adhesive deformities were analyzed so as to take differe nt resolutions for the complication. Results It seemed that th e operative damage to the aponeurosis was the main reason for the complication. So it caused the scar adhesive deformities between the palpabralis aponeurosis a nd the orbital membrane or the skin of upper eyelid, resulting in the difficult y of the palpabralis muscle's contract and unablility of the muscle function in different degrees. Conclusion The ptosis can surely be av oided if we have good knowledge about the mechanism of double fold formation and the anatomy of eyelids and perform the operation meticulously.

Objective To investigate the red blood cell in urine after kidney puncture and know the best detection method.Methods Collected three urines of 31 patients after kidney puncture and detected the red blood cell by UF-100 and the hemoglobin by urine dipstick test.At the same time,detect the conductivity of the urine by UF-100.Results Both the UF-100 and urine dipstick test could reflect the decrease of red blood cell after kidney puncture.But both of them had some limits.Sometimes the results of the two method were not relative(κ=0.148).The agreement of two methods was 0.538.Neither of them could reflect the bleeding of the kidney accuracy.Conclusion By the best,we should use both the UF-100 and the urine dipstick test to detect the red blood cell in urine after kidney puncture,at the same time we should consider the conductivity of the UF-100.

Objective To observe the oculocardiac reflex during the ocular plasty and to study its possible mechanism. Methods A total of 132 cases were studied by dynamic electrocardiography to analyze the changing of heart rate before and after operations. Results Heart rate decreasing by over 10 times during operations was regarded as the positive standard of the oculocardiac reflex. Of all cases the oculocardiac reflex appeared in 41 cases and 53 times were observed including of continual two times happened in 6 cases. In all the reflex ,9 cases was observed in injection of anaesthetic, 5 cases in pulling eyelids, the orbicular muscle and the palpabralis muscle, 21 cases in resecting orbital fats,6 cases in stressing the eyeball after operations. The incidence of oculocardiac reflex was 31.1%. Conclusion Some steps in the process in ocular plasty are more possible to elicit the reflex, such as dragging peripheral eyeball muscles,stressing eyeball and pulling eyelid. The reflex will result in heart rate decreasing, heart rhythm and respiratory changing,even render heart to sudden stop . Therefore, we should operate gentlely and accurately,and also avoid overly stress on eyeball to elicit the oculocardiac reflex.

Objective To study the CT findings of small thyroid carcinoma. Methods The CT findings of 40 patients with histology-proven small thyroid carcinoma (diameter, 1.0 to 2. 0 cm) were retrospectively reviewed. Results (1)The single lesion was detected in 38 cases and two lesions in bilateral thyroid in 2 cases. Two cases were combined with contralateral nodular goiter and I case with contralateral thyroid adenoma. ( 2 ) Eight lesions showed smooth edge and complete envelope. Thirty-four lesions demonstrated foggy edge and incomplete envelope,but they didn't invade the surrounding soft tissues and important organs. ( 3 ) The density of all lesions were homogeneous or comparatively homogeneous without obvious hemorrhage or necrosis area on non-enhanced CT. Thirty lesions showed varied shape calcifications,with granular calcifications in 20 lesions being the most common. Irregular nodular,eggshell-like or mulberrylike calcifications were also detected. (4)Forty-one lesions showed marked enhancement on post-contrast CT and the amplitude of enhanced CT value was greater than 40 HU(range,90 to 140 HU). Thirty-eight lesions exhibited homogeneous enhancement, and other 3 lesions showed marked enhancement center with a ring-like low density edge and manifested as a characteristic damascene-like appearance. (5)Enlarged cervical lymph nodes were found in 24 cases ( 60. 0% ), which displayed solid, cystic-solid or cystic appearances on nonenhanced CT. They showed markedly homogeneous,irregular ring or wall-node enhancement on post-contrast CT. In 8 cases there were granular, nodular or eggshell-like calcifications within the enlarged lymph nodes.Conclusion A solid thyroid nodule with granular calcification, incomplete envelope and marked enhancement, companied with enlarged lymph nodes with calcification, cystic degeneration and obviously enhanced solid part are the relatively characteristic CT features of small thyroid carcinoma.

Objective To explore the mechanism of sulfotanshinone sodium injection in treatment of patients with sudden deafness (SD).Methods Sixty patients with SD admitted to the Department of Otorhinolaryngology of Wuxi Traditional Chinese and Western Medicine Hospital from January to December 2016 were enrolled, and they were randomly divided into a study group and a control group (each 30 cases). The same basic treatment was given in the two groups, the patients in the study group were treated with sulfotanshinone sodium 40 mg intravenous (IV) drip, while the patients in the control group were treated with vinpocetine sodium chloride 20 mg IV drip, once a day for consecutive 14 days to complete a therapeutic course, and two courses were carried out in bothgroups. Before and after treatment, the changes of hearing threshold, indexes of hemorheology and immune function were compared between the patients in the two groups, and the clinical efficacy and adverse reactions in the two groups were observed.Results After treatment, the hearing threshold, hemorheology indexes, immune function index of CD8+ were significantly lower than those before treatment, while the CD3+, CD4+, CD4+/CD8+ ratio were significantly higher than those before treatment in the two groups, and the above changes of indexes were more obvious in the study group than those in the control group hearing [threshold (dB): 16.63±2.04 vs. 17.15±1.88, plasma viscosity (PV, mPa·s): 1.27±0.14 vs. 1.31±0.11, whole blood middle shearing viscosity (mPa·s): 4.77±0.33 vs. 4.95±0.28, whole blood high shearing viscosity (mPa·s): 3.86±0.25 vs. 4.00±0.31, erythrocyte aggregation index (EAI): 1.57±0.29 vs. 1.72±0.34, CD3+: 0.70±0.05 vs. 0.64±0.05, CD4+: 0.43±0.04 vs 0.37±0.03, CD8+: 0.32±0.04 vs. 0.34±0.03, CD4+/CD8+: 1.36±0.32 vs. 1.18±0.27]; the degree of whole blood low shearing viscosity (mPa·s: 6.72±0.80 vs. 7.01±1.13) and hematocrit (HCT: 0.38±0.04 vs. 0.40±0.03) decreasing weremore significant in the control group than those in the study group. The total effective rate was higher in study group than that in the control group [86.67% (26/30) vs. 83.33% (25/30)], but the difference between the two groups was not statistically significant (P > 0.05); the incidence of adverse reactions in the study group was markedly lower than that in the control group [3.33% (1/30) vs. 20.00% (6/30),P < 0.05].Conclusions Sulfot anshinone sodium injection can effectively enhance the SD patients' hearing, and improve their hemorheology indexes and immune function; the therapeutic results of sulfotanshinone sodium injection in safety and improvement in immune function are superior to those of vinpocetine sodium chloride injection.

Objective To evaluate the efficacy of percutaneous CT guided interstitial 125Ⅰ seeds implantation treatment for refractory pelvic malignant tumors and discuss the procedure of technique.Methods Twenty-three patients with refractory pelvic malignant tumors(25 lesions,diameter 3.5-7.0 cm,mean 4.5 cm) showing failure response to full chemotherapy and/or radical radiotherapy after tumors resection were undergone percutaneous CT guided intratumoral 125Ⅰseed implantation.Treatment plan system(TPS)was used to design the distribution and number of 125Ⅰ seeds according to matched peripheral dose(MPD)1-3 days before the procedure.Of which 6 cases received combined internal iliac arterial infusion chemotherapy before or after the 125Ⅰ seed implantation procedure.Results 9 ～ 75(mean 27)125Ⅰ seeds were implanted into a single tumor at first time including 6 patients with intraarterial chemotherapy for 14 cycles(mean 2.3 cycles),showed relief of clinical pain symptoms in 16 of 23 cases,72 h ～ 4 w after the seeds implantation;with the an effective rate of 69.6%.Follow up for 2-34 months(median,21 months),CT or MRI performed 2 months after the seeds implantation showed no CR,but PR in 18 cases,SD in 4 cases,and PD in 1 case,with overall response rate of 78%(18/23),and no serious complication.In addition,20 cases survived,with the longest one of 34 months and the other 3 died.Conclusions Intratumoral 125Ⅰ seeds implantation under CT guidance for pelvic refractory malignant tumors is safe,minimally invasive,and effective.

Objective To establish the immunoprecipitation method to detect the activity of ?-glutamyltransferase (GGT) complexed to low-density-lipoprotein and very-low-density-lipoprotein in serum (LDL-LDL-GGT) and to observe the diagnostic efficiency in discriminating liver malignancies from other hepatopathy diseases.Methods Using apo B antibody as a precipitant to precipitate the parts of GGT which connected to LDL and VLDL. 65 cases of liver malignancies, 53 cases of cirrhosis, 32 of chronic hepatitis and 75 of health people were analysed. Results Apo B antibody can precipitate LDL and VLDL thoroughly with various serum samples. The withinrun CV are 3 6%～8 2% and the betweenrun CV are 5 5%～9 8%. The linearity range is 0～587 U/L. With the cut off value of 10 U/L, the sensitivity for liver malignancies is 86 2%, the specificities for discriminating liver malignancies from cirrhosis and chronic hepatitis is 67 9% and 81 3% respectively. Conclusion It has important significance for the diagnosis of liver malignancies with the immunoprecipitation method for determination of serum LDL-VLDL-GGT activity.
