1.Quality evaluation of radix salvia miltiorrhizae tablets and the methods thereof

Objective To evaluate the quality of six kinds of radix salvia miltiorrhizae tables and verify the methods of the quality evaluation . Methods Identification, detection and quantitative test method in Chinese Pharmacopoeia Part One 2005 edition and the HPLC-FPC analysis method were used to evaluate the quality of six kinds of radix salvia miltiorrhiza tables.Results The content of tanshinone IIA of six samples is not in accordance with that in the Chinese Pharmacopoeia and the content of lithospermate B of three samples is not in accordance with that in the Chinese Pharmacopoeia.Both the content of lip-soluble component and the water-soluble component of all the samples differ from those of standard radix salvia miltiorrhiza tables.Conclusion The HPLC-FPC analysis method is useful and effective to identify and detect the quality of the HPLC-FPC analysis method.More attention shall be paid to the quality of the radix salvia miltiorrhiza tables and the supervision of the radix salvia miltiorrhizae tables market should be strengthened.
2.Study on Quality Standard of Sanyuan Rupixiao Gel Paste

Objective To establish the quality standard for Sanyuan Rupixiao Gel Paste. Methods Sparganii Rhizoma, Gleditsiae Sinensis Fructus, Cyperi Rhizoma and Impatientis Semen were identified by TLC method. The content of tetrahydropalmatine was determined by HPLC. Waters symmetry column was used with the mobile phase of acetonitrile-0.1% phosphatic acid in a gradient manner (pH was adjusted to 6.4 by triethylamine) (55:45) at the detection wavelength of 280 nm. The flow rate was 1.0 mL/min at the column temperature of 30 ℃. Results The spots in TLC were clear without any interference;tetrahydropalmatine showed a good linear relation in the range of 0.092–1.84 μg;the average recovery was 100.15%with RSD of 1.58%(n=6). Conclusion The method is simple and accurate with high reproducibility, which can be used for the quality control of Sanyuan Rupixiao Gel Paste.
3.Construction and validation of a risk assessment model for frailty in elderly patients with lower extremity osteoarthritis

Objective To analyze the influencing factors of frailty in elderly patients with lower extremity osteoarthritis,and to construct and validate the risk assessment model.Methods Convenient sampling method was used to select 535 elderly patients with lower extremity osteoarthritis from tertiary hospitals and community health service centers in Hangzhou from January to September 2022 as the survey subjects including 357 in the modeling group and 178 in the validation group.Univariate and multivariate logistic regression analysis were used to determine the risk factors of frailty,construct a risk assessment model and draw a nomogram.The discrimination and calibration of the model were evaluated by the area under the receiver operating characteristic curve and the Hosmer-Lemeshow test.The Bootstrap method was used for intemal validation of the model,and the time verification method was used for external validation.Results The model variables included the number of affected joints,age-adjusted Charlson comorbidity index,pain,nutritional status,sedentary time,activity of daily living,osteoarthritis index,lower limb muscle strength,and Social Support Rating Scale score.The Hosmer-Lemeshow test results of the model showed that P=0.202,the area under the receiver operating characteristic curve was 0.942,the optimal critical value was 0.392,the sensitivity was 0.914,the specificity was 0.893,and the accuracy rate was 0.902.The internal and external validation showed that the C-statistics were 0.935 and 0.919,respectively,and the calibration curve showed good fitting.Conclusion The risk assessment model has a good degree of discrimination and calibration,which can more intuitively and easily screen elderly patients with lower extremity osteoarthritis at high risk of frailty,and provide references for early monitoring,identification,prevention and control.
4.Preliminary study on thyroid ultrasound image restoration algorithm based on deep learning

Objective:To explore the feasibility of deep learning-based restoration of obscured thyroid ultrasound images.Methods:A total of 358 images of thyroid nodules were retropectively collected from January 2020 to October 2021 at Sir Run Run Shaw Hospital, Zhejiang University School of Medicine, and the images were randomly masked and restored using DeepFillv2. The difference in grey values between the images before and after restoration was compared, and 6 sonographers (2 chief physicians, 2 attending physicians, 2 residents) were invited to compare the rate of correctness of judgement and detection of image discrepancies. The ultrasound features of thyroid nodules (solid composition, microcalcifications, markedly hypoechoic, ill-defined or irregular margins, or extrathyroidal extensions, vertical orientation and comet-tail artifact) were extracted according to the Chinese Thyroid Imaging Reporting and Data System (C-TIRADS). The consistency of ultrasound features of thyroid nodules before and after restoration were compared.Results:The mean squared error of the images before and after restoration ranged from 0.274 to 0.522, and there were significant differences in the rate of correctness of judgement and detection of image discrepancies between physicians of different groups(all P<0.001). The overall accuracy rate was 51.95%, the overall detection rate was 1.79%, there were significant differences also within the chief physicians and resident groups (all P<0.001). The agreement rate of all ultrasound features of the nodules before and after image restoration was higher than 70%, over 90% agreement rate for features such as solid composition and comet-tail artifact. Conclusions:The algorithm can effectively repair obscured thyroid ultrasound images while preserving image features, which is expected to expand the deep learning image database, and promote the development of deep learning in the field of ultrasound images.
5.Research advance in pharmacokinetic/pharmacodynamic characteristics and dose adjustment of ceftazidime-avibactam in special populations

Ceftazidime-avibactam (CAZ/AVI)is a novel β-lactam antibiotic with broad-spectrum antibacterial activity and good tolerability. However, the physiological and pathological differences in special populations [e.g. augmented renal clearance (ARC) patients, undergoing continuous renal replacement therapy (CRRT) patients, neonates and children, obese patients, undergoing extracorporeal membrane oxygenation (ECMO) patients, elderly patients and liver dysfunction patients] may affect the pharmacokinetic (PK) properties of CAZ/AVI, leading to treatment failure. At present, there is currently a lack of corresponding guidelines or consensus on dose adjustment of CAZ/AVI in special populations. This article summarizes the research on PK/ pharmacodynamic (PD) characteristics and dose adjustment of CAZ/AVI in special populations and recommends the following dosing regimens: for ARC patients, the recommended dose is 2.5 g, q8 h; for undergoing CRRT patients with infections caused by sensitive strains (i.e. MIC<4 mg/L) and infections at sites where hydrophilic antibiotics distribute well, a dose of 1.25 g, q8 h may be used; for undergoing CRRT patients with less sensitive strains or sites with poorer drug distribution, a dose of 2.5 g, q8 h or continuous infusion may be considered; for children aged 6 months to <18 years with normal or mildly impaired renal function, a dose of 62.5 mg/kg, q8 h is infused for 2 h (maximum dose not exceeding 2.5 g per dose); for infants aged 3~6 months with normal or mildly impaired renal function, a dose of 50 mg/kg, q8 h is infused for 2 h; for obese patients, the recommended dose is 2.5 g, q8 h, with therapeutic drug monitoring recommended;undergoing ECMO patients, elderly patients, and those with impaired liver function may also use the recommended dose of 179368757@qq.com 2.5 g, q8 h.
6. Study on the prediction of cervical lymph node metastasis risk in preoperative thyroid papillary carcinoma by ultrasonic elemental observation of thyroid nodules

Objective:
To evaluate the correlation between ultrasound features of papillary thyroid carcinoma (PTC) and lymph node metastasis by preoperative ultrasound elemental observation of thyroid nodules.
Methods:
Three hundred and seventy-six patients who underwent primary thyroid surgery and confirmed by ultrasound and pathological data as single-focal PTC from Jannary to December 2017 in Sir Run Run Shaw Hospital of Zhejiang Univbersity College of Medicine were retrospectively analyzed. According to the presence or absence of lymph node metastasis, they were divided into central and lateral lymph node metastasis group and non-metastasis group. Independent risk factors for central lymph node metastasis (CLNM) and lateral lymph node metastasis (LLNM) were analyzed by χ2 test and multivariate Logistic regression.
Results:
Multivariate analysis showed that the posterior margin of the cancer was <0.25 cm from the posterior wall of the thyroid gland as an independent risk factor for CLNM (
