Patient′s expectation has been defined as the anticipation that given events were likely to occur during, or as an outcome of, health care. What people expect from their health care are potentially important in predicting patient satisfaction and dissatisfaction with their care, treatment and health outcomes. There was widespread recognition of the importance of evaluating services from consumer perspectives. In this article, domestic and foreign research in recent years, expectations status, significance and various influencing factors in relation to the expectations in processes of health care were summarized. Consumer evaluations of their health care are now an established component of quality assessment, planners need to understand the expectations underlying patients′views in order to interpret their feedback. Thus leading to improve patient satisfaction and the social evaluation of medical units.

Objective To extract principal components from valuable indicators on conventional ultrasoundand contrast-enhanced ultrasound (CEUS) in the differential diagnosis of thyroid benign and malignant lesions and to discuss the diagnostic value of each indicator. Methods One hundred and three patients with 125 thyroid lesions (65 malignant lesions and 60 benign lesions) underwent preoperative grayscale ultrasound (GSUS),color Doppler ultrasound (CDUS) and CEUS examinations.Eighteen indicators were chosen to evaluate every lesion and principal components were extracted by principal component and valuable indicators were ordered by importance. Results There were significant differences on GSUS and CEUS indicators between benign and malignant lesions. The first principal component was the representation of contrast enhanced ultrasound and was valuable in the differential diagnosis of thyroid benign and malignant lesions. The rank of valuable CEUS indicators were homogeneity of enhancement, filling defect, relative arrival time of periphery, peak interior echogenicity, relative arrival time of interior, peak peripheral echogenicity and ring enhancement. Conclusions GSUS and CEUS are valuable in the differential diagnosis of thyroid benign and malignant lesions.

BACKGROUND:The cross-linking reagent is the main unit to support the skeleton of molecularly imprinted polymers, which is closely related to the bio-friendly adaption of polymers. The biocompatible and biodegradable capacities of common crosslinking agents are unclear.
OBJECTIVE:To prepare a novel biodegradable molecularly imprinted polymer and to evaluate the adsorption and biodegradability properties of the polymer.
METHODS:The biodegradable molecularly imprinted polymer was synthesized with the acrylated polyε-caprolactone as the cross-linking reagent under the UV polymerization. The adsorption of the polymer was evaluated by adsorption isotherm, Scatchard analysis and kinetic curves. The biodegradation of the polymer was observed in the simulated physiological environment system in vitro.
RESULTS AND CONCLUSION:Both the molecularly imprinted polymer and non-molecularly imprinted polymer showed an adsorption effect on theophyl ine, the template. But the adsorption amount of the molecularly imprinted polymer was significantly more than that of the non-molecularly imprinted polymer. The drug loading and encapsulation efficiency of the molecularly imprinted polymer were 1.54%and 12.48%, respectively. The degradation rates of molecularly imprinted polymer and poly(ε-caprolactone) diol were 6.60%and 1.33%, respectively, within the observation time in vitro. The obtained molecularly imprinted polymer not only exhibited specific adsorption to certain molecules but also showed good biodegradable properties in the simulated physiological environment system, which is necessary to be a potent drug carrier.

Objective To explore the application of submental ultrasonongraphy (SUS) in the assessment of oropharyngeal swallowing disorders in children with cerebral palsy. Methods Seventeen children with cerebral palsy and oropharyngeal swallowing difficulties (7 on nasal feeding,10 on oral feeding) constituted the treatment group while 20 normal counterparts formed the control group.SUS was applied to measure any changes in the thickness of the tongue muscle and the range of hyoid bone displacement when they swallowed 5 ml of water. The results were compared with those assessed using the functional oral intake scale to decide the best cut-off point for detecting tube-feeding-dependent dysphagia. The intraclass correlation coefficient (ICC) of the 20 children in the control group was calculated to evaluate the intra-rater and inter-rater reliability of SUS.Results The average tongue muscle thickness change and hyoid bone displacement amplitude of the children on nasal feeding were significantly smaller than those of the children without nasal feeding and the normal children. The best cut-off point for the tongue muscle thickness change data was 1.0 cm,and that of the hyoid bone displacement amplitude was 1.5 cm.All of the ICCs were above 0.4,indicating good intra-rater and inter-rater reliability for the SUS examination. Conclusion Submental ultrasonongraphy can help assess the swallowing function of children with oropharyngeal swallowing disorders.

Objective:To observe the effects of low intensity pulsed ultrasound(LIPUS) on the osteogenic differentiation of rat bone marrow mesenchymal stem cells(BMSCs) on titanium surface. Methods:BMSCs from Wistar rat bone marrow were respectively cultured on the flat titanium surface and the large grain blast acid etched(SLA) titanium surface, and induced by mineralization medium. Then, the cells were interfered by LIPUS and a control condition.Alkaline phosphatase(ALP) were quantitative determinated after 3 and 7 d mineralization induction respectively,ALP staining were observed after 14 d induction.Alizarin red staining were observed after 21 d mineralization induction.Osteogenic related protein and gene expressions were detected after mineralization induction. Results:ALP in culture medium of LIPUS group was higher than that of the control group after 3 d and 7 d mineralization induction(P<0.05).LIPUS group showed stronger ALP staining and alizarin staining, and more mineralized nodules than control group. The expression of osteogenic related proteins, including Runx2,BMP2,OPN in LIPUS group increased.Osteogenic related genes expression, including ALP,Runx2,BMP2,OPN,OCN and Col-1 of the LIPUS group increased. Conclusion:The osteogenic differentiation of BMSCs on the fiat titanium surface or SLA titanium surface can be promoted by LIPUS.
