Objective:
To improve the knowledge and experience of ibrutinib combined with CAR-T cells in the treatment of high-risk chronic lymphoblastic leukemia (CLL) patients or small lymphocytic lymphoma (SLL) with TP53 gene aberration.
Methods:
One case of del (17p) CLL patients with BCL-2 inhibitor resistance was treated with ibrutinib combined with CAR-T cells, successfully bridged to allogeneic hematopoietic stem cell transplantation (allo-HSCT) , and the relative literatures were reviewed.
Results:
The patient was a young female with superficial lymph node enlarging at the beginning of the onset. Lymph node biopsy was confirmed as small lymphocytic lymphoma (SLL) without del (17p) . The disease progressed rapidly to CLL/SLL with del (17p) and bone marrow hematopoietic failure 2 years later. Firstly, the patient was treated with BCL-2 inhibitor (Venetoclax) , and the enlarged lymph nodes shrank significantly 2 months later. After 3 months, the disease progressed rapidly. The spleen was enlarged to 16 cm below the ribs, the neck lymph nodes was rapidly enlarged, and the superior vena cava syndrome appeared, which were mainly attributed to venetoclax resistance; so BTK inhibitor (ibrutinib) was used continuously after venetoclax discontinuation. Partial remission (PR) was achieved without lymphocytosis after 2 months, then ibrutinib was combined with CAR-T cells targeting CD19 antigen. Grade 1 of cytokine release syndrome (CRS) appeared after CAR-T cells infusion, and the complete remission (CR) was achieved after 1 month both in bone marrow and peripheral blood, with minimal residual disease (MRD) negative, then bridging allo-HSCT after 2 months of combined therapy.
Conclusion
CLL/SLL patients with TP53 aberration have poor prognosis because of rapid progression, drug resistance, etc. Ibrutinib combined with CAR-T cell therapy can quickly achieved complete remission.

Objective:Investigate the relationship between dietary patterns and sarcopenia in a Chinese elderly population. Methods:Participants in this cross-sectional study were recruited from the Tianjin Chronic Low-grade Systemic Inflammation and Health Cohort Study. The study population comprised 2 423 participants, with mean age of (67.6±5.2) years. Sarcopenia was defined based on the guidelines of the Asian Working Group for Sarcopenia. Three dietary patterns were derived by factor analysis: fruit and sweet pattern, traditional oriental pattern, and animal food pattern. The association between quartile categories of dietary pattern scores and the presence of sarcopenia was analyzed using multiple logistic regression models. Odds ratios ( OR) and 95% confidence interval ( CI) were calculated. Results:The prevalence of sarcopenia was 16.1%. After adjusting for confounding factors, compared with the lowest quartile, the adjusted OR ( 95%CI) of sarcopenia for the highest quintile of Fruit and sweet pattern score, Traditional oriental pattern score and Animal food pattern score were 1.06 (0.74, 1.50), 0.54 (0.34, 0.86), and 0.50 (0.33, 0.74), ( P for trend were 0.87,<0.01, and<0.001), respectively. Conclusions:The current study found that the traditional oriental pattern and animal food pattern has a protective relation for sarcopenia in elderly adults, which suggests its potential to attenuate or prevent the progression of sarcopenia.

Objective:To analyze the expectations and their influencing factors of gynecological examination among female physical examination population. Methods:Convenience sampling method was used to select women undergoing gynecological examinations in the health management center of the Shandong Provincial Hospital Affiliated to Shandong First Medical University between April to June 2021. A face to face interview was conducted with a 20-item structured questionnaire including general information, the women?s feelings about a gynecological examination, expectations of doctors, preferences for the doctor ?s gender, and the presence of chaperones in male doctor examination scenarios. Of the 1 600 questionnaires distributed, 1 583 were valid, with a recovery rate of 98.93%. To compare the preferences for a doctor ?s gender and chaperones with different demographic characteristics, an χ2 test was used. Logistic regression was used to analyze the influencing factors of preferences for doctors ?gender and for chaperones. Results:For a gynecological examination, women were mainly worried about the possibility of bad results (77.4%), while the main expectation to doctors was communication before the examination (49.2%); most women wanted to be examined by a female doctor (85.5%), mainly because they felt relaxed (76.0%); most women did not want chaperones when examined by a female doctor (56.5%), while most of them preferred to be accompanied by a nurse when examined by a male doctor (50.1%). Women whose family income was lower than expenditure and had gynecological examination experiences with male doctors tended to have no preference for a doctor ?s gender. Additionally, there was a strong inverse association between the education level and likelihood of no preference for a doctor ?s gender, that is, the lower the education level, the more likely they were to have no preference for the doctor ?s gender ( P<0.05), when examined by a male doctor. Furthermore, women older than 55 years and who had a gynecological examination more than twice a year, was less likely to need chaperones, while women who had gynecological examination experience with male doctors tended to have no preference for chaperones. Conclusions:Women preferred gynecological examinations to be performed by female doctors who provide gentle and companion staff support during the examinations. Education level, family income, and examination experience of male doctors are the influencing factors for gender preferences of a doctor. Age, examination frequency, and examination experience of male doctors are the influencing factors affecting the preferences of choosing an accompanying person.
